Objective To improve the poor water-solubility of nimodipine,nimodipine-loaded PLEG copoly- met nanopartieles were prepared to avoid the sedimentation of its crystal from water,which could also shun the blood vessel stimulation caused by ethanol,a cosolvent,in the injection.Methods Solvent evaporation method was used to prepare water-dispersing nimodipine-loaded nanoparticles,whose lyophilized powder was further prepared,with PLEG2000 as drug carrier.Particle size,Zeta potential,embedding ratio,drug loading,shape and redispersion ability of the lyophilized powder were determined.Results Concerning the lyophilized nanoparticle powders,the particle sizes were 136nm,the Zeta potentials were -29.90mV,the embedding ratio was 67.9% and the drug loading was 6.17%.Conclusion Prepared nimodipine-loaded nanoparticles solved the problem of poor water-solubility of ni- modipine basically.It also avoided the recrystallization process of the drug and reduced the blood vessel stimulation caused by ethanol in the injection.

Objective To investigate the protective immunity in mice immunized with recombinantBifidobacteria bifidum(Bb)-Eg95 vaccine of Echinococcus granulosus (Eg) and challenged with Eg protoscoleces.Methods Fifty-six female BALB/c mice 12-14 weeks old and weighed 20-25 g were vaccinated with the recombinant Bb-Eg95 vaccine subcutaneously,intramuscularly,intranasally and orally,respectively,with blank vector,Bb and medium of solution(MRS) as control,8 mice in each group.Mice were challenged with Eg protoscoleces on week 8 after immunization and killed on week 25 after infection.The weight of hydatid cyst was measured and the decreased larva rate was calculated.Sera were collected to determine the levels of IgE,IgG and its subclasses by enzyme linked immunosorbent assay(ELISA).Splenocytes were collected and cultivated to test the proliferation of splenocytes using methyltetrazolium (MTT) assay under EgAg and concanavalin A (ConA) stimulation.The results were compared with analysis of variance and the comparison between two groups was performed with LSD-t test.Results There was significant difference in the weight of hydatid cyst between groups (F =11.062,P ＜ 0.05).Compared with MRS control group[(0.075 ± 0.019)g],the hydatid cyst weight decreased in subcutaneous group [(0.050 ± 0.013)g],intramuscular group[(0.050 ± 0.019)g],intranasal group[(0.028 ± 0.016)g] and oral group [(0.031 ± 0.018)g,all P ＜ 0.01).Compared with subcutaneous and intramuscular groups,the hydatid cyst weight decreased in intranasal and oral groups(all P ＜ 0.05).The decreased larva rate was inversely proportional to the weight of hydatid cyst.There was significant difference in the levels(obsorbancy,A) of IgG,IgG2a,IgG2b,IgG1,IgG3 and IgE between these groups(F =21.774,36.977,27.071,14.746,10.131,9.444,P ＜ 0.05 or P ＜ 0.01).Compared with MRS control group (0.015 ± 0.002,0.002 ± 0.001,0.003 ± 0.001),the levels of IgG,IgG2a and IgG2b increased in subcutaneous group(0.022 ± 0.004,0.007 ± 0.002,0.008 ± 0.002),intramuscular group (0.023 ± 0.003,0.008 ± 0.002,0.007 ± 0.002),intranasal group(0.032 ± 0.007,0.012 ± 0.002,0.013 ± 0.004)and oral group(0.028 ± 0.006,0.010 ± 0.003,0.010 ± 0.002,P ＜ 0.05 or P ＜ 0.01).Compared with subcutaneous and intramuscular groups,the levels of IgG,IgG2a and IgG2b increased in intranasal and oral groups(P ＜ 0.05 or P ＜ 0.01).Compared with MRS control group(0.009 ± 0.001,0.009 ± 0.002,0.009 ± 0.001),the levels of IgG1,IgG3 and IgE decreased in subcutaneous group(0.022 ± 0.004,0.007 ± 0.002,0.008 0.002),intramuscular group(0.004 ± 0.001,0.004 ± 0.001,0.004 ± 0.002),intranasal group(0.005 ± 0.002,0.005 ± 0.003,0.005 ± 0.002)and oral group(0.005 ± 0.001,0.004 ± 0.002,0.004 ± 0.003,all P ＜ 0.01).There was significant difference in the proliferation of splenocytes in the supernatant of cultured splenocyte,of cultured splenocyte + EgAg and of cultured splenocyte + ConA(F =63.975,359.833,167.399,P ＜ 0.01).There was significant difference in the proliferation of splenocytes inside groups(F =6741.955,4953.667,869.320,201.235,175.413,139.653,169.994,all P ＜0.01).Compared with the cultured splenocyte the proliferation of splenocytes increased in the cultured splenocyte +EgAg and splenocyte + ConA (all P ＜ 0.01).Compared with the cultured splenocyte + EgAg,the proliferation of splenocytes increased in the cultured splenocyte + ConA(P ＜ 0.01).Conclusion An effective and protective immunity is induced by the recombinant Bb-Eg95 vaccine of Eg in mice.

Objective To explore the effect of preemptive anti-cytomegalovirus therapy on prognosis of patients with acquired immunodeficiency syndrome (AIDS) and cytomegalovirus (CMV) viraemia.Methods A non-random prospective study of male hospitalized patients were carried out who were newly diagnosed as AIDS during July 1,2011 to June 30,2013 in the First Affiliated Hospital of China Medical University.There were 69 patients who were divided into treatment group (57 cases) and control group (12cases).The duration of anti-cytomegalovirus therapy was a week after plasma CMV-DNA becoming undetectable.Baseline age,CMV-DNA,human immunodeficiency virus (HIV)-RNA,CD4+T cells,the rate of pneumocystis carinii pneumonia (PCP) and tuberculosis were evaluated between two groups.After 12-month follow-up,CD4 + T cells,the rate of rehospitalization,CMV retinitis,and mortality were evaluated.Results There were no difference in CMV-DNA,HIV-RNA,CD4 + T cells,the rate of PCP,and tuberculosis(P ＞ 0.05).At the end of 12 months,there were higher CD4 +T cells and lower rehospitalization rate in treatment group than control group(t =-3.850,P ＜ 0.05;x2 =6.078,P ＜ 0.05).CMV retinitis was not found.The mortality was not different between two groups(P ＞0.05).Conclusions Anticytomegalovirus therapy was beneficial to the increment of CD4 +T cells and decrement of rehospitalization,and prevention from CMV retinitis.

Objective To compare the effect of traditional open surgery and laparoscopic repair of gastric perforation on postoperative pulmonary function.Methods Fifty patients were divided into two groups according to therapeutic method.Group A underwent traditional open gastric perforation repair(n=25).Group B underwent laparoscopic repair of gastric perforation(n=25).The pulmonary functions were examined at 1 day,3 days and 7 days postoperatively.Results All patients had different degrees of postoperative pulmonary function decline.After operation,the forced vital capacity(FVC)and forced expiratory volume in 1 second(FEV1)and vital capacity(VC)in group A were significantly lower than those in preoperation(P<0.05).The patients in laparoscopic operation group decreased significantly only on the first day postoperative.The postoperative pulmonary function of patients undergoing open surgery was significantly lower than that of patients undergoing laparoscopic surgery(P<0.05).Conclusion Laparoscopic repair of gastric perforation has less damage to the lung function than traditional open gastric perforation repair.Laparoscopic surgery plays an important role in the recovery of postoperative pulmonary function.

Objective To investigate the types and frequency of gene mutations of β‐thalassemia in south of Sichuan area ,so as to provide basis for β‐thalassemia prevention plan and to help drop β‐thalassemia incidence effectively .Methods Polymerase chain reaction(PCR)and reverse dot blot(RDB)techniques were employed to perform diagnostic analysis for β‐thalassemia genes in sus‐pected thalassemia patient in south of Sichuan region.Results The detection rate of β‐thalassemia was 60 .1% among 319 cases ;9 genes mutation types was found among 17β‐thalassemia genes mutation types .Three types of gene mutations had highest frequency of occurrence ,followed by CD17 41 .3% ,CD41‐42 27 .39% ,IVS‐Ⅱ‐654 24 .35% .Conclusion The gene mutation rate of β‐thalasse‐mia are higher in south of Sichuan region .In order to prevent the birth of children with intermedial and major thalassemia ,the sig‐nificance is important to strengthen hematology screenings and genetic diagnosis for β‐thalassemia.

Objective To assess the safety and the efficacy of endoscopic ultrasound (EUS)-guided transmural drainage of pancreatic pseudocysts (PPC).Methods A total of 17 patients with PPC who underwent EUS to detect the optimal site and depth of puncture.The needle was punctured into the PPC cavity through endoscopic biopsy hole,cyst fluid was drained with a syringe.The guide wire was inserted along the pinhole under X-ray,and then the needle-knife was sent along the guide wire to cut the gastric wall and pseudocysts wall,followed by balloon dilation.The way of drainage was selected according to the cyst fluid properties.The technical success rate,treatment success rate,complication occurring rate and the skills were evaluated.Results Four patients were with nasalcystic catheter drainage,9 patients with double pigtail stents internal drainage,and 4 patients with nasal-cystic catheter and double pigtail stents combination drainage.The treatment success rates were 3/4,7/9,and 4/4 respectively.Only 1 patient subsequently developed bleeding from puncture site after stent successively placed,and was turned to surgery because of ineffective endoscopic treatment.Infection occurred in 4 patients during drainage,two of those were switched to surgical resection due to poor medical treatment response,and the other 2 were cured with intravenous infusion of antibiotics sensitive to cyst fluid bacteria and metronidazole rinse PPC.The median follow-up duration was 28.5months,and there was none of recurrence.Conclusions EUS-guided transmural drainage of PPC is safe.Stent placement and nasal-cystic catheter play an important role in PPC treatment.

Objective To evaluate the status of iodine deficiency disorders(IDD) in Huai'an City in 2011 and to provide a scientific basis for adopting target prevention strategies timely and adjusting the intervention policy scientifically.Methods According to The National Project of Surveillance on IDD,probability sampling method (PPS) was used to select 30 counties in Huai'an City,and one primary school was selected randomly from each county,then 40 children aged 8 to 10 were randomly selected in each school.Children's thyroids were examined by palpation and the iodine in salt samples collected in their home was tested by direct titration.At the same time,urine samples from 12 children of the 40 selected children were collected and urinary iodine was tested by As(Ⅲ)-Ce4+ catalytic spectrophotometry.Questionnaire survey of IDD was conducted among 30 students of grade 5 and 5 housewives around that school.Results A total of 1200 children aged 8 to 10 were examined,and the goiter rate was 1.3％(15/1200).All the goiters were level Ⅰ.The goiters of each age group was 0.8％(3/390),1.8％(7/390) and 1.3％(5/420),respectively.The 360 urine samples were collected from children aged 8-10,and the median of urinary iodine was 171.5 μg/L with the proportions of ＜ 20 μg/L,＜ 50 μg/L and ＜ 100 μg/L were 0(0/360),1.1％ (4/360) and 5.6％ (20/360),respectively.A total of 1200 salt samples were tested in children's family.The coverage rate of iodized salt was 99.4％ (1193/1200) and the qualified rate of iodized salt was 97.7％ (1165/1193).The consuming rate of qualified iodized salt was 97.1％(1165/1200),and non iodized salt rate was 0.6％(7/1200).The knowledge rates of IDD were 82.7％ (2234/2700) and 69.8％ (314/450) in 900 students and 150 housewives,respectively.Conclusions In Huai'an City,except the knowledge rate of IDD,the goiter rate and urinary iodine of children aged 8-10,iodized salt consumption maintain at the stage of the target level.In addition to strengthen the monitoring of iodized salt and to supply qualified iodized salt,it should also increase the focus on health education propaganda in the future.

Objective: To evaluate the bioavallability and bioequivalence of rabeprazole sodium enteric-coated pellets capsules. Methods:A randomized crossover design was performed in 32 healthy male volunteers. A single oral dose of 20 mg rabeprazole sodium enteric-coated pellets capsules ( test preparation) or enteric-coated shell capsules ( the reference capsules) was administrated under fed conditions. The wash period was 7 days. The blood samples were collected at different time points. The concentration of rabeprazole in plasma was determined by an LC-MS/MS method. The pharmacokinetic parameters were calculated by DAS 3. 0 software and the bio-equivalence was evaluated. The analysis showed that the maln pharmacokinetic parameters of the two formulations had no significant differences(P>0. 05) except for Tmax(P<0. 05). The relative bioavallability of rabeprazole sodium enteric-coated pellets capsules was (99. 80 ± 7. 20) %. Conclusion:Compared with the reference capsules, rabeprazole sodium enter-ic-coated pellets capsules show the property of higher dispersion degree, milder influence from food, more rapid release and absorption. The enteric-coated pellets capsules and the reference capsules are bioequivalence.
