2.Study on vasodilatation effects of total flavonoid from polygonum aviculare on blood vessel of rat in vitro

Objective To investigate the vasodilatation effects of total flavonoid from polygonum aviculare on rat thoracic aorta and its underlying mechanisms. Methods Total flavonoid from polygonum aviculare was gotten by extracted with 65% alcohol, gathered with polyamide, and eluted with 75% alcohol.The content of flavone was determined with rutoside as standard preparation. Normal rats thoracic aorta in vitro used as sample, BL-420E biological functional experiment system was utilized to record dilatation effect of total flavonoid on PE affecting pre-contracting blood vessel and the relation between dilatation effect of total flavonoid on blood vessel and calcium influx. Results Total flavonoids from polygonum aviculare can diastole contractions of thoracic aorta caused by PE. In calcium-free perfusion, gradually adding CaCl2 induced calcium influx. Clinical data showed dose-effect relation between drug and blood vessel contraction decreased in the total flavonoids from polygonum aviculare incubation rats than normal rats. Besides, total flavonoids from polygonum aviculare can obviously inhibit contraction of blood vascular circle induced by calcium releasing.Conclusion FP exerted a dose-dependent vasodilatation effect on rat isolated aorta rings by inhibiting Ca2+influx via L-type voltage-gated Ca2+ channels and Ca2+ release from sarcoplasmic reticulum, consequently decrease Ca2+ in vascular smooth muscle cells.
3.Effect of purslane decoction on oxidative stress products and soluble protein of senile cataract patients tears

Objective To study the effect of purslane decoction on oxidative stress products and soluble protein of senile cataract patients tears.Methods 120 cases of cataract patients (160 eyes) were selected, according to different drugs were randomly assigned to two groups.The control group were treated with Imidacloprid sinnock sodium drops eye drops, 1 drop each time, 4 times per day,the patients in the experimental group on the basis of the control group with Portulaca oleracea decoction 100 mL, for three consecutive months.SOD, MDA, soluble protein level and visual improvement were compared after the end of treatment.Results Compared with control group, the visual acuity of the experimental group was higher(P<0.05), MDA was lower, SOD was higher(P<0.05), soluble protein content was higher(P<0.05).Conclusion Portulaca oleracea decoction can apparently improve the cataract patients with tear of SOD, MDA, soluble protein level and improve visual acuity.
4. Surgical procedure and prognotic analysis of extrahepatic cholangiocarcinoma: A report of 161 cases

Objective: To explore the impact of clinical features and surgical procedure on survival of extrahepatic cholangiocarcinoma (EHCC) and the prognostic factors in patients with EHCC after curative resection. Methods: From 1995 to 2006, 161 EHCC patients undergoing operations were included in this study. The clinical features, diagnosis, surgical procedure and follow-up data were retrospectively analyzed. Factors influencing postoperative survival were analyzed using COX multiple stepwise regression model. Results: Of the 161 patients, 110 underwent radical resection and 32 underwent palliative resection, 19 underwent drainage or laparotomy. The overall 1-, 2-, 3-,and 5-year survival rates were 74.9%, 45.3%, 36.5%, and 11.1%, respectively. COX analysis showed hepatic infiltration, portal vein and/or hepatic artery invasion, and lymph node metastasis were independent prognostic factors after radical resection (P < 0.05). Conclusion: The radical resection is the key factor for increasing the long-term survival rate and improving the life qualities of EHCC patients. Partial hepatectomy or/and pancreatoduodenectomy in combination with skeletonization may be helpful for increasing radical resection rate and long-term efficacy.
5.Lichtenstein tension-free hernioplasty repair in adult patients with inguinal incarcerated hernia

Objective To investigate the preoperative management and the clinical effeciency of Lichtenstein tension-free hernioplasty in adult patients with inguinal incarcerated hernia.Methods Clinical data of 86 patients with inguinal incarcerated hernia were analyzed retrospectively.Hernia was repaired with Lichtenstein tension free after reposition.Results There were 59 male patients and 27 female patients with median age of (63 ± 18) years.There were 8 patients with liver cirrhosis.The operation was performed successfully in all patients.Segmental bowel resection with end-to-end anastomosis was performed in 38 emergency cases.Operative time was 20-120 min,with an average time of 54 min.The postoperative hospitalization was 5-17 d,with an average of 8 d.There were 7 cases of skin ecchymosis at the scrotum,there were no intestinal perforation,hepatic encephalopathy and upper gastrointestinal hemorrhage after operation.In early series of 24 cases without drainage tube left in place,there were 10 cases of fat liquefaction,10 cases of hydrops of hernial sac,6 cases of seroma and 3 cases of wound infection after operation.After 12 to 48 months of follow-up,there was no mortality after 2 years,no hernia recurrence.Conclusions Tension free repair in the treatment of incarcerated inguinal hernia is safe and feasible.
6.Diagnosis and treatment of renal tuberculosis

Objective To discuss the diagnosis and treatment of renal tuberculosis.Methods A retrospective study was made on 96 cases.Results Frequency (51.8%), urgency (37.2%),odynuria (33.4%),lumbodynia (41.0%),and hematuria(48.1%)were the most common symptoms. The diagnostic accuracy of IVU, B-type ultrasonography, CT and biopsy of mucous membrane of urinary bladder were 69.1%,12.5 %,37.5% and 33.3% respectively. 96 cases were given medicine (INH+RFP+PZA or PIA for 6-8 months).38 cases(39.6%)have been cured, while symptoms of 43 cases (44.8%) have been improved. Operation was performed on 15 cases that were ineffective treated by chemical therapy. Conclusions Urine routine, IVP, cystoscopy+biopsy of mucous membrane of urinary bladder provide important information for the diagnosis of renal tuberculosis. INH, REP and EMB or PIA combination therapy yields satisfactory outcome for early cases.
