1.Research in rehabilitation and nursing care of myogenic fecal incontinence

Objective To explore the methods and key points of nursing care for myogenic fecal incontinence.Methods There were 65 patients with myogenic fecal incontinence in this study.All these patients had undergone sphincter reconstruction surgery.Holistic nursing and biofeedback training were provided for all these patients after surgery.Results The complications in these patients included diaper rash of 32 cases,perianal ulcer 12 cases,urinary infection 7 cases,postoperative wound infection 16 cases.After treatment and holistic care,all the diaper rash disappeared and all the peroanal ulcer healed.The urinary infection for those 7 patients were controlled.Fifteen postoperative wound infection were cured,and one recovered after intestinal fistula.Six months to two years follow-up revealed that the clinical score after treatment was significantly higher than that before treatment (1.72±0.22 vs 4.01±0.36).Conclusions Enema,holistic nursing care and rehabilitation training are important for rehabilitation of myogenic fecal incontinence.
3.Correlation study between hope and depression with the elderly in a welfare institution in Henan Xingyang city

Objective To investigate the status of the level of hope and depression with the elderly in a welfare institution in Henan Xingyang city,and to explore the relationship between them. Methods A total number of 200 elderly people were recruited conveniently from a welfare institution in Henan Xingyang city. They were investigated by General Material Questionnaire, Herth Hope Scale and Geriatric Depression Scale. Results The total score of hope and depression with the elderly in a welfare institution were (21.28±3.07) , (13.15±2.77) points respectively, the depression detection rate was 82.38%(159/193);the level of hope was negatively related with depression (r=-0.447,P<0.01). Conclusions The hope of the elderly in welfare institution is not optimistic,and the depression detection rate is high,the level of hope is positively related with their depression,so we should take effective measures to improve their mental health according to the emotional state to give emotional care and improve the level of their hope.
4. Preparation, characterization and dissolution characteristics of curcumin solid lipid nanoparticles modified with PEG

Objective: The sustained release curcumin solid lipid nanoparticles (Cur-SLN) and long circulating solid lipid nanoparticles (LSLN) were prepared, and the physicochemical properties of the two nanoparticles were investigated. Methods: Cur-SLN was prepared by emulsification ultrasonic method, and then the entrapment efficiency and drug loading of Cur-SLN prepared under the optimal formulation were determined. Cur-LSLN was prepared by back-extrapolation method, and the physicochemical properties of Cur-SLN and Cur-LSLN were evaluated by entrapment efficiency, drug loading, particle size, and Zeta potential; DSC was used to analyze, in vitro release characteristics and the transmission electron microscope (TEM) was used to observe particle appearance. Results: Based on the optimal conditions, TEM showed that the appearance of Cur-SLN and Cur-LSLN were spherical or nearly spherical, the entrapment efficiency respectively were (89.15 ± 0.66)% and (92.97 ± 0.27)%, drug loading were (1.72 ± 0.08)% and (1.98 ± 0.08)%, average diameters of particles were (144.5 ± 4.1) nm and (155.0 ± 2.6) nm, and the mean Zeta potential were (-23.6 ± 0.2) mV and (-47.8 ± 1.8) mV. Through DSC detection, it can be determined that Cur in nanoparticles had been transformed into amorphous state. In vitro release test showed that the drug release of the two preparations was divided into two stages: burst release phase and sustained released stage, the release rate was fast in 12 h, and the cumulative release of Cur-SLN in 96 h was 86.63%, and Cur-LSLN was 76.98%, so Cur-LSLN showed better sustained-release effect. Conclusion: Cur-SLN and Cur-LSLN can be successfully prepared by emulsification ultrasonic method, and PEG modified nanoparticles have better sustained-release properties and prolong the time of the presence of drug in vivo, providing reference for the development of targeted drugs.
5. Profile of new natural active compounds in 2014

The standardization of compound nomenclature is of vital importance. The principles of standardization include scientificity, systematization, monosemy, openness, and back-translatability. The standardized compound names should also be cross-translatable from roots of the words and easy to remember. The article indexed 592 new active natural compounds, referencing 222 papers from 11 leading foreign periodicals and four domestic journals. The names of new compounds in the paper may be controversial, but the nomenclature was complied with the above principles of standardization.
6.Correlation analysis of impact positions on the head and hyperextension severities of cervical spinal cord

Objective To analyze the correlation between impact positions on the head and hyperextension severities of cervical spinal cord,as well as their treatment.Methods A retrospective analysis was done for 41 cases of hyperextension of cervical spinal cord without fracture-dislocation who had received management in our department from January 1999 to January 2004.All the patients had no cervical disorders before injuries.They were injured by impact mostly on the forehead,cheek or lower mandible.Their neurological findings at admission and 6-month follow-ups were assessed according to ASIA (American Spinal Injury Association) rating system.Results Of the 20 patients who were injured by impact on the forehead,four were rated in neural function as grade B,11 as grade C,and five as grade D at admission,while one as grade C,11 as grade D,and eight as grade E at 6-month follow-up.Of the 12 patients who were injured by impact on the cheek,one was rated in neural function as grade B, four as grade C,and seven as grade D at admission,while one as grade C,two as grade D,and nine as grade E at 6-mouth follow-up.Of the nine patients who were injured by impact on the lower mandible,five were rated in neural function as grade B,three as grade C,and one as grade D at admission,while two as grade B,five as grade C,and two as grade E at 6-month follow-up.The 6-month follow-ups after treatment showed that the neural function was rated below grade C in nine patients,seven of whom had been injured by impact on mandible.Two patients of grade B who showed little improvement after treatment had been injured also by impact on mandible.Conclusion Impact on the lower mandible causes mote severe hyperextension than impact on tbe cheek or forehead.Both rea- sonable non-operative and operative treatments can lead to satisfactory outcome for patients with hyperextension of cervical spinal cord but without fracture-dislocation.
7.Analysis of characteristics of blood loss for laboratory test for 246 cases of critically ill premature infants

Objective To analyze the characteristics of blood loss for laboratory test of critically ill premature infants,and to seek feasible measures to reduce the blood loss.Methods Two hundred and forty-six cases of critically ill premature infants admitted to the neonatal intensive care unit from April 2012 to April 2013 were analyzed the blood loss for test during the hospitalization and the clinical features of blood loss with different gestational age,different weight within the first four weeks after admission.Then the application of blood loss according to test category was described.The blood volume demanded in theory was determined by the formula B =5 (∑ s + 0.1),then calculated the phlebotomy overdraw on the basis of the practical blood loss and analyzed the characteristics of overdraw per patients per day in first two weeks after admission.Results Among 246 patients,The median blood loss figure was 25.57 ml for each infants with the range between 7.10 ml ~ 119.20 ml,and the blood loss concentrated in first four weeks,which showed a decreasing trend with time.There was a statistically significant difference(P <0.05)that the smaller gestational age,the lower birth weight,the more daily blood loss per patient per day in first two weeks,but no significant differences(P > 0.05) between the third and fourth week.The largest proportion of the blood samples was used for clinical chemical tests(31.49%),followed by blood gas analysis (19.03%),immunoassays (12.69%),blood cultures (12.63%),hematology (12.28%).The practical blood loss was about twice times of blood volume demanded for tests in theory,which the median was 7.8 times to the latter(25.57 ml vs 3.26 ml).It showed statistically significant difference(P <0.05) between blood overdraw per patients per day in the first week and the second week.Compared with different gestational age and birth weight,the difference of overdraw was also statistically significant(P <0.05).According to test category,blood culture was the most significant samples of phlebotomy overdraw,followed by biochemical,other,blood gas analysis,the percentage was 76%,64%,45% and 41% respectively.Conclusion The blood loss for laboratory test and the phenomenon of blood waste is serious in critically ill preterm infants.The smaller the gestational age is,the lower the weight is,the amount of blood loss and phlebotomy overdraw are more significantly.Biochemical and blood gas analysis are the main items of blood loss.

9.Relationship between liver enhancement in hepatobiliary phase of gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid-enhanced MRI and liver function

Objective To evaluate the effect of liver function on liver enhancement in hepatobiliary phase of gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid(Gd-EOB-DTPA)-enhanced MRI.Methods Sixty-seven patients who suffered from cirrhosis and received enhanced MRI with Gd-EOB-DTPA were retrospectively analyzed,and divided into three subgroups according to ChildPugh score(45 patients in group A,20 in group B,5 in group C).All the individuals of both groups had MRI before injection,and hepatobiliary phase images were obtained at 5,10,and 20 minutes after bolus administration of Gd-EOB-DTPA.The relative enhancement(RE) was calculated by dividing the signal intensity of liver(SI) at t min after injection(SIt) by precontrast SI(SI0).The total serum bilirubin level(TB),serum albumin level(Alb) and prothrombin time(PT) were recorded.The one-way ANOVA was used to compare the RE among three groups at 5,10 and 20 minutes.SNK was used for further pairwise comparison.The effect of liver function on RE was assessed with the generalized linear model.Pearson correlation coefficients were measured between each biochemical test result(TB,Alb,PT) and RE at different time points.Results The RE at 5,10 and 20 minutes were 1.59±0.20,1.65±0.22,1.69±0.25 of group A; 1.47± 0.14,1.48±0.18,1.50±0.22 of group B,1.35±0.07,1.27±0.06,1.26±0.06 of group C.There were statistically significant differences of RE among groups at 5,10 and 20 minutes(F=5.854,11.207,9.666,P<0.01).Statistically pairwise comparison differences of RE were found between group A and C at 5,10 and 20 minutes(P<0.01),between B and C at 10 and 20 minutes(P<0.05),between A and B at 10 minutes(P< 0.05).There were statistically significant differences of TB,Alb and PT among groups(P<0.01).RE at 10 and 20 minutes had moderate negative correlation with TB(r=-0.483,-0.500; P<0.01),low negative correlation with PT(r=-0.326,-0.351;P<0.01) and weak positive correlation with Alb(r=0.290,0.292;P<0.05).Conclusions There are differences of RE among patients with different liver function,and the RE is associated with TB,Alb and PT.Thus,it may allow us to estimate the liver function.
10.The effect of brain atrophy on the motor recovery following spinal cord injury

Objective To investigate the structural changes during the early stage of spinal cord injury,and the relationships between these structural changes and patients' motor recovery.Methods The clinical data of 25 patients with spinal cord injury collected from 2012 October to 2014 March were analyzed retrospectively.The spinal cord injury patients were split into two groups (Good recoverers group and Poor recoverers group) according to the clinical outcome of motor recovery at 6 months follow-up,and there were 10 Good recoverers (ASIA grade:1 A,4 B,3 C,and 2 D) and 15 Poor recoverers (ASIA grade:7 A,3 B,3 C,and 2 D).The study also recruited 25 matched healthy controls.The mean age of Good recoverers,Poor recoverers and healthy controls were 37.9± 13.9,35.8± 11.5 and 36.5±9.3,respectively.Structural MRI was used to investigate the regions with brain structural changes among the three groups.Pearson correlation analysis was used to explore the relationships between these structural changes and patients' motor recovery.Results Compared to healthy controls group,both poor recoverers and good recoverers had significantly decreased cortical thickness in the bilateral primary motor cortex.Poor recoverers exhibited more serious and widespread structural damages,in addition to reduced cortical thickness in the primary motor cortex,poor recoverers also showed decreased cortical thickness in the right SMA and premotor cortex when compared to healthy controls.Compared to the healthy controls group,poor recoverers showed reduced white matter volume in the right primary motor cortex and posterior limb of the internal capsule;good recoverers showed no significant difference in white matter microstructure.Furthermore,these structural changes at the internal capsule and primary motor cortex were associated with the motor recovery rate at 6 months follow-up (r=0.75,P< 0.001;r=0.76,P< 0.001,respectively).Conclusion These findings suggest that spinal cord injury causes significant anatomical changes in the human sensorimotor system in the early phase,and these structural changes directly affect the motor recovery of spinal cord injury.Future treatment aimed at promoting neural function recovery of spinal cord injury patients should pay close attention to the injured brain.
