1.Progress of mechanical ventilation during cardiopulmonary resuscitation

Mechanical ventilation is regarded as an effective means of replacing artificial ventilation during cardiopulmonary resuscitation (CPR), and has been widely used in the treatment of cardiac arrest (CA) patients. However, there are still some controversial issues remaining to be settled, such as the assessmentof the effectiveness of mechanical ventilation, the selection of the optimal oxygen concentration (FiO2), tidal volume (VT), respiratory frequency, and mode of ventilation during the CPR process. The pros and cons of positive pressure ventilation are also inconclusive. We reviewed and summarized the related research in recent years, and recommended that the intermittent positive pressure ventilation (IPPV) of volume controlled with small VT (6-7 mL/kg), low ventilation rates (10 times/min), and pure oxygen could be applied during the mechanical ventilation of CPR. The best mechanical ventilation strategy still needs further experimental researches to discover and explore.
2.Advances in Bosentan Treatment for Pediatric Pulmonary Arterial Hypertension

Bosentan is a non-selective endothelin receptor antagonist,which plays an important role in the treatment of children with pulmonary arterial hypertension.Bosentan has shown to improve exercise capacity,hemodynamics and reduce pulmonary vascular resistance in pediatric patients.The main adverse effect is less severely comparing with the adult.The combination of bosentan with other drugs can improve the life quality furtherly.Further study of large-scale trials needs focus on the long-effects of bosentan,the ideal period of therapy and the effects of combination therapy in children.
3.Effects of mild moxibustion on imbalance of Th1/Th2 in rabbits with atherosclerosis and its regulating rules.

OBJECTIVETo observe effects of mild moxibustion and lovastatin on immunologic function in rabbits with chronic hyperlipidaemia and atherosclerosis (AS) to initially explain regulating rules of mild moxibustion on immunologic function.
METHODSAmong thirty-two Japanese male big-ear rabbits, 8 rabbits were randomly selec ted as a blank group, the rest 24 rabbits were fed with method of endothelial injury and high-fat diet to establish AS model. The blank group was raised with normal diet and free water. After ten weeks of model establishment, the rest 24 rabbits were randomly divided into a model group, a moxibustion group and a medicine group, eight rabbits in each one. Moxibustion was applied at "Shenque" (CV 8) and "Zusanli" (ST 36) for 10 min per acupoint per day in the moxibustion group, while intragastric administration of 3.6 mg/kg lovastatin capsule was applied in the medicine group. After treatment, serum was acquired. Spectrophotometry method was adapted to measure cholesterol (TC) and high-density lipoprotein (HDL-C) and evaluated atherosclerosis index (AI), while enzyme-linked immunosorbent assay method was used to measure interferon-gamma (IFN-gamma) and interleukin-4 (IL-4).
RESULTS(1) The serum TC and HDL-C in the model group were significantly higher than those in the blank group, moxibustion group and medicine group (all P < 0.01). The mean value of AI was 1.683 +/- 0.486 in the moxibustion group, which was obviously lower than 20.301 +/- 4.022 in the model group (P < 0.01). (2) The ratio of Th1/Th2 was 0.569 +/- 0.143 in the moxibustion group and 0.445 +/- 0.079 in the medicine group, which were significantly lower than 0.917 +/- 0.255 in the model group (both P < 0.01), but there was no statistically significant difference between the moxibustion group and the medicine group (P > 0.05).
CONCLUSIONThe moxibustion for AS could reduce atherosclerosis index, influence drift and bias of helper T cell and regulate balance between humoral immunity and cellular immunity. As a result, status of relative balance of immunity is acquired, which could slow down the development of atherosclerosis and process of thrombus burst.

4.Analysis of the misdiagnosis and missed diagnosis of prenatal ultrasonography in screening fetal heart disease

Objective To investigate the application of prenatal ultrasonograpy in diagnosis of fetal heart disease during the medium-term and terminal pregnancy,and to avoid misdiagnosis.Methods The prenatal ultrasonographic screening was conducted in 7020 pregnant women with medium-term and terminal pregnancy.All confirmed and suspicious cases of fetal heart disease were observed by echocardiography once 4 weeks until postnatal sixth month to 1 year while normal cases was traced at the intervals of 8 weeks.All the cases were confirmed by autopsy or surgery while part of them were confirmed by postnatal angiocardiography before surgery.The reasons of misdiagnosis were analyzed.Results In fifty-three cases of fetal cardiovascular abnormality,43 cases were found,2 cases were misdiagnosis while 8 cases were missed diagnosis.2 cases of misdiagnosis included 1 case of pulmonary valve stenosis and 1 case of tetralogy of Fallot.8 cases of miss diagnosis included 2 cases of inferior vena cava in ectopic drainage and 6 cases of ventricular septal defect (defect diameter < 5 mm).The accuracy rate of diagnosis of fetal heart disease was 81.1 % by echocardiography.Conclusions Using high frequency probe,image zoom,enhancing the detection of vein-atrium connection and the structure of right ventricular outflow with pulmonary artery are important factors to avoid missed diagnosis and incomplete diagnosis.
5.Application of transthoracic echocardiography and CT in diagnosis of complex congenital heart disease

Objective To investigate the application of transthoracic echocardiography (TTE) and 320-row CT in diagnosis of complex congenital heart disease(CCHD).Methods A retrospective analysis was performed in 68 patients with CCHD,which contained the echocardiographic apprarance,320-row CT and surgical outcoming.All of malformations were divided into three groups:intracardiac group,heart-large vascular connecting group and extracardiac group.Results One hundred and eighty-four malformations were comfirmed by surgery in the 68 cases.The misdiagnosis and missed diagnosis rate of CT and TTE:for the intracardiac anomalies were 12% (11/93),1% (1/93) respectively (P < 0.05),for the heart-large vascular connecting malformation were 4% (2/49),6% (3/49) respectively (P > 0.05),and for the extracardiac anomalies were 2%(1/42),19%(8/42) respectively (P <0.05),for the total anomalies were 8%(14/184),7% (12/184) respectively (P >0.05).While it was 1% (2/184) in diagnosis of total anomalies by both CT and TTE.Conclusions Both 320-row CT and TTE have their own advantages and shortcomings in the diagnosis of CCHD.Joint application of two methods could reduce the rate of misdiagnosis and missed diagnosis of CCHD.
6.Effect of Tonifying Kidney using Chinese Medicine(Guzhen Recipe) to Retard Aging of the Hippocampus and Hypothalamus-Pituitary-Adrenal-Thymic(HPAT) Axis in Senile Rat.

According to the TCM theory, human growth, development and aging are closely related to kidney function and the main cause of human senility is deficiency of “kidney essence”. Recent studies showed that the “kidney” in TOM include the regulatory function of the neuroendocrine and immune systems.
In this paper, the effect of Guzhen Recipe (_??__??__??_), an aging retarding prescription composed of kidney tonifying herbs such as Radix Polygoni Multiflori (_??__??_), Herba Cistanchis (_??__??_) Rubus Chiagii (_??__??__??_) etc. on the hippocampus and HPAT axis in senile rats were observed. The results showed : (1) With age the number of glucocorticoid receptors (GCR) in the hippocampus, the concentrations of NE, DA, 5-HT in the hypothalamus were markedly decreased, while the plasma corticosterone concentration in senile rats was obviously increased. (2) The thymocyte GCR nuclear transition rate was increased significantly ; (3) The contents of cytoplasmic protein, nuclear RNA and DNA, the thymus weight/body weight ratio, the serum thymic factors as well asthe rate of lymphocyte transformation, the activity of IL-2 and NK cells in senile rats were obviously decreased compared to those of young control rats. Guzhen recipe however, was capable of improving these changes in the neuroendocrine-thymic net work in senile rats to varying degrees, suggesting that the kidney with Chinese Medicine might retard the functional degeneration of thymic endocrine partly by improving the feedback regulation of the hippocampal GCR and increasing the concentrations of NE and 5-HT of hypothalamus, thus decreasing the plasma corticosterone ; Simultaneously weakening the nuclear transition rate of thymocyte GCR, which in turn improving the immune function of the senile animal.
We concluded that Guzhen Recipe may retard the functional degeneration of both the hypo-thalamuspituitary-gornadal-thymic and hypothalamus-pituitary-thyroidal-thymic axis. All the above results revealed that retarding the aging effect using TCM in volves multiple-leves, multiple-systems and links. Using holistic regulation, imbalance links in aging process are modulated and the equilibrium of Yin and Yang are maintained. This is an important characteristic of the TCM to retard aging.

7.Prospective cohort study on the predictive value of serum uric acid levels to the incident nonalcoholic fatty liver disease

Objective To assess the predictive value of serum uric acid levels to the incident nonalcoholic fatty liver disease(NAFLD)in a cohort of healthy population. Methods A prospective cohort study of NAFLD incidence was conducted in Xinjiang province, from 2012 to 2014. A cohort study was performed on 2 207 subjects with no evidence of fatty liver disease by liver ultrasound and with no major risk factors for liver disease at baseline. All participants were interviewed to carry out the epidemiological questionnaire survey, physical examination, abdominal ultrasonography, as well as blood biochemistry measurements. The hazard ratios of NAFLD were compared among groups with different uric acid levels. All subjects were classified according to serum uric acid (within normal range grouped by quartile: Q1-Q4 group; above the normal range: Q5 group). Results NAFLD was newly diagnosed in 13.72% subjects within the 3-year period. The incidence of NAFLD was increased with elevated serum uric acid quartiles(P<0.01). In multivariate logistic regression, after adjustment for gender, age, race, metabolic syndrome and its components, OR for incident fatty liver in Q2 to Q5 of serum uric acid concentration as compared to Q1 were 2.509, 3.172, 3.392, and 4.041 respectively. Conclusion Elevated serum uric acid concentrations may predict NAFLD in the subjects for a regular health checkup.
8.Study of the process parameters in pharmaceutical air jet pulverization

Objective:To study the influence of some process parameters on the product granularity in pharmaceutical air jet pulverization process.Methods:The method of quadratic regressive orthogonal experiment was used.Results:The mathematics model describing the relationship between the parameters studied and product granularity was established.Conclusion:The conclusions in the experimental study have instructional significance for pharmaceutical production.
9.Diagnostic value of Sj26, Sj32 and Sj14-3-3 coding gene of Schistosoma japonicum amplified by PCR

Objective To investigate the diagnostic value of coding gene of Sj26, Sj32 and Sj14-3-3 amplified by PCR for chronic Schistosomiasis japonica. Methods The DNA was extracted from sera of 40 patients with chronic Schistosomiasis japonica, the coding gene of Sj26, Sj32 and Sj14-3-3 was amplified by PCR and identified by 1.2% agarose gel electrophoresis. DNA from the sera of 21 patients with Clonorchiasis sinensis, 13 patients with Parogonimiasis westermani and 43 healthy donors was taken as control. Results A total of 399 bp coding gene of Sj14-3-3 was amplified successfully from sera of the patients with chronic Schistosomiasis japonica,but Sj26(676 bp) and Sj32( 1270 bp) coding gene were not obtained. Control groups were all negative. Conclusions Sj14-3-3 coding gene amplified by PCR can be used for genetic diagnosis of chronic schistosomiasis.
10.The diagnostic value of rSj26-Sj32-IgG-ELISA for acute schistosomiasis japonica

Objective To study the diagnostic value of rSj26-Sj32-IgG-ELISA for acute schistosomiasis japonica. Methods Purified rSj26-Sj32 fusion protein and crude Schistosoma japonicum antigen (SjAWA)were used to establish IgG-ELISA to detect serum of patients with acute schistosomiasis, and clonorchiasis sinensis,paragonimiasis westermani, alveolar echinococcosis, cystic echinococcosis, type B hepatitis, lung tuberculosis patients and healthy human serum were used as control. Results The sensitivity and specialty were 90.00%(45/50) ,97.67% (42/43) and 92.00% (46/50),97.67% (42/43) in detection of acute schistosomiasis japonica with rSj26-Sj32and SjAWA, respectively, and the difference was not statistically significant(x2 were both 0.0, all P >0.05). The serum cross-reaction reactivity was 20.00%(2/10) in patients with alveolar echinococcosis with SjAWA,but no cross-reaction with rSj26-Sj32, the difference was not statistically significant(x2 = 0.5, P > 0.05). The serum cross-reactivity were 14.29% (3/21 ), 7.69% (1/13) and 19.05% (4/21 ), 7.69% (1/13) among patients with clonorchiasis sinensis and paragonimiasis westermani by rSj26-Sj32 and SjAWA, but no cross reaction with type B hepatitis and lung tuberculosis patients, the difference was not statistically significant (x2 were both 0.0, all P > 0.05). The positive predictive value, the negative predictive value and the diagnostic efficiency with acute schistosomiasis japonicum by rSj26-Sj32-IgG-ELISA and SjAWA-IgG-ELISA were 97.83% (45/46),89.36% (42/47),93.55% (87/93)and 97.87% (46/47),91.30% (42/46),94.62% (88/93), respectively, and the difference was not statistically significant (x2 were both 0.0, all P > 0.05). Conclusion rSj26-Sj32 fusion protein can be used for the immune diagnosis of acute schistosomiasis japonica.
