1.Efficacy observation of acupuncture for dry eye syndrome of lung-yin deficiency pattern

Objective: To observe the clinical efficacy of combining two needling manipulations, Er Long Xi Zhu (two dragons playing with a pearl) and Guo Yan Re (heat produced to reach the eyes), in treating dry eye syndrome (DES) of lung-yin deficiency pattern. Methods: Fifty-six eligible DES patients of lung-yin deficiency were randomized into an observation group and a control group, with 28 cases in each group. Same acupoints were selected in the two groups: Cuanzhu (BL 2), Sizhukong (TE 23), Taiyang (EX-HN 5) and Fengchi (GB 20) were chosen as the major points and Feishu (BL 13) and Chize (LU 5) as the adjuvant. Er Long Xi Zhu and Guo Yan Re needling manipulations were applied in the observation group while twirling reinforcing manipulation was used in the control group. Treatment was conducted once per day in both groups, for two sets of 15 consecutive days at a 2-day interval. Changes in the symptom score, tear break-up time (BUT) and tear production were observed afterwards, and the clinical efficacy was also compared between the two groups. Results: The total effective rate was 92.8% in the observation group, higher than 71.4% in the control group, and the between-group difference was statistically significant (P<0.05). After treatment, the symptom score, tear BUT and tear production showed significant improvements in both groups (all P<0.05); the symptom score, BUT and tear production in the observation group were significantly different from those in the control group (all P<0.05). Conclusion: Given the same acupoint selection, combining Er Long Xi Zhu and Guo Yan Re needling manipulations can produce more significant clinical efficacy than twirling reinforcing manipulation in treating DES of lung-yin deficiency pattern.
2.Effect of Galvanoiontophoresis Combined with Ultrashort Wave Therapy on Cervical Vertebrae Disease-derived Arrhythmia

ObjectiveTo observe the curative effect of galvanoiontophoresis combined with ultrashort wave therapy on cervical vertebrae disease-derived arrhythmia.Methods112 patients with cervical vertebrae disease-derived arrhythmia were randomly divided into the treatment group and control group with 56 cases in each group. All patients of two groups were treated with antiarrhythmia drug, Sibelium and traditional Chinese medicine. While, the patients of the treatment group were added with galvanoiontophoresis combined with ultrashort wave therapy to eliminate the aseptic inflammation.ResultsThe cure rate and total effective rate of the treatment group were 51.8% and 94.6%. Those of the control group were 16.1% and 76.8%. The effect of the treatment group was better than that of the control group significantly ( P<0.01).ConclusionThe galvanoiontophoresis combined with ultrashort wave therapy has a better effect on cervical vertebrae disease-derived arrhythmia.

The complex level of constructing biopharmaceutics classification system of Chinese materia medica CMMBCS) was the study of traditional Chinese compound, on the premise of insisting that the multicomponent simultaneous determination, when carrying out the study of intestinal permeability, the primary task was to define the source of the components that was absorbed through the intestinal wall, namely, which medicinal material the components belonged to in traditional Chinese compound. The technology of chemical fingerprint and in vitro everted gut sac model were used in this research to make multicomponent an intuitive source attribution which permeated the intestine in the classic formula Gegen Qinlian decoction, and to lay the foundation for the further qualitative and quantitative research of intestinal permeability.

4.Glycosides from Machilus wangchiana.

Ten glycosidic compounds were isolated from an ethanol extract of Machilus wangchiana by a combination of various chromatographic techniques including column chromatography over silica gel and Sephadex LH-20 and reversed-phase flash chromatography and HPLC. Their structures were identified by spectroscopic data analysis (IR, MS, and NMR) as icariside B1 (1), boscialin-3-O-β-D-glucopyranoside (2), pisumionoside (3), isolariciresinol-9'-O-β-D-xylopyranoside (4), 5'-methoxyisolariciresinol-9'-O-β-D-xylopyranoside (5), lyoniresinol-9'-O-β-D-xylopyranoside (6), (E) -4-hydroxyphenylprop-7-ene 4-O-β-D-glucopyranoside (7), (E) - 4-hydroxy-3-methoxyphenylprop-7-ene 4-O-α-L-rhamnopyranosyl-(1 --> 6) -β-D-glucopyranoside (8), 4-hydroxy-3-methoxyphenylprop-8-ene 4-O-β-D-xylopyraosyl-(1 --> 6) -β-D-glucopyranoside (9), and 4-hydroxy-3,5-dimethoxyphenylprop-8-ene 4-O-α-L-rhamnpyranosyl-(1 --> 6)-β-D- glucopyranoside (10), respectively.

5.Clinical and genetic analysis of a family with familiar acute necrotizing encephalopathy due to mutation in the RANBP2 gene

Objective To analyze the clinical manifestations of familial acute necrotizing encephalopathy (ANE)and to improve the recognition of this disease. Methods The clinical data of a 25 - month - old girl with fa-milial and recurrent ANE with evidence of mutation in the RANBP2 gene were collected and analyzed,and the gene examination of their family members was performed. Results A previously healthy girl experienced recurrent ANE epi-sodes at the ages of 8 months,18 months and 25 months,respectively. At each beginning of each episodes the patient presented with lethargy and tremor of limbs following febrile illness of 3 - 4 days,even developed coma and convulsions in the last time. Brain magnetic resonance imaging showed bilateral and high T2 signal changes in thalamus,cerebellum and hippocampus. Abnormal signals also appeared in the brainstem,claustrum,corpus scallosum and cortex(temporal, parietal and cingulate)also appeared abnormal signals. Spinal MRI showed spinal cord involvement. The girl recovered after her first episode;she could speak but could not walk steadily after the second time;after the third episode,al-though she regained consciousness from coma,she could no longer speak or walk. The patient's sister died of encephali-tis at the age of 18 months. Her paternal uncle had suffered from dysnoesia from meningitis at his 17 months of age. The patient and her grandmother,father,uncle and one of her aunts harbored a mutation(c. 1754C ﹥ T)in RANBP2 gene. Conclusions Familial ANE has typical clinical manifestations and characteristic MRI findings. The patient with recur-rent history,especially with positive family history,should have the mutation in RANBP2 gene detected earlier in order to clarify the diagnosis of ANE.
6.Effects ofYangyin Yiqi Mixture on Bleomycin-induced Pulmonary Fibrosis in Rats

Objective To investigate the effects ofYangyin Yiqi Mixture on pulmonary fibrosis caused by bleomycin in rats;To discuss its possible mechanism.Methods SD rats were randomly divided into normal control group, model group, dexamethasone acetate group, low-dose, middle-dose and high-doseYangyin Yiqi Mixture groups. Bleomycin was used to establish pulmonary fibrosis rat models through endotracheal instillation. One day after molding, each medication group received gavage with relevant medicine. Lung tissues were taken on the 7th, 14th and 28th day. Masson staining was used to observe pathological change;alkalinehydrolysis was used to detect HYP content;Western blot was used to detect the protein expressions of MMP-2 and TIMP-1.Results Compared with the normal control group, on 7th day, collagen in lung tissue of rats in model group markedly increased;the content of HYP in lung tissue significantly increased (P<0.01). On 28th day, collagen was deposited diffusely and the alveolar was destroyed;content of HYP in lung tissue increased significantly (P<0.01). Compared with model group, deposited collagen in lung tissue was alleviated, and the content of HYP were remarkable reduced in middle and high doseYangyin Yiqi Mixture groups and displayed dose-dependent. Western blot results showed that the protein expressions of MMP-2 and TIMP-1 increased quickly compared with the low expression of normal lung tissue in the 7th day of molding (P<0.05), and reached high expression in the 28th day. Compared with the model group, the level of MMP-9 in high-dose Yangyin Yiqi Mixture group decreased significantly in the 14th day;the expression of TIMP-1 decreased significantly in the 7th, 14th and 28th day (P<0.05).ConclusionYangyin Yiqi Mixture can effectively resist pulmonary fibrosis, which may be realized by inhibiting the expressions of TIMP-1 and MMP-9, regulating and promoting collagenic hydrolysis and restraining deposition of collagen.
7.Preliminary investigation and analysis of irregular antibodies of blood group before patients′transfusion

Objective Irregular antibody screening and identification are very important to reduce hemolytic reaction.The dis-tribution of irregular antibodies of blood group was analyzed in patients, to provide evidence for using Micro column gel test to detect and identify irregular antibodies transfusion safety. Methods Micro column gel anti-globulin technique was used to screen the irregular an-tibodies and cross-matching in 29770 patients.If the serum had irregular antibodies, the spectrum cells were used to identify the specific-ity of irregular antibodies. Results Among 29 770 patients, 120 cases had irregular antibodies of blood group.The positive rate was 0.4%.The irregular antibodies include Rh system(n=67),MNS system (n=7), Lewis system(n=5),cases of Kidd system(n=3) and undefined specified antibodies(n=38).Among the 120 antibody-positive patients, 70 were female patients and 50 were male patients. Further study showed that the pregnancy history was to the main factor causing irregular antibodies in women (46 positive cases among 70 female patients) . Conclusion The study showed that some patients had irregular antibodies of blood group, especially female patients with pregnancy history.These results suggested that irregular antibody screening and cross-matching by micro column gel method in pa-tients before transfusion had great significance for ensuring the blood transfusion safety.
8.Ambispective Cohort Study on Treatment of Cervical Spondylotic Myelopathy with Qi-blood Regulation Method

This study was aimed to observe the natural history and evolution of cervical spondylotic myelopa-thy , explore development law of the disease incidence in order to improve the understanding of cervical spondylotic myelopathy and provide a theoretical basis for Chinese medicine treatment . Complete clinical data of 80 cervical spondylotic myelopathy cases were collected from March 1998 to July 2007 in special and ex-pert practice of Prof . Shi's out-patient department . Chinese medicine for q i-blood regulation was given in the treatment . Related measuring scales were designed . More than two-month follow-up was given through means of returning visit, telephone calls and letter inquiries. Changes of clinical symptoms, and scores of different types of evaluations were observed . The SPSS software was used in the data analysis of results from measuring scales for the objective evaluation . The results showed that there were statistical differences on the 40-score and JOA total score between initial visiting and first time follow-up ( P < 0 . 05 ) . It suggested that through Chinese medicine qi-blood regulation treatment, the scores on spinal cord function were increased compared with the previous visiting . For patients who continued long-term Chinese medicine q i-blood regulation treat-ment, there were statistical differences on the 40-score and JOA total score between the first-time follow-up and second-time follow-up ( P < 0 . 05 ) . It suggested that through continued long-term Chinese medicine q i-blood regulation treatment , the scores on spinal cord function were increased compared with the previous visit-ing . In this group of patients , there were no statistical differences on the 40-score and JOA total score be-tween the patient's initial visiting , first time follow-up , and the first-time follow-up and second-time follow-up . The scores on spinal cord function suggested that there was no obvious difference on the relieving degree between the first-time follow-up and second-time follow-up . For patients who were unable to continue the Chinese medicine qi-blood regulation treatment, no matter if it was full score on the spinal cord function in the first-time follow-up , there were statistical differences on the 40-score and JOA total score between the first-time and second-time follow-up . It suggested that the disease became worse when patients were unable to continue the Chinese medicine qi-blood regulation treatment. It was concluded that Chinese medicine qi-blood regulation treatment for cervical spondylotic myelopathy has a significant effect in both short-term and long-term treatment with steady effectiveness. Patients who continue the Chinese medicine qi-blood regulation treatment not only can prevent disease recurrence , but also can promote further relieving of disease condition .
9.Pulmonary membrane diffusing capacity and pulmonary capillary blood volume in patients with stable COPD

Objective To study the clinical significance of pulmonary membrane diffusing capacity(Dm) and pulmonary capillary blood volume(Vc) in patients with stable chronic obstructive pulmonary disease(COPD). Methods Spirometry was performed in 38 patients with stable COPD and 35 healthy individuals in resting condition.The changes of pulmonary parameters were obtained and compared between groups. Results Spirometry test revealed that the percent predicted forced expired volume in one second(FEV1),FEV1/forced volume capacity(FVC)and the percent predicted maximal ventilatory volume(MVV) were declined from stage Ⅰin patients with COPD in comparison with healthy individuals,while diffusing capacity for carbon monoxide of lung(DLCO),carbon monoxide diffusing capacity per liter of alveolar(DLCO/VA),Dm and Vc were declined from stage Ⅱ.Dm in patients with COPD of stageⅠwas sig-nificantly decreased compared with the controls,while Vc was increased compared with the controls(both P
10.Chinese expert consensus on the diagnosis and treatment of IgG 4 related diseases

IgG 4 related disease (IgG 4-RD) is an immune medicated rare disease, characterized with chronic inflammation and fibrosis in the involved organs, it is a systemic disease affected nearly every anatomic site of the body, usually involvement of multiple organs, and with diverse clinical manifestations. Due to the the relative novelty of the disease and under-recognition, the overall level of diagnosis and treatment in China is uneven. Till now, there is no relevant expert consensus or guidance of IgG 4-RD in China. In order to further improve the understanding and standardize the management of IgG 4-RD, on the basis of summarizing domestic and international experience, the China Alliance For Rare Diseases, together with the Chinese Rheumatology Association, organized an expert group and established the Chinese expert consensus on the diagnosis and treatment of IgG 4 related diseases.
