Objective To improve the poor water-solubility of nimodipine,nimodipine-loaded PLEG copoly- met nanopartieles were prepared to avoid the sedimentation of its crystal from water,which could also shun the blood vessel stimulation caused by ethanol,a cosolvent,in the injection.Methods Solvent evaporation method was used to prepare water-dispersing nimodipine-loaded nanoparticles,whose lyophilized powder was further prepared,with PLEG2000 as drug carrier.Particle size,Zeta potential,embedding ratio,drug loading,shape and redispersion ability of the lyophilized powder were determined.Results Concerning the lyophilized nanoparticle powders,the particle sizes were 136nm,the Zeta potentials were -29.90mV,the embedding ratio was 67.9% and the drug loading was 6.17%.Conclusion Prepared nimodipine-loaded nanoparticles solved the problem of poor water-solubility of ni- modipine basically.It also avoided the recrystallization process of the drug and reduced the blood vessel stimulation caused by ethanol in the injection.

Objective:To observe the clinical effect of needling gluteus medius muscle trigger point (TrP) plus chiropractic for sacroiliac joint subluxation.Methods:A total of 124 cases conforming to the inclusion criteria of sacroiliac joint subluxation were classified by anterior subluxation and posterior subluxation,and randomized into a TrP group and a conventional group respectively.There were 63 patients with anterior subluxation who were randomized into a TrP group of 32 cases (including 4 dropped out cases) and a conventional group of 31 cases (including 3 dropped out cases);and 61 patients with posterior subluxation who were randomized into a TrP group of 31 cases (including 3 dropped out cases) and a conventional group of 30 cases (including 3 dropped out cases).Patients in the TrP group received the treatment of needling gluteus medius muscle TrP plus chiropractic,while patients in the conventional group received conventional acupuncture treatment plus chiropractic.The treatment was done twice a week for a succession of 8 weeks.Then,the pain visual analog scale (VAS),Oswestry disability index (ODI) and therapeutic efficacy were evaluated.Results:After treatment,the total effective rate of TrP group with anterior subluxation was 96.9％,higher than 77.4％ in the conventional group,the difference showed a statistical significance (P＜0.05);the total effective rate was 93.5％ in the TrP group with posterior subluxation,higher than 73.3％ in the conventional group.After treatment,the VAS and ODI scores in both groups dropped obviously,the differences showed statistical significance (all P＜0.05);the scores of VAS and ODI in the TrP group were obviously lower than those in the conventional group (all P＜0.05).Conclusion:Needling gluteus medius muscle TrP plus chiropractic had a better therapeutic effect than conventional acupuncture plus chiropractic for sacroiliac joint subluxation.

On the basis of the Uncaria transcriptome, specific primers were designed for UrSTR. The full-length cDNA of UrSTR (GeneBank: OL310251) was 1 541 bp, encoding 345 amino acid residues, and the promoter region sequence of UrSTR (GeneBank: OL310252) was 1 179 bp. Phylogenetic tree is revealed that UrSTR had a closest relationship with STR from Ophiorrhiza pumila and Ophiorrhiza japonica. Localization of UrSTR protein is revealed located in the vacuole membrane. Plant-care analysis indicated that the promoter region sequence of UrSTR, covering multiple light, stress and hormone-response cis-regulatory elements, and verified transcriptional activity. The results of SDS-PAGE show that pET-28a-UrSTR recombinant protein was successfully expressed, and the size was anticipated. The UrSTR prokaryotic expression system needs to be optimized in the later stage. The research lays the foundation for further purification to study its structure and functional characterization of the UrSTR protein.

ObjectiveTo observe the rehabilitative effect of patients with shoulder-hand syndrome after stroke by manipulation treatment. MethodsThe patients with shoulder-hand syndrome were randomly divided into two groups, manipulation group (180cases) and control group (128 cases). Patients in the manipulation group were regularly given a passive quantitative movement on shoulder, elbow and hand joints,while patients in the control group were irregularly given a passive movement or ordered to perform an autonomic movement. The signs and symptoms of patients in these two groups were not much different. The rehabilitative effects were compared 3 months later. ResultsSigns and symptoms in the manipulation groups improved much better than that of the control group. Conclusions The manipulation treatment for the post-stroke patients with shoulder-hand syndrome is the method that is simple, effective and easy to perform.

OBJECTIVETo study modified ilioinguinal approach through the retrospective analysis on the surgical treatment of 63 patients with pelvic and acetabular fractures through anterior approach.

METHODSFrom January 2006 to January 2013, 63 patients with pelvic and acetabular fractures were treated with the ilioinguinal anterior approach, including 45 males and 18 females, ranging in age from 12 to 68 years old, with an average of (37.71 +/- 13.41) years old. All the patients were divided into two groups: standard ilioinguinal anterior approach group (group A) and modified ilioinguinal anterior approach group(group B). In group A, there were 26 males and 11 females, with an average age of (38.49 +/- 13.64) years old. In group B, there were 19 males and 7 females, with an average age of (36.62 +/- 13.29) years old. Intraoperative and postoperative indicators in group A and B were observed and compared, including operation incision exposure time (from skin incision to complete the ilioinguinal in front of three "windows"), the blood loss, incision close time and treatment effect of Majeed function score.

RESULTSCompared to group A, the incision exposure time of patients in group B was shorter, the blood loss (bleeding during exposure process) was less, and the close incision time was shorter, but the treatment effect of Majeed function score had no significant differences between two groups. All the patients were followed up, and the during ranged from 3 to 36 months, with an average of (18.6 +/- 9.2) months. According to Matta standard assessment reduction of pelvic and acetabular fracture, there were 28 patients got an excellent result, 8 good, and 1 fair in the group A; and 20 patients got an excellent result, 5 good, and 1 fair in the group B. According to Majeed function score for hip function, 20 patients got a satisfactory result, 12 good,4 fair and 1 poor in group A, and the mean score was 82.51 +/- 9.72; and 13 patients got an satisfactory result, 10 good, 3 fair and 0 poor in group B, and the mean score was 80.54 +/- 10.79.

CONCLUSIONThe modified approach has several advantages as follows: providing a good surgical exposure; preventing from the injury of femoral nerve, femoral artery and vein under the inguinal ligament; not needing to open the inguinal canal, which can avoid the occurrence of inguinal hernia, reduce operation prodedures and shorten operation time.

Objective To explore the optimized range of normal tissue ablation around the tumor by HIFU in combination with UCA for VX2 liver tumors in rabbits. Methods In this study,51 rabbit models with implanted fiver VX2 tumors were randomly divided into 3 groups:ablating the tumor only(group A)、ablating the tumor and 2 mm eircumferential normal tissue(group B)、and 4 mm normal tissue around the tumor(group C).After HIFU+UCA treatment,the pathology of the targeting tissues,the level of serum ALT,the metastases in the liver、lungs and abdominal cavity,and the survival time were compared with each other. Results After treatment coagulative necrosis was observed in all the target tissues.Serum ALT level between each two groups were statistically different on d3 and d5(F=12.933、18.37,P＜0.01、＜0.01),and the level of ALT in group C was the hightest.The ALT level in group A was significantly higher than group B or group C on the 14th day(F=17.2,P＜0.01).Metastases in group A was significantly higher than group B or group C,and there were no statistically different between group B and C (P=0.353、0.332、0.380).The survival time both in group B and group C were significantly longer than group A(48±13.6,51±12.1 vs 29±5.4),and there were no statistically difference between group B and C(P＞0.05). Conclusions Tumor eradication can be significantly promoted when 2mm normal liver tissue around the tumor is ablated by HIFU+UCA.
