1.Clinical Efficacy of Foam Sclerotherapy Combined with Melilotus Extract Tablets for Varicose Veins of Lower Extremities

Objective To compare the short-term clinical effect of the foam sclerotherapy combined with Melilotus Extract Tablets vs foam sclerotherapy alone in treating varicose veins of the lower extremities.Methods A retrospective cohort study was carried out in 120 cases receiving foam sclerotherapy in the Invasive Technology Department of Panyu District Central Hospital.Seventy-two patients in trial group wore graduated compression stockings and received oral use of Melilotus Extract Tablets after the operation,while 48 cases in the control group only wore graduated compression stockings after the operation.The follow-up for the two groups lasted for 3 months.We recorded the relief of symptoms (pain,soreness and distention) and physical signs (edema,hyperpigmentation,ulcer) after the operation.The grading levels of clinical-etiology-anatomic-pathophysiologic classification system (CEAP) for lower limb venous disease,venous clinical severity scores (VCSS) and disease quality-of-life questionnaire CIVIQ scores were observed before and after the operation.Results (1) Three months after the operation,the illness state of the two groups was improved to certain degree,and the differences of CEAP grading levels were insignificant(P > 0.05).(2) VCSS and CIVIQ scores of the two groups were improved after the operation (P < 0.01 compared with those before the operation),and the trial group had better effect on improving CIVIQ scores than the control group (P < 0.01).(3) The successful rate of the trial group was 88.89%,and that of the control group was 81.25%,the difference being insignificant (P > 0.05).(4) Time for obvious relief of lower limb symptom in the trial group was shorter than that in the control group (P < 0.05),but the time for hyperpigmentation and ulcer disappearing as well as the medical expenses in the trial group did not differ from those in the control group (P > 0.05).Conclusion Foam sclerotherapy combined with Melilotus Extract Tablets is effective,safe and economical for the treatment of varicose veins of the lower extremities,and its efficacy is superior to foam sclerotherapy alone.
2.Repairing atrial septal defects with bilateral lung transplantation in treatment of ASD with Eisenmenger's syndrome

Objective To investigate the feasibility and the perioperative treatments of repairing atrial septal defects and bilateral lung transplantation in treatment of ASD with Eisenmenger's syndrome.Methods On May 24th,2016,one case of atrial septal defect with Eisenmenger's syndrome underwent repair for the atrial septal defect with bilateral sequential lung transplantation aided by extracorporeal circulation ECMO in Zhongshan People's Hospital.In strict accordance to the procedure,the lung was collected and preserved with the HTK solution.The operation went smoothly,the cold ischemic time for the right lung was 120 min and that for the left lung was 260 min.Postoperatively,Tacrolimus (FK506) + Mycophenolate Mofetil (MMF) + adrenal cortical hormone Sanlian immunosuppression was used.Results At 60th h post-operation the patient was hemodynamically stable,with good oxygenation after removal of the ECMO.At 7th day post-operation the patient was extubated.At 12th-20th day post-operation bacterial culture of the sputum was found to be positive and the patient was treated with the corresponding sensitive antibiotics and recovered.Cardiac ultrasound before discharge showed that there was no residual shunt.One month after discharge the cardiopulmonary function of the patient was quite good,chest X-ray showed that the bilateral transplanted lungs were clear and the patient's quality of life was much better.Conclusion Cardiac malformation repairs with bilateral lung transplantation is a feasible treatment for patients with end-stage Eisenmenger's syndrome with bi-directional shunts.The key to success is sufficient preoperative preparation,and a smooth operation with the timely and effective treatment of postoperative complications.

Third molars, late-eruption permanent teeth in humans, have commonly been extracted in clinical treatments. However, with the development of oral medicine, the value of maxillary third molars in clinical treatments, as well as in oral prosthetics, orthodontics, and oral implant applications, has gradually become recognized. This paper summarizes the research on the morphology of the crown, root, root canal and root tip of the maxillary third molar in a review to facilitate related research and clinical treatments.

Objective To investigate the clinical effectiveness of foam sclerotherapy for lower extremity varicose vein with deep venous valve incompetence.Methods A retrospective study was carried out in 124 patients (185 limbs) receiving foam sclerotherapy in our department.Sixty-one limbs diagnosed as deep venous valve incompetence were categorized as the observation group,while 124 limbs without that were the control group.The main outcome were success rate,remission rate of various symptoms and signs,the scores of CEAP and CIVIQ.Results The follow up rate was 69.3%,and the follow-up period ranged from one to nine months.There was no statistical difference in the success rate between the observation group (93.4%) and control group (94.4%)(P>0.05).However,the symptom of heaviness and oedema and the CIVIQ scores in observation group be inferior to the control group,and the difference of them were statistically significant (P<0.05).Conclusion Deep venous valve incompetence could not decrease the success rate of foam sclerotherapy,but may influence the remission of some symptoms.It should be supplemented with medical elastic stockings or ringing of femoral veins.

Objective:To investigate the clinical efficacy of middle-preserving pancreatectomy (MPP).Methods:The retrospective and descriptive study was conducted. The clinicopathological data of two patients who underwent MPP in Fujian Provincial Hospital from July 2019 to October 2019 were collected. Case 1 was a 52-year-old woman with multiple serous cystic neoplasms of the pancreatic head and tail. Case 2 was a 20-year-old man with chronic pancreatitis and multiple pancreatic duct stones. After comprehensive evaluation, two patients were performed laparoscopic pancreatic head tumor resection+ pancreatic tail resection+ splenectomy+ pancreaticojejunostomy and duodenum-preserving pancreatic head resection+ pancreatic tail resection+ lithotomy by longitudinal pancreatic duct incision+ side-to-side pancreaticojejunostomy, respectively. Observation indicators: surgery, operation time, volume of intraoperative blood loss, blood transfusion, postoperative fasting blood glucose, postoperative complications, duration of postoperative hospital stay, postoperative pathological examination, and follow-up. Follow-up was performed using outpatient examination and telephone interview to detect new-onset diabetes mellitus, pancreatic enzyme replacement therapy and disease recurrence up to March 2020.Results:Two patients successfully underwent MPP. The operation time of case 1 and case 2 were 470 minutes and 400 minutes, the volume of intraoperative blood loss were 200 mL and 100 mL, respectively. No blood transfusion was performed in either patient. The postoperative fasting blood glucose fluctuated between 5.4 and 11.8 mmol/L in case 1, and fluctuated between 5.9 and 11.3 mmol/L in case 2. Case 1 developed abdominal infection after operation, and was discharged after anti-infective treatment. Case 2 had good recovery with no complication. No pancreatic fistula or perioperative death occurred in two patients. The duration of postoperative hospital stay were 12 days and 8 days of case 1 and case 2, respectively. The pathological examination of case 1 showed serous cystadenoma of the pancreatic head and pancreatic tail. The pathological examination of case 2 showed that slightly dilated pancreatic ducts of the pancreatic head and tail with multiple stones and chronic inflammatory cells infiltration around the pancreatic ducts. The length of pancreas preserved was 8.5 cm and 8.3 cm of case 1 and case 2 on postoperative computed tomography (CT) reexamination. Two patients were followed up for 5 months and 7 months, respectively. During the follow-up, both patients had no new-onset diabetes and they didn′t require pancreatic enzyme replacement therapy. Both patients underwent upper abdominal CT examination at postoperative 5 months, which showed good blood supply in middle pancreas and no signs of recurrence of cystadenoma or stones.Conclusions:MPP is a safe and feasible procedure for the treatment of multifocal pancreatic lesions. The procedure can eradicate the lesions and ensure good control of blood glucose in patients.
