ObjectiveTo evaluate the performance of diagnosis-related groups,Beijing version (BJ-DRGs) which were locally developed in Beijing.MethodA total of 1.3 million inpatient records from 149 hospitals in Beijing in 2008 were drawn from Beijing Public Information Center.Coefficient of variation (CV) was used to measure the performance of DRGs system.ResultsBJ-DRGs produced the best CV results for expenditure.ConclusionsUnder Beijing's medical information condition,BJ-DRGs produced good performance.

This article reviewed the connotation,developing process and utilization theory of diagnosis related groups (DRGs).DRGs,which is one of ease mix systems,is now used widely in healthcare management.DRGs classify inpatient based on diagnose and procedure.Practices have shown that it has relative high capacity for the risk adjustment of acute care cases; therefore,it can improve the efficiency of expenditure and performance management of inpatient services.Currently,there are more than 30 countries and regions using DRGs,and the theories and methodologies of DRGs get mature.China's new round healthcare reform have developed in depth.The study and development of DRGs related health management tool will promote China' s healthcare system becoming more scientific,normalized and systematic.
3.DRGs-based methodology for medical performance evaluation and case studies

Authors described characteristics of medical service performance evaluation,and strategy options for scientifically evaluating such performance,and systematically presented the conditions and methodology of DRGs-based evaluation of medical service performance.Six evaluation indicators were cited in terms of the capacity,efficiency and medical safety of medical service providers,with detailed introduction to the computation process of such indicators.Cases in hand indicate that the first-page data completeness and accuracy of medical records are preconditions for using DRGs in medical service performance evaluation.

Objective Beijing plans to run a DRGs-PPS pilot.The relative height of each DRG group produced by the price of charge categories.Because of serious distortions of medical services prices,DRGs'weight must be adjusted.Methods Dividing medical service into 5 major class-medical,nursing,technique,drugs and consumables,management,a conversion with currency were established to produce the cost.Results The weight of groups involve intervention from MDCF are drop.ConclusionThe weight regulated can optimize resources allocation and reflect the ture value of medical service.

Diagnosis related groups (DRGs)have been widely used in many countries and regions.The versions of DRGs in different regions are modified according to the local circumstances and conditions.Beijing version DRGs (BJ-DRGs)is the first DRGs system in Chinese,which was developed locally based on the conditions of medical information system and health relative policies in Beijing.This paper introduced the grouping principles,logic and methodology of BJ-DRGs.Taking“ear,nose,mouth and throat disorders”for example,this paper demonstrated the grouping process of BJ-DRGs.
6.Anti-tumor Effects of Total Saponins Extract from the Aerial and Underground Parts ofParispolyphyllavar.yunnanensis

Objective To investigate the anti-tumor effects of total saponins,extracted from stems and leaves ofParispolyphyllavar.yunnanensis. Methods Taking mouse stomach carcinoma MFC cell line,human mammary cancer cell line(MCF-7)and human cervical carcinoma cell line(Hela),and their tumor-bearing mice as models,the antitumor activity was carried out in vitro and in vivo. CCK-8 assay was used to determine the inhibitory effect in vitro. The tumor-bearing mice model was induced by tumor cell vaccination in normal mouse forelimb left axillary subcutaneous and these mice were randomized into NS group,cytoxan group(30 mg/kg),saponins high-dose,medium-dose and low-dose groups(60,30 and 15 mg/kg). They were given intraperitoneal injection once a day for consecutive 15 days. The tumor inhibition rate and survival of the tumor-bearing mice were measured. Results Saponins,extracted from both aboveground and underground ofP.polyphyllavar.yunnanensis could significantly inhibit the growth of MFC,MCF-7 and Hela cells in time- and dose-dependent manners. The tumor weight in each drug treated group was significantly lower than that in the control group. Conclusion Saponins,extracted from both aboveground and underground ofP.polyphyllavar.yunnanensis have antitumor activity.
7.Effect of Qizhen capsule combined with docetaxel injection on neoadjuvant chemotherapy in patients with breast cancer and its safety evaluation

Objective To study the effect of Qizhen capsule combined with docetaxel injection on the neoadjuvant chemotherapy of breast cancer and its effects on cyclooxygenase (COX-2), cancer antigen 15-3 (CA15-3),and vascular endothelium growth factor (VEGF).Methods 84 patients with breast cancer were selected in our hospital from January 2015 to August 2016, those patients were divided into observation group and control group according to coin method.The control group received docetaxel injection, epirubicin injection, cyclophosphamide neoadjuvant chemotherapy, the observation group was treated with Qizhen capsule on the basis of the control group.The clinical efficacy, COX-2, CA15-3 and VEGF levels, quality of life and adverse reactions were compared between the two groups.Results After treatment, the total effective rate in the observation group (90.48%) was significantly higher than the control group (57.14%) (P<0.05).After treatment, the levels of COX-2, CA15-3 and VEGF in the observation group were significantly lower than the control group (P<0.05).The scores of cognitive function, emotional function, social function, role function and physical function of life quality in the observation group were significantly higher than the control group ( P<0.05 ) .Cardiac toxicity, nausea and vomiting, thrombocytopenia, alopecia, neutropenia and leukopenia were significantly lower in the observation group than the control group (P<0.05). Conclusion Qizhen capsule combined with docetaxel injection can significantly reduce the levels of COX-2, CA15-3 and VEGF in the neoadjuvant chemotherapy of breast cancer, and the clinical curative effect is good, which can effectively improve the quality of life of patients and reduce the adverse reaction rate.
8.Effect of intrathecal transplantation of different quantities of neural stem cells on neuropathic pain in rats

Objective To investigate the effect of intrathecal (IT) transplantation of different quantities of neural stem cells (NSCs) on neuropathic pain (NP) in rats.Methods Eighty-four adult pathogen-free male SD rats weighing 150-180 g were randomly divided into 7 groups ( n = 12 each) : group sham operation (S group) , NP group, NP+ NSCs 103 , 104 , 105 , 106 , 107 groups (N1-5 groups) . NP was induced by partial transection of right sciatic nerve. NSCs were transplanted into subarachnoid space in N1-5 groups. Paw withdrawal threshold to mechanical stimulation (MWT) and paw withdrawal latency to nociceptive thermal stimuli (TWL) were measured at 1 day before (baseline) and 1, 3, 7, 14 and 21 days after operation. Brain derived neurotrophic factor ( BDNF) expression in spinal dorsal horn and dorsal root ganglia (DRG) was detected by immuno-histochemistry and RT-PCR on the 7th and 21st day after operation. Results Partial transection of the sciatic nerve gradually reduced MWT and TWL after operation starting from day 1 until day 7 and 14 as compared with the baseline in group NP. IT NSC transplantation significantly increased MWT and TWL and expression of BDNF in spinal dorsal horn and DRG in a dese-dependent manner at day 7 after operation in N1-5 groups as compared with group NP. There were no significant differences in MWT and TWL and BDNF expression among N3, N4 and N5 groups at day 21 after operation.Conclusion The proper quantity of transplanted NSCs which are able to ameliorate NP induced by partial transection of sciatic nerve in rats is 105 .
10.Protective Effects of Rosemary's Extract on Cerebral Ischemia in Mice and Acute Toxicity Evaluation

Objective To evaluate the protective effects of Rosemary's chloroform extract on cerebral ischemia in mice and acute toxicity.Methods The protective of rosemary's chloroform extract on cerebral ischemia was observed and compared by the mice models of cerebral anoxia and ischemia,thrombus formation in vivo,and chloroform induced arrhythmias.Rosemary's Chloroform Extract was given orally to mice to evaluate acute toxicity.Results Rosemary's chloroform extract had different degrees of inhibition of collagen-the adrenaline induced thrombus formation,and prolonged acute ischemic mice brains off mouth breathing time and increase the number of mouth breathing (P <0.05 or P <0.01);Chloroform extract significantly reduced the incidence of chloroform induced ventricular arrhythmias.Acute toxicity results suggested that a female rat died in the next day of chloroform extract group,no additional toxicity was observed.Conclusion Rosemary's chloroforrm extract has significant protective effect on cerebral ischemia and hypoxia in mice.
