Juvenile idiopathic arthritis(JIA)is the most common rheumatology disease in childhood period with poor prognosis.The biological agents are newly developed drugs with features of clear therapeutic targets and fast effects.But its use in JIA is still limited,so this article focuses on the clinical use experience,timming and sideffects of the biological agents on JIA.
2.Effect of Ligusticum wallichii-containing serum on expressions of Toll-like receptor 4 and myeloid differentiation factor 88 in hepatic stellate cells.

To observe the effect of Ligusticum wallichii-containing serum on the expressions of Toll-like receptor 4 and myeloid differentiation factor 88 in hepatic stellate cells. Clean-grade SD rats were randomly divided into 5 groups and orally given L. wallichii decoction, colchicine and normal saline for 7 d to prepare L. wallichii-containing serums. Except for the blank group, all of the remaining groups were stimulated with LPS 1 mg x L(-1) for 24 h. After being intervened, the L. wallichii-containing serums were cultured in 5% CO2 incubator at 37 degrees C for 24 hours. The expression of TLR4 and MyD88 were detected by RT-PCR and Western blot. After HSC was stimulated with LPS, TLR4 and MyD88 mRNA and protein expressions were significantly higher than the blank control group (P < 0.01). After being intervened with L. wallichii-containing serum, TLR4 and MyD88 mRNA and protein expressions were notably lower than the model group (P < 0.05 or P < 0.01). In conclusion, L. wallichii-containing serum could regulate the TLR4 signaling pathway and show the anti-fibrosis effect by inhibiting the expression of TLR4 and MyD88 in LPS-induced HSCs.

3.Effect of Danshen-containing serum on expression of SuFu and DYRK2 in HSCs.

To observe the effects of Danshen-containing serum on SuFu and DYRK2 expression in the HSCs stimulated by leptin. SD rats (n = 60) were used to make danshen-containing serum by gastric perfusion for ten days with Danshen water decoction, normal saline and colchicine. The HSCs that were cultured in vitro would be stimulated for 24 hours by leptin (100 μg x L(-1)) except blank control group, after being intervened, the drug serum in each group would be cultured at 37 degrees C in 5% incubator. The cells would be collected after 24 hours, then the effects of danshen-containing serum on the proliferation of HSCs were detected by MTT, the expression of SuFu mRNA and DYRK2 mRNA were detected by RT-PCR, the expression of SuFu and DYRK2 proteins were tested by Western blot. Compared with blank control group, the expression of DYRK2 mRNA and DYRK2 proteins were enhanced obviously after stimulated the HSCs of rats by leptin (P < 0.01), but the expression of SuFu mRNA and SuFu proteins were decreased significantly (P < 0.01). Compared with the model group, after cyclopamine group (Hh pathway inhibitor), Danshen-containing serum and colchicine were interfered, the expression of DYRK2 mRNA and DYRK2 proteins were decreased clearly (P < 0.01), but the expression of SuFu mRNA and SuFu proteins were increased significantly (P < 0.01 or P < 0.05). Compared with model group, adding purmorphamine (Hh pathway agonist) to model group and making it activate could increase the expression of DYRK2 mRNA and DYRK2 proteins, but the expression of SuFu mRNA and SuFu proteins were decreased significantly (P < 0.01). Compared with the model group, using the Danshen-containing serum to interfere the purmorphamine group could make the expression of DYRK2 mRNA and DYRK2 proteins decrease and the expression of SuFu mRNA and SuFu proteins increase significantly (P < 0.01). Danshen-containing serum would inhibition the activation and increment of HSCs by interfering the expression of SuFu and DYRK2 which were induced by leptin.

4.Study on effect of fertilizers on spring shoots growth of Akebia trifoliate.

OBJECTTo study the effects of different kinds and amounts of fertilizers on spring shoots growth yield of Akebia trifoliate.
METHODBy applying orthogonal design method, the effects of fertilizers on spring shoots growth yield were analyzed.
RESULTThe optimization project of applying fertilizers was K3N3J2P2, the amount of potassium fertilizer was 0.075 kg per m2, nitrogenous fertilizers was 0.090 kg per m2, organic fertilizer was 0.300 kg per m2, phosphate fertilizer was 0.090 kg per m2. The effect sequence of different fertilizer on spring shoots growth was K > N > J > P.
CONCLUSIONSpring shoots growth yield of A. trifoliate can increase significantly by reasonable applying fertilizer.

OBJECTIVETo observe the serological features of hepatitis G virus (HGV or GBV) in selected population of Shaanxi, Qinghai and Xinjiang Provinces (region).
METHODSEnzyme-linked immunosorbent assay (ELISA) was used to determine IgG antibody against GBV (anti-GBV IgG) in serum specimens of 1469 individuals from the 3 provinces (region).
RESULTSThe positivity rate of serum GBV-IgG antibody in ethnic minorities (4.11% in Tibetan, 5.36% in Mongolian, 4.55% in Uigur, 4.00% in Hui population) was slightly higher than that in Han population (1.36%-1.73%), but the differences were not significant (P>0.05). The positivity rate of serum GBV-IgG antibody in drug abusers (11.30%, 34/301) was remarkably higher than that of the normal population (2.44%,18/736) (P<0.01). The positivity rate of serum GBV-IgG antibody in blood donors was 1.02%-7.68%.
CONCLUSIONSThe positivity rates of serum GBV-IgG antibody among ethnic groups in the 3 provinces (region) had no significant differences; blood-borne transmission seemed to be an important transmission route of GBV, therefore supervision of blood donors and drug abusers should be intensified.

7.Short-term clinical outcomes in extremely/very low birth weight infants with individual nutritional support

Objective: To explore the application value of bedside nasojejunal feeding tube placement using four steps of ultrasonic-assisted method in critical ill patients. Method: Thirty-three critical ill patients receiving enteral nutrition were enrolled. We adopt four steps of ultrasonic guidance to conduct nasojejunal feeding tube placement. The details were as followed: nasojejunal feeding tube placement within the esophagus, within Gastric body, within After the pylorus, at last the location of nasojejunal feeding tube. Results: The placement of nasojejunal feeding tube was successful in 28 patients, failed in 2 patients. The failure reason include that the false appearance of pylorus through and the lost gastrointestinal motility led to the sticking of nasojejunal feeding tube on arcus major ventriculi and it couldn't enter into gastric antrum and pylorus. Conclusion: The application of nasojejunal feeding tube placement by four steps of ultrasonic-assisted is a novel feeding technique, and it's simple, effective, noninvasive and repeatability procedure with a high success rate and low complications.

9.Use of indocyanine green staining technique for phacoemulsification in white cataract

AIM:To observe the application efficiency of 5g/L indocyanine green ( ICG ) staining technique for continuous circular capsulorhexis ( CCC ) during phacoemulsification in white cataract.
METHODS:Ninety-eight patients (98 eyes) with white cataract were randomly divided into staining group ( 50 cases, 50 eyes) and control group (48 cases, 48 eyes). The control group didn't do anterior capsule staining. The staining group was injected to fill the anterior chamber, 5g/L ICG 0.1mL was applied on the central surface of the anterior capsule, using a 27G blunt needle through the side-port after 30s, and the redundant ICG was replaced by BSS, and continuous curvilinear capsulorhexis was accomplished using capsulorhexis forceps.
RESULTS: In staining group: after ICG staining, the capsule, which presented uniform light green and visualization of the anterior capsule was significantly improved. There are 48 eyes capsulorhexis success. The rate of success was 96%. Meanwhile, in control group, there was 29 eyes capsulorhexis success. The rate of success was 60%. The difference was statistically significant (P<0. 05). During follow-up, no dye particle and the complications of iritis or high intraocular pressure were found in the eyes of staining group. Anterior chamber inflammation, corneal opacity compared with control group, the difference was not statistically significant.
CONCLUSION:Indocyanine green staining increases the visibility of anterior capsule in over mature cataract, and it should be an effective and helpful method which can increase the success rate of capsulorehxis. At the same time, it can reduce the incidence of intraoperative complications. This will help beginners quickly grasp continuous curvilinear capsulorhexis, and shorten the learning curve.
10.Comparison of posterior capsular opacification models by phacoemulsification with or without intraocular lens implantation in rabbit

Background Prevention and treatment of posterior capsular opacification (PCO) is a hot issue.To establish a PCO animal model is the basis of relevant studies.The most common methods of creating a PCO model are phacoemulsification surgery with or without intraocular lens (IOL) implantation.But the suitability of different methods is unclear.Objective This experiment was to compare the outcome between the two methods of establishing a PCO model in rabbit eyes.Methods Twenty New Zealand white rabbits were collected and randomized into 2 groups.Phacoemulsification of cataract with IOL or without IOL implantation was performed on the right eyes of rabbits in these two groups.The operative eyes were examined under the slit lamp from day 1 through 3 months after surgery.The inflammatory response was evaluated and compared between the two groups,and the extent of PCO was graded based on Odrich's criteria.The use of the animals complied with the Regulations for the Administration of Affairs Concerning Experimental Animals by State Science and Technology Commission.Results The inflammatory response,including conjunctival congestion,corneal edema and aqueous flare were less severe in the model eyes with IOL implantation than the eyes without IOL implantation 1-3 days after operation.Inflammatory response gradually disappeared and showed the same degree in the PCO grade from 2 weeks through 3 months in both groups.The numbers of eyes with 1-3 grade of PCO were 8 and 9,and those with 0 grade of PCO were 2 and 1 in the with IOL implantation group and without IOL implantation group,respectively,showing a significant difference (P=0.39).PCO appeared at 1 month,extended at 2 months and formed dense fibrosis following operation.Conclusions The model outcome of phacoemulsification combined with IOL implantation is better than without IOL implantation one.It is the ideal animal model for the study of after cataract.
