1.Diagnosis of primary neurogenic tumor on chest wall using CT imaging

Objective To discuss manifestations of neurogenic tumors on chest wall on CT imaging and pathology.Methods 7 patients with neurogenic tumors on chest wall confirmed surgically and pathologically were reviewed.To facilitate differential diagnosis,3 cases of malignant sarcoma were reviewed for their features on CT.Results 4 out of 5 cases of neurinoma were benign,with one case showing even density on plain CT scan,3 were poorly even,one case showed obvious evenness after enhancement,and 1 case was poorly evenly enhanced,2 benign cases had compression absorption of surrounding bone of scapula or ribs.One case of moderate malignancy was uneven in plain scan and slightly enhanced after enhancement procedure,and neighboring bone of scapula and rib were compressed and destroyed and absorbed,and neighboring muscular interspace and upper mediastinum were involved.One case showed single lesion of neurofibroma,displaying relatively even density on plain scan and moderate unevenness after enhancement procedure,as well as compression and absorption of neighboring ribs.One case of neurofibroma had multiple lesions,showing uneven density on plain scan.This case did not undergo enhanced scan.The remaining 3 cases were all sarcoma including 2 cases of fibrosarcoma and one case of synovial sarcoma.Conclusion Mass of soft tissues that are situated on chest wall or paravertebral area and with smooth edge are suggestive for benign neurogenic tumor.On the contrary,rough edge and coarse shape and infiltration into neighboring tissues are characteristic for malignant ones.
2.Incidence of Nosocomial Infection with Objective Monitoring Method and Prospective Overall Monitoring Method:A Comparison

OBJECTIVE By investigating the incidence of nosocomial infection with objective monitoring method and prospective overall monitoring method, to analyze the advantage and significance between two methods. METHODS Using the objective monitoring method to study the incidence of nosocomial infection of patients after neurosurgical operation from May to December in 2003 and using the prospective overall monitoring method to study the incidence of nosocomial infection of patients after neurosurgical operation from May to December in 2002. All data were analyzed with Stata 7.0 soft. RESULTS There were 100 cases occurring nosocomial infection in 267 neurosurgical operation patients. The patient incidence of nosocomial infection was 37.5% and the rate of nosocomial infection cases was 50.9% with objective monitoring method, that was significant higher than those data coming from May to December in 2002 with 248 cases under prospective overall monitoring method, with which the patient incidence of nosocomial infection and the rate of nosocomial infection cases were 20.2%(50 cases ) and 21.4%(53 cases), respectively . 5.Study on quality standard for Qiankun Liquid

Objective: To establish the quality standard for Qiankun Liquid.Methods: Fructus Lycii、Fructus Cnidii、Radix Ginseng in Qiankun Liquid were identified by TLC, and the content of icariin was determined by HPLC.Results: Fructus Lycii、Fructus Cnidii、Radix Ginseng could be identified by TLC. Icariin showed a good linear relationship at a range of 0.126~1.008?g, r =0.9999. Conclusion: The method is accurate and can be used for the quality control of Qiankun Liquid.
6.Nosocomial Infection in Patients with Hematological Malignancies:Targeted Monitoring and Risk Factor Analysis

OBJECTIVE To investigate the characteristics and risk factors of nosocomial infection(NI) in patients with hematological malignancies(HM),and provide the bases for making the controlling measures of NI.METHODS Using the targeted monitoring to study NI in patients with HM,and recording 14 factors such as the time of hospitalization,chemotherapy,count of leukocyte and absolute neutrophil count(ANC) and so on.The data were analyzed with unifactorial ?2 test and multifactorial Logistic-regression analysis.RESULTS Among 242 patients with HM the prevalence of NI was 35.5%(86/242) and the prevalence of NI time-cases was 52.9%(128/242).Among 86 patients of NI there were 27 patients occurred multiple sites NI(31.4%).The main infection sites were upper respiratory tract,gastrointestinal tract,lower respiratory tract,oral cavity and blood.66.7% Of NI happened in the period of chemotherapy and 7 days after chemotherapy.The time of hospitalization and ANC were independent risk factors of NI in patients with HM.CONCLUSIONS The patients with HM are susceptible population of NI,and NI often occurs in the period of chemotherapy and 7 days after chemotherapy.So medical staff should strengthen monitoring,and shorten the time of patient hospitalization and of recovery of ANC to reduce the prevalence of NI efficiently.
7.Correlation analysis of efficacy of yiqi chutan recipe in treating NSCLC and P4HB expression.

OBJECTIVETo study the predicting effect of proly 4-hydroxylase beta polypeptide (P4HB) in treating non-small cell lung cancer (NSCLC) patients by Yiqi Chutan Recipe (YCR).
METHODSTotally 46 stage III and IV NSCLC patients were treated by YCR for 4 therapeutic courses. Effect was assessed by RECIST of solid tumor. P4HB expression was detected in the lung cancer tissue by immunohistochemical assay. Factors affecting disease control rates (DCR) of YCR were analyzed by Logistic regression analysis. The correlation between P4HB expression and the effect of YCR was analyzed.
RESULTSThe DCR of advanced NSCLC treated by YCR was 36.96% (17/46 cases). P4HB was high expressed in advanced lung cancer tissue (6/15 cases). Gender, initial treatment, and retreatment are independent factors for affecting DCR of treating lung cancer by YCR.
CONCLUSIONP4HB might be taken as a factor for predicting the effect of YCR in treating NSCLC.

9.Effects of riboflavin combined with photosensitization on reduction of Gram-positive and Gram-negative indicating germs in plasma and P-selectin expression of apheresis platelet concentrates.

This study was purposed to confirm the practical efficacy of reducing indicating germs suspended in plasma by riboflavin and photosensitized inactivation and to evaluate its influence on activation of apheresis platelet concentrates. The synergistic effects of riboflavin combined with ultraviolet irradiation on inactivation of germs were investigated by using Escherichia Coli (E. coli) and Staphylococcus Aureus (S. aureus) as Gram⁻ and Gram(+) indicating germs, respectively. The activation status of apheresis-platelet concentrates treated with riboflavin combined with ultraviolet irradiation was detected by flow cytometry. The results showed that when 50 μmol/L of riboflavin was combined with 6.2 J/ml of ultraviolet irradiation, the T/E ratios reached 1.42 for E. coli and 1.68 for S. Aureus, and reduction of E. Coli and S. Aureus were 3.87 Logs and 3.82 Logs respectively; the CD62p expression level on germ-inactivated platelets stored at 22 degrees C for 0 and 5 days were 4.92% and 36.18% respectively, which slightly increased as compared with controls (3.94% and 32.03)% (p < 0.05). It is concluded that combination of riboflavin with ultraviolet irradiation displays well synergistic effects which can reduce E. Coli and S. Aureus counts, but no significantly influence on platelets. The partial activation of liquid platelets mainly presents metabolism damage during storage, which is found at an acceptable level.

10.Case report of congenital glucose-galactose malabsorption in China and literature review

Objective To study the clinical features of congenital glucose-galactose malabsorption (CGGM),and to improve the understanding of CGGM.Method Clinical manifestations and treatment process of one patient with CGGM in our hospital were retrospectively analyzed.From 1966 to 2016 May,Chinese medical database and PUBMED were searched using Malabsorption syndrome,dehydration,hypernatremia , diarrhea , newborn , carbohydrate metabolism ,andglucose/galactose malabsorption as key words.The clinical features of CGGM reported in literatures were summarized.Result The patient in our hospital was a full-term female infant naturally delivered.The onset of the disease was on the 9th day after birth,and the clinical manifestations included severe diarrhea,severe dehydration,hypernatremia,metabolic acidosis and malnutrition.After intravenous infusion and symptomatic treatment,dehydration,hypernatremia and metabolic acidosis were corrected.However,there was no improvement of diarrhea characterized with watery and acidic stools,and neither was weight gain.Glucose loading test was negative,and fructose loading test was positive.Diarrhea was improved markedly using diagnostic carbohydrate-free formula,so CGGM was diagnosed clinically.SLC5A1 homozygous IVS7-2 A > G mutation was detected which confirmed the diagnosis of CGGM.With carbohydrate-free formula feeding,the body weight of the infant was increased.Followed up for 2 months now,her body length and body weight were at P25 and P22 on growth curve respectively,and no obvious neurological sequela was observed.Our literature review revealed 7 reports including 48 cases of CGGM children.Literature review showed that:most children with CGGM (79.2%) had the onset within 7 days of life;main clinical features included diarrhea (100%),dehydration (100%),and malnutrition (54.2%);22.9% of patients with carbohydrate-free formula and 27.1% with fructose matrix formula were fed well;no death was detected,77.1% had normal weight gain,and 91.7% had normal development of the nervous system.Conclusion CGGM is rare.The symptoms include severe watery and acidic stools with onset during neonatal period.CGGM is associated with severe complications such as hypertonic dehydration and hypernatremia.The diagnosis is established based upon typical clinical manifestations,sugar loading test and SLC5A1 gene detection.Carbohydrate-free formula feeding is effective.
