1.Chemical constituents from stems of Hedyotis hedyotidea and their immunosuppressive activity.

Hedyotis hedyotidea has been traditionally used for the treatment of arthritis, cold, cough, gastro-enteritis, headstroke, etc. But few studies have screened the active compounds from extracts of H. hedyotidea. In this study, the structure of the chemical constituents from stems of H. hedyotidea were determined and the immunosuppressive activity of the compounds was evaluated. The compounds were separated and purified with silica gel, gel column chromatographies and preparative HPLC, and their structures were identified by spectral methods such as MS and NMR. Eleven compounds were obtained and identified as(6S,9S) -vomifoliol (1), betulonic acid (2), betulinic acid (3), betulin(4), 3-epi-betulinic acid (5), ursolic acid (6), β-sitosterol (7), stigmast-4-en-3-one (8), 7β-hydroxysitosterol (9), (3β,7β) -7-methoxystigmast-5-en-3-ol (10) and morindacin (11). This is the first report of compounds 1, 2, 4, 8, 9, 10 and 11 from H. hedyotidea. Compounds 1, 2 and 8-11 were firstly isolated from the genus Hedyotis, and compounds 9 and 10 were isolated from the family Rubiaceae for the first time. The immunosuppressive activity of these compounds was tested using the lymphocyte transsormationtest. Compounds 4, 6 and 9 showed significant immunosuppressive activity.

2.Recent advance in genetic study of hereditary autosomal recessive cerebellar ataxia.

Autosomal recessive cerebellar ataxias (ARCA) are a highly heterogeneous group of rare neurodegenerative diseases affecting both central and peripheral nervous systems. Based on pathological mechanisms, five major types of ARCA may be distinguished, which include mitochondrial ataxia, metabolic disorder, DNA repair defect ataxia, congenital ataxias and degenerative ataxia. This review summarizes clinical features, molecular genetics and recent advances in DNA sequencing of common types of ARCA.

Objective To determine the value of acute phase protein in the early diagnosis of recurrence after curative gastric cancer surgery. Methods Acute phase serum protein level was measured before and after the gastric cancer surgery in 120 patients,and was compared with that of the control group. At 1, 3, 6, 9, 12 months after the operation in 87 patients undergoing curative gastric cancer surgery, the levels of acute phase proteins were measured. They were followed up for at least 12 months or until death. Results The levels of serum C reactive protein(CRP), ?1 antitrypsin (?1 AT) and ? acid glycoprotein (? AG) in gastric cancer group were significantly higher than those in the control group (P0.05). In patients who underwent curative gastric cancer surgery in a certain period after the operation, the serum levels of CRP, ?1 AT and ? AG were significantly lower than those before the surgery(P0.05).There were significant difference in CRP,?1 AT and ? 4.Effects of Shuangdanmingmu capsule on retinal vascular morphology and VEGF expression in rats with diabetic retinopathy

Abstract?AlM: To observe the effects of Shuangdanmingmu capsule on VEGF expression and retinal vascular morphology in rats with diabetic retinopathy ( DR) .?METHODS: DR rats were fed with Shuangdanmingmu capsule. By comparing with the normal group, the model control group, and positive control group, the effect of Shuangdanmingmu capsule on retinal tissue of DR rats was observed under electron microscopy. After HE staining, retinal structure was observed under the light microscope. lmmunohitochemical staining was used to detect the VEGF expression in retina.?RESULTS:Two months after treatment, the layers tissue of retina presented mild edema, capillary pericytes performed edema, mitochondria showed mild swelling and less clear structure, some endothelial cells showed slight proliferation in Shuangdanmingmu group. Compared with the normal group, the expression level of VEGF in retina increased in the other groups, especially in model control group. A significant differential in expression of VEGF was found between Shuangdanmingmu group, positive control group and model control group (P<0. 01).? CONCLUSlON: Shuangdanmingmu capsule can effectively improve the retinal microvascular, reduce edema and necrosis of each layer of retina, improve the ultrastructure of retina's tissue and inhibit VEGF expression in DR rats.
5.Influence of Wnt/beta-catenin signal transduction pathway on the differentiation of umbilical cord mesenchymal stem cells into hepatocyte-like cells

BACKGROUND:Umbilical cord mesenchymal stem cells can be induced to differentiate into hepatocyte-like cells in vitro and in vivo.However,the exact mechanism is still unknown. Existing studies have shown that the Wnt/β-catenin signaling pathway is closely related to this process. OBJECTIVE: To explore the effect of Wnt/β-catenin signaling pathway on the differentiation of umbilical cord mesenchymal stem cells into hepatocyte-like cells and its potential molecular mechanism. METHODS: Human umbilical cord mesenchymal stem cells were extracted from the neonatal umbilical cord by tissue adherent method. After being cultured and purified, the umbilical cord mesenchymal stem cells at passages 4-6 were divided into four groups: control group (DMEM culture group), hepatocyte-like differentiation group, activator Wnt3a group (adding 20 μg/L Wnt3a, an activator of Wnt/β-catenin signaling pathway, under the differentiation condition), and inhibitor Dkk-1 group (adding 20 μg/L Dkk-1, an inhibitor of Wnt/β-catenin signaling pathway, under the differentiation condition). Induced cells were collected respectively on days 7, 14, 21, 28. Their mRNA and protein expressions of α-fetoprotein (AFP), albumin (ALB), hepatocyte nuclear factor 4α (HNF4α) and Cytokeratin-19 (CK-19) in the cells were detected by real-time quantitative PCR and western blot respectively. Meanwhile, Periodic Acid-Schiff staining, low-density lipoprotein uptake test and indocyanine green absorption test were applied to detect the function of hepatocyte-like cells. RESULTS AND CONCLUSION: Compared with the control group, expressions of AFP and HNF4α mRNA and protein as well as ALB mRNA were significantly up-regulated in the hepatocyte-like differentiation group, activator Wnt3a group and inhibitor Dkk-1 group (P < 0.05). Whereas, there was a decrease in the CK-19 expression at mRNA and protein levels (P < 0.01) in these three groups. Compared with the hepatocyte-like differentiation group, the mRNA and protein expressions of AFP and HNF4α, and the mRNA expression of ALB were significantly down-regulated in the activator Wnt3a group (P < 0.05). Compared with hepatocyte-like differentiation group and activator Wnt3a group, the inhibitor Dkk-1 group had higher expression of AFP, HNF4α mRNA and their proteins as well as the mRNA expression of ALB (P <0.05). Findings from the Periodic Acid-Schiff staining, low-density lipoprotein uptake test and indocyanine green absorption test showed more positive cells in the inhibitor Dkk-1 group than in the hepatocyte-like differentiation group and least positive cells in the activator Wnt3a group. Overall, these findings suggest that the inhibition of Wnt/β-catenin signaling pathway promotes the differentiation of umbilical cord mesenchymal stem cells into hepatocyte-like cells;conversely,the cell differentiation can be inhibited via the Wnt/β-catenin pathway.
6.Application of bedside Ipad health education combined with health diary in inpatients with typeⅡdiabetes

Objective? To explore the application effect of bedside Ipad health education combined with health diary in the inpatients with typeⅡ diabetes. Methods? By convenience sampling, 56 patients admitted in Ward I, Department of Endocrinology, Chengdu First People's Hospital from April to September, 2018 were enrolled into the control group, and 60 patients in Ward II into the observation group. The control group received the conventional diagnostic and therapeutic nursing care and bedside Ipad health education, on the basis of which, the observation group was instructed by nurses to keep health diaries. On the first day and 14th day upon admission, Diabetes Mellitus Self-Management Efficacy Scale(DMSES), the Summary of Diabetes Self-care Activities(SDSCA) were used to measure and compare the patients' self-management efficacy and behaviors. Results? On the first day of hospitalization, there was no statistical difference between the two groups in the scores of DMSES and SDSCA(P>0.05). On the 14th day, the scores of the 3 dimensions:"diet control", "regular exercise" and "blood glucose monitor" of the observation group and the total score of DMSES were both higher than the control group with statistically significances (P<0.05); the scores of 4 dimensions:"appropriate diet", "regular exercise, "blood glucose monitor" and "foot care" as well as the total score of SDSCA in the observation group were all higher than the control group with statistical significances (P<0.05). Conclusions? The method of bedside iPad health education combined with health diary can help hospitalized patients with typeⅡ diabetes develop reasonable diet, regular exercise, blood sugar monitoring, foot care and other life and behavior patterns consistent with health management goals, and enhance patients' self-confidence and ability to participate in their own health management and improve the efficacy of self-health management.
7.Design and clinical application of a self-designed acromion & clavicle anatomic plate with combinatorial hinge

Objective To design and evaluate a new type of acromion & clavicle anatomic plate with combinatorial hinge more suitable for acromioclavicular joint dislocation and distal clavicular fracture.Methods The new type of acromion & clavicle anatomic plate with combinatorial hinge we had designed was used in 21 patients (21 shoulders) who were treated for acromioclavicular joint dislocation or distal clavicular fracture from October 2016 to April 2017 at Department Ⅰ of Orthopaedics,The 2nd People's Hospital of Lu'an.They were 15 men and 6 women,aged from 24 to 63 years(average,41.5 ±4.2 years).Of the 14 cases of acromioclavicular joint dislocation,7 were type Ⅲ,5 type Ⅳ and 2 type Ⅴ,according to the Rockwood classification;all the 7 distal clavicular fractures were Neer type Ⅱ.They were all treated by open reduction and internal fixation with our self-designed acromion & clavicle plate with combinatorial hinge.They were followed up at the outpatients department at 3 weeks,3,6,12 and 15 months postoperatively for evaluation of their shoulder functions by Constant-Murley score,changes in coracoclavicular clearance before and after operation and loss of coracoclavicular clearance after removal of internal fixation by X-ray.Results All the 21 patients were effectively followed up for 15 to 20 months (average,18 months).As for their Constant-Murley scores,preoperation (49.7±4.9) <3 weekspostoperation (57.6±5.9) <3 monthspostoperation (83.2±5.7) <6 months postoperation (90.4 ±4.0) < 12 months postoperation (94.3 ±4.2) < 15 months postoperation (98.1 ±4.2),with significant differences between different time points (P < 0.05).The postoperative coracoclavicular clearances at different time points were all significantly decreased than the preoperative value (P < 0.05).With the aid of our new type of acromion & clavicle anatomic plate,the shoulder functions of the patients were gradually improved,the postoperative acromioclavicular joint and reduction of the distal clavicular fracture were stable,and no re-dislocation occurred after removal of internal fixation.Conclusion Our new handy type of acromion & clavicle anatomic plate with combinatorial hinge can provide more effective internal fixation for acromioclavicular joint dislocation and clavicular distal fracture,facilitating functional recovery of the shoulder.

Objective To identification and analysis Aichi virus from diarrhea and normal children in Lanzhou, and discuss the relationship between Aichi virus and Infant Diarrhea. Methods According to the literature published data, Using RT-PCR method to amplified Aichi virus 3CD fragment and the positive products were sequenced and determined, and made the alignment analysis between the nucleotide sequences of the amplified fragment with the known sequence of this virus. Results There was 1 case detection of Aichi virus in the 46 hospitalized children with diarrhea and 299 children with diarrhea out-patients specifically, Overall detection rate was 0.06% , and there was no Aichi virus was detected in normal control children. 2 viral 3CD gene and the known reference strains of nucleotide sequences were 97% , while phylogenetic analysis showed that genotype of 2 viral belongs to the B. Conclusions There existed B Genotype of Aichi virus in China, and more research is needed to clarified the etiology and epidemiology of Aichi virus characteristics.
9.Epidemiologic analysis on severe fever with thrombocytopenia syndrome in Hubei province, 2010

Objective To identify the epidemic characteristics and risk factors of an emerging infectious disease-severe fever with thrombocytopenia syndrome (SFTS) in Hubei province.Methods Active surveillance program on SFTS was set up in monitoring sites-hospitals,at the township level or above,in Suizhou,Huanggang and Wuhan from January to December,2010.Specific surveillance program on SFTS was launched across the province in hospitals above the county level.Cases that matched the definition of surveillance case were identified and reported to Centers for Disease Control and Prevention (CDCs).Cases were interviewed and their blood samples collected and detected using PCR and virus isolation.We also conducted serum antibody surveys among healthy population and livestock and surveillance on vector ticks in those high-epidemic areas.Results 188 cases that matched the definition of surveillance case and 21 deaths were reported in 11 cities,32 countries and 100 towns in 2010,with an incidence rate of 0.33/106.The fatality rate was 11.2%.Data showed that the patients were from hilly areas at the altitude elevated between 28-940 meters.The epidemic period was between April and December with the peak from May to September.The youngest case was an 11-year old,while the eldest was 81 with median age as 56-year old.95.3 % of the patients were farmers.All Patients did not have the history of traveling,two weeks before the onset of SFTS.93.6% of the patients engaged in different kind of work which was associated with agriculture.52.8% of the patients had been exposed to ticks.22.0% of the patients had been bitten by ticks.Skin injury was found in 64.2% of the patients.Samples from 129 cases (68.6%) were collected and detected,with 67.4% of them (87 cases) showed positive by Real time-PCR for SFTS virus.An elevation in antibody titer by a factor of four or evidence of sero-conversion was observed in 11 patients; SFTS virus was isolated from 2 patients.The total antibody positive rates were 3.8%,55.0% (6/11 ),36.7% (2/3) and 80.0% (4/5) respectively in healthy population,dogs,sheep and cows.Ticks from grass,cattle and sheep were detected positive by Real time-PCR.Conclusion Most cases of SFTS in Hubei were infected by SFTS virus,and cases of livestock were infected by SFTS virus.Ticks might serve as an important vector.Skin injury,exposure to tick bites seemed to be the risk factors.
10.Short term prognosis comparison of transcatheter aortic valve replacement through the femoral artery for patients with pure aortic valve regurgitation of different annulus girths

Objective To evaluate and compare the success rate and short-term clinical prognosis of transfemoral transcatheter aortic valve replacement(TF-TAVR)for patients with pure aortic regurgitation(PAR)of different annulus sizes.Methods This study is a single center retrospective study,selecting symptomatic PAR patients who received TF-TAVR treatment at Zhongshan Hospital Fudan University from September 2019 to September 2023.Based on preoperative CT results,all patients were divided into three groups:Group A(aortic annulus circumference<80 mm),Group B(80 mm≤aortic annulus circumference<85 mm),and Group C(aortic annulus circumference≥ 85 mm).The primary endpoint was success rate and 30d all-cause mortality,while the secondary endpoint was TAVR related complications.Results A total of 61 PAR patients were included in this study,including 27 in Group A,21 in Group B,and 13 in Group C.The overall success rate is 82.0%,and the 30 d all-cause mortality rate is 3.3%.The success rate of Group C patients was significantly lower(P=0.012),with higher rates of conversion to surgery and valve-in-valve implantation(P=0.022 and P=0.040).In terms of secondary endpoint events,there were no significant differences among the three groups in major bleeding events,major vascular complications,stroke,myocardial infarction,newly developed atrial fibrillation,implantation of new pacemakers,coronary artery occlusion,and postoperative moderate to severe perivalvular leakage(all P>0.05).Conclusions The circumference of the aortic valve annulus is a key factor affecting the success rate of TF-TAVR in PAR,and PAR patients with an aortic valve annulus circumference less than 85mm may be more suitable for TF-TAVR.
