3. Intervention mechanisms of Chinese materia medica on inflammatory injury of renal tissue with chronic nephropathy

Chronic kidney disease is closely related to inflammation. Chinese materia medica (CMM) or their extracts can intervene in some signal transduction paths such as nuclear factor-kappa B (NF-κB) path, mitogen activated protein kinase (MAPK) path, Toll-like receptor 4 (TLR4) path, and regulate cytokines, adhesion molecules, transcription factors, acceptor molecules, and enzyme molecules to resist inflammatory injury of renal tissue from multiple target points. Because of the interaction, network, and magnification of inflammatory injury factors, the action of CMM shows the characteristics, such as multi-point, pleiotropy, overlap, and so on. The target points of CMM on inflammatory injury are explored and their molecular mechanism plays an important role in the prevention and control of chronic kidney disease and study on new drug.
5.Application of high frequency color Doppler ultrasonography in emergency operation of flexor tendon injuries in the hand

Objective To observe the value of the high frequency color Doppler uItrasonography in emergency operation of flexor tendon injuries in the hand.Methods Totally 21 patients of flexor tendon injuries in the hand underwent surgical exploration with high frequency probe.The extent of the tendon injuries,the retracted parts of the ruptured tendon and the blood supply were observed.Results The extent of the tendon injuries,the poison of broken ends were clearly distinguished with high frequency probe.The continuous tendon fibrous of layered high echo and sheath thin layer low echo complete broke and replaced with low or no echo.Conclusion Intraoperative uhrasonography of flexor tendon iniuries is important tO shorten operation time,reduce the injury of surrounding tissues,relieve local adhesion and ensure vascular recanalization after operation.

8.EFFECTS OF TAURINE ON BLOOD GLUCOSE AND PLATELET AGGREGATION IN STREPTOZOTOCIN-INDUCED DIABETIC RATS

Objective: To investigate the effects of taurine on blood glucose, number of platelets in plasma, pletelet aggregation and metabolism of oxygen free radicals in diabetic rats. Methods: Diabetes was induced by a single injection (ip) of streptozotocin (STZ,70 mg/kg body weight). The rats were maintained on drinking water with or without taurine (1.5 g/kg body weight per day). The concentration of blood glucose, maximal aggregation rate of platelets induced by adenosine 5 diphosphate(ADP) or thrombin(Thr) and the number of platelets in plasma were measured. Metabolism of oxygen free radicals in platelets was assessed as superoxide dismutase(SOD) activity and content of malondialdehyde(MDA) in platelets. [WT5FZ]Results: [WT5BZ]The concentration of blood glucose and maximal aggregation rate of platelets induced by ADP or Thr were significantly higher in diabetic rats than in non diabetic rats. The diabetic rats showed decreased platelet number and SOD activity with higher MDA level. In taurine treated diabetic rats concentration of blood glucose was obviously decreased, but no significant change was found in platelet number, SOD activity, MDA content and aggregation of platelets induced by ADP or Thr. [WT5FZ] Conclusion: [WT5BZ]There is a correlation between the increase in platelet aggregability and the abnormal metabolism of oxygen free radicals in platelets and taurine can decrease the blood glucose level but has no significant effect on platelet aggregability and metabolism of oxygen free radicals in diabetic rats.
9.CT scan findings of primary pulmonary non-Hodgkin's lymphoma and their relation to pathological features.

OBJECTIVETo investigate the CT scan findings of primary pulmonary non-Hodgkin's lymphoma (PPNHL) and their relation to pathological features.
METHODSThe CT scan findings of 17 PPNHL cases confirmed pathologically from August 1992 to June 2007 were analyzed in relation to their pathological foundation.
RESULTIn 17 cases the CT findings showed: 1) Five were single mass or nodule, including 3 with air-bronchograms, 1 with straddling-lobe distribution and 3 with slight enhancement; 2) One was multi-nodules with air-bronchograms; 3) Six were consolidation with air bronchogram,including 3 with straddling-lobe distribution and 3 with slight enhancement; 4) Five were combined changes, including 5 with air-bronchograms, 2 with straddling-lobe distribution and 4 with slight enhancement. The CT features of PPNHL corresponded to their pathological process and pathological features.
CONCLUSIONPrimary pulmonary non-Hodgkin's lymphoma should be considered when CT scan shows variety of images found in one patient, middle lobe of right lung involved, air-bronchograms and straddling-lobe distribution in consolidation with slight enhancement.

10.Relationship Between Blood Product Transfusion and Post-operative Prognosis in Children With Pediatric Open Heart Surgery by Cardiopulmonary Bypass

Objective: To explore the relationship between intra-operative blood product transfusion and post-operative prognosis in children with pediatric open heart surgery by cardiopulmonary bypass (CPB).
Methods: A total of 1028 consecutive pediatric patients received the open heart surgery by CPB in our hospital were retrospectively studied. Uni- and multivariate Logistic regression analysis were conducted to analyze the relationship between intra-operative blood product transfusion and post-operative in-hospital death and complication rates.
Results: There were 45/1028 (4.4%) of patients died, 143 (13.9%) with low cardiac output syndrome, 43 (4.2%) received dialysis, 26 (2.5%) with sustained pulmonary failure, 17 (1.7%) with infection, and 28 (2.7%) developed neurologic complications. Multivariate Logistic regression analysis indicated that duration and the lowest temperature of CPB, 24-hour post-operative pediatric risk of mortality (PRISM-III) score and intra-operative transfusion amount of RBC > median (20.3 ml/kg) were the independent risk factors for post-operative in-hospital death. Congenital heart disease (CHD) with pre-operative cyanosis, duration of CPB, 24-hour post-operative PRISM-III score, 6-hour post-operative mediastinal drain loss and intra-operative transfusion amount of RBC > 20.3 ml/kg were the independent risk factors for post-operative complication occurrence.
Conclusion: Large volume of RBC transfusion in the open heart pediatric surgery with CPB may increase the risk of post-operative in-hospital death and complication occurrence, reducing RBC transfusion might be improving the post-operative prognosis in CHD patients.
