1.Advances of target therapy in hormone-receptor-positive breast cancer
Chinese Journal of Clinical Oncology 2017;44(13):635-639
Hormone therapy is a major treatment option for hormone-receptor-positive (HR+) breast cancer. Resistance to hormone therapy is the major reason for disease recurrence and progression. Recent studies have identified several resistance mechanisms that lead to the estrogen-independent growth of HR+breast cancer and were exploited to develop novel target drugs, including cyclin-de-pendent kinases 4 and 6 (CDK4/6) inhibitor, mammalian target of rapamycin (mTOR) inhibitor, epidermal growth factor receptor family inhibitor, vascular endothelial growth factor inhibitor, histone deacetylase inhibitor, fibroblast growth factor receptor inhibitor, insulin-like growth factor receptor inhibitor, and checkpoint inhibitor. These inhibitors are being developed to block resistance pathways and improve the efficacy of hormonal therapy. Among these drugs, the mTOR inhibitor everolimus and the CDK4/6 inhibitor palbociclib are currently approved in the United States to treat metastatic HR+breast cancer. In this study, we summarize the frontier advances in the combination of hormone and target therapies.

Replacement therapy is the most effective method for the treatment of end-stage liver disease, and decellularized liver bioscaffold broadens the research field of the replacement therapy. The present liver bioscaffold preparation is to perfuse chemical reagents (detergents, enzymes,et al) into the vascular structure of the liver under certain physical conditions,so as to remove cellular components and retain extracellular matrix and microvascular structure.Cells were reseeded into the decellularized liver scaffold to obtain the recellularized liver, which can be cultured and evaluated in vitro or in vivo by observing the adhesion of seeded cells, detecting the synthesis and secretion of the recellularized liver.Currently, the selection of seed cells, recellularization protocol and recellularized liver transplantation are still under exploration. In this review, the preparation, evaluation, detection and application of the decellularized liver bioscaffold are introduced for the further experimental study and clinical research.

The effects of positive end-expiratory pressure (PEEP) on pulmonary shunt were studied during gen- eral anesthesia and postoperative period.Twenty cholecystectomy patients were randomly divided into experiment group (group P) and control group (group Z). PEEP and ZEEP were used separately after induction. Artery blood and mixed blood from the right ventricle were taken for blood gas analysis and determine the amount of pulmonary shunting before anesthesia. half and hour, one and half an hour and two and half an hour after anesthesia and one hour after the operation. The results showed that shunt in group P decreased gradually during general anesthesia and returned to the level of preoperation at an hour after operation. Shunt in group Z was increased continually and the level was significantly higher than preoperation an hour after operation. Shunt between two groups was significant difference (P

Objective To investigate the delayed alteration of hippocampus proteome after an-esthesia with isoflurane in aduh and aged rats. Methods Ten 8-month-old SD rats were randomly divided into group Caduh and group Iadult (5 in each group) , and another ten 22-month-old SD rats were randomly divided into group Caged and group Iaged (5 in each group). The rats in group Iadult and group Iaged received 2 h anesthesia with 1.2 % isoflurane. The rats in group Cadult and group Caged inhaled 40% oxygen for contrast. The hippocampal proteome of each rat was measured by 2-dimensional gel electrophoresis and mass spectrometry. Results The vital signs of the rats in group Iadult and group Iaged were stable. There were 878±34 protein spots in group Cadult, 864±49 protein spots in group Iadult, 834±47 in group Caged, and 819±24 in group Iaged. There were 12 (4/8)different protein spots between group Iadult and group Cadult. There were 11 (3/8) different protein spots between group Iaged and group Caged. All of the protein spots were identified by MALDI-TOF-MS. Most of the different proteins were related to metabolism, anti-oxidation, and signal conditioning of synapse. Conclusion Isoflurane may cause the alteration of hippocampal pro-teome in rats, which is age-related.

Objective To translated the Male Osteoporosis Assessment Questionnaire (OPAQTM) into Chinese and test the validity and reliability of it.Methods Translation and cross-cultural adaptation of the original OPAQTM was performed according to the published guidelines.A total of 122 male osteoporosis patients in Xi'an city were included in item analysis, exploratory factor analysis and criterion-related validity and reliability analysis of this Chinese version of OPAQTM.Results A 7-factor structure with 37 items was identified through item analysis and exploratory factor analysis, which explained 73.943% of the total variance. The ratio of convergent validity of each dimension was exceeded 0.90 and exceeded 0.95 for discriminant validity. The Cronbach's α coefficient for the total OPAQTM score was 0.956.The split-half for the overall scale was 0.858.The test-retest reliability was 0.760 for the overall scale. In relation to criterion validity, the Spearman correlation coefficients ranged from 0.485 to 0.867 between the Chinese version of OPAQTM and the corresponding dimensions of 12-Item Short-Form Health Survey Questionnaire (SF-12),which were significantly correlated,P < 0.01.Conclusions The Chinese version of OPAQTM appears to possess adequate validity and internal consistency. The newly translated Chinese version of OPAQTM may be used to assess the quality of life of male osteoporosis patients in China.

Objective To explore the correlation of empowerment with illness perception among inpatients with type 2 diabetes mellitus. Method The convenience sampling method was used to investigate the status of empowerment and illness perception among 102 patients with type 2 diabetes mellitus from October 2014 to July 2015, followed by analyzing the association between the two variables. Results The total score of empowerment was (4.00 ± 0.65), and the total score of illness perception was (38.00 ± 2 . 33 ) . The empowerment was positively correlated with illness perception ( P < 0 . 001 ) . The empowerment was positively correlated with the timeline, personal control, treatment control, illness concern and illness comprehensibility, respectively (all P<0.001). Conclusions The empowerment of patients with type 2 diabetes mellitus is at a high level, and positively correlated with illness perception, more pronounced chronic nature, the better personal control and treatment control, a higher degree of concern about the disease and better personal understanding of the illness contributed to a higher degree of empowerment. Medical staff should evaluate the illness perception of the patients with type 2 diabetes mellitus regularly in the daily work so as to improve the illness perception , improve their empowerment and promote the patients′quality of life eventually.
