BACKGROUND: Prospective studies concerning bone metabolism and bone mineral density variation after fractures have been reported from the 1960s, but these studies are mainly focused on tibia and fibula and ankle fractures in patients with low sample size.
OBJECTIVE:To observe the changes in bone mineral density and bone metabolism indexes in elderly women within 6 to 12 months after hip fractures, and to analyze the correlation.
METHODS:We selected 48 elderly women with hip fractures admitted in the Department of Orthopedics, Beijing Aerospace General Hospital in China from May 2011 to July 2013. Standards for fol ow-up were developed to measure the bone mineral density and bone metabolism indexes in the L 1-4 spinal segments and both sides of the hip. The bone metabolism indexes included bone alkaline phosphatase, osteocalcin, cross-linked C-terminal peptide of col agen I, and serum tartrate-resistant acid phosphatase 5b levels. Multiple linear regression analysis was performed based on measurements of bone mineral density and bone metabolism indexes after fracture healing.
RESULTS AND CONCLUSION:After fracture healing, bone mineral density of the fractured hip and lumbar vertebra was significantly lower than the baseline value. There was no statistical difference in bone mineral density between the healthy hip and the baseline value. At 6 months after fractures, bone alkaline phosphatase, osteocalcin, cross-linked C-terminal peptide of col agen I, and serum tartrate-resistant acid phosphatase 5b levels were significantly higher than the baseline values. At 12 months after fractures, osteocalcin level was significantly higher than the baseline value, while other indexes of bone metabolism measurements showed no statistical difference from the baseline values. When healing of hip fractures met the clinical and radiographic standards, the partial regression coefficient of delta-Z score reached peak in the changes of serum osteocalcin and bone mineral density of the fractured hip. Under clinical healing of fractures, serum osteocalcin level exhibits a higher value for the assessment of recovery speed of bone mineral density. Monitoring corresponding bone metabolism indexes after fracture healing can improve the accuracy of judging bone mineral density changes to reduce the risk of secondary fractures.
4.Determination of Bergenin in Dicha Kechuan oral liquid by HPLC

Objective:To establish a determination method of Bergenin content in Dicha Kechuan oral liquid by HPLC.Methods:The separation was performed on SHIMADZU VP-ODS C18 column(150mm?4.6mm,5?m),the mobile phase was carbinol-water(20:80) with a flow rate of 1.0ml/min.The column temperature was room temperature and the detection wavelength was 275nm.Results:The linear range of Bergenin was 0.2575-5.150?g,the regression equation of working curve was Y=812702.2X-80626.3,r=0.9998,the average recovery was 101.4% and RSD 1.67%(n=5).Conclusion:This method is simple,reproducible,and can be used for quality control of Dicha Kechuan Oral Liquid and the semifinished product.
5.Correlation of bone metabolic index with osteoarthritis and postmenopausal osteoporosis

BACKGROUND:Increasingly studies report that the normal balance of bone metabolism may be destroyed in the case of postmenopausal osteoporosis or osteoarthritis. The concrete metabolic process of bone turnover could be revealed sensitively by measuring the bone turnover markers in the serum or urine.
OBJECTIVE:To study the bone density and bone metabolic index of knee osteoarthritis (KOA) and postmenopausal osteoporosis (PMO), and to discuss the characteristics of bone density and bone metabolic index in KOA and PMO.
METHODS:A total of 248 postmenopausal women were detected for bone mineral density and knee X-ray. Final y 180 patients were included in this study and were divided into three groups:KOA group, PMO group, and control group. The levels of bone turnover markers (bone alkaline phosphatase, bone gla protein, col agen type I cross-linked C-telopeptide, and tartrate-resistant acid phosphatase 5b) in serum from the participants were measured. The correlation between bone turnover markers and the disease progression was analyzed by Logistic regression analysis.
RESULTS AND CONCLUSION:The bone mineral density in the KOA group was higher than the control group but col agen type I cross-linked C-telopeptide was lower. The levels of bone gla protein, col agen type I cross-linked C-telopeptide, and tartrate-resistant acid phosphatase 5b in serum from PMO group were higher than the control group. The decrease of col agen type I cross-linked C-telopeptide was associated with the incidence of KOA, and the increases of bone gla protein, col agen type I cross-linked C-telopeptide, and tartrate-resistant acid phosphatase 5b were associated with the incidence of PMO. The lower bone absorption can be seen in postmenopausal women with KOA. PMO patients showed a higher bone turnover rate. The difference of bone metabolism between patients with KOA and PMO led to negative relationship of bone mineral density. The serum levels of bone gla protein, col agen type I cross-linked C-telopeptide, and tartrate-resistant acid phosphatase 5b can assist clinical diagnose and therapeutic effect detection of both KOA and PMO.

7.Effect of ginsenoside Rb1 in ameliorating insulin resistance and ectopic fat deposition in obese mice induced by high fat diet.

Ginsenoside Rb1 is an active component in ginseng. Previous in vitro experiments showed that ginsenoside Rb1, could inhibit lipolysis and promote glucose transporter in adipocytes. This study focused on the effect of ginsenoside Rb1 in insulin resistance and ectopic fat deposit in obese mice induced by high fat diet and its molecular mechanism. Obese male C57/L mice induced by high fat diet were randomly divided into the diet-induced obesity group (DIO group), the ginsenoside Rb1 group (Rb1 group) and the rosiglitazone group (Rog group), and continuously fed with high fat diet. In addition, male C57/L mice fed with normal diet were selected as the normal group (NC group). Mice in Rb1 group and Rog groups were intraperitoneally injected with ginsenoside Rb1 and rosiglitazone with the dosage of 20 mg x kg(-1) and 10 mg x kg(-1), respectively. NC and DIO groups were intraperitoneally injected with the same amount of saline. Two weeks later, the intraperitoneal glucose tolerance test (IPGTT) was performed. Three days later, the mice were killed, and their serum samples were collected to detect insulin and free fatty acid (FFA). Their livers were weighed to examine the triglyceride content, and a pathological detection was performed. Epididymal adipose tissues were weighed, and PDE3B, HSL and perilipin were detected by Western blotting. The results showed that the treatment with ginsenoside Rb1 for two weeks could improve the glucose tolerance of obese mice. Except for 0-120 min, the areas under the glucose tolerance curve (0-30 min, 0-60 min and 0-90 min) in the Rb1 group were less than that in the DIO group (P < 0.05, n = 5), with a much lower HOMA-IR (P < 0.05, n = 5). The fat level of obese mice was significantly reduced by Rbl (P < 0.05, n = 5), and so were liver weight/weight (P < 0.05, n = 8). The increased serum FFA of obese mice declined after the treatment of Rb1 (P < 0.05, n = 8). Rb1 could partially recover the expression of perilipin in adipose tissues, but without obvious change in the expressions of PDE3B and HSL and the phosphorylated activation. The above findings indicated that ginsenoside Rb1 could reduce the release of FFA and alleviate the ectopic deposit of triglyceride by up-regulating the expression of perilipin in adipose tissue, which may be one of its mechanisms for improving the insulin resistance and abnormal glucose metabolism of organisms.

Objective This report describes the mechanism of Ginsenoside Re to resist Exercise-induced fatigue based on the observation of effects on MDA content and SOD activity.Methods Thirty male SD rats were divided into 3 groups randomly:Ginsenoside Re group,model group and control group,10 rats in each group.Rats in Ginsenoside Re group were given gastric gavage of Ginsenoside Re once a day,while rats in model and control groups were given the same volume of normal saline.One hour after administration,rats of Ginsenoside Re groups and Model groups received medium-intensity treadmill exercise for 20 minutes at the speed of 15m/min,with a slope of 0 degree,and after 40-mimute break repeated it for another 20minutes.Fourteen days later,MDA content and SOD activity have been tested.Results MDA content in the serum,liver tissue and muscle tissue of Ginsenoside Re group was obviously lower than that in the model group (P

10.Clinical characteristics of dyskinetic cerebral palsy

ObjectiveTo study the risk factor, semeiology and neuroimaging abnormalities of dyskinetic cerebral palsy.MethodsA hospital-based study, 136 children with dyskinetic cerebral palsy were examined neurologically and their perinatal history was reviewed. Their cranial CT or MRI findings were studied. The association between the gestational ages, CP types and the radiological appearances were analyzed.Results124 cases (91.18%) were found obviously risk factors, including asphyxia (34 cases), pathological hyperbilirubinemia (70 cases), both asphyxia and hyperbilirubinemia (11 cases) and others (8 cases). According to the clinical and neurological features, 60 (44.12%) were chorea-athetoid cerebral palsy, 26 (19.12%) were dystonic cerebral palsy, and 50 (36.76%) were athetoid-spastic cerebral palsy. Those with asphyxia were mainly athetoid-spastic whereas cases with pathological hyperbilirubinemia were mainly chorea-athetoid cerebral palsy. The abnormal rates of cranial MRI scans was 52.9%, and it was higher in the group of asphyxia than pathological hyperbilirubinemia, preterm than term. The main findings on MRI scans were as follows: periventricular leucomalacia(PVL) 28 (38.8%), diffuse bilateral atrophy 20 (27.8%), focal abnormalities in the basal ganglia1 and/or temporal lobe 18 (24.0%).ConclusionMRI abnormalities of the brain were correlated with semeiologic subtypes, risk factors, and the gestational age at birth.
