2.Recent Research on CCR5 and Its Antagonists
China Biotechnology 2006;0(11):-
Chemokine receptor 5 (CCR5 ),as a member of the G protein-coupled receptor(GPCR)superfamily,is a membrane protein on cell surface and one of the major of coreceptors for HIV-1infection.CCR5 has became a molecular target for the novel drugs against HIV-1,and antagonists for CCR5 could be grouped as following, chemokine derivatives, small molecule non-peptide compounds, molecular antibodies and peptides. These compounds with high anti-viral activity and affinity are at different stages, and some have been under clinical studies. Therefore, the development research in the different kind of CCR5 antagonists is reviewed.

Protein tyrosine phosphatase (PTP) 1B is a potential target for the treatment of diabetes and obesity. We have previously identified the benzoyl sulfathiazole derivative II as a non-competitive PTP1B inhibitor with in vivo insulin sensitizing effects. Preliminary SAR study on this compound series has been carried out herein, and thirteen new compounds have been designed and synthesized. Among them, compound 10 exhibited potent inhibition against human recombinant PTP1B with the IC50 value of 3.97 micromol x L(-1), and is comparable to that of compound II.

Objective: To evaluate the bioavallability and bioequivalence of rabeprazole sodium enteric-coated pellets capsules. Methods:A randomized crossover design was performed in 32 healthy male volunteers. A single oral dose of 20 mg rabeprazole sodium enteric-coated pellets capsules ( test preparation) or enteric-coated shell capsules ( the reference capsules) was administrated under fed conditions. The wash period was 7 days. The blood samples were collected at different time points. The concentration of rabeprazole in plasma was determined by an LC-MS/MS method. The pharmacokinetic parameters were calculated by DAS 3. 0 software and the bio-equivalence was evaluated. Results:The maln pharmacokinetic parameters of the two formulations were shown as follows:T1/2 of (2. 20 ± 0. 83)h and(1. 951 ± 0. 515)h,Tmax of (3. 88 ± 1. 11)h and(4. 64 ± 1. 504)h,Cmax of (401. 06 ± 170. 75)ng·ml-1 and(394. 63 ± 215.64)ng·ml-1,AUC0→t of (918.42 ±427.39)ng·h·ml-1 and (994.49 ±520.73)ng·h·ml-1, and AUC0→∞ of(937.30 ± 445.13)ng·h·ml-1 and(1 011.69 ±534.77)ng·h·ml-1. The analysis showed that the maln pharmacokinetic parameters of the two formulations had no significant differences(P>0. 05) except for Tmax(P<0. 05). The relative bioavallability of rabeprazole sodium enteric-coated pellets capsules was (99. 80 ± 7. 20) %. Conclusion:Compared with the reference capsules, rabeprazole sodium enter-ic-coated pellets capsules show the property of higher dispersion degree, milder influence from food, more rapid release and absorption. The enteric-coated pellets capsules and the reference capsules are bioequivalence.

Objective To better promote the health education and health promotion among patients with chronic disease, general practitioners (GPs) in the community should master the knowledge of non-communicable diseases (NCDs). This paper aimed to describe the current situation of GPs' knowledge about prevention and treatment of NCDs. Methods Self-designed questionnaires were issued to 1 077 GPs in community health service centers, and all GPs finished the questionnaire without assistance. The content of questionnaire includes the general information of GPs and six questions focusing on NCDs. Results Overall, 1 076 GPs completed the questionnaire, and the response rate was 99.9% (1 076/1 077). With regard to the six questions about NCDs, 90.4% (973/1 076) of GPs were well aware of the diagnosis of hypertension. Apart from that, the knowledge of diabetes mellitus (60.9%, 655/1 076), factors related to NCDs (70.2%, 755/1 076), the concepts of mental illness (52.0%, 560/1 076) and the clinical significance of tumor markers (87.9%, 946/1 076) among GPs were adequate, while more than half of the GPs did not know a lot about home oxygen therapy. Conclusion The mastering level of knowledge about prevention and treatment of NCDs among GPs varies that they need further improvement.

Objective To compare the fetal and maternal outcomes of labor undergoing epidural block anesthesia with and without using hand-and-knee position in the second stage of labor. Methods A total of 1,158 subjects were included in the study. There were 560 subjects in the study group and 598 cases in the control group. All the subjects were divided into study and control group according to the randomized numbers. Both groups underwent epidural block anesthesia and used the same labor way in stage I and stage 3.In the study group, the hand-and-knee position was used for 15~30 minutes combined with uterine contractions and the delivery was done in the lithotomy position. In the control group, the gradidea took the semi-recumbent position with bent knees and bed elevated 40°,combined with uterine contractions until the delivery was done in the lithotomy position. The labor time of stage1,stage2,stage3 and all abor, labor way, perinaueum, complications and newborns were compared. Results The average time of the second stage labor and all labor in the observation group were significantly shorter than those of the control group (P<0.05).The labor ways in the observation group was better than that of the control group (P<0.05).The incidence of postpartum urinary retention in the observation group was significantly lower than that of the control group (P<0.05).The differences of labor time for stage 1,stage2,perinaueum,blood loss rate, newboms' s tomor and neonatal asphyxia between two groups were not statistically significant (P>0.05).Conclusions Under continuous epidural block anesthesia, the use of the hands-and-knees position in the second stage of labor can reduce the incidence of operative delivery, shorten the labor process, and reduce maternal complications. The method is worthy of promotion.

Chinese Journal of Medical Education Research 2021;20(4):406-408
In recent ten years, general practitioner (GP) training and practice develops rapidly in China. It has established a relatively complete GP training system, including medical college education and standardized residency training. However, the path of continuing professional development and clinical skills improvement for GP after standardized residency training is still unclear. The concept of GP with special interest (GPSI) has been proposed these days, and it has put forward a new development direction for the career path of GP. In this article, on the basis of understanding the current status of GPSI in China, we try to discuss its advantages and problems, and then propose our suggestions for its future development.
