1.Serum 1,5-anhydroglucitol in pregnant women with different glucose tolerance status

Objective To explore the diagnosing value of serum 1,5-anhydroglucitol(1,5-AG)for gestational diabetes mellitus(GDM)by determining 1,5-AG in pregnant women with different glucose tolerance status.Methods The levels of serum 1,5-AG,fasting plasma glucose(FPG),1 hour plasma glucose(1hPG)and glycosylated hemoglobin(HbA1c)were determined in 30 cases GDM(GDM group),32 cases gestational impair glucose tolerance(GIGT group)and 43 cases gestational normal glucose tolerance (GNGT group).Results The level of serum 1,5-AG in GDM group[(11.69 ± 3.92)mg/L]was obviously lower than that in GIGT group[(20.93 ± 3.43)mg/L]and GNGT group[(27.83 ± 4.03)mg/L](P < 0.01),GIGT group was obviously lower than that in GNGT group(P <0.05).The level of serum 1,5-AG had significantly negative correlation with FPG,1hPG and HbA1c respectively(P< 0.01).Conclusion The level of serum 1,5-AG has significantly correlation with glucose tolerance,1,5-AG can be used as useful indicator in the diagnosis of GDM.

3.The hemodynamic effects of hypotension induced by PGE_1.

The hemodynamic effects of deliberated hypotension induced by PGE1 or SNP on 14 dogs anesthetized by ketamine were studied separately.MAP was decreased by 30%~40% after infusing PGE,or SNP Hemodynamic variables were measured before and 15 min 30 min after hypotension and 15 min after the discontinuation of infusion.The results indicated that SVRI, PVRI, MPAP, LVSWI and RVSWI all decreased significantly during PGE1-or SNP-induced hypotension. CVP decreased and HR increased significantly during SNP-induced hypotension, but with no significant change during PGE1-induced hypotension and with SVI. CI. PCWP remained unchanged in these two groups throughout the experiments.

5.Role of platelet endothelial cell adhesion molecule-1 in the lung injury caused by mechanical ventilation

ve To investigate the possible role of platelet endothelial cell adhesion molecule-1 (PECAM-1) in the lung injury induced by mechanical ventilation. Methods Twenty-four healthy small pigs of either sex, weighing (17.5?2.5) kg were randomly divided into four groups according to the tida volume of mechanical ventilation: control group without mechanical ventilation; low tidal volume group (A) VT=6 ml?kg-1; normal tidal volume group (B) VT=12ml?kg-1; high tidal volume group (C) VT =25ml?kg-1. The animals were anesthetized with intramuscular ketamine 15-20mg?kg-1 and diazepam 1.5-2.0mg?kg-1 and intubated and mechanically ventilated. Anesthesia was maintained with infusion of ketamine, procaine and succinylcholine. Respiratory rate was set at 18 bpm, I:E=1:2 and FiO2=35% , Femoral artery and vein were cannulated for intra-arterial pressure monitoring, blood sampling and infusion of fluid and anesthetics. ECG, rectal temperature and urine output were monitored. Venous blood samples and lung tissue from right lower lobe were obtained on 1st, 3rd and 7th day of mechanical ventilation for determination of serum and lung myeloperoxidase (MPO) activities, expression of PECAM-1 protein on pulmonary vascular endothelium cells and electron microscopic examination. The Baculoviridae are a large family of enveloped DNA viruses exclusively pathogenic to arthropods. Baculoviruses have been extensively used in insect cell-based recombinant protein expression system and as biological pesticides. They have been deomostrated to be safe to mammals, birds and fish. Recently, baculoviruses has been shown to transduce different mammalian cells in spite of the fact that they cannot replicate in mammalian cells (11, 73, 76). This has resulted in the development of baculoviruses as mammalian expression systems and even as vestors for gene therapy.
7.Early changes of angiopoietin-2 in multiple trauma patients and its clinical significance

Objective To investigate early changes of angiopoietin-2(Ang-2)in multiple trauma patients and assess its clinical significance.Methods Forty-five multiple trauma patients aged 2060 years admitted to the hospital within one hour after injury were randomly divided into three groups according to injury severity score(ISS).Blood specimens were obtained immediately upon arrival in the emergency department and plasma samples were assayed for comparing changes of Ang-2,TNF-a and IL-6.Meanwhile,plasma level of Ang-2 was measured and analyzed under different oxygenation index,shock index and base deficit.Results Plasma level of Ang-2 was positively correlated with ISS(P < 0.01)and was concordant with the plasma levels of TNF-a and IL-6(P<0.01).Furthermore,plasma level of Ang-2 was elevated upon increase of shock index or decrease of oxygenation index(P < 0.01).Plasma level of Ang-2 was elevated with the increase of base deficit(P < 0.01).Conclusions High level of Ang-2 is a marker of endothelial activation and dysfunction early after trauma.Ang-2 is related tightly with the injury severity,inflammation factors,systemic oxygenation and tissue hypoperfusion and may have a tight relation with pathophysiological development and clinical outcome after trauma.
8.Association of serum osteoprotegerin levels with bone mineral density and bone turnover markers in patients with hyperthyroidism before and after methimazole treatment

Serum bone alkaline phosphatase (BAP) , osteocalcin (OC) , C-telopeptides of type-1 collagen (CTx) , osteoprotegerin (OPG) levels, and bone mineral density (BMD) were evaluated in 62 hyperthyroid patients and 60 healthy subjects matched for sex and age. In hyperthyroid patients, the biochemical evaluations and bone density were performed before and after 6 months of methimazole ( MMI) treatment. Results showed that the BMD in lumbar spine L2 - L4, neck of femur, Ward's triangle and greater trochanter of patients before treatment were all significantly lower than those in healthy controls, and improved markedly after MMI treatment. The serum bone turnover parameters BAP, OC and CTx in patients before treatment were all significantly higher than those in control group, and were decreased markedly after treatment. The serum OPG level in patients with hyperthyroidism was significantly higher than that in healthy controls, and decreased markedly after treatment. The serum OPG levels were significantly correlated with bone turnover parameters ( BAP, OC, CTx) and BMD, which indicates that serum OPG level can reflect the abnormality of bone metabolism in patients with hyperthyroidism.
9.The research of acetated Ringer solution and succinylated gelatin in the fluid resuscitation of sepsis patients

Objective To research the clinical significance of acetated Ringer solution and succinylated gelatin in the fluid resuscitation of sepsis patients.Method Fifty-nine sepsis patients were divided into two groups:lactic acid Ringer solution and hetastarch (group A,27 cases)and acetated Ringer solution and suecinylated gelatin (group B,32 cases),compared the fluid resuscitation and other parameter.Results There were no significant difference in the central venous pressure,mean arterial pressure and the dose of noradrenaline (P > 0.05).The activated partial thrombeplastin time in group A was longer than that in group B[(58±10),(74±13)s vs(48±7),(54 ±11) s in 6,24 hours],fibrinogen concentration was lower than that in group B [(3.3 ±0.8),(1.6±0.3)g/L vs (4.2±1.1),(2.1±0.2)g/L in 6,24 hours] (P<0.05).Serum lactic acid,creatinine,calcium concentration in group A [(6.9±0.8)retool/L,(289 ~ 27)μ mol/L,(2.1 ±0.3)mmol/L]were higher than those in group B [(3.2 ±1.1)mmol/L, (193 ±42)μmol/L, (1.7±0.2)mmol/L](P<0.05),there was no difference in APACHE Ⅲ scores on the third day between two groups (P>0.05).Conclusions Acetated Ringer solution and succinylated gelatin in the fluid resuscitation of sepsis patients has the same effect,but fewer impacts on serum coagulation,serum lactic acid and renal function than lactic acid Ringer solution and hetastareh.They may be more suitable for fluid resuscitation of sepsis patients,but hypecalcemia is easy to happen,and there is no significant evidences proving the prognosis.
10.Comparative study of three recruitment maneuvers in acute respiratory distress syndrome caused by extrapulmonary disease

Objective To evaluate the effects and side effects of three different recruitment maneuvers (RM) in acute respiratory distress syndrome (ARDS) caused by extrapulmonary disease.Methods Forty-four patients of extrapulmonary ARDS, according to crossover design methods, were undergone three RM in different periods, including sustained inflation (SI), increase progressively positive end expiratory pressure (IP), pressure control ventilation (PCV). Heart rate (HR), mean arterial blood pressure (MAP), central venous pressure (CVP), oxygenation index, lung compliance were recorded before and after RM, and were analyzed for statistical analysis. Results Oxygenation index and lung compliance were increased obviously in a short time after RM, improvement of IP method were better more obviously than the other two methods [1 h oxygenation index after RM:227 ± 42 vs 190 ± 19,186 ± 21; lung compliance:(59.4±12.5 ) ml/cm H2O(1 cm H2O = 0.098 kPa) vs (50.1 ± 9.3 ), (49.7 ± 10.6) ml/cm H2O;P< 0.05], 2h after RM,there were no statistical difference among the three methods (P>0.05). After RM,HR and CVPwere increased, MAP was decreased in a short time, changes of SI method were smaller than the other two inethods [10 min HR after RM: (94.0±10.3 ) beats/min vs (116.0 ± 14.8 ), ( 107.0 ± 5.7 ) beats/min; CVP:(13.7±3.1 )cm H2O vs ( 18.4 ± 6.7 ), ( 15.4 ± 2.7 )cm H2O; MAP: ( 87.0 ± 12.1 ) mm Hg( 1 mm Hg = 0.133 kPa) vs (73.0 ± 4.8), (81.0 ± 6.6) mm Hg;P< 0.05), 20 min after RM, there were no statistical difference among the three methods (P> 0.05). Conclusion When extrapulmonary ARDS undergo RM ,IP method is the most effective on increasing oxygenation index and lung compliance, SI method has the smallest side effect on hemodynamics.
