Objective: To compare the advantages and disadvantages of the intraluminal suture, electrocautarization of middle cerebral artery combined with unilateral ligation of common carotid artery and electrocautarization of middle cerebral artery combined with bilateral ligation of common carotid artery for establishment of focal cerebral ischemia models in rats. Methods: The focal cerebral ischemia rat models were induced by 3 means. Twenty rats were randomly allocated into intraluminal suture group (n = 6), electrocautarization with unilateral ligation group (n = 7), and electrocautarization with bilateral ligation group (n = 7) according to the different methods for establishment of the rat model. A laser Doppler flow metry was used to detect the relative values of the cerebral blood flow before and after cerebral infarction. The neurological deficit scores of models were measured after 24 h, and the brain slices were stained with 2,3,5-triphenyl tetrazolium chloride. The volume of cerebral infarction was calculated. Results: Circled digit oneThe neurological deficit scores in the intraluminal suture group (9.6 ± 0.6) and the electrocautarization with bilateral ligation group (9.8 ± 0.6) were higher than those in the electrocautarization with unilateral ligation group (5.7 ± 0.7). The differences were statistically significant (P < 0.01). Circled digit twoThe volume of cerebral infarction in the intraluminal suture group (131.5 ± 15.8) mm3 and the electrocautarization with bilateral ligation group (141.7 ± 19.8) mm3 were larger than that in the electrocautarization with unilateral ligation group (39.9 ± 11.4) mm3. The differences were statistically significant (P < 0.01). Circled digit threeThe relative value of cerebral blood flow (CBF) in the electrocautarization with bilateral ligation group was lower than that in the electrocautarization with unilateral ligation group. The difference was statistically significant (P < 0.05). After reperfusion, the relative value of CBF in the intraluminal suture group was higher than that in the electrocautarization with bilateral ligation group. The difference was statistically significant (P < 0.05). Conclusion: The three methods had their advantages and disadvantages in inducing focal cerebral ischemia model in rats. A different model should be used according to the purposes of research.

Objective To investigate the efficacy and safety of autohemotherapy therapy in ASST positive chronic idiopathic ur‐ticaria .Methods One hundred and twenty cases of chronic idiopathic urticaria treated in our department from April ,2012 to Janu‐ary ,2013 were divided into treatment group and control group ,60 cases in each group .Patients in treatment group were given oral ioratadine (10 mg/d) ,supplemented by autohemotherapy ;patients in control group were only given oral ioratadine (10 mg/d) .Both of the two groups were treated with twelve weeks ,then observed the effectiveness and safety of two kinds of treatment .Results The effective rate of treatment group and control group were 68 .33% and 48 .33% respectively ,and there was significant difference between the two groups (P<0 .01) .Conclusion Autohemotherapy combined antihistamine was of high efficiency and safety in the treatment of ASST positive chronic idiopathic urticaria ,while the long‐term curative effect remains to be observed .

Objective To analyze the mutations in nucleotide sequences of transfusion transmitted virus(TTV) in neonatal infection.Methods Neonatal serum TTV-DNA was detected by a nested PCR technique.Fifteen Chinese neonates with positive TTV-DNA were diagnosed as TTV infection.ORF1 sequences of TTV-DNA from these neonates were determined.Results Homology of Chinese TTV(C01-C15) and Japanese TTV(N22)isolated ranged from 87.1%-97.7% at nucleotide level,but there were point mutations in Chinese TTV,such as GG→TT in locus 112 and 113,TTATC→CCTAT in locus 236-240.Conclusions Chinese and Japanese TTV isolated had the same genotype.Some gene mutations may increase the TTV pathogen,and result in neonatal hepatitis syndrome or hyperbilirubinemia.

Objective To compare the diagnostic value of 64-slice helical CT cholangiography and MR cholangiopancreatography (MRCP) for pancreaticobiliary obstructive diseases. Methods Thirty-six patients with pathologically proved pancreaticobiliary obstruction or endoscopic retrograde cholangiopancreatography (ERCP) were examined with MRCP and routine enhanced CT scanning. CT row data of portal venous phase were reconstructed with 0.625 mm thickness and intervals. Then multiplanar reformation (MPR) of intra- and extrahepatic biliary duct, gallbladder and pancreas was generated, and curved planar reformation (CPR) was performed when necessary. The accuracy of MPR (and CPR) and MRCP in evaluating the site and nature of obstruction was compared. Results The accuracy of MPR and MRCP was 97.22% and 94.44% in evaluating the site of obstruction, respectively. In evaluating the nature of obstruction, the accuracy of MPR and MPCP was 83.33% and 80.56%, respectively, and the accuracy of MPR increased to 88.89% in combination with CPR in some patients. There was no statistical difference between the accuracy of MPR and MRCP in evaluating the site and nature of obstruction, while their diagnostic consistency was medium (Kappa=0.471). Conclusion Both MSCT cholangiography and MRCP have high diagnostic value in pancreaticobiliary obstruction, while the former gets some advantages in images review for clinicians.

Objective To investigate the relationship between ultrasonographic features of papillary thy-roid carcinoma(PTC)and lateral cervical lymph node metastasis (LCLNM).Methods A total of 449 eligible ca-ses with 633 PTC nodules was selected and divided into two groups:LCLNM group included 135 nodules(91 pa-tients);Without metastatic groups included 498 nodules(135 patients).Ultrasonographic features of PTC nodules and the relationship with LCLNM were analyzed by χ2 test and logistic regression analysis .Results In univariate analysis,the significant factors were male,age<45yrs,multifocal,maximal diameter(d>1 cm),multi-microcal-cificaltion(d<2 mm),mixed flow,wider than tall,CCLNM,extrathyroid extension,located in upper(P<0.05 or P<0.017).Multivariate analysis showed that multi -microcalcificaltion(d<2 mm),wider than tall,CCLNM,ex-trathyroid extension,located in upper pole were predictive factors of LCLNM (P<0.005,OR=1.626,2.644, 0.479,2.579).Conclusion High-frequency ultrasound can accurately describe the sonographic appearance of thyroid nodules;it can be used to remind the risk of LCLNM before operation ,and will be informative for the treat-ment of surgery .

Objective To investigate the preoperative management and the clinical effeciency of Lichtenstein tension-free hernioplasty in adult patients with inguinal incarcerated hernia.Methods Clinical data of 86 patients with inguinal incarcerated hernia were analyzed retrospectively.Hernia was repaired with Lichtenstein tension free after reposition.Results There were 59 male patients and 27 female patients with median age of (63 ± 18) years.There were 8 patients with liver cirrhosis.The operation was performed successfully in all patients.Segmental bowel resection with end-to-end anastomosis was performed in 38 emergency cases.Operative time was 20-120 min,with an average time of 54 min.The postoperative hospitalization was 5-17 d,with an average of 8 d.There were 7 cases of skin ecchymosis at the scrotum,there were no intestinal perforation,hepatic encephalopathy and upper gastrointestinal hemorrhage after operation.In early series of 24 cases without drainage tube left in place,there were 10 cases of fat liquefaction,10 cases of hydrops of hernial sac,6 cases of seroma and 3 cases of wound infection after operation.After 12 to 48 months of follow-up,there was no mortality after 2 years,no hernia recurrence.Conclusions Tension free repair in the treatment of incarcerated inguinal hernia is safe and feasible.

Background Excimer laser corneal refractive surgery is widely used because it reduces the dependency of refractive error eye to glasses.However,the adverse effects of anti-inflammatory drugs is a problem after operation.Lotemax eyedrops is a kind of steroidal anti-inflammatory drug,with a good effect on the operative eye.But its influence on intraocular pressure(IOP) after long-term topical administration is less studied.Objective This study was to investigate the influence of lotemax eyedrops on IOP after long-term topical administration in myopic eyes received excimer laser corneal refractive surgery.Methods An one-month retrospective case serial study was adopted.The case history data of 1552 eyes from 1552 Patients who received excimer laser corneal refractive surgery were collected in Tangshan Eye Hospital from 2011 January to 2012 January,including 321 eyes with laser in situ keratomileusis(LASIK),608 eyes with sub-Bowman keratomileusis (SBK) and 623 eyes with femtosecond laser for flap excimer laser in situ keratomileusis(FS-LASIK).Lotemax eyedrops was topically administered after operation 4 times per day initial and then decreased 1 time weekly for 1 month.IOP was measured in before,1 week,2 weeks,3 weeks,4 weeks after operation with non-contact tonometer.The increasing range of IOP was divided into ＜5 mmHg,5-15 mmHg and ＞ 15 mmHg after operation.Optical head parameters were measured by Heidelberg retina tomography when IOP increasing after operation.Results Within 1 month,elevated IOP was found in 47 of 1552 eyes after administration of lotemax eyedrops,including 3.12％ (10/321) in the LASIK group,3.29％ (20/608) in the SBK group and 2.73％ (17/623) in the FS-LASIK group,without significant difference among them(x2 =1.95,P＞0.05).The IOP elevated 1 week after operation and remained normal after that till drug withdrawal in all eyes.There were no significant differences in the disk area (1.65 ±0.44) mm2 vs.(1.66 ±0.44) mm2,disk volume (0.40 ±0.09)mm3 vs.(0.39±0.08) mm3 and mean nerve fiber layer thickness (0.28 ± 0.08) mm vs.(0.29 ± 0.07) mm in IOP elevation duration compared with 1 month after IOP recovery(t =0.34,0.81,0.48,P＞0.05).Conclusions Topical administration of 0.5％ lotemax eyedrops after excimer laser corneal refractive surgery leads to reversible high IOP.It is very important to monitor the IOP during use of the drug.
