Objectives:To understand the patients' cognition and satisfaction with public welfare in public hospitals.Methods:A questionnaire survey was conducted among 600 patients in Beijing municipal public hospitals and the data was statistically analyzed by SPSS 22.0.Results:Patients' understanding of public welfare was not ideal,the satisfaction was quite well,and the satisfaction of the patients in the experimental hospitals was more satisfactory than those in non-experimental hospitals.The patients have urgent needs on rational drug use,mutual recognition of medical results,smooth channels of patient rights,reduced medical expense and shortened waiting time.Conclusions:It should strengthen the publicity and governance of the public welfare of public hospitals to enhance the patients' understanding,breakthrough the urgent needs of public welfare to improve patients' satisfaction,and intensify the policy linkage and explore diversified implement forms of public welfare to improve service level.

Analysis of the nature, tastes, channel distributions and effects of the frequently used herbal medicines in the prescriptions involved in the clinical literatures about treatment of gastric cancer published from 1988 to 2007 was made in the paper. The literatures were categorized into three types: 1) treatment of middle- and late-stage gastric cancer; 2) prevention and treatment of the recurrence and metastasis after operation; 3) Chinese herbal medicines combined with chemotherapy for enhancing efficacy and reducing toxicity. The most frequently used herbal medicines in the three literature types were qi-invigorating herbs, such as Atractylodes, Astragalus, Codonopsis, Glycyrrhiza and Ginseng, etc. The herbal medicines for promoting urination to subside swelling such as tuckahoe and Semen Coicis, etc were used more frequently than the herbal medicines for regulating qi such as dried orange peel and putchuck, etc, as well as for clearing away heat to remove toxin such as spreading hedyotis herb, Herba Scutellariae Barbatae, yangtao actinidia root, and Rhizoma Paridis, etc. From another angle, the most frequently used herbal medicines for the treatment of gatric cancer were those cold, warm and neutral in nature, sweet, bitter and pungent in taste, and distributed to spleen and liver channels.

To determine whether p53 gene transcripts were altered in thyroid tumor and adjacent tissues of thyroid tumor, total RNA from 12 tumor specimens (9 carcinomas and 3 adenomas) and 4 adjacent tissues of thyroid tumor was exmined by Northern blot hybridization and reverse transcription——polymerase chain reaction. It was found that p53 gene transcripts were present in all tumors and the adjacent tissues of tumor and were significantly more obvious in the samples of carcinomas as compared with those of adenomas and in the samples of tumor as compared with those of the adjacent tissues of tumor. These data suggest that excessive p53 gene expressions are involved in thyroid carcinogenesis.

Objective To investigate the effect of old myocardial infarction on the heart rate variability.Methods Totally 86 patients with old myocardial infarction and 78 healthy people were enrolled in the study.Twenty-four-hour heart rate variability in the time domains including NN,SDNN,SDANN,RMSSD,and PNN50 and the frequency domains including VLF,LF,HF,and LF/HF were analyzed.Results Both the time domain and frequency domain indexes of the heart rate variability in patients with old myocardial infarction significantly decreased than those in the healthy peolple（P 0.05）.Conclusion There might be autonomic nervous system dysfunction in the patients with old myocardial infarction.

OBJECTIVETo evaluate the impacts on the short-term efficacy and the long-term prevention of recurrence of allergic rhinitis treated with the triple-strong stimulation at Dazhui (GV 14) so as to provide the convenient and long-term effective therapy of acupuncture and moxibustion for allergic rhinitis.

METHODSOne hundred and twenty cases of allergic rhinitis were randomized into an acupuncture group, an acupuncture + medication group and a triple-strong stimulation group, 40 cases in each one. In the acupuncture group, acupuncture was applied at Dazhui (GV 14), Fengchi (GB 20), Baihui (GV 20), Yintang (GV 29) and the others, stimulating with reinforcing manipulation for the deficiency and reducing manipulation for the excess, once every day. In the acupuncture + medication group, on the basis of acupuncture therapy, claritin (loratadine tablets) was supplemented for oral administration, 10 mg, once every two days, continuously for 30 days. In the triple-strong stimulation group, on the basis of acupuncture therapy, the strong needling, strong cupping and strong moxibustion were applied at Dazhui (GV 14). This combined therapy was given once every day in the first 3 days and once every two days afterwards. The 10 day treatment made one session, at the interval of 3 days between the sessions and totally 3 sessions were required in the three groups. Separately, before treatment, after treatment and in 6 months after treatment, the changes of symptom and physical sign score and value of single item symptom including nasal itching, nasal blockage, sneezing and rhinorrhea were observed in the patients of the three groups. And the long-term clinical efficacy was compared among the three groups.

RESULTSThe symptom and physical sign score and the value of single item symptom were all reduced in the three groups after treatment and in 6 months after treatment (P < 0.001, P < 0.01, P < 0.05). The results in the triple-strong stimulation group were superior to the other two groups (all P < 0.05). In the triple-strong stimulation group, the total effective rate was 92.5% (36/40) in the follow-up of 6 months after treatment, which was better than 60.5% (23/38) in the acupuncture group and 69.2% (27/39) in the acupuncture + medication group (both P < 0.01).

CONCLUSIONThe combined therapy of acupuncture and the triple-strong stimulation at Dazhui (GV 14) achieves the reliable and effective result in the clinical treatment of allergic rhinitis and displays the good role on the prevention from long-term recurrence.

Objective To investigate the effect of serum-deprivation on the changes of [ Ca2+ ] i and the protein release of S100A13 and fibroblast growth factor 1 ( FGF-1 ) in thyroid cancer TT cell,and to reveal the role of Ca2+in the protein release of S100Al3 and FGF-1.Methods The protein expressions of FGF-1 and S100A13 in TT cells under serum-deprivation were detected by Western blot.The released FGF-1 protein from TT cells in the supernatant fluid was detected by ELISA.Realtime dynamic examinations on the change of 1 h [ Ca2+ ] i in TT cells under serum-deprivation were detected by confocal laser scanning microscopy.Then,the effect of EGTA( 2.5 mmol/ L),BAPTA-AM (2.5 μmol/L)on distributions of the fluorescence of S100AI 3 and FGF-1 in TT cells under serumdeprivation for6 h were detected by indirect immunofluorescence.Results The expressions of FGF-1 and S100A13 in TT cells after serum-deprivation for4 h and 6 h were reduced( P＜0.05 or P＜0.01 ),but the released FGF-1 protein from TT cells in the supernatant fluid was elevated ( P＜0.05 or P＜0.01).Confocal laser scanning of Ca2+ imaging indicated that [ Ca2+ ] i of serum-deprivation TT cells maintained the relative stabilization within 23 win,but the rapid rise of [ Ca2+ ] i achieved peak value 1.6 μmol/L after 30 min,and remained stable for about 17 win,and thereafter 40 win slowly dropped to a low level From 40 win to 60 win the [ Ca2+ ] i was about 0.3-0.6 μ mol/L.The average [ Ca2+ ] i was higher than that in normal group,EGTA group,and BAPTA-AM group within 1 h.The protein expressions of S100A13 and FGF-1 did not drop obviously in EGTA group and BAPTA-AM group.Conlusion The release of S100A13 and FGF-1 from TT cell under serum-deprivation is possibly related with the change of [ Ca2+ ]i.Both Ca2+-chelating agents EGTA and BA PTA-AM are able to inhibit the rise of [ Ca2+ ] i and release of S100A 13 and FGF-1 from TT cells under serum-deprivation.

Endoplasmic reticulum stress(ERS) is a kind of subcellular pathological state,and associated with many diseases.Recently,the research of ERS on chondrocyte is at the beginning stage,and may be involved in pathogenisis of rheumatoid arthritis(RA) and osteoarthritis(OA).It has been proved that ERS can interfere the differentiation of chondrocyte,decrease the synthesis of abnormal protein,attenuate the injury of cell.But overreaction of ERS can cause chondrocyte apoptosis through an independent pathway without of Fas and NO.There are three signal transmission passages in ERS:ATF-6(activating transcription factor 6)、Ire 1(inositol-requiring 1)and PERK(PKR-like ER kinase).The three protein molecules activate apoptotic genes by TRAF-2(TNF receptor-associated factor 2)and GADD153(growth arrest and DNA-damage-inducible gene 153),initiate the chondrocyte apoptosis.Therefore,ERS may effect the pathogensis of RA and OA by modulating chondrocyte function and inducing apoptosis,but more research are needed to reveal the mechanism.

Objective: To investigate the protein basis and molecular mechanism of combination therapy with epigallocatechin gallate (EGCG), taurine, and genistein on CCl4-induced liver fibrosis in rats using isobaric tags for relative and absolute quantitation (iTRAQ). Methods: The SD rats were randomly divided into normal control, model, and combination therapy (taurine 200 mg/kg + EGCG 30 mg/kg + genistein 20 mg/kg) groups. The rats in the model and combination therapy groups were induced by ig administration of 50% CCl4 for 14 weeks, and the rats in the normal control group were given peanut oil. The combination therapy was started at week 9 for 6 weeks. HE staining of liver tissue was performed to evaluate histopathologic changes. The high-abundance proteins in the serum of model and combination therapy groups were depleted by ProteoMiner Protein Enrichment Kit. Proteins differentially expressed in the serum of those two groups were identified with iTRAQ coupled with LC-ESI-MS/MS technology and analyzed with bioinformatics tools. The expression of representative differential protein (Txn1) was validated by ELISA. Results: A total of 359 proteins with the confidence coefficient above 95% were identified, of which 78 were differential expressed proteins, including 51 up-regulated ones and 27 down-regulated ones. The serum level of Txn1 increased significantly in the combination therapy group compared with the model group (P < 0.05). Conclusion: The combination therapy can alleviate the progression of liver fibrosis effectively by affecting multiple biological processes. The regulation of anti-oxidant defense system and coagulation cascade pathway may be the molecular mechanisms of the combination therapy against liver fibrosis.
