Objective To compare the clinical efficacy and tolerability of 0 75% ropivacaine(8～10ml) versus 1% lidocaine plus 0 2% dicaine in epidural anesthesia cesarean section.Method Parturients for elective cesarean were randomly designed to receive 0 75% ropivacaine(groupⅠ,n=15) or 1%lidocaine plus 0 2% dicaine(groupⅡ,n=15) epidural anesthesia. Sensory block,intraoperative pain(VAS score) and abdominal wall relaxation were assessed together with adverse reaction. As a new method of modern teaching,the computer assisted instruction(CAI) is widely used in every field in modern education. In this paper,the author explores the existed problems and the necessary qualities of using CAI for college teachers in microbiology experimental teaching.

OBJECTIVETo explore the expression patterns of the chemokine CXCL12 and its receptor CXCR4 in human sperm.

METHODSWe collected semen samples from 10 fertile men, performed density gradient centrifugation, and then determined the expressions of both CXCL12 and CXCR4 in the sperm by RT-PCR and immunofluorescence staining.

RESULTSRT-PCR revealed the mRNA expressions of CXCL12 (0.641 +/- 0.180) and CXCR4 (0.464 +/- 0.100) in the sperm. However, only CXCR4 rather than CXCL12 was expressed at the protein level, and the positive staining for CXCR4 was observed mainly in the posterior part of the acrosome.

CONCLUSIONCXCL12 and CXCR4 are involved as important molecules in regulating the function of human sperm.

Objective To discuss application value for detecting sentinel lymphatic node(SLN) in patients with breast mass by subcutaneous injection SonoVue. Methods Forty-two cases with breast masses who were detected by conventional techniques were injected subcutaneously with 1.25 ml Sono Vue to the borderline of breast tumor at each 3,6,9, 12 points. The changes of contrast-enhanced ultrasound (CEUS) of the ipsilateral axillary SLN were observed and the start time of enhancement of lymphatic channel and lymph nodes, the position and numbers of enhancement of the lymph nodes, the time of regression were recorded. Compared with Methylene blue and surgicalbiopsy, the accuracy of detection of SLN by CEUS were evaluated. Results SLN were successfully detected in 30 of 42 patients, the detection rate was 71.4%. The total number of detected SLN was 40.Five nodes were detected in 5 of 10 (50%)patients with benign masses. Thirty-five nodes were detected in 25 of 32 (78.1%) patients with breast cancer. Fifty-seven SLN were successfully detected in 39 of 42 patients with Methylene blue, the accurate rate was 92.9%. Seven of 10 patients with benign masses and all 32 patients with breast cancer were Methylene blue positive. There were total 64 SLN found, including 11 with benign masses and 53 with lymph nodes metastasis of breast cancer cells. Conclusions The subcutaneous injection SonoVue is effective to detect SLN in patients with breast mass and apply clinical value for forecasting whether the lymph node metastases in patients with breast cancer.

Objective To study the feasibility of contrast-enhanced ultrasonography (CEUS) in detecting sentinel lymph node(SLN) in patients with colorectal cancer during operation and their clinical significance.Methods Twenty-one patients who underwent radical resection of a primary colorectal cancer were enrolled.During operation,1 ％ methylene blue was injected around the tumor,SLN mapping was performed.Then the SonoVue was injected at the same sites,CEUS was performed to observe changes of SLN in real time,and marked them.The SLN detection rate and accuracy of CEUS were compared to those of conventional dye guided method.Results The rectal cancer of below peritoneal reflex,the detection rate of CEUS for SLN was 92.31 ％,and the accuracy was 88.89％.The detection rate of dye guided method for SLN was 84.62％,and the accuracy was 93.10％,there was no statistically significant difference between CEUS and dye guided method (P ＞0.05).Conclusions When injected SonoVue in submucosal layer,the good mapping of SLN of patients with rectal cancer of below peritoneal reflex can be obtained,it will play an important role in clinical application to map SLN of patients with rectal cancer of below peritoneal reflex.

Objective To investigate the correlation between long-term blood pressure variability (BPV) and leukoaraiosis (LA). Methods Blood pressure was measured monthly in 209 patients visiting Zhejiang Hospital during June 2011 to June 2013, and they were followed up for 12 months. Te patients were divided into LA 0-3 groups according to the MRI results and Fazekas scale. Te clinical information, long-term blood pressure, and the index of long-term BPV were compared between LA groups and non-LA group, and among patients with different levels of LA, and then the correlation was regressively analyzed. Results One hundred and thirty-eight patients in LA group, and 71 in non-LA group. Regression analysis revealed that signifcant differences in age and the history of hypertension and occurrence of stroke between LA groups and non-LA group (P<0.05). Te maximal systolic blood pressure, the mean value of long-term blood pressure, the standard deviation (SD), coefficient of variation (C V), and variation independent of mean (VIM) of blood pressure were higher in LA groups than in non-LA group (P<0.05). Tere was signifcant difference in maximun systolic pressure between every level of LA groups (P<0.05). Te mean long-term systolic blood pressure and SD, CV and VIM of long-term blood pressure were signifcantly higher in the patients of LA3 group than that in the patients of LA1 group (P<0.05). Logistic regression analysis showed that the mean long-term diastolic blood pressure (OR=1.048, 95% CI 1.028-1.067), systolic blood pressure SD (OR=1.064, 95% CI 1.001-1.131), CV (OR=1.086, 95% CI 1.002-1.177), VIM (OR=1.060, 95% CI 1.001-1.122), and diastolic blood pressure SD (OR=1.111, 95% CI 1.000-1.233) and CV (OR=1.091, 95% CI 1.001-1.190) were the independent risk factors of LA (P<0.05). Conclusion Tere is a signifcant correlation between long-term diastolic blood pressure and long-term BPV and LA.

Objective To investigate the effects of miR-19 on the migration ability of lung adenocarcinoma cell line A549. Methods The expressions of miR-19 in lung epithelial cell line BEAS-2B and lung adenocarcinoma cell line A549-Luc were detected by qRT-PCR.The A549-Luc cell line which over-expressed miR-19 was established. The expression levels of miR-19 in A549/RFP+/H2B and A549/RFP+/m19 were identified by qRT-PCR. The morphology of A549/RFP+/m19 was observed,and the migration ability of A549/RFP+/m19 was detected by transwell migration assay. Results The expression levels of miR-19 differed significantly between BEAS-2B cells and A549-Luc cells (t = -20.954, P < 0.001). The lung adenocarcinoma cell line A549/RFP+/m19 which over-expressed miR-19 was successfully established. Changes in A549/RFP+/m19 cell morphology were found. As compared with A549/RFP+/H2B cells, A549/RFP+/m19 had an increased migration ability (P <0.01). Conclusions miR-19 enhances the migration ability of lung adenocarcinoma A549-Luc cells.

Objective To evaluate the renal function response to vasopressin in obstructed and non-obstructed kidney in vasopressin-deficient rats by using dynamic enhanced MRI. Methods This study included 26 vasopressin-deficient Brattleboro(BB)rats.Unilateral ureteral obstruction(UUO) model was set up in 12 BB rats.After releasing UU0 24 h,treatment of AVP was given in 7 non-obstrueted and 7 obstructed BB rats for 6 d.Seyen normal Wistar rats and 7 BB rats were used as control.Dynamic enhanced MRl was performed at 0-60 min following the administration of GadoliniumDTPA(0.05 mmol/kg)and the mean relative signal intensity(RSI)was measured. Results BB rats RSI increase occurred during the first minute,with a maximum mean RSI of(149±10)%in cortex,(155±13)%in medulla and(146±13)%in pelvis.Then,a gradual decrease in the cortical RSI occurred,after which a small increase was observed in Phase 3.BB rats RSI patterns were different from Wistar rats.After AVP treatment,mean RSI of BB rats in cortex,medulla and pelvis reached a maximum of(180±6)%,respectively at 30 sec,followed by a fast decrease at 1 min.Then,RSI decreased gradually.Phase 3 demonstrated a moderate increase in medullary and a relative large increase in pelvic RSI until 30 min after injection,after which RSI returned to baseline.BB rats RSI patterns after AVP treatment were similar to that of Wistar rats.Cortical,medullary and pelvic mean RSI increased slowly compared to non UUO rats.In phase 3,mean RSI of all segments decreased gradually.UUO rats RSI patterns were different from non UUO rats.After AVP treatment in UUO rats,mean RSIs of all segments increased faster compared to UUO rats without AVP.Cortical mean RSI reached a maximum of(193±7)% at 2 mim Mean RSI of medulla were unchanged,but the pelvic mean RSI deereased gradually to a minimum of(77±5)%at 5 min.Phase 3 was characterized by a slight recovcry of all RSIs.UUO rats RSIs were recovered after AVP treatment.The renal blood flow(RBF)of UUO rats was decreased to(1.1±0.1)m1·min-1·100g-1,wherease the RBF was increased to (1.9±O.3)ml·min-1·100g-1 after injecting AVP. Conclusion Dynamic enhanced MRI may provide useful information for distinguishing between kidneys having changes in the renal vasopressin concentration and the kidneys suffer from the obstructed renal damage.
