2.Comparison of DWI and CT perfusion in diagnosis of liver fibrosis
Chinese Journal of Medical Imaging Technology 2010;26(2):297-300
Objective To explore the diagnostic value of diffusion weighted imaging (DWI) and CT perfusion imaging (CTPI) in diagnosis of liver fibrosis. Methods Fifty-seven hepatic fibrosis patients and 23 normal controls received DWI (b=500 s/mm2), ADC value of different fibrosis stages was measured, while 35 patients and all 23 normal controls received CTPI. The parameters of CTPI including blood flow (BF), blood volume (BV), mean transit time (MTT), hepatic arterial fraction (HAF) and permeability surface (PS) were measured. Analysis of variances was performed to compare the difference among the groups in both examinations. ROC curve was used to analyze the sensitivity and specificity of DWI and CTPI. Results The difference of ADC value between control group and S1 group was not significant, but between control group and S2, S3, S4 group and among group S2, group S3, group S4 was significant. In the parameters of CTPI, only the difference of HAF between control group and S3-S4 group was significant. Sensitivity and specificity of DWI and CTPI was 78.90%, 82.60% and 66.67%, 73.91%, respectively. Conclusion DWI is superior to CTPI for early diagnosis and degrading of liver fibrosis.

Objective To evaluate the diagnostic value of quantitative analysis parameters of dynamic contrast-enhanced magnetic resonance imaging (MRI) in differential diagnosis of prostatic disease. Methods 12 patients with prostate cancer and 16 patients with benign disease underwent DCE-MRI examination.Region of interest (ROI) were drawn in areas of cancerous foci, normal prostate peripheral zone and normal prostate central gland to measure the values of Ktrans (transport constant),Kep (rate constant),Ve (extravascular extracellular volume fraction).Values of the three parameters were analyzed by ANOVA and the differences were compared between the different tissues. Results The values of Ktrans,Kep and Ve were (0.42±0.10)/min,(0.64±0.11)/min,(0.61±0.14) respectively in the area of cancerous foci, while (0.17 ±0.08)/min,(0.32 ±0.10)/min,(0.59±0.11) respectively in normal prostate peripheral zone, and (0.33±0.12)/min,(0.50± 0.16)/min,(0.62 ±0.07) respectively in normal prostate central gland. The differences in Ktrans,Kep values were statistically significant between cancerous foci, normal prostate peripheral zone and normal prostate central gland (F =31.646,27.196,respectively,all P < 0.05),but no statistically significant difference was found in the Ve value between the three different tissues (F=0.533,P> 0.05).Conclusions Quantitative analysis of DCE-MBI can be used to differentiate benign from malignant prostatic diseases, which hasa great application prospect.

Aim To research the protective effect and mechanisms of taurine on myocardial ischemia and reperfusion injury in rats. Methods Myocardial ischemia reperfusion models were established in SD rats. The effects of taurine on the size of myocardial infarction and the changes of activity of SOD , and the levels of MDA and NO in myocardium and plasma of rats with myocardial ischemia and reperfusion injury were observed. Results In rats with myocardial ischemia and reperfusion injury, the taurine could reduce the size of myocardial infarction (P

Objective To assess the value of MSCT in the evaluation of the anatomy and variation of hepatic veins for living donor liver transplantation (LDLT) donors and the significance of vessel variation in surgical operation. Methods A total of 238 subjects who wanted to be the donors of LDLT underwent MSCT plain and enhanced examination, and the hepatic veins were evaluated. Results Among all 238 subjects, according to Nakamura's classification of hepatic veins, 164 were type Ⅰ, 60 were type Ⅱ, 14 were type Ⅲ. The left hepatic vein (LHV) shared a common trunk with the middle one in 167 subjects. Branches of Ⅷ going along the cross section and diameter larger than 5 mm were detected in 105 subjects. The Ⅳ segment veins drained into MHV in 68, into LHV in 7 subjects. Right inferior hepatic vein with diameter larger than 3 mm was found in 108 subjects, while the distance between RHV and IRHV were larger than 4 cm in 55 subjects. Conclusion MSCT can offer details and exact information about the donors pre-operation, and is an important non-invasive method for the evaluation of hepatic veins of potential LDLT donors.

Objective To systematically evaluate the diagnostic value of magnetic resonance cholangiopancreatography (MRCP) in the investigation of bile duct anatomy of liver transplantation living donors. Methods A search in Cochrane library,MEDLINE,EMBASE,CBMdisc (China Biology Medicine disc) was performed to identify relevant English and Chinese-language abstracts, supplemented by Springer,OVID,Sciencedirect full text database, etc.Criteria for inclusion were based on validity criteria for diagnostic research published by the Cochrane collaboration. With Meta analysis package for Stata10.1,heterogeneity of the included articles was tested, which was used to select proper effect model to calculate pooled weighted sensitivity and specificity, positive likelihood ratio, negative likelihood ratio. Summary receiver operating characteristic (SROC) curve was performed and the area under the curve (AUC) was calculated. Finally, sensitivity analysis was performed. Results Seventeen articles with 34 studies were included.Heterogeneity analysis revealed heterogeneity between studies and the source was MRCP imaging methods spotted by meta-regression analysis. Subgroup analysis according to MRCP imaging methods showed homogeneity within subgroups. The pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, diagnostic odd ratio of breath-holding thick slice MRCP,3D MRCP,the combination of the prior two methods, contrast enhance MRCP were 0.89,0.78,4.1,0.14,29; 0.92,0.80,4.5,0.10,45;0.95,0.82,5.2,0.06,85; and 1.00,0.76,4.1,0,1228,respectively with fixed effect model analysis. The area under the SROC curve was 0.83,0.92,0.96 and 0.99 respectively. Conclusion The combination of thick slice and 3D MRCP is a practical and effective method with good sensitivity and specificity to investigate bile duct anatomy of living liver transplantation donors, which fully meets the requirements of the preoperative assessment of bile duct structure.

Objective To investigate the effects of Helicobacter priori (H.prlori) strains from the patient with gastric cancer on the expression of DNA mismatch repair (MMR) genes in AGS cells in vitro. Methods AGS cells were co-cultured with ten strains of H.prlori from five patients with gastric cancer and five patients with gastritis respectively. The expressions of DNA MMR genes (hMSH2 and hMLH1) in mRNA and protein levels were determined by RT-PCR and Western blot. The enteropathogenic E.coli served as a bacterial control. Results E.coli and H.priori strains from gastritis have no effects on the expression of hMSH2 and hMLH1 in both protein and mRNA levels on the whole, while all the H.prlori strains from gastric cancer reduced expression levels of hMSH2 and hMLH1.Conclusions There are different effects between H. prlori strains from gastric cancer and those from gastritis on DNA MMR in AGS cell line, indicating that infec- tion of some H.prlori strains might lead to the inhibition of DNA MMR,and that in turns increases the risk of gastric carcinogenesis during chronic H.prlori infection.
