2.Gene mutation and enzymatic activity analysis of α-galactosidase A in a patient with Fabry disease

Objective To analyse the clinical characteristics,gene mutation and enzymatic activity of αgalactosidase A(α-GalA)in a 15-year-old male patient with typical Fabry disease,whose mother was without any clinical manifestations.Methods Clinical features and laboratory data were collected from the patient and his mother.Genomie DNA was extracted from peripheral blood of the patient.his mother,and a healthy control subject.Seven exons of the GLA gene were amplified by PCR.PCR products were purified.cloned into T vector,and then sequenced.The enzymatic activity of α-GalA Was measured by fluorimetrie substrate assay. Results DNA sequencing results showed that a missense mutation of 10036-10038delAAG in exon 7 WaS identified in the patient,resulting in the replacement of 374 lysine and 375 glyeine by arginine,which Was not previously reported.The patient Was a hemizygote with gene mutation,his mother WaS a heterozygote carrying gene mutation,and the healthy control without mutation.α-GalA enzymatic activity assay showed that the enzymatic activity of the patient with GLA gene mutation was only 50%of the healthy control subject,while the enzymatic activity of the patient's mother Was about 70%of the heahhy control SObject.Conclusiolls Detecting GLA gene mutation and α-GalA enzymatic activity in patients with Fabry disease who have been clinically diagnosed seelns to be helpful in finding other patients in the family and in further understanding the molecular pathogenesis of that disease.
3.Expression of SCC-Ag and CEA and its clinical significance in patients with lung cancer

Objective To investigate the ex pr ession of squamous cell carcinoma antigen (SCC-Ag) and carcinoembryanic antigen (CEA) and its clinical significance in patients with lung cancer. Meth ods The pre-treatment serum from 134 patients with lung cancer from 1998 to 2002 were analyzed for the SCC-Ag level and CEA level by IMX, and the correlation of the SCC-Ag level and CEA level with the clinicopathologic charac teristics were also detected. Results Significant correlations were found between the pre -treatment SCC-Ag level, and CEA level and the pathologic classification, clin ical stage, the positive rate of diagnosing lung cancer was raised by combined a ssay with serum SCC-Ag and CEA. Conclusion Pre-treatment and after-treatment measurement of serum SCC-Ag level and CEA level can be used for diagnosis, prediction of pr ognosis and monitor of after-treatment recurrence in patients with lung cancer.
4.Application of pre-hospital emergency care intervention procedures in patients with acute cerebral hemorrhage

Objective To explore the effect of emergency pre-hospital care intervention procedures on patients with acute cerebral hemorrhage treatment. Methods One hundred and eighty patients with acute cerebral hemorrhage rescued in the ambulance of our hospital from June 2011 to June 2014 were selected as the observation group , and another 120 patients of acute cerebral hemorrhage escorted by family members to our hospital during the same period were set as a control group. The patients in the observation group were given guidance nursing, on-site emergency care, emergency care and hospital transit emergency care, and the patients in the control group were given rescue in the hospital. The two groups were compared in terms of effective transit time. Results The effective transit time for the patients in the observation group was (0.87 ± 0.19)h, significantly shorter than the control group (1.26 ± 0.35) h (P<0.05). The complication rate in the observation group was 22.22%, significantly lower than that of the control group (54.17%) (P<0.05). Conclusion Pre-hospital emergency care intervention for patients with acute cerebral hemorrhage can effectively shorten transit time, reduce complications and improve outcome and improve survival, worthy of promotion.
5.The analysis of clinical features of 140 cases with primary hyperparathyroidism

Objective:To compare the clinical characteristics in primary hyper(-) parathyroid hormone (PHPT) of the different patholog-ic types. Methods:Clinical data of 140 patients with PHPT proved by operation and pathology during January 2010 to June 2016 were retrospectively analyzed. Results:A total of 140 PHPT patients, including 13 (9.29%) cases of parathyroid carcinoma (PC), 27 (19.29%) cases of parathyroid hyperplasia (PH), and 100 (71.43%) cases of parathyroid adenoma (PA). The duration of the PC group was longer than the PH group and the duration of the parathyroid adenoma (PH) group was longer than the PA group (P<0.05). The percentage of young patients with PC was higher than in the other two groups (P=0.003). The diameters of the PC group were larger than those of the other two groups, and those of the PA group were larger than those of the PH groups (P<0.05). Blood calcium, parathyroid hor-mone (PTH), AKP, fasting blood glucose (FBG), alanine aminotransferase (ALT), aspartate aminotransferase (AST), glutamete transpepti-dase (GGT), blood urine nitrogen (BUN), creatine (CRE), urinary calcium, and phosphorus of the PC group were higher than in the oth-er two groups (P<0.05). The blood calcium, PTH, alkaline phosphatase (AKP), urinary calcium of the PH group were lower than those in PA group (P<0.05). The proportion of ostalgia was 46.15%(6/13), 44.44%(12/27), and 49.00%(49/100). No statistical difference was observed (P>0.05). The postoperative calcium level of PC group was lowest (P<0.001), and the highest was of PTH (P<0.001). The pro-portions of clinical manifestation of the urinary system, digestive system, and nervous system in the PC group were 76.92%(10/13), 76.92%(10/13), and 15.38%(2/13), respectively, and these values were the highest in the three groups (P<0.05). The proportion of the clinical manifestation of the urinary system of the PH group was higher than that of the PA group. The fracture rate (30.77%, 4/13) and constipation rate (38.46%, 5/13) of the PC group were the highest among the three groups (P<0.05). Conclusion:The duration of patients with PC was the longest among the three groups. The percentage of young patients with PC was the highest. The abnormal parathyroid glands in the PC group were the heaviest. The PC group exhibited the lowest postoperative calcium level and the highest PTH level. The biochemistry and clinical manifestations of PC were obvious.
6.Analysis of Electrocardiographic Characteristics in 84 Patients With Primary Hyperparathyroidism

Objective: To analyze the electrocardiographic (ECG) characteristics in patients with primary hyperparathyroidism (PHPT) and meanwhile, to explore the relationship between ECG abnormality and serum calcium level in relevant patients. Methods: Our study included 2 groups: Observation group,n=84 PHPT patients treated in our hospital from 2010-01 to 2014-04 and Control group,n=80 normal subjects from regular physical examination at the same period. The ECG manifestation was compared between 2 groups. Meanwhile, according to tertile levels of serum calcium, PHPT patients were further divided into 3 subgroups:①Ca2+ level (2.0-2.8) mmol/L,②Ca2+ level (2.8-3.1) mmol/L,③Ca2+ level >3.1 mmol/L andn=28 in each subgroup. The ECG abnormalities were compared among 3 subgroups and the relationship between ECG changes and serum calcium levels were studied. Results: The ECG abnormality rates in Observation group and Control group were as 83.3% vs 18.8%, P<0.001. Compared with Control group, Observation group presented more frequent ST-T changes, QRS duration widening, high left ventricular pressure and QT interval shortening,P<0.001 orP<0.05. In observation group, with the rising of serum calcium, the incidences of ST-T changes and high left ventricular pressure were increased accordingly, the ST-T changes among 3 subgroups were significant,P<0.05; while with the rising of serum calcium, the QT interval was shortened and QRS duration was widened accordingly, the differences were obvious among 3 subgroups, allP<0.05.Conclusion: PHPT patients have the higher incidence with abnormal ECG, while ECG abnormality has beenrelated to serum calcium level in relevant patients.
7.Expression and significance of Smad4 in mice with hepatic alveolar echinococcosis ZHANG Ya-lou,MA

Objective To investigate the expression and significance of Smad4 in peripheral hepatocytes of lesions in mice with hepatic alveolar echinococcosis.Methods Eight mice in the test group were inoculated with alveolar echinococcosis and 8 mice in the control group were injected with normal saline.The expression of Smad4 protein in the hepatic tissue of the mice was detected by immunohistochemistry method,and the data were analyzed by chi-square test.The effect of alveolar echinocoeeosis on the expression of Smad4 protein was investigated.Results Smad4 was detected in cell nuclei and partly in the cytoplasm.Six months after the establishment of the mice model for alveolar echinococeosis,the expression of Smad4 in the hepatic tissue in the test group was significantly higher than in the control group(x2=19.869,P<0.05).The number of Smad4 with positive expression in the hepatocytes in the test group was slightly higher than in the control group,and the expression intensity of Smad4 in the test group was greater than in the control group(x2=58.3 10,P<0.05).Conclusions The increase of the expression of Smad4 protein in the periphery hepatocytes and tissues of hepatic alveolar echinococcosis lesions may play a role in hepatic cirrhosis and immune evasion.
8.Study on 1H-NMR fingerprinting of Rhodiolae Crenulatae Radix et Rhizoma.

Nuclear magnetic resonance (1H-NMR) fingerprint of Rhodiola rosea medicinal materials was established, and used to distinguish the quality of raw materials from different sources. Pulse sequence for water peak inhibition was employed to acquire 1H-NMR spectra with the temperature at 298 K and spectrometer frequency of 400.13 MHz. Through subsection integral method, the obtained NMR data was subjected to similarity analysis and principal component analysis (PCA). 10 batches raw materials of Rhodiola rosea from different origins were successfully distinguished by PCA. The statistical results indicated that rhodiola glucoside, butyl alcohol, maleic acid and alanine were the main differential ingredients. This method provides an auxiliary method of Chinese quality approach to evaluate the quality of Rhodiola crenulata without using natural reference substances.

9.Effect of integrative medical therapy on post-angioplasty arterial restenosis in lower limbs of diabetic patients.

OBJECTIVETo evaluate the effect of integrative medical therapy on lower limb post-angioplasty arterial restenosis in diabetes patients.
METHODSThirty-six patients with lower limb diabetic arterial disease of Fontaine III/IV stage after successful balloon angioplasty were assigned randomly and equally to two groups, the control group and the treatment group, both were treated with basic therapy for 3 months, but Chinese drugs were given to the treatment group additionally. Level of acroesthesia was determined before and after treatment with simplified McGill pain questionnaire (SF-MPQ) scoring, ankle brachial index (ABI), toe brachial index (TBI), mean flow velocity of dorsal artery (MDA) and tibial artery (MTA) were measured by ultrasound Doppler meter, and blood preparation was determined. Moreover, the ulcer remission rate, ulcer cure rate, restenosis rate, and amputation rate were determined in the 6-month and 12-month follow-up studies.
RESULTSSignificant improvements of ABI, TBI, MDA and MTA, as well as on blood levels of high-sensitivity C-reactive protein (hs-CRP) and fibrinogen (FIB) were shown in the treatment group after treatment, showing significant difference in comparing with those before treatment (P < 0.05 or P < 0.01), also in comparing with the control group (P < 0.05 or P < 0.01) respectively, while in the control group, although a trend of improvement was seen, these indices were insignificantly changed statistically (P > 0.05). Comparisons between groups after 3-month treatment showed significant difference in ischemia remission rate and pain relief rate (P < 0.05). Outcomes of 6-month follow-up displayed that the differences between groups were insignificant in terms of the ulcer cure rate the and amputation rate (P > 0.05), but did show significance in the ulcer remission rate and the restenosis rate (P < 0.05), while all the 4 parameters were insignificantly different between groups after 12-month follow-up (P > 0.05). Overall incidence of adverse event was somewhat higher in the treatment group, but there was no significant statistically difference.
CONCLUSIONIntegrative medical therapy could improve the hemodynamic indices and pain symptoms, raise the ulcer cure rate and reduce the incidence of restenosis in patients with diabetic arterial disease after balloon angioplasty.

10.Protective effects of ulinastatin on intestinal barrier damaged after cardiopulmonary resuscitation in rats

Objective To investigate the protective effects of Ulinastatin on intestinal barrier damaged after cardiopulmonary resuscitation (CPR) in rats in order to illustrate the possible mechanism.Methods Twenty-one male SD rats were divided into three groups randomly (random number) including control group (sham group, n =7), cardiopulmonary resuscitation group (CPR group, n =7) and ulinastatin group (UTI group, n =7).The rats were anesthetized with pentobarbital sodium (45-60 mg/kg) by intraperitoneal injection.The rats of sham group were only treated with endotracheal intubation.Ulinastatin (100 000 U/kg) were injected via caudal vein 2 hours prior to CPR, and cardiac arrest was made in rats and cardiopulmonary resuscitation was carried out in the UTI group, while equivalent volume of sterile saline was used instead in the CPR group.Blood and ileum samples were obtained at 48 hour after restoration of spontaneous circulation (ROSC).The levels of tumor necrosis factor alpha (TNF-α) and interleukin-1β (IL-1β) were assayed by ELISA (enzyme-linked immunosorbent assay), the protein levels of caspase-3 were determined by western blot, the intestinal mucosa were stained by terminaldeoxynucleoitidyl transferase mediated nick end labeling (TUNEL) and ileac mucosa were observed under transmission electron microscope.Data were processed with SPSS 17.0 software.Results The plasma levels of TNF-α and IL-1β were dramatically higher in CPR group than those in other two groups (CPR vs.sham, P < 0.01;CPR vs.UTI, P < 0.05).Moreover, the tight junctions between cells obviously broadened and loosened in the CPR group were found under electron microscope, however, this phenomenon was not obvious in the UTI group.A large number of apoptotic cells were observed by TUNEL assay in the CPR group, but a small number of apoptotic cells were observed in the UTI group.The protein levels of caspase-3 in the UTI group were higher than those in sham group, but lower than those in CPR group (both P < 0.05).Conclusions Ulinastatin has protective effects on the intestinal barrier damaged after cardiopulmonary resuscitation in rats by decreasing the proinflammatory mediators in the blood, reducing the expression of caspase-3and then reducing the numbers of apoptotic intestinal cells.
