Objective To explore the correlative factors and management of secondary high intraocular pressure(IOP) after vitreoretinal surgery for proliferative diabetic retinopathy(PDR). Methods The data of 51 patients(54 eyes) with PDR after vitreoretinal surgery were collected.The incidence of secondary high IOP was obtained,and the correlative factors leading to high IOP were analysed.Besides,the therapeutic effects of management were observed. ?AlM: To compare clinical efficacy between Ahmed glaucoma valve implantation and trabeculectomy in patients with primary open angle glaucoma ( POAG) .
?METHODS: This retrospective study included 58 eyes from 45 patients with primary open angle glaucoma. And 32 eyes underwent penetrating trabeculectomy ( group A), while 26 eyes were performed Ahmed glaucoma valve implantation ( group B) . lntraocular pressure ( lOP) , best-corrected visual acuity ( BCVA ) , visual field and post-operative complications were observed between these two groups.
?RESULTS:(1) lOP:the lOP of post-operation in group A were (13. 56±4. 91), (14. 47±4. 03), (17. 56±5. 74), (18. 25±5. 49), (18. 13±4. 24), (19. 68±4. 55) mm Hg at 2d, 1, and 2wk, 1, 3 and 6mo respectively; and that were (13.58±4. 16), (16. 00±4. 83), (18. 00±5. 05), (19. 42±5. 41), (18. 42±3. 37), (20. 00±5. 37) mm Hg in group B. There was no statistically significant difference in lOP between the two groups ( P> 0. 05 ). ( 2 ) BCVA: the number of visual acuity decreased eyes, with 6mo follow-up, was 7 (22%) in group A; and that was 5 in group B (19%). There was no statistically significant difference in vision loss postoperatively between the two groups (χ2=0. 061, P>0. 05). (3) Visual field: with 6mo follow-up, there were 13 eyes ( 41%) which had constricted visual field in group A, while those were 10 eyes (38%) in group B; the difference of visual field loss between the two groups was not statistically significant (χ2 = 0. 028, P>0. 05 ) . ( 4 ) Complications: six-month follow-up after operation, there were 4 eyes with shallow anterior chamber, 4 eyes with complicated cataract and 1 eyes with Descemet’s membrane detachment in group A, while that was 1 eyes with shallow anterior chamber and 1 eyes with complicated cataract in group B; there was statistically significant difference in the rate of complications between the two groups (χ2 = 4. 144, P<0. 05 ) . ( 5 ) Surgical success rate: six month after operation, the completed success rate was 37% (12 eyes) in group A, and that was 35% (9 eyes) in group B. The partial success rate was 19% (6 eyes) and 15% (4 eyes) in group A and group B respectively. The difference of success rates between the two groups was not statistically significant (χ2=0. 225, P>0. 05).
? CONCLUSlON: Both Ahmed glaucoma valve implantation and trabeculectomy are effective methods for the treatment of POAG. The clinical efficacy was no difference between the two methods. However, compared with trabeculectomy, Ahmed glaucoma valve implantation was safer and had fewer complications.

Objective To evaluate the cost-effectiveness of early enteral nutrition (EEN) in elder patients undergoing gastrectomy for gastric carcinoma. Traditionally, a novice ear doctor acquires the skillfully operative ability through dissecting cadaveric temporal bone in ear surgical training lab. For conceivable reasons donated bodies are always short in supply, virtual reality-based simulator provides a new model for surgical training. It has been proven that the simulator is a useful tool for surgical training. And it can be used in the rehearsal of operation procedures, real-time operation guidance, and remote surgery. Our design is different from the existing virtual surgical system. It has a parallel, distributed multi-workstation with the vivid three-dimensional model of temporal bone, and it can be used not only to learn the ear surgical skills in the virtual environment, but also preoperative plan and surgical consultation.

9.Changes of Ultrastructure of Xenograft Model of Human Hemangioma on Nude Mice

Objective To investigate the development of ultrastructure in grafts from a xenograft model of human hemangioma on nude mice.Methods Human infantile hemangioma tissue was obtained in surgery and planted into subcutaneous layer of nude mice in small pieces.The grafts were harvested on the 2~(nd),5~(th),15~(th),30~(th),45~(th) day and then at 2~(nd),3~(rd),4~(th),6~(th) month for transmission electron microscopic view.Results In early stage after transplantation,signs of degeneration and necrosis were more evident.On the 30~(th) day,the number of endothelial cells increased and some of the endothelial cells were larger in size with high nuclear to plasm ratio.At the 2~(nd) month,the number of endothelial cells increased,while some of these cells turned flat.Later,high endothelial cells decreased in number and collapse and pyknosis of plasm,formation of secondary lysosome,margination of chromatin,fragmentation of nucleus were evident with surrounding deposition of adipose and collagen.Conclusions With xenotransplantation of human infantile hemangioma tissue onto nude mice,the grafts may survive ischemia and proliferate,and then gradually involute.The development resembles a natural course of human infantile hemangioma.
10.Cloning and Characterization of Swamp Buffalo SRY Gene

The SRY gene from buffalo (Bubalus bubalis) genome was amplified by the polymerase chain reaction ( PCR) with primers based on the sequence of Hostein SRY gene. The amplified fragment was 2005 bp include 5UTR ( 1 - 504bp) and 3'UTR(1196 - 2005bp). And the amplified fragment was cloned and sequenced. Sequence analysis showed that the coding region of SRY gene (505 - 1195bp) from buffalo was highly homologous with those of other bovine counterpart genes (96% homology) , especially in the HMG box region (99%homology). It was found that there were only signal on male buffalo genome on Southern blot,which indicate SRY gene are highly conservative on evolves.
