3.Application of titanium cages and poly(ether-ether-ketone) cages in anterior cervical discectomy and fusion surgery

BACKGROUND:The two main cages used in anterior cervical surgery are titanium cages and poly(ether-ether-ketone) cages, but it is stil controversial in imaging performances and clinical outcomes.
OBJECTIVE:To systematical y evaluate imaging performances and clinical outcomes between titanium cages and poly(ether-ether-ketone) cages in anterior cervical discectomy and fusion surgery.
METHODS:The Cochrane library (issue 5, 2013), Medline database, EMBASE, CNKI and Wanfang database were retrieved by computer. The deadline of al the retrieves concerning control study of uses of titanium cages and poly(ether-ether-ketone) cages was June 1st, 2013. Two reviewers assessed literatures independently, and the meta-analysis was conducted by RevMan 5.2 software.
RESULTS AND CONCLUSION:A total of four literatures including 235 patients, 128 of titanium cages and 107 of poly(ether-ether-ketone) cages, were included. Meta-analysis results showed that there was no statistical y significant difference between the groups in fusion rates, subsidence and dislocation rates of short-term fol ow-up and clinical outcomes (al P>0.05). There were statistical y significant differences between the groups in subsidence and dislocation rates of long-term fol ow-up (P<0.05). Poly(ether-ether-ketone) cage group was superior to titanium cages group in maintenance of cervical curvature and intervertebral height. This meta-analysis showed that poly(ether-ether-ketone) cages had good fusion rates and clinical outcomes, less subsidence and dislocation rates, and can better maintain cervical curvature and intervertebral height compared with titanium cages.
4.The analysis of related factors leading to hip fracture in the elderly

Objective To observe the BMD threshold of hip fracture in the elderly, analyze the pre-imposing factors leading to femoral neck fracture or intertrochanteric fracture, and provide some evidences for fracture prevention. Methods The BMD examination were performed on 496 fractured patients over 60 years old which were grouped after admission according to their age, gender and fracture types, and then the results were statistically analyzed. Results The BMD of fracture cases in the elderly over 60 years old was 2.5 s less than peak bone volume. There is no obvious difference in BMD between femoral neck fracture and intertrochanteric fracture among various age groups. According to our study, most hip fractures happened at the age of 60-79 years, accounting for 73%of all hip fractures. Conclusions Decreased bone mass or osteoporosis is an important factor leading to hip fracture in the elderly. The fracture types (femoral neck fracture or intertrochanteric fracture) depend mainly on the impaction force to the hip, not only on the BMD change.

6.Correlation between tumor necrosis factor-α and interleukin-1β in exhaled breath condensate and pulmonary function in chronic obstructive pulmonary disease

Abstract: Objective To explore the correlation between lung function in patients with chronic obstructive pulmonary disease (COPD) and tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β) levels in exhaled breath condensate (EBC), and to provide a convenient methodological basis for the diagnosis and treatment of COPD and the determination of its efficacy. Methods A total of 81 COPD patients and 40 healthy controls were selected from the respiratory department of the Fourth Affiliated Hospital of Guangzhou Medical University from August 2020 to February 2022 as the research subjects. The COPD patients were divided into 41 cases in the acute exacerbation group and 40 cases in the remission group according to their status. All participants underwent lung function detection, venous blood and EBC collection, and the levels of TNF-α and IL-1β in EBC and venous blood were analyzed by enzyme-linked immunosorbent assay (ELISA), and correlation analysis was performed by Pearson correlation analysis method. Results The levels of TNF-α and IL-1β in EBC of in the acute exacerbation group, the healthy control group, the remission group were (5.16±0.18) pg/μL and (7.75±0.27) pg/μL, (2.66±0.31) pg/μL and (2.41±0.24) pg/μL, (3.61±0.29) pg/μL and (3.17±0.38) pg/μL, respectively. Compared with the healthy control group, the levels of TNF-α and IL-1β in EBC in the COPD acute exacerbation group were significantly higher than those in the healthy control group and the COPD remission group (F=9.451, 8.217, P<0.001). Serum tests were consistent with this result. Correlation analysis showed that the levels of TNF-α and IL-1β in EBC were significantly positively correlated with the level of serum inflammation levels (P<0.001), while significantly negatively correlated with lung function (P<0.001). Conclusions TNF-α and IL-1β in EBC are potential biomarkers of inflammation in patients with COPD, and their detection can be used to effectively assess lung function in patients with COPD.
7.Clinicopathological Analysis of 155 Patients with Persistent Isolated Hematuria

Objectives To reveal etiologies of persistent isolated hematuria (PIH) through ultrastructural pathological examination, to disclose clinicopathological correlation in cases with PIH, and to summarize appropriate management of patients with PIH.
Methods we retrospectively studied 155 PIH patients receiving renal biopsy between January, 2003 and December, 2008 in Peking Union Medical College Hospital. All the clinical data and follow-up result were analyzed.
Results All subjects included 38 children and 117 adults, with mean age of 11.38±3.25 years for children and 35.17±8.44 years for adults. Thin basement membrane nephropathy (TBMN) was the most common pathology (55.3% of children and 49.6% of adults), followed by IgA nephropathy (18.4% of children and 32.5% of adults, mainly grade 2-3) and mesangial proliferative glomerulonephritis (MsPGN) without IgA deposition (13.2%of children and 12.8%of adults). Besides, Alport syndrome (2.6%of children) and membrane nephropathy (2.6%of children and 0.9%of adults) were demonstrated as other causes of PIH. Elevated mean arteral pressure or protein excretion rate, as well as episodic macrohematuria, indicated higher risk for MsPGN rather than TBMN. On the other hand, severity of microhematuria was irrelevant to pathological types of PIH. Totally, 86 patients were followed up and 37 cases therein stayed on track for long term (mean duration 41.11±28.92 months, range 8-113 months). Most cases had benign clinical course except 3 cases with TBMN, 5 cases with IgA nephropathy, 1 case with MsPGN (without IgA deposition), and 1 case with Alport syndrome, who developed hypertension or proteinuria. All of them were administered timely intervention.
Conclusions Close follow-up should be required as the primary management for PIH. Equally important is careful monitoring for early identification of undesirable predictors;while renal biopsy and other timely intervention are warranted if there is hypertension, significant proteinuria or renal impairment.
8.Therapy strategies for transplant candidates of multiple myeloma.

Multiple myeloma (MM) is a malignant plasma cell neoplasm that can not be cured with the conventional chemotherapy. Although new drugs, such as bortezomib, are highly effective in controlling the disease, the hematopoietic stem cell transplantation (HSCT) still should be performed, so as to increase the patients response and survival time. For over a decade, autologous HSCT has been a critical component in the treatment plan for newly diagnosed myeloma. The survival outcome with auto-HSCT seems to be highly dependent on the ability of this approach to enhance the depth of response. Maintenance therapy prolongs the responses after auto-HSCT by continuous administration of drug, such as thalidomide. However, patients invariably relapse after single auto-HSCT or double auto-HSCT. Allogeneic HSCT for myeloma has a potential for curing MM with the presence of a graft versus myeloma effect. Currently, allogeneic approaches should be considered in MM transplant procedure. Clinical data suggest that the mortality after allo-HSCT is definitely lower with the reduced-intensity regimens, but the relapse rate was higher than myeloablative regimens. Individual-adapted treatment strategy is benefited for MM transplant candidates. This article reviews the whole process of MM transplant candidates, including regimens for initial therapy, time of transplantation, choice of transplantation procedure and maintenance therapy.

9.Study on the Chemical Constituents of Arundina graminifolia

Objective To study the chemical constituents from the flowers of Arundina graminifolia and to find its bioactive compounds.Methods The compounds were extracted by 95% alcohol and isolated by column chromatography on silica gel and Sephadex LH-20.The structures were identified by spectroscopic analysis (1H NMR,13CNMR and EIMS).Results Eight compounds were obtained and identified as (1) 7-hydroxy-2,4-dimethoxy-9,10-dihydrophenanthrene;(2)coelonin;(3)4,7-dihydroxy-2-methoxy-9,10-dihydro-phenanthrene;(4)ephemeranthoquinone;(5)densiflorol B;(6)4-(3-hydroxyphenyl)-2-butanone;(7) stigmasterol,(8) β-sitosterol.Conclusion Among them,compound 6 is discovered for the first time from the plant.
10.Value of serum lipoprotein (a) in diagnosing and monitoring early diabetic nephropathy

[Summary] According to urinary albumin excretion rates ( UAER) , 256 patients with type 2 diabete mellitus (T2DM) were divided into normal albuminuria (NA), microalbuminuria (MA), and clinical nephropathy (CN) groups while 108 healthy subjects as control group. The analysis of variance of single factor was applied to examine lipoprotein(a), triglyceride, total cholesterol(TC), low-density lipoprotein cholesterol(LDL-C), cystatin C, and homocysteine. The correlations of lipoprotein ( a ) with urinary albumin excretion rate ( UAER ) and glomerular filtration rate ( eGFR ) were analyzed by Pearson correlation analysis. The sensitivities of lipoprotein ( a ) were evaluated in diagnosis of diabetic nephropathy ( DN) by receiver operating characteristic curve. The results showed that lipoprotein ( a) levels in NA, MA, and CN groups were gradually increased, with a significant increase in CN group(P<0. 05). Pearson correlation analysis revealed that lipoprotein (a) was positively correlated with systolic pressure, TC, cystatin C, and UAER(all P<0. 05) and negatively correlated with fasting blood glucose and eGFR (P<0. 05). The area under the ROC curve of lipoprotein(a) was 0. 639, with the sensitivity 66. 4% and specificity 55. 9% in the optimal cutoff value of 8. 41 mg/dl. These results suggest that lipoprotein(a) may serve as an index for monitoring DN based on its better correlation with UAER and eGFR.
