1.In Vitro Study on the Transdermal Permeation Enhancing Effect of N-trimethyl Chitosan on Estradiol Gels

OBJECTIVE: To study the enhancing effect of N-trimethyl chitosan with degree of quaternization of 60% (TMC60) on the estradiol gel in vitro. METHODS: TMC60 was synthesized and the degree of quaternization was determined by 1H-NMR. Three different kinds of estradiol gels, 2% TMC60 group, 2% azone group and blank control group, were studied respectively. The cumulative penetration Q and steady penetration rate J in vitro were used as the indices. The cumulative penetration was calculated. RESULTS: The degree of quaternization of TMC60 was 67.2%. Significant difference was found the value Q and J of TMC60 group when compared to blank control group(P0.05). CONCLUSION: 2% TMC60 is a penetration enhancer with similar enhancing effect as 2% Azone, which deserves further research.
2.Application of antihistamines in special populations

Antihistamines,a group of drugs most commonly used for the treatment of allergic diseases in dermatology,exert favorable efficacy and are well tolerated in most people.Due to their wide application,the safety of medication is particularly important.When they are used in some special populations with allergic diseases,such as children,pregnant and lactating women,the elderly and people with hepatic or renal insufficiency,their pharmacodynamics,metabolic characteristics and interactions with other drugs should be fully considered,and profits of medication and potential adverse effects should be well weighed before choosing relatively safe antihistamines.In addition,decreasing the routine dose or prolonging intervals between the administration of antihistamines may also be attempted to achieve maximum safety.
3.Preparation and Quality Evaluation of O/W-Type Nifedipine Transdermal Nanoemulsion

Objective:To optimize the formula of nifedipine nanoemulsion (NFNE) to lay foundation for the development of nifedipine transdermal preparation.Methods:The content of nifedipine as the index,the compositions in the formula were screened out by single factor experiments,and then the formula of NFNE was optimized by orthogonal tests with the ratio of emulsifier to co-emulsifier,ratio of mixed emulsifiers to oil phase and the amount of water as the influencing factors.N-Trimethyl chitosan (TMC) was used as the transdermal enhancer.The formula of TMC-NFNE was optimized by orthogonal tests with the degree of quaternization of TMC,the concentration of TMC solution and the amount of TMC solution as the influencing factors.The quality evaluation of TMC-NFME was determined including appearance,type identification,particle size and drug content.Results:The optimal formula was as follows:7% ethyl oleate,32% Tween 80,16% alcohol,45%water and the mass ratio of TMC of 0.45%.The type of the emulsion was O/W.The content of NFNE,TMC20-NFNE,TMC40-NFNE and TMC60-NFNE was (12.76±0.22),(14.76±0.16),(12.26±0.18) and (12.92±0.27) mg·ml-1,respectively.The particle size was 13.62,17.10,16.26 and 17.25 nm,respectively.The morphology of the nanoemulsion was spherical.Conclusion:The optimal formula and preparation process of TMC-NFNE are simple with high drug content,which is worthy of further studies.
4.Application of contrast-enhanced ultrasound in brain perfusion

Ultrasound brain perfusion imaging can evaluate brain perfusion in normal and pathological conditions. After basilar stent implantation, contrast-enhanced ultrasound (CEUS) can show normal visualization of color flow within the stent and provide essential functional information by evaluation of blood flow velocities. Furthermore, CEUS can estimate the volume of brain hemorrhage and readily identify residual tumor. CEUS can be used in thrombolysis, and will be applied in cerebral trauma. The value of CEUS in brain perfusion imaging was reviewed in this article.
5.Advance in Rehabilitation of Upper Limb Function in Hemiplegic Patients after Stroke (review)

The incidence of upper limb dysfunction of stroke patients was high, and the recovery was difficult, which seriously impact-ed their activities of daily living and quality of life. This article reviewed the new techniques and methods for hemiplegic patients after stroke. The main techniques and methods includeed bilateral isokine-matic training, mirror neuron system, gymnastics rods training, music therapy, motor imagery, upper limb rehabilitation robot, virtual reality, constraint induced movement therapy, functional electrostimulation, electromyographic biofeedback therapy, transcranial magnetic stimulation, neurophyisological therapy and so on.
6.In vitro Corneal Permeation and Antibacterial Activity of N-Trimethyl Chitosan-coated Sparfloxacin Lac-tate Nanoliposomes

Objective:To study the in vitro corneal permeation and antibacterial activity of N-trimethyl chitosan(TMC)-coated sparfloxacin lactate(SL)nanoliposomes. Methods:Franz diffusion cells were used with rabbit cornea as the barrier to study the in vitro corneal permeation of TMC-coated SL nanoliposomes,and the permeation parameters were calculated. Eseheriehia eoli,staphylo-eoeeus aureus,pseudomonas aeruginosa and baeillus subtilis were used as the tested bacterial strains and the in vitro antibacterial activity of the SL preparations was investigated to obtain the minimum inhibitory concentration(MIC),the minimum bactericidal concentration (MBC)and the relationship of bacterial inhibitory rate and time. Results:The order of steady permeation rate(J),corneal perme-ation coefficient(P)and corneal retention rate was TMC-coated SL nanoliposomes > SL nanoliposomes > SL eye drop. The order of lag time(τ)and corneal diffusion coefficient(D)was SL nanoliposomes > SL eye drop > TMC-coated SL nanoliposomes. The in vitro an-tibacterial activity showed the order of TMC-coated SL nanoliposomes > SL nanoliposomes > SL eye drop. Conclusion:Compared with the eye drop,liposomes can enhance the corneal permeation and storage of SL with improved antibacterial activity,and TMC-coating can further improve the permeation and antibacterial activity,which is worthy of further study.
7.Reflections on the Operating Mechanism of Experimental Teaching in Preclinical Period

To find a good model for experimental teaching in medical college we established five experimental teaching centers five years ago,and up to now the experimental teaching centers are operated well,and the teaching environment has been improved,the teaching cost has been reduced,and teachers'research abilities enhanced.What's more,the students'hands-on abilities are well developed,and more attention has been paid to experimental teaching.
8.Surgical effect observation of lens dislocation greater than 2 quadrants

AIM: To investigate the clinical effect of cataract combined lens dislocation greater than 2 quadrants` patients after having phase I intracapsular cataract extraction and anterior vitrectomy,postoperative corrected vision greater than 0.3,and then taking phase II small incision suture-fixation of intraocular lens suspension.METHODS: Totally 34 cases with cataract combined lens dislocation greater than 2 quadrants had been treated in our hospital from July 2014 to December 2016.We investigated their 1wk,1 and 3mo postoperative complications and the uncorrected visual acuity,best corrected visual acuity,intraocular pressure,corneal astigmatism who took phase II small incision suture-fixation of intraocular lens suspension with corrected vision greater than 0.3 after having phase I intracapsular cataract extraction and anterior vitrectomy.RESULTS: Along with the recovery time extension,patients` uncorrected visual acuity and best corrected visual acuity increased obviously than preoperative in each phase.On the 3mo postoperatively,the best corrected visual acuity of 1 eyes was between 0.3 and 0.1,8 eyes was between 0.5 and 0.3,16 eyes was between 0.7 and 0.5,and 9 eyes was better than 0.7.The best corrected visual acuity was achieved or near the best corrected visual acuity before surgery.Intraocular pressure rechecked became in the normal range at 1wk,1 and 3mo postoperative.Surgery did not significantly increase the corneal astigmatism.CONCLUSION: It is a safe and effective way in improving eyesight effectively,with stable intraocular pressure and fewer complications towards cataract combined lens dislocation greater than 2 quadrants` patients having phase I intracapsular cataract extraction and anterior vitrectomy,whose postoperative corrected vision greater than 0.3,and then phase II small incision suture-fixation of intraocular lens suspension.

10. Application of 64-slice spiral coronary CT angiography in diagnosis of restenosis in patients with coronary heart disease after stenting

Objective: To evaluate the clinical value of 64-slice spiral coronary CT angiography (CTA) in diagnosis of in-stent restenosis (ISR) after percutaneous coronary intervention (PCI) in patients with coronary artery disease. Methods: CTA was used to reconstruct and analyze the 345 segments (each stent was divided into three segments: proximal,middle and distal) of 115 stents in 60 patients with coronary heart disease after PCI. The results of selective coronary angiography (SCA) were taken as the golden standard to evaluate the sensitivity and specificity of CTA in diagnosis of ISR after PCI. Results: CTA clearly showed the location and length of the stents,the stenosis at stent and the characteristics of restenosis plaques. Thirty-nine in-stent restenosis lesions were found by CTA, including 8 calcified lesions and 31 non-calcified lesions. There were 25 lesions at the proximal end of the stent, 7 at the middle and 7 at the distal end. Forty-two lesions were found by CTA, including 9 calcified lesions and 33 non-calcified lesions, with 26 at the proximal end, 8 at the middle and 8 at the distal end. CTA correctly diagnosed 36 segments, missed 6, and misdiagnosed 3. The sensitivity, specificity,PPV,NPV and accuracy of CTA were 85.71%,99.01%,92.31%,98.04% and 97.39%, respectively, CTA had the highest sensitivity and specificity for diagnosis of the proximal ISR,being 96.15% and 100%, respectively. Proximal stent restenosis accounted for 64.10% of the total. Conclusion: CTA can clearly demonstrate the in-stent restenosis and has a high accuracy in diagnosing restenosis after PCI. CTA is a safe, simple and reliable noninvasive diagnostic method for diagnosis of in-stent restenosis after PCI.
