In order to improve the metabolism yield of cordycepin and adenosine,we studied nitrogen sources ,the levels of nitrogen sources and carbon sources,dynamic development in liquid culture ,as well as the total yield of cordycepin and adenosine in culture system through determining the contents of cordycepin and adenosine by HPLC .The results are as follows: not only animal protein but also some plant protein is very good nitrogen sources; in culture solution the suitable levels of nitrogen sources and carbon sources is respectively 3% and 4%; the content of adenosine in culture solution is very low, while it is quite high in mycelia; it is much higher that the total yield of cordycepin in culture solution than in mycelia; when the culture system is oscillated and cultured after 7~9 days, the total yield of cordycepin and adenosine are both high.

Objective To summaize the experience of application of direct digital radiography and electrohydraulic shockwave lithotripsy in treatment of residual bile duct stones.Methods A retrospective analysis of clinical records of 60 patients with residual bile duct stones treated in our hospital from 2003 to 2006 was made.Results In 59(95.9%) of 60 cases the stones were removed completely,including successful removal in one time of application of this method in 57 Patients(95.0 %),and 2 applications in 2 Patients ﹙3.3 %).One case with bile duct stone complicated with dile duct cancer was unsuccessful.The biggest stone was 3.5 cm?2.5 cm?2.0 cm.No serious complications occurred and changed to operation.The 59 cases were followed up for 6 mo to l yr,and all had a successful outcome.Conclusions The method of combination of direct digital radiography and electrohydraulic shockwave lithotripsy in treatment of postoperative residual bile duct stones after operation can facilitate removal of residual bile duct stones.The procedure is a safe way of treating residual stones.

Objective To investigate the effects of diclofenac sodium suppositories on emergence agitation after sevoflurane maintenance in children undergoing adenotonsilleetomy. Methods Forty-five patients selected for adenotonsillectomy were randomly divided into three groups (n=15). Patients in group 1 were given diclofenac sodium suppositories 12.5 mg right after intubation, those in group 2 were given diclofenac sodium suppositories 12.5 mg immediately after operation, while those in control group were not treated with diclofenac sodium suppositories before or after operation. The extubation time and time spent in post-anesthctic ICU (PACU) were recorded, the modified Aldrete score and pain score were assessed after entrance into PACU, and pediatric anesthesia emergence delirium (PAED) scale was administered 10, 20 and 30 rain after entrance into PACU. Results There was no significant difference in extubation time and time spent in PACU among three groups(P>0.05). Ten minutes after entrance into PACU, the PAED score of group 1 was significantly lower than those of group 2 and control group(P<0.05), while there was no significant difference between the latter two groups(P>0.05). Twenty and thirty minutes after entrance into PACU, the PAED scores of group 1 and group 2 were significantly lower than that of control group(P<0.05). There was no significant difference in the modified Aldrete score right after entrance into PACU among three groups. The pain scores of group 1 and group 2 were signifiantly lower than that of control group(P< 0.05). Conclusion Diclofenac sodium suppositories can decrease the incidence and severity of emergence agitation after sevoflurane maintenance in children undergoing adenotonsillectomy.

Background Many kindof cells,cytokineand extracellulamatrix components,such aconnective tissue growth facto(CTGF) and fibronectin (FN),can regulate and controll the formation and developmenof epiretinal membrane.Iiimportanto investigate the morphology and pathology of epiretinal membrane fothe prevention of relevanretinopathies.Objective Thistudy wato investigate the expression of CTGF and FN in differentypeand stageof epiretinal membranes.MethodEpiretinal membranewere collected during vitrectomy and silicon oil removal surgery from 24 eyeof 24 casewith rhegmatogenouretinal detachment(RRD) (including 14 eyewith courses＜90 dayand 10 eyewith course≥ 90 days),20 eyeof 20 casewith diabetiretinopathy (DR)associated with vitreouhemorrhage and tractional RD (course≥ 90 days)and 7 eyeof 7 casewith silicon oil tamponade (course≥ 90 days).The grading of the proliferative vitroretinopathy (PVR) wa＞C2 in all the eyes.Histopathological examination waperformed to assesthe pathological elemenin the differentypeand stageof epiretinal membrane,and the differential expressionof CTGF and FN in the varioutypeof epiretinal membranewere detected by immunochemistry.The amounof cellpositive foCTGF and FN wacompared using the FisheExacProbability method.ResultHistopathological examination showed thaincrease in the numbeof RPE celland inflammatory cellwaprimary characteristiin the epithelial membrane of RRD with courses＜90 days,buthere were loof neurogliocyteand fibroblastin the RRD group with course≥ 90 days.In addition,the infiltration of inflammatory cellwapredominanin the RRD and Dgroups,bufibroplasiwaobviouin the silicon oil tamponade group.When the course wa≥ 90 days,7 eye(70.0％)in the RRD group wapositive foCTGF,and 18 eyes(90.0％)in the Dgroup and 2 eyes(28.6％)in the silicon oil tamponade group were positive foCTGF,with significandifference among them(P=0.037).In regardto FN,9 eyeshowed expression in the RRD group(90.0％),18 eyein the Dgroup(90.0％)and 7 eyein the silicon oil tamponade group(100.0％),withousignificandifference among them (P =0.379).In the RRD group,7 eyewith course≥ 90 dayand 13 eyewith courses＜90 days(92.9％)were positive foCTGF,showing significandifference between them(P =0.032).AfoFN,9 eye(90.0％)with course≥ 90 dayshowed positivity,which wasignificantly highethan the 7 eyewith courses＜90 day(50.0％) (P =0.019).ConclusionThe expression of CTGF in epiretinal membranevariewith the disease type and course duration,buthe expression of FN in epiretinal membrane varieonly with the course duration.

Objective To evaluate the efficacy and safety of photodynamic therapy(PDT)combined with intravitreaIinjection of bevacizumab for choroidal neovascularization(CNV)caused by agerelated macular degeneration(AMD). Methods A total of 21 eyes of 21 patients with AMD,which was diagnosed by examination of visual acuity,intraocular pressure,ocular fundus,fundus color photography,fundus fluoreseein angiography(FFA),indocyanine green angiography(ICGA)and optic coherence tomography(OCT),were underwent PDT combined with intravitreal injection of Bevacizumab.The patients,15 males(15 eyes)and 6 females(6 eyes),aged from 56 to 78 years,with the average of 68.6years.The best corrected visual acuity:counting fingers/10cm-0.9,logMAR was 1.04±0.41.CNV located in below or side central fovea of macula.There was obvious leakage of fluorescein which examined by FFA and ICGA.The average of retinal thickness of macular foveal was(258.91±78.66)μm.The treatment method of PDT has to according to the way of PDT for TAP and Verteporfin PDT for VIP.Intravitreal infeetion with 1.5mg bevacizumab was performed after three days under surface anesthesia.Follow-up time was 1,3,6,12 months after the treatment. Resuits At last visit,the best corrected visual acuity:counting fingers/10 cm-1.5,logMAR was 1.04±0.41,and the differences are statistically significant compared with before.The BCVA improved four or more lines in 6 eyes(28.57%),improved two to four lines in 9 eyes(42.86%),stabilized(±1 line or no change)in 6 eyes(28.57%)and decreased in none.The average intraocular pressure was(15.20±2.41)mmHg after surgery,and the differences was not statistically significant compared with before(P＞0.05).FFA and lCGA showed CNV complete closure in 13 eyes(61.90%).partial closure in 8 eyes(38.10%).The average of retinal thickness of macular foveal was(127.38±20.14)μm(P＜0.01). Conclusion Combining treatment with PDT and intravitreal injection of Bevacizumab is safe and effective for CNV which caused by AMD.It has significant improvement in BCVA.1eakage of CNV and retinal edema.
