1.Clinical analysis of 163 cases of pulmonary thromboembolism

Objective To explore the role and significance of standardized scheme for diagnosis and treatment of pulmonary thromboembolism(PTE).Methods The clinical data of 163 consecutive PTE patients who were treated in our hospital from Jan.1972 to Dec.2006 were retrospectively reviewed.The patients were divided into group A and group B based on the time of application of standardized treatment and diagnosis for PTE.The clinical data of the two groups were analyzed and compared.Results The main risk factors included deep vein thrombus,operation,injury,fracture and tumors,etc.Dyspnea was the most important clinical symptoms.The incidences of dyspnea in group A and B were 92.5% and 84.6%,respectively.The diagnostic case and the ratio of final diagnosis in group B was increased compared to those in group A on an annual basis.The median time for diagnosis was shortened(P

Objective To investigate the relationship between tubular cells transdifferentiation and renal interstitial fibrosis in patients with chronic aristolochic acid nephropathy (CAAN). Methods Specimens from renal biopsies of 10 CAAN patients with serum creatinine level of (309. 41 ? 164. 44) ?mol/L were performed to examine the extent of renal interstitial fibrosis by Masson staining, the expression of collagen types Ⅰ and Ⅲ by sirius red staining, and the expression of cytokeratin(CK), ?-smooth muscle actin (a-SMA), vimentin (Vim) and transforming growth factor-? 1 (TGF-?1) by immunohistochemical staining. 4.Delayed scanning method in multi-slice spiral CT for diagnosis of pulmonary embolism

Objective To compare different delayed scanning methods in multi-slice spiral CT angiography (MSCTA)in diagnosis of pulmonary embolism (PE). Methods Sixty patients with suspected PE were divided into three groups (A, B and C). MSCT with same Iodine concentration, injection rate, contrast medium but different delayed scanning methods was performed after injection of contrast medium. Patients in group A were examined with fixed time method (15 s), in group B with small dose-density curve method, while in group C with contrast medium track and triggering technoligy. The number, position and the shapes of emboli were evaluated with MIP, MPR and VR. Results The successful examination rate of group A was 55.00% (11/20), while of group B and C was both 100%. The coincidence rate of MSCTA compared with DSA in each group was 96.04% (291/303). Conclusion The best delayed scanning method in MSCTA for diagnosis of pulmonary embolism is contrast medium tracking and triggering technology.
5.Expression and purification of Litopenaeus vannamei allergen protein Lit v1.2

Objective:To obtain purified recombinant Litopenaeus vannamei allergen protein Lit v 1.2.Methods: The target gene of Lit v 1.2 was inserted into clone vector pGEM-T and then ligated to the expression vector pET 44a.The pET44a-Liv 1.2 was transformed into Rosetta and screened by ampicillin resistance .The recombinant protein was expressed by IPTG induction .The protein was purified by 6-His tag affinity chromatography and the purification was analyzed by SDS-PAGE gel electrophoresis .Results:The ex-pression plasmid pET44a-Lit v 1.2 was constructed.SDS-PAGE showed that expressed Lit v 1.2 was efficient and soluble in E.coli Rosetta.The protein molecular weight was consistent with the theoretical value .The highly purified target protein was obtained.Conclusion:In this study ,we successfully gained highly purified recombinant allergen protein Lit v 1.2 which was expressed in prokaryotic system and purified by affinity chromatography column .The purified Lit v 1.2 protein will facilitate us to further study its role in immunological responses .
6.Comparison of two measures assessing prospective memory impairment in elderly people with schizophrenia

Objective:To compare the results of two different measures assessing prospective memory (PM) in elderly schizophrenia,and explore their diagnostic validity.Methods:Fifty patients who were diagnosed as schiz-ophrenia according to the criteria of Diagnostic and Statistical Manual of Mental Disorders,Fourth Edition (DSM-Ⅳ)and 50 normal elderly people were enrolled in the study.Laboratory paradigm prospective memory task and the Chinese version of the Cambridge prospective memory Test (C-CAMPROMPT)were administered to measure thelevel of EBPM and TBPM.The Wechsler Adult Intelligence Scale-Fourth Edition,Continuous Performance Test-I-dentical Pairs were used to evaluate IQ and attention.Results:Whether in the laboratory paradigm or C-CAM-PROMPT,schizophrenia group performed worse than normal controls on PM total scores[7(0,16)vs.12(0,16),14 (4,34)vs.25(11,36)],EBPM[3(0,8)vs.6(0,8),7(2,16)vs.14(4,18)]and TBPM [3(0,8)vs.6(0,8),6(2,18) vs.12(4,18)],and patients had comparable performances in PM subtypes[-1.0(-2.2,1.0)vs. -1.0(-2.1, 0.8),-1.9(-3.4,0.8)vs. -1.8(-2.9,1.6)].In the C-CAMPROMPT,the scores of EBPM and TBPM were negatively related to age(r=-0.36 --0.40,P<0.001 ),but there was no significant relationship between PM performance and age in the laboratory paradigm task.The area under ROC curve of the laboratory paradigm and C-CAMPROMPT were 0.73 and 0.85.While 8 and 19 as the cut off value,the sensitivity were 0.60 and 0.74,and the specificity were 0.76 and 0.90,respectively.Conclusion:The two measures have confirmed that the elderly schizophrenia have comparable performances in EBPM and TBPM.Both the C-CAMPROMPT and the laboratory paradigm have moderate level of diagnostic validity,but the former is slightly higher and more susceptible to aging.
7.Rhythmic changes of ambulatory blood pressures in pre-eclampsia patients

Objective To study the rhythmic changes of 24 h ambulatory blood pressure in pre-eclampsia(PE)patients and come up with coping strategies. Method The levels and circadian variations of blood pressures in 240 pre-eclampsia patients were monitored and observed with 24 h ambulatory blood pressure monitoring. Results Among the 240 PE cases,121 were diagnosed as mild PE and 119 as severe PE.There were significant differences in the average of 24 h circadian variation,the average of date time variation,decreased blood pressure at night time between the two groups(all P<0.05). Conclusion Through ambulatory blood pressure monitoring,nurses can monitor the rhythmic changes of 24 h ambulatory blood pressure,make reasonable arrangements for the treatment and care operations,reduce the factors that induce eclampsia and ensure the safety of mother and baby .
8.Expressions of Protease Activated Receptors on Human monocyte.

Objective To explore the expression of protease activated receptors (PARs) on human monocytes. Methods Flow cytometry and RT - PCR was used to detect the expressions of PARs on human monocytes that purified by MACS. Results FACS analysis showed that monocytes expressed PAR - 1 , PAR - 3 , PAR - 4, but not PAR - 2.RT - PCR revealed that monocytes expressed PAR - 1 and PAR -3, but not PAR -2 and PAR -4 mRNA. Conclusion PARs is expressed on human monocyte cells,which may facilitate further investigation of the potential functions of PARs on monocytes.
9.Endoscopic submucosal dissection in patients with high grade intraepithelial neoplasia and/or early cancer of remnant stomach after partial gastrectomy

Objective To evaluate the feasibility,safety and efficacy of ESD for precancerous lesions and early cancer of remnant stomach after partial gastrectomy.Methods ESD was performed in 11 cases of high grade intraepithelial neoplasia/early cancer of remnant stomach.The short-term and long-term indices including time of procedure,complication,En Bloc resection rate,R0resection rate,local recurrence rate as well as lymph node metastasis were recorded and analysed.Results ESD was completed in all patients with only one case of delayed massive bleeding which was controlled by endoscopy successfully.Average procedure time,En Bloc resection rate,R0 resection rate were 85.5 minutes,100% and 90%,respectively.No local recurrence or lymph node metastasis was detected during post-ESD surveillance (15 ~ 51 months).Conclusion High grade intraepithelial neoplasia and early cancer of remnant stomach after partial gastrectomy might be indication for ESD because of its safety and definite effect.Additionally,careful management of the fibre tissue is the key to procedure success.
10.A prospective study of endoscopic ultrasound-guided celiac plexus neurolysis for pain management in patients with pancreatic carcinoma

ObjectiveTo prospectively assess the efficacy and safety of endoscopic ultrasoundguided celiac plexus neurolysis (EUS-CPN) in the management of pain caused by pancreatic carcinoma.MethodsPatients with confirmed un-resectable pancreatic malignancy were treated with EUS-CPN once or twice.The data including age,gender,pain duration,opioid consumption,tumor location,pain scores before and after the procedure,number of treatment,duration of pain relief and survival time were collected.ResultsA total of 27 cases with cancerous abdominal pain were recruited.The median pain scores were significantly lower after EUS-CPN,and pain relief was obtained in 81.5% (95% CI,66.8% -96.1% ) patients with a median duration of 56.0 days (95% CI,17.1-94.9),whose confidence interval was comparable to that of survival time (97.0 days,95 % CI,82.7-111.3 ).No clinical factors could predict post-procedure duration of pain relief (x2 =6.757,P =0.239).Procedure-related transient diarrhea and fever were noted only in 1 patient.No major complications occurred.ConclusionEUS-guided CPN is safe and effective in alleviating abdominal pain associated with pancreatic cancer,even at the late stage.Moreover,no clinical factor could predict post-procedure degree or duration of pain relief.
