2.Clinical value of infusion gemcitabine via artery port and catheter system for 37 cases with ad-vanced panceratic cancer

Purpose:To investigate the efficacy of infusion gemcitabine via artery port and catheter system (PCS) for advanced pancreatic cancer.Methods:57 cases advanced pancreatic cancer proved by pathology were implanted Implantofix PCS.The number of ports implanted subcutaneously in inferior right abdomen and subcutanceusly in inferior chest was 48 and 9 cases respectively.The first total does in experience group was gemcitabine 3.0,5-FU 3.0,IL-2 6 million u via three times infusion.Results:The overall implanted successful rates was 100% and indwelling catheter tip dislocation rate was 0. Two PCS were pulled out in 2 fat old female patients because of bad wound healing after two months.In the experimental and control group according to the standard of clinical benefit-responces were 37 and 20 respectively.The obvious improve- ment in pain in experimental and control group was 27.2% and 4.8%,respectively.The duration of clinical benefit-re- sponces were 18 weeks in experimental groups,13 weeks in control group.The midian survival in experimental group was 6.25 months,4.41 months in control group.The 6-,9-,12-months survival rates in experimental group were 46%,24%, 18% more thans 31%,6% and 2% in control group.Conclusions:Infusion gemcitabine via artery PCS can not only relieve pain,prolong survival rate but also provides a long-term intra-arterial chemotherapy and effctive access for advanced pancre- atic cancer.
3.Effects of intensive insulin therapy on the intestinal permeability in sepsis patients

Objective: To discuss the effect of intensive insulin therapy on D-lactic acid and diamine oxidase level in patients with sepsis.Methods: 48 patients was divided into control group and conventional group randomly.Content of D-lactic acid and diamine oxidase in serum was detected with absorption spectrometry before and after therapy.Result: D-lactic acid and diamine oxidase in patients decreased significantly in control group compared to that of conventional group.Conclusion: Insulin can depress interstinal permeability and ameliorate sepsis symptom resulted from bacterial translocation from intestine.
4.Clinical application of self-expandable metallic stent implantation through bronchoscope in the patients with malignant airway stenosis

Objective To evaluate the short and long term efficacy of self-expandable metallic stent implantation through bronchoscope in the management of the patients with malignant airway stenosis.Methods Seventy-four hospitalized patients with malignant airway stenosis during a 10-year period were selected as our subjects from Dec.2002 to Nov.2012 in the Respiratory Department of the Central Hospital of Wuhan.All these patients underwent bronchoscope for nitinol self-expandable metallic stent implantation.Clinical improvement,6min walking distance (6MWT),mMRC index,arterial blood gas,as well as early and late postoperative complications were investigated.Results A total of 77 stents were implanted,including 34 covered metallic stents and 43 uncovered metallic stents.All patients showed the obvious relief of dyspnea after the operation.6MWT at pre-operation was (172 ± 45)m and (288 ± 36)m at 28 week post-operation.Arterial blood gas was (61.34 ±6.44)mmHg at pre-opemtion and (72.12 ±6.87) mmHg at 28 week post-operation.mMRC was 3.56 ± 0.44 at pre-operation and 1.96 ± 0.37 at 28 week post-operation.All these differences were significant(P < 0.01).The operation succeeded in releasing bucking of patients with esophago-tracheal fistula.Symptoms such as fever,cough,pharyngodynia,foreign body sensation and hemoptysis were observed,and all the above side effect could be solved while no serious complications arose.Conclusion Bronchoscopic insertion of metallic airway stents for malignant airway stenosis is a safe and effective procedure that could obviously relieve respiratory distress and improve the quality of life.
5.A preliminary clinical study on p53 gene in the therapy of advanced hepatocellular carcinoma

Objective To preliminarily investigate curative effect of hepatic arterial perfusion of p53 gene in the therapy of advanced hepatocellular carcinoma.Methods Tirty cases of advanced hepatocellular carcinoma were divided into experimental group and control group,p53 gene was perfused into target artery confirmed by angiography.In experimental group,10~(12)Vp of p53 gene and 20 mg OPT were employed every week as a course for 21 days and 1 to 4 courses time the cases of the study.In control group,only 20 mg OPT was used.Results The survival period was 14 to 405 days in experimental group and 18 to 167 days in control group.There was a significant difference between the two groups(P<0.05). Conclusion p53 gene is efficacious in the therapy of advanced hepatocellular carcinoma.(J Intervent Radiol,2007,16:127-129)
6.Advances in the study of visual pathway injury in glaucoma and the neuroplasticity of the visual system after injury

Glaucoma is a group of diseases characterized by progressive optic nerve atrophy and visual field defects. Some evidence demonstrate that glaucoma's pathological damage affects the retina and optic nerve and involves the entire visual pathway. Recent studies have found that the nerve injury is not completely irreversible but has certain plasticity. Furthermore, studies have shown that neurons in the visual pathway of glaucoma can be repaired and remodeled under certain conditions to restore visual function, which offers a new way to treat glaucoma.
7.Investigation and anaIysis of bacteriaI spectrum and drug sensitivity test in conjunctivaI sac of cataract patients of Kazak

· ATM:To analyze bacterial spectrum and drug sensitivity in conjunctival sac of cataract patients of Kazak.
· METHODS:A total of 538 cases of conjunctival sac secretion in cataract patients of Kazak were collected.The samples were cultured and their sensibilities to antibiotics were tested.
· RESULTS: The bacterial culture was positive in 214 cases.The positive rate was 39.8%. The variety of pathogenic bacteria were mainly made up of gram positive cocci ( 88.3%), and most of them were Staphylococcus epidermidis ( 66.4%), followed by Micrococcus(9.8%).Sex had no effect on conjunctival bacteria rate in the cataract patients of Kazak, while age, place of residence had an effect on camier rate. The camier rate of conjunctival bacteria was significantly higher in people over 60 years old than that in people with age between 40 to 59 years old.And the people from city had a significant lower bacteria positive rate than those from countryside and pastoral. Most of grams were sensitive to Vancomycin, Teicoplanin, Rifampicin, Duly cloth mildew mutual and Amikacin, the tolerance was less than 20%, and they usually had higher tolerance to Penicillin, Erythromycin, Tetracycline and Chloramphenicol (>70%) .
·CONCLUSlON:Gram positivecocci is the most common bacteria in conjunctival sac in cataract patients of Kazak. Staphylococcus epidermidis was most common, followed by Micrococcus.The germ-carrying rate of conjunctival SAC in Kazakh population is associated with the patient’s age and area of residence.The clinical use of antibacterial drugs should be strictly grasp the indications, to reduce the incidence of bacterial resistance.
8.Expression and clinical signiifcance of PTK7 in ovarian serous tumors

Background and purpose: The protein tyrosine kinase-7 (PTK7) gene may be related to the occurrence and progression of many tumors. This study was aimed to explore the expression of PTK7 in ovarian serous tumors and its relationship with clinical stage, histological grade, metastasis and prognosis indicators linkages, and analyze the diagnostic and prognostic value of PTK7 in ovarian serous tumors. Methods:Expressions of PTK7 in 3 ovarian cell lines (HO8910, SKOV3, A2780), 14 cases of normal fallopian tube epithelium, 6 cases of benign serous ovarian tumors, 51 cases of borderline serous ovarian tumors and in 97 cases of ovarian serous carcinoma were detected by immunohistochemical EliVision two-step method. Statistical analysis of the relationship between the expression of PTK7 and the pathological indicators was performed byχ2 test, Fisher exact test and Kaplan-Meier method. Results:PTK7 was negatively expressed in HO8910 and A2780, but weakly positively expressed in SKOV3. The positive rates of PTK7 in normal fallopian tube epithelium, benign serous ovarian tumors, borderline serous ovarian tumors and serous ovarian cancer were 92.86%(13/14), 83.33%(5/6), 45.10%(23/51), and 28.87%(28/97), respectively. The expression of PTK7 had no difference between normal fallopian tube epithelium and benign serous tumors, benign serous tumors and serous borderline tumors (P=0.521, P=0.102). The PTK7 expression showed signiifcant differences in serous ovarian carcinoma compared with those in normal epithelium, benign serous tumors and borderline serous tumors (P=0.000, P=0.012, P=0.048). Expression of PTK7 in borderline serous ovarian tumors was signiifcantly with clinical stage, metastasis (lymph node and/or peritoneum metastasis) (P=0.038, P=0.038), rather than its location, age (P=0.088, P=0.896). Expression of PTK7 in ovarian serous carcinoma had a signiifcant relation with its clinical stage, WHO grade, MDACC grade (P=0.011, P=0.004, P=0.000), rather than its location, metastasis, tumor diameter and age (P=0.326, P=0.524, P=0.588, P=0.584). The survival rate of PTK7 positive group in ovarian serous carcinoma was signiifcantly higher than that in the negative control group (P=0.017). Conclusion:The expressions of PTK7 in normal ovarian epithelium, benign serous ovarian tumors, borderline serous ovarian tumors and epithelial serous carcinoma show a gradual downward trend. The expression of PTK7 in ovarian serous tumors has a positive correlation with late clinical stage, high histological grade and poor prognosis. PTK7 can be a new indicator of clinical diagnosis and prognosis in ovarian serous tumors.
9.Status of pesticide registration and residue analysis for traditional Chinese medicine in China.

The present paper outlined pesticide registration status for traditional Chinese medicines (TCMs) and summarized the characteristics of pesticide contamination in different regions of some widely used TCMs by retrieving last 10 years' literatures. At present, the problems of pesticide residues for TCM include less pesticide registrations, widespread high-residue organochlorine pesticides contamination, pesticide abuse, irregular GAP bases and imperfect pesticide limit standards, etc. According to the current situation, we should adopt some control measures to strengthen the quality control of TCMs so as to ensure the safety of TCMs and related products.

10.Effect of berberine on PPARalpha/delta/gamma expression in type 2 diabetic rat retinae.

Retinopathy is a major cause of morbidity in diabetes and remains the primary cause of new blindness. Therefore, it is necessary to find new drug to treat diabetic retinopathy. Type 2 diabetes mellitus (T2DM) rats were induced by injection (ip) with streptozotocin (STZ) 35 mg x kg(-1) and fed with a high-carbohydrate/high-fat diet 2 weeks later. From week 17 to 32, diabetic rats were given different doses of berberine 75, 150, and 300 mg x kg(-1), fenofibrate 100 mg x kg(-1) and rosiglitazone 4 mg x kg(-1), separately. Retinal structure was observed with hematoxylin-eosin staining and peroxisome proliferator-activated receptors (PPARs) alpha/delta/gamma protein expressions were detected by immunohistochemistry. The retina of control rats was thicker than that of other groups, 16 weeks treatment with berberine (150 and 300 mg x kg(-1)) and rosiglitazone 4 mg x kg(-1) thickened the diabetic retina, but no difference existed in retinal structure among groups. Both berberine (150 and 300 mg x kg(-1)) and rosiglitazone 4 mg x kg(-1) significantly decreased PPARy expression in diabetic retina; while berberine (150 and 300 mg x kg(-1)) and fenofibrate 100 mg x kg(-1) obviously increased both PPARalpha and PPARdelta expressions in diabetic retina. Berberine modulates PPARalpha/delta/gamma protein levels in diabetic retina which may contribute to ameliorate retinopathy complication induced by STZ and a high-carbohydrate/high-fat diet. It is expected that berberine might be a more beneficial drug to treat diabetic retinal complication comparing with fenofibrate and rosiglitazone.
