Objective To evaluate the imaging features of patients with vascular cognitive impairment (VCI) induced by Subcortical Ischemic Vascular Disease (SIVD), through magnetic resonance diffusion tensor imaging ( DTI ) and proton spectroscopy ( MRS ) technology. Methods A total of 52 patients with SIVD were enrolled. After analysis of scale score, 32 patients with cognitive impairment were assigned to VCI group and 20 patients with no cognitive impairment were assigned to control group. Both group received DTI and MRS examination. The mean values of apparent diffusion coefficient (ADC) and fractional anisotropy (FA) of the bilateral temporal, frontal, parietal and occipital white matter regions as well as in the bilateral centrum semiovale were calculated. The peak value of MRS of N-acetyl aspartate (NAA) , choline ( Cho), creatine (Cr) and phaseomannite ( mI ) were calculated. Results Compared with control group, FA decreased in the region of temporal, frontal, parietal as well as in the centrum semiovale , and ADC increased in VC[group ( P < 0.05 ) . In the frontal regions and centrum semiovale, the VCI patients had a significant FA decrease. The ADC value increased obviously in the temperal lobe.Spectrum analysis results showed, NAA/Cr was lower than control group in VCI group in the frontal lobe (1.43 ±0.08 vs 1.53 ± 0. 92), while mI/Cr was higher than control group in the temporal lobe(0. 51 ±0. 06 vs 0. 46 ± 0. 07 ) ( P < 0. 05 ). Conclusion FA in the temporal and centrum semiovale regions of VCI group and NAA in the temporal white matter regions decreased obviously. DTI and MRS could provide a reference value for early diagnosis and assessment of VCI.

Objective To study the distribution of Montreal cognitive assessment(MoCA)scores in terms of age and educational level in Chinese residents aged 50 years and over and to offer a benchmark for a cutoff score. Methods A total of 281 residents aged 50 years or older was drawn randomly in the urban areas of Beijing, including 215 healthy elderly controls(NC)and 66 patients meeting the clinical criteria of mild cognitive impairment(MCI).The final scores for MoCA were given in the form of mean percentage distributions specific for age,sex and educational level so as to compare the validity of MMSE mini-mental state examination and MoCA in detecting MCI.By a fitting muhiple regression model the influenee of the factors on MMSE and MoCA was assessed. Results Using a cutoff score of 26.MMSE had a sensitivity of 24.2%to detect MCI.whereas MoCA detected 92.4%of the MCI subiects. We found a bivariate correlation between MoCA scores and both the factors of age and educational level(P<0.001).Conclusions MoCA iS a brief cognitive screening tool with high sensitivity and specificity for detecting MCI as currently conceptualized in patients performing normally on MMSE.Our adiustment in the cutoff scores would improve the detection of MCI and Alzheimer'S disease by reducing the number of false negatives.MoCA scores should be used to identify current cognitive difficulties but not to make formal diagnoses.

Objective To assess the right ventricular (RV) performance in patients with type 2 diabetes mellitus (2-DM) by two-dimensional speckle tracking imaging (2D-STI),and to explore the clinical value of RV longitudinal strain and strain rate. Methods Thirty-seven patients with 2-DM only and thirtyone patients coexisting diabetes and hypertension (DM + HTN) were studied.Thirty-nine healthy age matched persons served as control subjects. In each patient a conventional two-dimensional echocardiography was performed and also an echocardiographic study with strain/strain rate imaging was studied.Analysis of RV longitudinal systolic strain were obtained in the apical four-chamber view of the RV for the assessment of the RV free wall(F-PLSS),interventricular septum (S-PLSS) and the global RV wall (G-PLSS).The entire RV longitudinal peak systolic strain rate (G-SRs),peak early-diastolic strain rate (G-SRe) and peak late-diastolic strain rate (G-SRa) were performed in the apical four-chamber view. Results Compared with controls,F-PLSS,S-PLSS,G-PLSS,G-SRs and G-SRe were decreased in 2-DM and DM + HTN (all P <0.01),with lower values in DM + HTN (P <0.05).In the patients coexisting diabetes and hypertension,G-SRa was significantly lower than those in the control group (P < 0.05).Compared with 2-DM,this difference was not significant in controls and DM + HTN (P >0.05).Conclusions No matte the patients of 2-DM with or without hypertension, the early changes of RV dysfunction can be found by 2D-STI.Patients coexisting diabetes and hypertension may have worse RV dysfunction.

Journal of International Oncology 2006;0(06):-
Objective To evaluate the efficacy and toxicity of weekly docetaxel and cisplatin in previously untreated patients with advanced non-small-cell lung carcinoma. Methods Between January 2002 and December 2003 ,34 patients with pathologically comfirmed advanced non-small-cell lung carcinoma who had not received treatment were enrolled. The mean age was under 66 years. The patients received intravenous infusions of docetaxel(25 mg/m2,dayl ,8,15) with dexamethasone premedication and cisplatin(25 mg/m2,dayl ,8,15) ,followed by a week of rest. The remedies which were less than 6 regimens lasted to disease progression or severe toxicity. Therapeutic effect was evaluated by CT scan every two courses . The patients were followed up for 24 months. Descriptive statistics and SPSSIO. 0 software were used to analyse the results. Results 34 patients finished 90 courses. The mean was 2. 6 courses. All patients were followed up. Two patients achieved complete responses, ten patients achieved partial responses, ten patients achieved stable disease. An objective response rate of 35. 29% (95% confidence interval 19. 25%-51. 33% )was obtained. Patients life quality was significantly improved. The median time to progression was 4. 1 months, and median overall survival was 11 months. The 1-year survival rate was 47. 06% , the 2-year survival rate was 11.76% . Toxicities were mild. Grade 3 to 4 neutropenia (11.76%), anemia (5.88%), hyponatremia (5.88%), alopecie (17.64%) and nausea/vomiting (5. 88% ) were observed. Conclusion Weekly Cisplatin plus docetaxel is an effective and well-tolerated regimen in chemo-naive patients with advanced NSCLC. Well-designed clinical trials should be conducted.

Journal of Shanghai Jiaotong University(Medical Science) 2006;0(11):-
Objective To explore the correlation between high-risk human papillomavirus(hr-HPV) viral load and severity of cervical lesion. Methods One thousand eight hundred and six women undergoing both hr-HPV DNA test by hybrid capture Ⅱ(HCⅡ) and colposcopic biopsy for histologic results were enrolled in this study.Correlation between hr-HPV viral load and pathological findings was investigated. Results Of 1 806 cases,641(35.5%) patients were positive for HPV DNA.23.2%(301/1299) of women with normal diagnosis had HPV infection, significantly lower than cervical lesion including cervical intraepithelial neoplasia(CIN) and cervical cancer(P=0.000).Associations(odds ratio) among severity of cervical lesion and viral load were calculated. The significantly increased risk only existed between cervical cancer and CIN1 on high viral load(odds ratio,8.5;95% confidence interval,1.0-71.4;P=0.049).Furthermore,viral load values in CIN1,2,3 and cervical cancer were calculated and median analysis revealed non-significant difference(P=0.712). Conclusion The hr-HPV viral load can distinguish cervical lesion as CIN and cervical cancer from normal one, while the prediction of the severity of cervical lesion may be inadequate and need further investigations.
