1.Changes in perioperative level of hepatitic B virus DNA in patients with hepatocellular carcinoma and their influences

Objective To investigate the influences in perioperative variations in serum levels of hepatitis B virus (HBV) DNA in patients with HBV-related hepatocellular carcinoma (HCC),and to observe the differences in postoperative recovery of liver function between the antiviral treatment group and the non-antiviral treatment group of patients.Methods From Feb.2012 to Nov.2012,55 patients whose preoperative serum levels of HBV DNA were below the recommended level of antiviral treatment were included into the study.The serum levels of HBV DNA,IL-6,IL-10 and IL-27 preoperatively and on postoperative day 3 were measured.Using the postoperative serum levels of HBV DNA,the patients were divided into 2 groups:the high level group and low level group.Patients in the high level group received antiviral treatment,but the low level group did not.The changes in preoperative and postoperative liver function and other data of the patients were recorded and analyzed by the SPSS 13.0 software.Results (1) The reactivation rate of serum HBV DNA in the HBsAg-positive HCC patients was 45% (25/55).In patients with a preoperative HBV DNA level < 1 × 104 IU/ml the postoperative HBV reactivation rate was up to 76% (19/25).(2) Logistic regression analysis showed tumor diameter (P=0.037,0.006) and injecting anhydrous alcohol into the resection margin (P=0.004) were independent risk factors of postoperative HBV reactivation.(3) Postoperative serum IL-10 elevation was associated with HBV reactivation (P=0.001).On the contrary,serum IL-6 level elevation was associated with HBV reactivation (P<0.01).(4) When compared with the low level group,postoperative serum alanine aminotransferase,total bilirubin and albumin in the high level group showed no significant difference (P>0.05).Conclusions Hepatectomy could reactivate HBV replication during the perioperative period.A close monitoring of HBV DNA during the perioperative period was necessary,especially in patients with low HBV DNA levels.The tumor diameter and injecting anhydrous alcohol into the resection margin were independent risk factors of postoperative increase in HBV DNA.Changes in serum IL-10 and IL-6 levels after hepatic resection might be related to the elevation of HBV DNA level.In addition,reactivation of HBV did not aggravate damages in postoperative liver function.Antiviral treatment did not promote recovery of liver function in the early stage.
2.Clinical Observation of Alprostadil Combined with Salvia Ligustrazine in the Treatment of Aged Patients with Unstable Angina Pectoris

OBJECTIVE:To observe clinical efficacy and safety of alprostadil combined with salvia ligustrazine in the treat-ment of aged patrents with unstable angina pectoris. METHODS:A total of 150 patients with unstable angina pectoris department of our hospital during Oct. 2011-Mar. 2015 were randomly divided into alprostadil group,salvia ligustrazine group and combination group according to random number table,with 50 cases in each group. Three groups received routine treatment. Alprostadil group additionally received Alprostadil injection 100 μg added into 0.9% Sodium chloride injection 250 mL,ivgtt,qd,on the basis of routine treatment. Salvia ligustrazine group additionally received Salvia ligustrazine injection 10 mL added into 0.9% Sodium chlo-ride injection 250 mL,ivgtt,qd,on the basis of routine treatment. Combination group additionally received constant dose of Al-prostadil injection and Salvia ligustrazine injection. Hemorheological indexes (high shear whole blood viscosity,low shear whole blood viscosity,plasma viscosity,hematocrit,fibrinogen),cardiac function indexes(LVEF,SV,LVEFD,LVST),serum CRP, NO,ET,SOD and clinical efficacies were observed in 2 groups before and after treatment;the occurrence of ADR was compared between 2 groups. RESULTS:Before treatment,there was no statistical significance in hemorheological indexes,cardiac function indexes or serum CRP,NO,ET,SOD level between 2 groups (P>0.05). After treatment,plasma viscosity,the whole blood high and low shear viscosity,hematocrit,fibrinogen,serum CRP and ET levels of 3 groups were decreased significantly,while LVEF,SV,serum levels of NO and SOD were increased significantly,combination group was significantly better than alprostadil group and salvia ligustrazine group,with statistical significance (P<0.05). There was no statistical significance in above indexes between alprostadil group and salvia ligustrazine group (P<0.05). Total response rate of combination group was 90.00%,which was significantly higher than 74.00%of alprostadil group and 72.00%of salvia ligustrazine group,with statistical significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Alprostadil combined with salvia ligustrazine can effectively reduce the blood viscosity of patients with unstable angina pectoris,improve cardi-ac function and endothelial function,reduce myocardial ischemia injury and show significant therapeutic efficacy and safety without increasing the incidence of ADR.
3.Molecular mechanisms of TRP channels in mechano-sensory transduction.

Channels from the TRP superfamily have essential roles in a wide variety of sensory transductions, especially in mechano-sensation, such as hearing, touch and mechanical pain. TRP channels are also implicated in major channelopathies, including deafness, chronic pain, autosomal dominant polycystic kidney disease (ADPKD) and ventricular hypertrophy. As the leading candidates for mechano-sensitive channels, some TRP channels appear to be mechano-receptor, which can be activated by mechanical forces directly, such as C. elegans TRPN homolog TRP-4; whereas others may act as signal modulators, receiving and amplifying signals indirectly. This review is to introduce the function of TRPs in mechano-sensory transduction and to discuss the underlying molecular mechanisms.

4.Study on limit detection of flavones in diterpene ginkgolides meglumine injection materials by LC-MS and HPLC-DAD.

Limit test of flavones in diterpene ginkgolides meglumine injection materials by UV-Vis and HPLC-DAD method was studied in this essay. The HPLC-DAD method has lower LOD (about 1% of the UV-Vis), that is, the sensitivity is higher than UV-Vis method. Through the analysis of the kinds of flavonoids ingredients in the samples by LC-MS, the three compounds with highest contents are kaempferol, quercetin and isorhamnetin. Kaempferol, quercetin and isorhamnetin were chosen as reference compounds for HPLC analysis, and the HPLC separation analysis was carried on an Agilent Eclipse plus C18 column (4.6 mm x 250 mm, 5 μm) with methanol and water containing 0.4% phosphoric acid (50: 50) as mobile phase, and the flow rate was 1.0 mL x min(-1). The detection wavelength was set at 360 nm. This method has good specificity, precision and reproducibility. The LODs of quercetin, kaempferide and isorhamnetin were 27.6, 22.3, 29.5 μg x L(-1). The average recovery was 87.9% (RSD 3.3%), 91.7% (RSD 3.1%), 88.3 (RSD 1.3%) for quercetin, kaempferide and isorhamnetin, respectively. Based on the 10 batches of sample results and sensitivity of different HPLC, the content of total flavonoids ingredients of diterpene ginkgolides meglumine injection materials was limited no more than 2 x 10(-5). This method is simple, quick and has good maneuverability, and could be used to the limit test of flavonoids in the diterpene ginkgolides meglumine injection materials.

Objective To investigate the effects of tetramine to cardiac/skeletal muscle in the rats and elucidate the relationship of the effects and the elevated sero-enzyme.Method The 30 homogeneous SD rats were invided into three groups:the control group,the half-lethal dose group and the lethal dose group.The number of female and male rats was equal.The tetramine powder is dissolved into 0.9% NS and puured into the stomach of the objects in medication groups;0.9%NS was poured into the rats of control group.Once the rat died or one hours later,the related sero-enzyme was determined and the rat was executed,Immediately cardiac muscle and skeletal musclewas respectively drawn the materials from the rat and was pathologically examined.Results After the rats are intragastric administrated they spasm in 10-60 min;the 6 rats of the medial lethal dose group die in 20-60 min.Serum TnI/CK/AST/LDH/a-HBDH in medication groups is higher than in control group (P
6.MRI study of acute radiation-induced hepatic injury with super paramagnetic iron oxide enhancement in experimental model

Objective To investigate the MR imaging technique and features of acute radiation- induced hepatic injury with superparamagnetic iron oxide(SPIO)enhancement in a rabbit model.Methods On the 10th day after half-liver single 40 Gy X-ray irradiation,MR imaging before and after SPIO administration and pathologic study of 12 adult rabbits were performed.Results On the 10th post-irradiation day,TSE T_2 WI before SPIO enhancement,the irradiated liver of two rabbits showed relatively higher signal, and 1 showed slightly lower signal.With SPIO enhancement,the irradiated livers of 9 rabbits were found to be abnormal,manifesting as higher or slightly higher areas on multiple MR imaging sequences,especially the TFE T_1 WI sequence.Histological specimen with HE stain under light microscopy revealed occlusive injury of central veins(veno-occlusive disease,VOD)in each irradiated liver to some extent.Electron microscopy investigation of the irradiated liver disclosed intracellular edema,fibrin deposition,and widening of the Disse-space.Conclusion The early pathologic feature of the irradiated liver is occlusive injury of the central vein.MR imaging with SPIO enhancement is effective to valuate the early irradiation-induced liver injury.
7.The influence of infrasound on pathological and ultrastructural changes in the renal tissue of rats

Objective To investigate renal tissue changes in rats exposed to 16 Hz/130 dB infrasound.Methods Twenty male Sprague-Dawley adult rats were randomly divided into a control group,a pseudo-infrasound group,and two infrasound groups-A and B.Groups A and B were exposed to 16 Hz/130 dB infrasound for 2 h or4 h daily respectively over 7 days.Pathological and uhrastruetural changes in their renal tissues were observed with a light microscope and an electron microscope.Results Under the light microscope,Bowman's capsules expanded,epithelial cells were shed,and a little secretion was found in the renal tubules of infrasound group A.Slight degeneration and necrosis of the renal tubules and glomerular exudation could be observed in group B.Under the electron microscope,a large number of lysosomes displayed hyperplasia,there was interstitial edema,and leukocyte pavementing wag found in group B.Other changes such as swelling of podocytes' processes,fusion of foot processes,and vacuolization in the mitochondria could be observed in both infrasound groups.Conclusion Lengthy exposure to 16 Hz/130 dB infrasound can cause severe pathological and uhrastruetural changes in renal tissue,at least in rats.
8.Role of imaging diagnosis in surgical treatment of primary aldosteronism

Objective To evaluate the role of imaging diagnosis in surgical treatment of primary aldosteronism(PA). MethodsFrom Jan 1995 to Dec 2004,245 patients with PA were hospitalized in our hospital.Before the operations,all the patients underwent B-ultrasonography and CT scaning,240 received intravenous pyelography and 75 MRI.The preoperative imaging diagnosis were compared with the findings during the operations and postoperative pathologic results. Results Compared with the findings during the operations,the accuracy rates of localized diagnosis for PA with B-ultrasonography,CT scanning and MRI were 92.7%,98.2% and 90.4%,respectively.Compared with the postoperative pathologic results,the accuracy rates of qualitative diagnosis for aldosterone-producing adenoma(APA) with B-ultrasonography,CT scanning and MRI were 83.0%,90.7% and 72.2%,respectively. Conclusion The comprehensive imaging data are helpful in the localized diagnosis of PA.Correct preoperative qualitative diagnosis of APA is the key step for the surgical treatment for PA.
9.Identification and transcriptional activity analysis of core regulatory region of human guanylate binding protein 5 gene promoter

Objective To construct luciferase reporter plasmids of truncated fragments of different lengths of human guanylate binding protein 5(GBP5)gene promoter and analyze the transcriptional activity of each fragment to determine the core regulatory region.Methods GBP5promoter sequence was amplified by PCR,truncated into five fragments of different lengths and connected to pGL3-basic plasmid.The constructed recombinant plasmids pGL3-GBP5-11/21/31/41/51were transfected into 293FT cells and detected for luciferase activity.The binding sites of transcription factors in GBP5promoter region were predicted by JASPAR software,and Yin-Yang transcription factor 1(YY1)targeting the core regulatory region was selected and verified for the transcriptional regulatory activity.The CDS sequence of YY1 was amplified by PCR to construct the overexpression plasmid pIRES2-EGFP-YY1,which was then co-transfected to 293FT cells with plasmids pGL3-GBP5-21(-1 623 ~ +47 bp)and internal reference plasmid pRL-CMV,and detected for luciferase activity to analyze the regulation of transcription factor YY1 on GBP5 promoter activity.Results Colony PCR and double enzyme digestion identification proved that the plasmid of human GBP5 promoter reporter gene was correctly constructed;JASPAR software predicted that there were multiple transcription factor binding sites such as STAT1,YY1 and Foxp3 in GBP5promoter region.Double luciferase activity assay showed that pGL3-GBP5-21(-1 623 ~ +47 bp)showed the highest promoter activity,while the promoter activity of pGL3-GBP5-41(-520 ~ +47 bp)decreased significantly,suggesting that the core region of GBP5 promoter was located at upstream-1 623 ~-520 bp of 5 'UTR;Overexpression of YY1 significantly activated the GBP5 promoter activity and regulated the expression of GBP5.Conclusion The core regulatory region of human GBP5 promoter was located in upstream-1 623 ~-520 bp of the 5 'UTR,with a binding site of transcription factor YY1 existing in this region.Meanwhile,overexpression of YY1 significantly effected the activity of GBP5 promoter.
10.Percutaneous hollow screws for treatment of the vertical sacrum longitudinal fracture.

OBJECTIVETo introduce percutaneous hollow screws for treatment of the vertical sacrum longitudinal fracture and evaluate the preliminary therapeutic outcomes.
METHODSFrom January 1999 to December 2006, 24 cases with vertical sacrum longitudinal fractures inchuded 15 males and 9 females were treated by percutaneous hollow screws, with an average age of 35 years ranging from 18 to 61 years. Accordng to Denis'classification of sacral fracture, there were 6 cases of type I, 11 of type 1 and 7 of type II.
RESULTSThe operation lasted for 30 to 65 minutes (averaged 48 minutes). All of them were followed up for 3 to 36 months (averaged 18.6 months). According to improved effective standard of pelvic trauma, the result of radiography was excellent in 18 cases, good in 5 and poor in 1, and the clinical evaluation was exellent in 16, good in 8.
CONCLUSIONTreatment of the vertical sacrum longitudinal fracture with percutaneous hollow screws is a comparatively reliable method and has the advantages of more precise with few postoperative complications and allows the patient early mobilization.
