1.Effects of xinshuai mistura on plasma angiotensin II, atrial natriuretic peptide and N terminal pro-brain natriuretic peptide in patients with heart failure.

OBJECTIVETo observe the effects of Xinshuai Mistura (XM) on plasma angiotensin II (Ang II), atrial natriuretic peptide (ANP) and N terminal pro- brain natriuretic peptide (Nt-proBNP) in patients with heart failure (HF).
METHODSFifty-nine patients with HF (NYHA class II-IV) were randomly divided into two groups, the treated group treated with XM and the control group with Wuling Pulvis (WP), both on the base of captopril treatment. The treatment course was two weeks. Plasma levels of Ang II , ANP and Nt-proBNP were observed.
RESULTSThe plasma levels of Ang I, ANP and Nt-proBNP showed an ascending tendency along with the increasing in severity of HF(P < 0.01), and significantly decreased after treatment in the treated group (P < 0.01), but with no remarkable difference as compared with those in the control group (P > 0.05).
CONCLUSIONXM had definite therapeutic effects on excessive neuroendocrine activation in HF patients.

2.Study on the competence of master of public health

A questionnaire was used to survey the perspective of the 22 mentors,24 students, 27 graduate students and 27 public health units on the competence of MPH student. The result showed that the practice skill,analyzing skill,program plan and practice skill,and emergency reaction abil-ity got more attention. This suggested that to the training of MPH,the training objective should focus on the ability to resolve problems; the curriculum should focus on the characters of public health and on the practice training.
3.Effects of anti-somatisation therapy on the quality of life in patients with functional dyspepsia

Objective To explore somatic symptoms of patients with functional dyspepsia (FD) and effects of combined anti somatisation therapy on the quality of life in patients with FD.Methods Two hundred and nineteen patients with FD were enrolled.Patient Health Questionnaire-15 (PHQ-15) and Nepean Dyspepsia Index (NDI) were completed,and the relation between them was analyzed.Lansoprazole,mosapride and flupentixol melitracen (the anti-somatisation medication) were taken for four weeks by 131 FD patients who had poor response to four-week conventional treatment with proton pump inhibitors (PPI) and/or prokinetic medication and who got more than 4 scores in PHQ 15.And then therapeutic effects were observed and the changes of NDI before and after treatment were analyzed.The t test,chi-square test,Spearman's correlation and linear regression were performed for statistical analysis.Results The total PHQ-15 and NDI score of 219 patients with FD were 5.7±3.4 and 42.0± 10.4,and there was positive correlation between them (r=0.493,P<0.05).Stomach pain,headache,chest pain,dizziness,shortness of breath,fatigue,sleeping disorder,thirst and so on were positively correlated with totalNDIscore (r=0.262,0.230,0.241,0.243,0.352,0.385,0.266,0.281,all P<0.05).Chest pain,stomach pain,shortness of breath,dizziness and fatigue were the influence factors of NDI (t=2.340,2.488,5.278,2.167,2.443,all P<0.05).Among 131 patients with FD who received anti somatisation combination therapy,six patients did not complete the therapy because of the side effects,and finally 125 patients were enrolled and analyzed.Before and four weeks after the treatment,the total PHQ-15 scores of 125 patients with FD were 7.3±3.2 and 2.9±2.2,respectively; and the difference was statistically significant (t =12.653,P<0.01).There were significant differences in the influence of stomach pain,headache,chest pain,dizziness,shortness of breath,fatigue,sleeping disorder and thirst before and after the treatment (not bothered at all,bothered a little,bothered a lot) (x2=39.231,6.796,6.693,15.520,6.698,51.572,44.390 and 16.506,all P<0.05).The total NDI score before and after the treatment were 44.3 ± 11.7 and 29.2 ± 6.9,and the difference was statistically significant (t=12.503,P<0.01).Conclusions The quality of life in patients with FD is affected by somatic symptoms such as chest pain,stomach pain,shortness of breath,dizziness,fatigue and so on.For FD patients with poor response to conventional treatment,the quality of life could be improved by the combination of anti-somatisation therapy and the conventional therapy.
4.Effect of Probiotics on Feeding Intolerance in Infants with Very Low Birth Weight

Objective To explore effect of probiotics on improving the feeding intolerance in very low birth weight infants(VLBWI). Methods Sixty cases of VLBWI were randomly divided into two groups, 30 infants as therapy group,another 30 infants as control group.All infants in two groups were given aggressive intravenous nutrition and enteral feeding with preterm formulars. At the same time, probiotics were administered combined with preterm formulars to infants in therapy group. Objective To evaluate the training quality of anaesthesiology residents in different hospitals. Methods The scores of 125 residents,attending Shanghai resident clinic skill examination from 2005 to 2007,were analyzed. ResultsThe scores of examinees from municipal hospitals were higher than those from district hospitals,municipal specialized hospitals and traditional Chinese medicine hospitals.The scores of practice part were higher than those of theory part in all groups. Conclusion The training quality is related to the comprehensive strength of the hospital in which examinees work,but the training of theory knowledge still need to be strengthened.

Objective To investigate the relationship between apolipoprotein (Apo) e4 allele and endothelium dependent arterial dilation in females with type 2 diabetes mellitus. Methods One hundred and one female type 2 diabetic patients without angiopathy, aged 40 71 years, and 95 healthy female individuals were selected. PCR/allele specific oligonucleotide probes were used to determine Apo e genotypes, and high resolution ultrasound was applied to measure brachial artery flow mediated endothelium dependent dilation and glyceryltrinitrate (GNT) mediated endothelium independent dilation. Objective To investigate the value of intraoperative ultrasound in the resection of hemangioblastoma.Methods Intraoperative ultrasound was applied in 17 patients who underwent resection. The size,number,location ,depth,feeding arteries and draining veins of the tumors were clearly displayed.Results Intraoperative ultrasound could real-time locate the tumor with high accuracy. Totally there were 35 tumors in 17 patients, and intraoperative ultrasound found 32 of them.The diameter of thr smallest tumor was 7 mm. Intraoperative ultrasound could displayed clearly the feeding arteries and draining veins in bigger tumors. Conclusions Intraoperative ultrasound should be routinely used in the operation of hemangioblastoma for its high detection rate.

Objective To study the feasibility and characteristic findings of contrast-enhancedultrasound(CEUS)on peripheral pulmonary tumors.Methods Twenty patients with peripheral pulmonarytumors proven by pathology were studied.They were divided into two groups:primary pulmonary tumors (n=15),and metastatic pulmonary tumors(n=5). The dynamic enhancement images were stored and Strip enhancement was dominant in the primary pulmonary tumors,while dot enhancement was dominant in ascended quickly and descended slowly,while ascended slowly and descended slowly in majority of metastaticThe modes of enhancement and time-intensity curves were all different in primary and metastatic pulmonary tumors.Contrast-enhanced uhrasound is promising in the diagnosis of peripheral pulmonary tumors.
