Objective To investigate the clinical efficacy of noninvasive positive pressure ventilation for eld-erly patients with chronic obstructive pulmonary disease and respiratory failure.Methods A total of 64 elderly patients with chronic obstructive pulmonary disease and respiratory failure were collected.The patients were treated with conventional therapy,while using BiPAP ventilator line through the nose and mouth mask bilevel noninvasive pos-itive pressure ventilation therapy,and to give effective care.After treatment for 3d,the pH,PaO2 ,PaCO2 ,SaO2 index changes,respiration,heart rate changes and APACHE Ⅱ,GCS and accessory muscle score were observed.Adverse reactions were recorded.Results Before treatment the PH value of patient was (7.30 ±0.06),which significantly increased to (7.36 ±0.03 )after treatment,PaO2 values was significantly increased to (43.58 ±5.54)from (84.52 ±8.27),SaO2 value was significantly increased to (94.93 ±5.61)from (74.86 ±7.25),PaCO2 value from (71.69 ±8.48)decreased to (42.55 ±6.23).There was statistically significant before and after treatment (t =5.98,6.35,4.86,7.23,all P <0.05),respiratory rate,heart rate were significantly improved,showing a significant difference (t =4.87,5.32,all P <0.05 ),APACHE Ⅱ,GCS and accessory muscle scores were significantly improved,which APACHEⅡ,accessory muscle score was significantly lower,GCS score was significantly increased, were statistically significant (t =4.09,4.01,4.33,all P <0.05).Conclusion It shows that noninvasive positive pressure ventilation combined with conventional drug therapy can improve elderly patients with chronic obstructive pulmonary disease and respiratory failure in patients with clinical symptoms,increase ventilation and improve hyper-capnia,clinical efficacy is better,with no serious adverse reactions.

Objective To investigate the mechanism of Glutathione in the prevention of ventilator-associated lung injury.Methods 30 mechanical ventilation patients who were healthy before were selected and randomly divided into the two groups.Group A was the high tidal volume group:10mL/kg;group B was used the tidal volume plus the injection of Glutathione(2.4g/d).After 48 hours,we collected lavage fluid and use Coomassie brilliant blue to detect the protein content of alveolar lavage fluid of the two groups.Results There was a significant difference (P<0.01)between the two groups.The protein content in group A[(9.90 ±2.25)g/L]was higher than that of group B [(4.17 ±0.39)g/L],and there was statistically significant (P<0.01).Conclusion Glutathione can reduce the incidence of ventilator-associated lung injury and prevent the occurrence of ventilator-associated lung injury.

Objective To explore the correlative factors and management of secondary high intraocular pressure(IOP) after vitreoretinal surgery for proliferative diabetic retinopathy(PDR). Methods The data of 51 patients(54 eyes) with PDR after vitreoretinal surgery were collected.The incidence of secondary high IOP was obtained,and the correlative factors leading to high IOP were analysed.Besides,the therapeutic effects of management were observed. Results Twenty-six patients(26 eyes) experienced postoperative high IOP,with the incidence of 48.15%(26/54).It was revealed that the preoperative retinal detachment,the combined performance of crystal phacoemulsification,the practice of intraocular tamponade and the postoperative posterioruveitis were related to the occurrence of high IOP(P

Objective To examine the expression of Toll-like receptor(TLR)4 protein and the transcription of TLR4 mRNA in the peripheral blood mononuclear cells(PBMC)from patients with hepatitis B virus(HBV)infection and explore the relationship between TLR4 and chronic HBV infec- tion.Methods The expression level and transcription level of TLR4 were determined by flow cytometre and reverse transcriptase polymerase chain reaction respectively in PBMC from 37 chronic hepatitis patients,28 liver cirrhosis patients,31 severe hepatitis patients and 27 healthy controls. AIM:To observe the effects of normal mesenteric lymph (NML) on the lung, heart and liver inju-ries and the phosphorylation levels of p 38 mitogen-activated protein kinase ( MAPK) , extracellular signal-regulated kinase (ERK) 1/2 and c-Jun N-terminal kinase (JNK) in the mice with endotoxic shock (ES).METHODS: The NML was drained form health male BALB/c mice for the intervention of ES after the removal of cellular constituent .Lipopolysaccha-ride (LPS, 35 mg/kg) was intraperitoneally injected into the mice for the establishment of ES model .After 60 min of LPS injection, the administration of NML (1/15 of whole blood volume) was performed through the femoral artery in NML +ES group.Meanwhile, the mean arterial pressure (MAP) was monitored during the experiment .At 6 h after intraperitoneal in-jection of LPS or the corresponding time point , blood samples were harvested from the heart through apical centesis for de-termination of the biochemical indexes to reflect myocardial and hepatocyte injuries .Simultaneously , the lung , heart and liver tissue specimens from a fixed location were harvested for the observation of histomorphology and the measurement of phosphorylation levels of p38 MAPK, ERK1/2 and JNK.RESULTS:Compared with sham shock (SS) group, MAP in ES group and NML+ES group remarkably decreased at multiple time points after intraperitoneal injection of LPS .However, MAP in NML+ES group at 80 min, 90 min, 190 min, 210 min, 240 min, 250 min, 340 min, 350 min, and 360 min were significantly increased compared with ES group .There were normal structures in the lung , liver and myocardium of the mice in SS group, while the morphological damages of these tissues appeared in ES group .Meanwhile, the damages were attenuated in the mice of NML +ES group.The activities of AST , ALT and CK-MB in the plasma in ES group were remark-ably higher than those in SS group .The CK-MB activity in NML+ES group was also increased compared with SS group , and the activities of AST and LDH-1 were lower than those in ES group .At 6 h after LPS injection , the phosphorylation levels of p38 MAPK, ERK1/2 and JNK in the lung tissues were remarkably increased .Meanwhile , no statistical difference of these indexes between the myocardial and hepatic tissues was observed .NML intervention decreased the phosphorylation levels of p38 MAPK in the lung tissues , and p38 MAPK, ERK1/2 and JNK in the myocardial tissues .CONCLUSION:The NML administration alleviates multi-organ injuries and reduces the phosphorylation level of p 38 MAPK in the lung tis-sues in the mice subjected to ES .

Objective To establish HPLC fingerprints for the identification of Ilicis Rotundae Cortex (Jiubiying), dried bark of Ilex rotunda Thunb., and its adulterant, dried bark of Ilex godajam (Colebr.) Wall.. Methods Some samples named Jiubiying were collected from some medicine markets and drugstores. The HPLC fingerprints were obtained to detect Jiubiying and Ilex godajam. The software Similarity Evaluation System of Chromatographic Fingerprints of Traditional Chinese Medicine (2004A edition) was used to evaluate fingerprints. Results The fingerprints of Jiubiying and Ilex godajam were established. Methodological study met the technical requirements of fingerprints. There were 31 and 28 common peaks in Jiubiying and Ilex godajam, respectively. Jiubiying and Ilex godajam could be classified into two clusters by principal component analysis and hierarchical clustering analysis. Conclusion Jiubiying and Ilex godajam can be identified by HPLC fingerprints. This method can provide an effective method for quality evaluation of Jiubiying.

An HPLC method for the determination of geniposide concentration in mouse plasma was developed and the pharmacokinetics after intranasal administration of Xingnaojing microemulsion (XNJ-M) and mPEG2000-PLA modified Xingnaojing microemulsion (XNJ-MM) were investigated. Eighty mice were treated by XNJ-M and XNJ-MM nasally. The plasma samples were collected at different times and the drug in samples was detected by HPLC. The pharmacokinetic parameters were calculated by the software of Kinetica. The pharmacokinetic parameters of geniposide of XNJ-M were C(max) (4.36 +/- 2.69) mg x L(-1), t(max) 1 min, MRT (29.73 +/- 4.54) min, AUC (53.63 +/- 14.03) mg x L(-1) x min. The pharmacokinetic parameters of geniposide of XNJ-MM were C(max) (9.75 +/- 4.14) mg x L(-1), t(max) 1 min, MRT(22.34 +/- 2.90) min, AUC (131.87 +/- 40.13) mg x L(-1) x min. Geniposide can be absorbed into blood in a higher degree after intranasal administration with XNJ-MM compared to XNJ-M, which maybe caused by its less irritating and more absorption.

Objective To observe the effects of acupuncture on synaptic transmission signal molecules in rats with vascular dementia (VD), such as PKC, CaMKⅡ and NR2B, and discuss the molecular mechanism of acupuncture treatment for VD.Methods The multi-infarct dementia model was established by injection of emboli into the internal carotid artery. Experimental rats were randomly divided into normal group, model group, acupuncture group and non-acupoint group. For acupuncture group, acupuncture needles were penetrated into bilateral Zusanli. Non-acupoint group was given acupuncture treatment at the bilateral hypochondrium (10 mm above iliac crest). The rats in normal group and model group were performed to the same amount of capture stimulation as the acupuncture and non-acupoint groups. After treatment, the hippocampal PKC activity was detected by ELISA. Western blot was used to detect CaMKⅡ expression, and the protein expression of NR2B in CA1, CA3 and DG zones was assayed by immunohistochemical staining.Results Compared with normal group, PKC activity and NR2B expression in the hippocampus significantly decreased in the model group (P<0.01). After the acupuncture treatment, PKC activity increased significantly (P<0.05), and the protein expression of NR2B showed a trend to increase. There was no obvious difference in CaMKⅡ expression among all groups.Conclusion Acupuncture at Zusanli can enhance the activity of hippocampal PKC, a synaptic transmission signal molecule, which maybe one of the important molecular targets for the treatment of VD.

The morphology,cultural characteristics,physiological and biochemical properties of phosphate solubilizing strain BL-11 were studied.Combine with its 16S rDNA sequence analysis result strain BL-11 was identified as Brevibacillus laterosporus.Results showed that the phosphate solubilizing efficiency of strain BL-11was up to 10.91% when with Ca_ 3 (PO_ 3 )_ 2 as the sole phosphorus source in the medium,while the efficiency of that was up to 1.56% when with sands as the sole phosphorus source.An optimal media of BL-11 strain for solubilizing phosphate was obtained by orthogonal test.It was composed of sugar 20g/L,(NH_ 4 )_ 2 HCO_ 3 0.3g/L,MgSO_ 4 .7H_ 2 O 0.5g/L,NaCl 0.3{g/L},KCl 0.5g/L,FeSO_ 4 0.03g/L,MnSO_ 4 .H_ 2 O 0.03{g/L.}_ While other conditions were consisted of initial pH7.0-8.0,180 r/min,and 30℃.
