Medical laser equipment are widely used in clinic with significant improvement in medical technologies.Security incidents in clinic take place frequently,so safety standards should be standardized.According to the experience in working,dynamic management processes to upgrade the safety control level in using medical laser equipment are expounded.The ultimate purpose is to achieve more effective safety and quality management.
2. Mechanism and safety of triptolide in treatment of rheumatoid arthritis

Objective: To explore the mechanism of triptolide in the treatment of rheumatoid arthritis (RA) and analyze its safety. Methods: A total of 60 rats were randomly divided into control group, model group, methotrexate (MTX) group and triptolide (TP) low, medium and high dose groups with 10 rats in each group. In addition to the control group, the rats in the other groups were established type II collagen-induced RA model. After the successful establishment of the model, rats in MTX group were given 0.4 mg/kg MTX by gavage from the 3rd week. Rats in TP high, middle, and low dose groups were given 0.1, 0.2, and 0.4 mg/kg TP by gavage. Rats in control group and model group were given equal volume distilled water once a day for 4 weeks. The paw swelling of rats in each group was compared. The percentage of CD4+CD25+and CD4+Foxp3+ Treg was detected by flow cytometry. The levels of IL-10, IL-17, TNF-α, VEGF, IFN-γ, TGF-β, ALT, and AST in the serum were detected. The pathological morphology of ankle joint was observed under microscope. The expression levels of IL-10, IL-17, TNF-alpha, VEGF, IFN-γ, and TGF-β were detected by immunohistochemistry. Results: Pathological sections showed that synovial cells were proliferated significantly in the ankle joint of rats in the model group, with infiltration of a large number of inflammatory cells such as monocytes and lymphocytes, new capillaries, thinning of bone trabeculae and serious erosion of cartilage surface. The degree of pathological changes in other groups was significantly less than that in model group. After treatment, the degree of joint swelling and arthritis index in MTX and TP groups were significantly decreased compared with those before treatment (P < 0.05). Compared with model group, CD4+Foxp3+Treg and CD4+CD25+Treg were increased in MTX group and TP all dose groups (P < 0.05). Compared with the model group, the serum levels of IL-10, TGF-β in MTX and TP all dose groups were increased, while the levels of IL-17, TNF-α, VEGF and IFN-γ were decreased (P < 0.05). Compared with model group, the expressions of IL-10 and TGF-β in ankle joint tissue of rats in MTX and TP all dose groups were increased significantly, while the expressions of IL-17, TNF-α, VEGF and IFN-γ were decreased significantly (P < 0.05). Compared with control group or model group, there was no significant difference in serum ALT and AST levels between MTX group and TP all dose groups (P > 0.05). There was no significant difference between MTX group and TP high dose group (P > 0.05). Conclusion: TP is effective in treating type II collagen-induced arthritis in rats, which can significantly improve joint swelling. Its mechanism is related to promoting the expression of IL-10 and TGF-β, increasing the proportion of Treg cells, inhibiting the expression of IL-17, TNF-α, VEGF and IFN-γ, and has no obvious hepatotoxicity.
3.Numerical study on the stability of micro-implant with different pitchs for immediate loading

Objective To study the stability of micro-implant orthodontic anchorage(MA)with different pitch in the case of immediate loading.Method Employing 3D finite element analysis method,the stress and dis-placement distribution on the bone interface of MIA with different pitch(0.3 mm、0.5 mm、0.7 mm and 1.0 mm,respectively),which was 1.47 N loaded vertically in the major axis direction,were analyzed.Result The pitch affected the stress distribution significantly,because the maximum stress increased with the pitch decreasing and the impact of pitch on stress distribution on neck and central locations of MIA were different;to decrease the pitch could reduce the max displacement of the jaw,but the impact of pitch on displacement distribution of MIA was not significant.Condusions In the case of immediate loading.MIA with pitch 0.5 mm-0.7 mm is suggested to be selected as orthodontic anchorage in the clinic.

5.Foot hyalohyphomycosis caused by Fusarium subglutinans:a case report

Objective To report a case of foot hyalohyphomycosis due to Fusarium subglutinans. Methods Medical history was collected and physical examination performed for this patient.Biopsy samples were obtained from the inner side of right ankle of this patient and subjected to pathological examination. Discharge was collected from the lesions for direct microscopic examination and culture.Results A 72-year-old woman presented with an ulcer on the right foot for 3 years.Physical examination disclosed an ulcer,measuring 3 cm x 1.5 cm,with a moist surface and obvious tenderness,at the inner side of the right ankle.Proliferation of dusky-red granulomatous tissue was observed at the base of the ulcer.Pathological examination revealed necrotic granulomatous tissue and slender,septate and hyaline hypha-like structure in the superficial dermis with scattered infiltration of inflammatory cells.PAS staining showed sausage-like hypha and scattered orbicular-ovate spores.Microscopic examination of lesional discharge exhibited septate, branching and hyaline hypha.The isolated fungus was identified as Fusarium subglutinans by culture,and appeared to be highly sensitive to terbinafine,nystatin and amphotericin B.The lesion completely healed after 2 months of treatment with oral terbinafine (0.25 g,twice a day).Conclusions This is a case of foot hyalohyphomycosis due to Fusarium subglutinans,and terbinafine is effective for this condition.
6.Static finite element analysis of the stability of osteotomy after hallux valgus surgery based on wrapped curtain method with external fixation

BACKGROUND:Integrated Traditional Chinese and Western Medicine minimaly invasive treatment for halux valgus based on wrapped curtain method with “8”-shaped bandage and sub toe pad external fixation has been used for a long time in the clinic. This method abandons the internal implant fixationandexternal plaster fixation. After surgery, patients could take care of themselves. However, theactivity of the broken end may cause fracture nonunion, which once aroused scholars’ question. Recently, with the continuous improvement of foot biomechanics research, foot finite element model and applications become a reality.
OBJECTIVE:To evaluate thestability of osteotomy after the operation of wrapped curtain method with“8”-shaped bandage and sub toe pad external fixation on the basis of static finite element method.
METHODS:A young female volunteer with halux valgus was selected, whose body weight was 58 kg, and right foot halux abductor valgus angle was 24°; intermetatarsal angle was 13°; proximal articulator set angle was 7°; distal articulator set angle was 7°. CT was used to scan the right foot. ABAQUS software was applied to establish a finite element model of right foot halux valgus bone, and model of the first metatarsal neck minimaly invasive osteotomy was simulated based on wrapped curtain method with external fixation. Von Mises stress and displacement at the osteotomy endwere calculated.
RESULTS AND CONCLUSION:(1) The maximum stress was 0.067 MPa without external fixation, and the maximum stress was 1.258 MPa with the external fixation. Stress was mainly distributed in the outer edge of the osteotomy. (2) The maximum absolute displacement was 0.363 mm without external fixation, and the maximum absolute displacement was 0.716 mm with external fixation. The two largest displacements were both in the Z-axis direction. Statistical analysis confirmed that the four nodes absolute displacement and stress were significantly different (P< 0.01). (3) The maximum relative displacement was 0.101 mm. The maximum relative displacement was 0.046 mm with external fixation. The maximum relative displacement without external fixation was-0.102 mm and occurred in the Z-axis. The maximum relative displacement with external fixation was 0.110 mm and occurred in the Y-axis. (4) One-way analysis of variance confirmed that the four nodes relative displacements were not statisticaly significant in X-axisand Y-axis (P> 0.05). The four nodes relative displacements were statisticaly significant in Z-axis (P< 0.05). (5) These findings suggest that the external fixation based on wrapped curtain method after halux valgus surgery could effectively reduce osteotomy displacement. The moderate stress and elastic fixation are conducive to fracture healing.
7.Clinical significance of peripheral blood neutrophil to lymphocyte ratio in patients with 2 type diabetes mellitus complicated acute cerebral infarction

Objective:To investigate the clinical significance of peripheral blood neutrophil to lymphocyte ratio in patients with 2 type diabetes mellitus complicated acute cerebral infarction, so as to provide theoretical basis for clinical diagnosis and treatment. Methods:From Jan. 2014 to Dec. 2016 year,136 patients with 2 type diabetes mellitus complicated acute cerebral infarction were selected as research group and 68 diabetic patients undergoing physical examination were selected as the control group. White blood cell count(WBC),neutrophil count(NC),lymphocyte count(LC) and neutrophil to lymphocyte ratio(NLR) were detected and calculated. The role of WBC,NC,LC and NLR in the diagnosis of type 2 diabetes mellitus combined with acute cerebral infarction were evaluated by receiver operating characteristic curve(ROC) and area under the curve(AUC). Results:The levels of WBC,NC and NLR in research group were significantly higher than those of in control group(P<0. 05),while the level of LC was significantly lower than that of in control group(P<0. 05). The levels of WBC,NC and NLR in research group increased with the increase of NIHSS score(P<0. 05),and the level of LC decreased with the increase of NIHSS score(P<0. 05). The area under the curve of NLR,WBC,NC and LC in diagnosing diabetes combined with acute cerebral infarction were respectively 0. 856,0. 717,0. 685 and 0. 321. The sensitivity were respectively 0. 838,0. 596,0. 517 and 0. 208. The specificity are respectively 0. 868,0. 838,0. 793 and 0. 559. Conclusion:NLR could be used as a marker for the diagnosis of type 2 diabetes with acute cerebral infarction.
8.A prospective study on the relationship between nutrition intervention and incidence of malignant tumors in Lin County, China.

OBJECTIVETo analyze the tendency in the incidence of malignant tumors in nutrition-intervened population of Lin County.
METHODSSubjects who had been enrolled in the General Population Trial, Lin County Nutrition Intervention Trial were followed up and the data of malignant tumors were collected with collaboration of National Cancer Institute/National Institute of Health. Incidences of malignant tumors during and after nutrition intervention were calculated and compared.
RESULTSEsophageal cancer, cardiac cancer, and stomach cancer were constantly the three leading cancers, accounting for 84.06% and 83.74% of the malignant tumors in men and women, respectively. The incidences of lung cancer and liver cancer in men showed increasing tendencies, while similar tendencies were found in those of stomach cancer, colorectal cancer, and liver cancer in women. With the prolonging of the follow-up, the incidences of upper digestive tract cancers declined (except stomach cancer in women).
CONCLUSIONSThe upper digestive tract cancers remain the leading malignant tumors, although their incidences may decline with longer-term follow-up. Prevention and control of tumors in Lin County should still be focused on the esophageal cancer, cardiac cancer, and stomach cancer, as well as lung cancer and liver cancer.

9.Study on recovery and its influencing factors of ferulic acid and tetramethylpyrazine in cerebral microdialysis probe.

To establish a method for detecting microdialysis recovery of tetramethylpyrazine (TMP) and ferulic acid (FA) and investigating the influencing factors, providing the basis for further in vivo microdialysis experiments. The concentration of FA and TMP in dialysates were determined by high pressure liquid chromatography ( HPLC) and probe recovery were calculated respectively. The influence of the flow rates, medium concentration, temperature and in vivo probe stability on the recovery of FA and TMP were investigated by using concentration difference method (incremental method and decrement method). The recovery obtained by incremental method were similar to by decrement method. The in vitro recovery rate of FA and TMP decreased with the increase of 1-2.5 μL min(-1), and increased obviously with the temperature of 25-42 degrees C under the same conditions. The concentration of FA and TMP had no obvious effect on the probe recovery under the same flow rate. In addition, the recovery of TMP and FA remained stable and showed similar trends under the condition of four concentration cycles, indicating that the intra day reproducibility of the concentration difference method was good. The recovery of brain microdialysis probes in vivo 8 h maintained a relatively stable, but certain differences existed between different brain microdialysis probes, demonstrating that each probe was required for recovery correction in vivo experiment. Microdialysis sampling can be used for the local brain pharmacokinetic study of FA and TMP, and retrodialysis method can be used in probe recovery of FA and TMP in vivo.

10.The effect of Chinese yam extract combined with tegafur on the cytotoxicity of HT-29 cell line in vivo and in vitro

Objective To study the inhibitory effects of Chinese yam combined with tegafu on colon cancer HT-29 cells in vivo and in vitro.Methods The changes of cell morphology were observed by the method of adopting 2×2 factorial designs,after HT-29 colon cancer cells were treated with physiological saline containing dimethyl sulfoxide(blank control), tegafur(36 mg/L)and Chinese yam(125 mg/L).MTT method was used to detect the proliferation of tumor cells,and flow cytometry was used to detect the expression of tumor stem cells(CD133+cells).The nude mouse model of colon cancer HT-29 cells was established. The treatment was given in the above method, and the tumor inhibition rate was calculated. The VEGF positive rate of the tumor was detected by immunohistochemistry. Results The inhibited proliferation of colon cancer HT-29 cells was significantly higher in the synergistic group than that in Chinese yam group and tegafur group,and the inhibitory rates were higher in the three treatment groups than that of blank control group.The proportion of CD133+cells was significantly lower in HT-29 cells in the synergistic group compared with that of the Chinese yam group and the tegafur group,and which was lower in these three groups than that of the blank control group.After the treatment in three treatment groups,the tumor quality and VEGF positive rate were significantly lower than those of the blank control group,and which was lower in the tegafur+Chinese yam group than that of Chinese yam group and the tegafur group. Conclusion Chinese yam combined with tegafu can inhibit the proliferation of colon cancer HT-29 cells and their stem cells.
