Objective To investigate the quality of life (QOL) of maintenance hemodialysis (MHD) patients and its influencing factors. Methods A total of 257 MHD patients in our hospital were recruited in this study.Clinical data of the patients were collected, and the QOL was assessed by MOS 36 item short form health survey(SF-36).Nutritional status of patients was evaluated by modified quantitative subjective global assessment (MQSGA).Univariate analysis of variance, pearson correlation analysis and multiple linear stepwise regression analysis were performed to determine the effect of related factors on QOL scores. Results The scores of all scales of SF-36 evaluation in MHD patients were relatively lower than that of general population as reported before.Their physiological component summary (PCS) score decreased gradually as age grew, nevertheless, the mental component summary (MCS) score was highest in the group aged 41-60.The score was lower in patients with moderate to severe malnutrition or diabetic nephropathy when compared with other patients.Univariate analysis of variance also revealed that high SF-36 scores associated with higher education or income.Multivariate analysis indicated that PCS score and total SF-36 score of MHD patients were positively correlated with body mass index (BMI) and cholesterol, but negatively correlated with diabetic nephropathy, pulmonary artery systolic pressure and MQSGA score (all P < 0.05).There was positive correlation between MCS score and income, yet negative correlation between MCS score and MQSGA score (all P < 0.05).Conclusion MHD patients had relatively poor QOL.Primary diseases and nutritional status were probably the main influencing factors.Age,educated level, family income and pulmonary artery systolic pressure might also have effects on their QOL.

To study the changes in serum immunosuppressive acidic protein (IAP) and T lymphocytes rDNA transcription activity in peripheral blood of patients with pancreatic cancer before and after operation. The level of IAP and T lymphocytes rDNA transcription activity in peripheral blood of 58 patients with pancreatic cancer were measured before and after opreration by one directional immunodiffusion test and a cell image analysis of Ag NORs, respectively, and the results were compared with that of healthy controls. The level of IAP was higher while T lymphocytes rDNA transcription activity was lower before operation compared with healthy controls( P 0 05). The levels of IAP and T lymphocytes rDNA transcription activity were closely related to tumor invasion, metastasis and clinical stages (TNM). Serial determination of the levels of IAP and T lymphocytes rDNA transcription activity might serve as an important index for evalution of tumor invasion and metatasis, and also prognosis for the patients with pancreatic cancer

Objective:To analyze the immune function regulation and clinical effect of dust mite drops desensitization treatment on children with allergic rhinitis.Methods:80 patients with allergic rhinitis and adenoid hypertrophy treated in our department from January 2012 to June 2014 were selected and randomly divided into experimental group and control group.Control group was treated with ebastine and levocabastine treatment, the experimental group was treated with sublingual containing Dermatophagoides Farinae Drops on the basis of control group. The level of IL-2,IL-6,house dust mite specific IgE (sIgE),dust mite IgE (T-IgE),house dust mite specific IgG4 ( sIgE in) ,blood addicted eosinophile cells ( EOS) ,and induced sputum EOS level were compared between the two groups before treatment and after 2 years treatment. Results:The level of IL-2 in the two groups was significantly higher than that before treatment, the level of IL-6 was significantly lower than that before treatment ( P<0.05 ) .After treatment, the level of IL-2 in the experimental group was significantly higher than that in the control group, the level of IL-6 was significantly lower than that in the control group( P<0.05 ).After treatment, the level of sIgE, T-IgE, blood EOS and induced sputum EOS in the two groups were significantly lower than before treatment, the level of sIgG4 were significantly higher than those before treatment (P<0.05).After the treatment, the level of sIgE, T-IgE, blood EOS and induced sputum EOS in experimental group were significantly lower than control group, and the level of sIgG4 was significantly higher than that in control group ( P<0.05).After treatment, the asthma symptom scores and nasal symptom scores of the two groups were significantly lower than before treatment (P<0.05),and those index of experimental group after treatment were significantly lower than control group ( P<0.05 ).Conclusion: Combine the basis of conventional therapy plus with Dermatophagoides Farinae Drops in the treatment of allergic rhinitis and adenoid hypertrophy children can effectively regulate the immune function of the patients, has good clinical efficacy.

Objective To test the efficacy of cognitive behavioral self help therapy for chronic insomnia (CBTI-SH) patients with hypnotic long-term use. Methods A total of 60 adults with chronic insomnia and common comorbidities were recruited.Participants were randomly assigned to either intervention group (IG,n =30) if they had used hypnotics more than 6 months,or control group(CG,n =30) if they had never used hypnotics in the past 6 months. All the patients were given CBTI-SH for 4 weeks. The primary outcome was self-report symptom, based on sleep diaries (including Sleep Latency(SL),Wake after Sleep Onset(WASO),Total Sleep Time(TST),Time In Bed(TIB),Sleep Efficiency(SE) which were evaluated on baseline and at the end of the 2nd,4th week treatment.Continuous variables were evaluated by repeated-measures multivariate analyses of variance (MANOVA).At the conclusion of treatment, each participant was asked how many days per week they enacted each of 6 core elements of the CBT regimen.Linear regression models were examined to determine net predictors of CBTI-SH adherence. Results The MANOVA showed a significant treatment group × time interaction,as well as time main effects for SE,TIB and WASO(P<0.05).The patients in CG made better improvements than those in IG on SE,TIB and WASO((0.19 ±0.08) vs(0.09 ±0.09),(92.48 ± 32.64) vs (78.06 ±60.05),(103.08 ±64.82) vs (64.92 ±56.36),respectively) (P < 0.05).Effect size were 1.14,0.85 and 0.62 respectively.Self-ratings of treatment adherence showed patients in the IG reported less adherence to standard rise time and TIB prescription than those in CG.(P < 0.05).According to the linear regression models, both age and the comorbidity of chronic somatic diseases explained net associations with CBTI-SH adherence (P < 0.05).Conclusion Chronic insomnia patients with hypnotic long-term use show less sleep improvement and less adherence to CBTI-SH compared with those who don' t use these medicines. Both age and the comorbidity of chronic somatic diseases explain net associations with CBTI-SH adherence.

Chinese Journal of Health Management 2011;05(3):134-136
Objective To investigate the association between obesity and benign prostate hyperplasia (BPH).Methods A total of 253 men (>40 year old) with no obesity related diseases were selected.Medical history, height, body weight, waist circumference (WC),body fat ratio, blood pressure, fasting blood glucose, blood lipid and prostate ultrasound were evaluated. The participants were then assigned to the normal group (body mass index 18.5 to 23.9 kg/m2),overweight group (24.0 to 27.9 kg/m2) and obesity group (≥28 kg/m2).The subjects were also divided into the normal group (WC<85 cm) and abdominal obesity group (WC≥85 cm) or normal group (body fat ratio≤25%) and excessive body fat ratio group (>25%).Prostate volume was analyzed in each group. Results The prostate volume was significantly different between the normal body mass index group and overweight or obesity groups, normal WC group and abdominal obesity group, and normal body fat ratio group and excessive body fat ratio group. The prostate volume was increased in individuals with higher body mass index,WC and body fat ratio.Of those with body mass index≥24 kg/m2 and normal WC or body fat ratio, the incidence of BPH was not significantly increased. Conclusion Obesity may be a risk factor for BPH; individuals with increased WC and body fat ratio may have a higher risk of BPH.
