Objective To discuss the reason of pain on the shoulders and epigastrium after the reprotenal laparoscopic operations. Methods One hundred and eight patients had undergone the laparoscopic adrenalectomy or laparoscopie renal cyst unroofing. They were divided into two groups at random, group A and group B. In group A, discharged CO2 as soon as finished the operation, but did not in group B. At the same period, there were 49 patients who had received retroperitoneal laparoscopic radical nephrectomy, divided into two groups by the CO2-pheunopertomeum time, group C and group D. In the group D, the CO2-pheunopertomeum time was over 90 minutes, and the time was less than 90 minutes in the group C. Evaluated the pain degree through the vision analogue score (VAS) and recorded the pH,PaO2, PaCO2,BE in the arterial blood in group A and B before and after operation. Results The incidence rate of pain postoperation was significantly higher in group B than A (P < 0.05). In group A and group B, there was no difference in PaCO2, PaO2, pH, BE in the arterial blood(P > 0.05 ). The incidence rate of pain and the grade of VSA both were significantly higher in group D than group C (P < 0.05). Conclusions The stimuli of acid materials such as carbonate that derived from postoperative residual CO2 is probably the main cause of the pain on the shoulders and epigastrium after the laparoscopicsurgery.

Objective To evaluate the perfectibility of colposcopy directed biopsy for the diagnosis of cervical intraepithelial(CIN)through cervial conization by loop electrosurgical excision procedure(LEEP).Methods Biopsy by colposcopy and biopsy after LEEP were analyzed retrospectively in 108 cases of cervical intraepithelial neoplasia to study the related factors of affecting the coincidence rate.Results Comparison of pathological hanges following both LEEP and colposcopy showed that 69 cases were the same,24 upgrade,and 15 downgrade.Conclusion LEEP is a kind of effective and perfect method for the specific diagnosis and treatment of CIN.Colposcopically directed biopsy has its limitations.

BACKGROUND:There is evidence that internal fixation through an anterior or posterior approach for treatment of severe kyphotic deformity in spinal tuberculosis exhibits good curative effects. However, few prospective, long-term fol ow-up case control studies are reported. OBJECTIVE:To investigate the efficacy and safety of cobalt al oy pedicle screw implantation for treatment of severe kyphotic deformity in spinal tuberculosis. METHODS/DESIGN:This is a prospective, single-center, self-control, open-label trial, which wil be performed at the Affiliated Hospital of Hebei University, China. Eighty-four patients with severe kyphotic deformity in spinal tuberculosis wil be included according to the diagnosis criteria. Among 52 patients with tuberculosis of the thoracic spine, 28 wil undergo surgery through a posterior approach, and 24 through an anterior approach. According to the Frankel Grade classification, grade C, D and E spinal cord function wil be assessed in 8, 31 and 13 patients, respectively. Surgery through a posterior and anterior approach wil be respectively performed in half of 32 patients with tuberculosis of the lumbar spine. Grade C, D and E spinal cord function wil be assessed in 7, 14 and 11 patients, respectively. The primary outcome measure of this study wil be the Cobb angle at the thoracic spine segments before and 2 years after surgery, which wil be used to evaluate the angle of the spine curvature at the thoracic segments. The secondary outcome measures wil be X-ray scan or MRI findings before and 2 years after surgery, which wil be used to evaluate vertebral fusion after internal fixation;and Frankel Grade before and 2 years after surgery, which wil be used to evaluate recovery of spinal cord function after injury. Other outcome measures wil include multiple logistic regression analysis results of the factors that influence patient's curative effects and the incidence of adverse events 2 years after surgery. The trial protocol has been approved by the Ethics Committee, Affiliated Hospital of Hebei University, China and wil be performed in strict accordance with the Declaration of Helsinki, formulated by the World Medical Association. Signed informed consent regarding the trial protocol wil be obtained from each participant. DISCUSSION:This study is to validate that cobalt al oy pedicle screw implantation shows precise curative effects in the treatment of severe kyphotic deformity in spinal tuberculosis and to analyze through what approach, posterior or anterior, internal fixation wil be more beneficial to surgery performance. Objective To study the stability of micro-implant orthodontic anchorage(MA)with different pitch in the case of immediate loading.Method Employing 3D finite element analysis method,the stress and dis-placement distribution on the bone interface of MIA with different pitch(0.3 mm、0.5 mm、0.7 mm and 1.0 mm,respectively),which was 1.47 N loaded vertically in the major axis direction,were analyzed.Result The pitch affected the stress distribution significantly,because the maximum stress increased with the pitch decreasing and the impact of pitch on stress distribution on neck and central locations of MIA were different;to decrease the pitch could reduce the max displacement of the jaw,but the impact of pitch on displacement distribution of MIA was not significant.Condusions In the case of immediate loading.MIA with pitch 0.5 mm-0.7 mm is suggested to be selected as orthodontic anchorage in the clinic.

Objective:To investigate the clinical significance of peripheral blood neutrophil to lymphocyte ratio in patients with 2 type diabetes mellitus complicated acute cerebral infarction, so as to provide theoretical basis for clinical diagnosis and treatment. Methods:From Jan. 2014 to Dec. 2016 year,136 patients with 2 type diabetes mellitus complicated acute cerebral infarction were selected as research group and 68 diabetic patients undergoing physical examination were selected as the control group. White blood cell count(WBC),neutrophil count(NC),lymphocyte count(LC) and neutrophil to lymphocyte ratio(NLR) were detected and calculated. The role of WBC,NC,LC and NLR in the diagnosis of type 2 diabetes mellitus combined with acute cerebral infarction were evaluated by receiver operating characteristic curve(ROC) and area under the curve(AUC). Results:The levels of WBC,NC and NLR in research group were significantly higher than those of in control group(P＜0. 05),while the level of LC was significantly lower than that of in control group(P＜0. 05). The levels of WBC,NC and NLR in research group increased with the increase of NIHSS score(P＜0. 05),and the level of LC decreased with the increase of NIHSS score(P＜0. 05). The area under the curve of NLR,WBC,NC and LC in diagnosing diabetes combined with acute cerebral infarction were respectively 0. 856,0. 717,0. 685 and 0. 321. Twenty Newcastle disease virus(NDV)strains were isolated from diseased chicken and geese in field outbreaks during 2005 and 2006 in some regions of Jiangsu and Guangxi,and the antigenic analysis of the all NDV isolates had been done based on the reaction spectrum with a panel of monoclonal antibodies to the HN glycoprotein.The entire ORFs encoding HN protein of these NDV isolates were amplified by RT-PCR successfully,cloned and sequenced.The resultant sequences of HN genes of 13 isolates of chicken origin and 7 isolates of goose origin were gained and analyzed.The results of reaction spectrum showed that there were some distinct differences in the antigenic epitopes among the 20 NDV isolates.And the sequences revealed that the coding regions of the HN genes of these isolates all consisted of 1716 nt characteristic of virulent strains of NDV,coding for 571 amino acids.Neucleotides sequence homology were found to be from 94.8%to 100%among 18 NDV isolates of genotypeⅦ,and the neucleotides sequence homology between all the isolates and the other genotypeⅦstrains of recent years in China ranged from 92.1%to 99.6%.The deduced amino acid sequences and the receptor-binding regions of HN proteins between the NDV isolates of chicken origin and of goose origin were compared and analyzed.The results showed that some unique amino acid substitutions were found in the genome of the NDV isolates,and the close genetic similarity provided evidence for epidemiological linkage between the NDV isolates of chicken origin and of goose origin in the same period.
