3.The Treatment of Mid-shaft Clavicle Fractures.

OBJECTIVEThrough reviewing the relevant literature from the past decades, to summarize the assessment and management of fractures of the clavicle, and provide an overview of the clinical results of a range of treatment options.
DATA SOURCESThe data analyzed in this review are mainly from articles included in PubMed and EMBASE, published from 1960 to 2015.
STUDY SELECTIONStudies involving assessment of fractures of the clavicle were reviewed. Further literatures were gathered regarding the conservative and surgical treatment of these fractures, including the methods of fixation and the surgical approaches used. Both conservative and surgical treatments were then compared and contrasted.
RESULTSThrough retrieving and reading the abstract, a total of 42 representative articles were selected, which covered all aspects of the conservative treatment and surgical treatment, and compared the advantages and disadvantages of different treatment options.
CONCLUSIONSAlthough the majority of recent data suggest that surgery may be more appropriate as it improves functional outcome and reduces the risk of complications, we recommend that the treatment should be individually assessed.

4.Protective effects of Schizonepeta volatile oil on endotoxin poisoning induced by lipopolysaccharide in mice.

In order to study the protective effects of Schizonepeta volatile oil (Sto)on endotoxin poisoning mice, and the relatively content of each chemical osubstance in Schizonepeta volatile oil was measured using GC-MS. The mare C57BL/6J mice were randomly divided into five groups including the normal group, model group, dexamethasone group (5 mg•kg⁻¹), and Sto (0.226 and 0.452 g•kg⁻¹, respectively) groups. The dexamethasone group was given the drugs once time by intraperitoneal injection on the 5th day, while the other mice were given drugs by oral administration once a day for 5 days. Then, the normal group was injected with the saline and the other groups were injected LPS (15 mg•kg-1) after 30 minutes of the last administration. After LPS injection twelve hours, the blood, serum, and lung tissue of mice were collected. The IL-18, IL-1β, IL-5, TNF-α, MCP-1, MIP-1β, M-CSF, and GM-CSF were measured in serum by ELISA and Luminex Magpix. The white cell (WBC) and platelet (PLT) in blood were counted and lung, spleen, and thymus index were calculated. The lung histopathology was performed at the same time. The GC-MS results showed that the relative content of menthone and pulegone are 46.67% and 33.92%, respectively. The Sto (0.452 and 0.226 g•kg⁻¹, respectively) reduced the levels of IL-1β, IL-5, TNF-α, MCP-1, MIP-1β, and M-CSF in serum (P<0.01 or P<0.05). The 0.452 g•kg⁻¹ Sto also reduced the levels of IL-18 and GM-CSF in the serum (P<0.01 or P<0.05). And the 0.226 g•kg⁻¹ Sto showed good anti-inflammatory effects by reducing neutrophil infiltration in the lung tissue. But the Sto had no effect on the increasing of WBC, spleen and lung index as well as decreasing of PLT and thymus index. The results showed that Sto has a protective effect in LPS-induced exdotoxin poisoning mice, its mechanism is related to inhibit the release of varies of inflammatory cytokines and reduce the inflammation reaction.

Objective Throush identify biochemical characteristics and virulence factors of 2 strains suspected Yersinia pestis(Y.pestis)isolated from the dead Marmota himalayana(M.himalayana)to confirm the nature epidemic focus in Dege County,Sichuan Province.Methods Y.pestis was analyzed by specific staining and shape,culturing characteristics,splitting-test by bacteriophage,test of biochemical characteristics and glycolysis ability,virulence factors,virulence,nutritional requirement,plasmid,genetic test and genetic type. Results The tested strains were Gram staining bacilus.The main biochemical characteristics were Arabinose(+)、 Rhamnose(-),Maltose(+),Melibiose(-),Glycerol(+),Denitrification(+).The virulence factors with FI+.VW+, Pgm+,Pst I+;and with the common 6.0×106,45.0×106,65.0×106 plasmids,also with the virulence-relative plasmid gene.Both their absolutely lethal dose(LD100)in mice were 50 bacteria.The nutritional requirement appeared which were depended on Phenylalanine and Methionine.With the Genomovar 5 genotype characteristics of M.himalayana plague foci of Qinghai-Tibet plateau.The difference between tested strains and Yersinia pseudotubercuosis on the 3 different culture medium was obvious.The tested strains had a Y.pestis' specific 3a fragment,Pst I and FI-Ag,at 22 ℃,the strains could be split by bacteriophage completely.Conclusions According to the diagnostic criteria of plague in China,the 2 suspected strains isolated from Dege County,Sichuan Province ale confirmed as Y.pestis.both with powerful virulenceand with the characteristics of the Y.pestis of M.himahtyana in Qinghai-Tibet plateau plague natural focus.
6. Current status and prospects of the survey of cardiac arrest and resuscitation

Cardiac arrest(CA)is a common serious clinical event. The high incidence and low survival rate make it one of the important public health problems in China. We urgently need to investigate the incidence, mortality and risk factors of cardiac arrest in Chinese population. Based on epidemiological data on cardiac arrest in China, early identification of cardiac arrest and early initiation of cardiopulmonary resuscitation can significantly improve outcomes of patients with in-hospital and out-of-hospital cardiac arrest. The article summarizes the definition of cardiac arrest, the epidemiology, domestic and international gap and prospects of cardiac arrest in and out of hospital.

To scientifically evaluate the intervention effect of Chinese medicine preventive administration(combined use of Huo-xiang Zhengqi Oral Liquid and Jinhao Jiere Granules) on community population in the case of coronavirus disease 2019(COVID-19), a large cohort, prospective, randomized, and parallel-controlled clinical study was conducted. Total 22 065 subjects were included and randomly divided into 2 groups. The non-intervention group was given health guidance only, while the traditional Chinese medicine(TCM) intervention group was given two coordinated TCM in addition to health guidance. The medical instructions were as follows. Huoxiang Zhengqi Oral Liquid: oral before meals, 10 mL/time, 2 times/day, a course of 5 days. Jinhao Jiere Granules: dissolve in boiling water and take after meals, 8 g/time, 2 times/day, a course of 5 days, followed up for 14 days, respectively. The study found that with the intake of medication, the incidence rate of TCM intervention group was basically maintained at a low and continuous stable level(0.01%-0.02%), while the non-intervention group showed an overall trend of continuous growth(0.02%-0.18%) from 3 to 14 days. No suspected or confirmed COVID-19 case occurred in either group. There were 2 cases of colds in the TCM intervention group and 26 cases in the non-intervention group. The incidence of colds in the TCM intervention group was significantly lower(P<0.05) than that in the non-intervention group. In the population of 16-60 years old, the incidence rate of non-intervention and intervention groups were 0.01% and 0.25%, respectively. The difference of colds incidence between the two groups was statistically significant(P<0.05). In the population older than 60 years old, they were 0.04% and 0.21%, respectively. The incidence of colds in the non-intervention group was higher than that in the intervention group, but not reaching statistical difference. The protection rate of TCM for the whole population was 91.8%, especially for the population of age 16-60(95.0%). It was suggested that TCM intervention(combined use of Huoxiang Zhengqi Oral Liquid and Jinhao Jiere Granules) could effectively protect community residents against respiratory diseases, such as colds, which was worthy of promotion in the community. In addition, in terms of safety, the incidence of adverse events and adverse reactions in the TCM intervention group was relatively low, which was basically consistent with the drug instructions.
