1.Progress in the treatment of cholecystolithiasis combined with choledocholithiasis

Cholecystolithiasis is a common disease of the digestive system. In recent years, due to the changes in diet and daily living habits, the incidence of cholecystolithiasis in the world is rising gradually. Choledocholithiasis can form initially in situ, mainly due to infection and cholestasis, but also secondary to cholecystolithiasis or intrahepatic bile duct stones.Symptomatic gallstone disease in patients with laparoscopic cholecystectomy is the standard treatment.However, for patients with common bile duct calculi, traditional open surgery, laparoscopic combined choledochoscope, laparoscopic combined endoscopic and laparoscopic combined laser lithotripsy are all effective ways of treatment. But which one specific operation method is the best choice has yet to agree a consensus. In this paper, several different surgical treatment methods of common bile duct calculi are reviewed in order to provide certain reference value for clinical practice.
2.ffect of basic fibroblast growth factor on rabbit lens epithelial cell proliferation

Objective To study the effect of basic fibroblast growth factor (bFGF) on proliferating of rabbit lens epithelial cells(RLECs).Methods The second and three generations of RLECs were exposed to different concentrations of bFGF, and the proliferation characteristics of the cells were measured with methyl thiazolyl tetrazolium (MTT) assay. The ultrastructure of cell were observed with transmission electron microscope (TEM).The changs of cells cycles were observed with flow cytometer (FCM).Results After the treatment of bFGF, RLECs showed the marked proliferation , especially in 10μg*L-1 of bFGF . TEM result showed that the cells were more active with bFGF, FCM result showed that the S phase cell obviously increased.Conclusion bFGF is an important factor that can promote proliferation of RLECs.
3.Calcium release-activated calcium channel modulator 1 promotes the migration and invasion of SW480 colon cancer cell line

Objective:To explore the effect of calcium release-activated calcium channel modulator 1 (ORAI1) on the migration and invasion of colon cancer cell line SW480 and its mechanism. Methods:The SW480 cells were infected with ORAI interference lentivirus. The expression of ORAI1 mRNA and protein was confirmed by quantitative real-time polymerase chain reaction and Western blot. Transwell chamber, adhesion, angiogenesis, and vasculogenic mimicry experiments were conducted to detect the ability of cell invasion, migration, and angiogenesis and the intercellular adhesion of homogeneous and heterogeneous cells among each group. Confocal microscopy was employed to detect the difference of store-operated Ca2+entry (SOCE) in each group. Western blott was used to detect the expression of ERK1/2, p-ERK1/2, MMP-2, VEGF, and E-cadherin protein. Results:After the infection of SW480 with the ORAI1 interference lentivirus for 72 h, significant fluorescence expression was observed. Compared with the empty vector group and control group, the expression of ORAI1 was lower in the interference group (P<0.01). Invasion and migration ability decreased (P<0.01); the intercellular adhesion ability of homogeneous cells increased (P<0.05); the intercellular adhesion ability of heterogeneous cells decreased (P<0.05);the angiogenesi and vasculogenic mimicry were enhanced (P<0.01);the internal flow peak of SOCE was low (P<0.05); the expression of p-ERK1/2, MMP-2, and VEGF proteins decreased (P<0.01); and the expression of E-cadherin protein increased (P<0.01). Conclusion:ORAI1 may promote the migration and invasion of SW480. This mechanism may be associated with the increase of SOCE.
4.Correlation between lactate/albumin ratio level and incidence of multiple organ dysfunction syndrome as well as mortality in patients with sepsis

Objective To study the correlation between lactate/albumin ratio level and incidence of multiple organ dysfunction syndrome (MODS) as well as mortality in patients with severe sepsis or septic shock.Methods In January 2012-September 2013, 54 patients who were admitted to the intensive care unit(ICU) of a hospital developed severe sepsis and septic shock on the first day of admission, clinical data of patients were analyzed.Results On the first and second days of admission, 30(55.56%)and 26(53.06%)patients developed MODS;lactate/albumin ratio between MODS group and non-MODS group on the first and second days of admission were both significantly different (both P<0.05).Multivariate logistic regression analysis showed that lactate/albumin ratio, oxygenation index (PaO2/FiO2), as well as acute physiology and chronic health evaluation (APACHE Ⅱ) were independent risk factors for predicting MODS in patients with severe sepsis.Lactate/albumin ratios between MODS group and non-MODS group, death group and non-death group were both significantly different (both P<0.05);lactate/albumin ratio was correlated with APACHE Ⅱand PaO2/FiO2, the higher the APACHE Ⅱ score and the lower the PaO2/FiO2, the higher of lactate/albumin ratio.The area under the receiver operating curve (ROC curve) analysis showed that incidence and mortality of MODS on the first day of admission predicted by lactate /albumin ratio were 0.85 and 0.84 respectively;sensitivity, specificity, positive predictive value, and negative predictive value of occurrence of MODS predicted by lactate /albumin ratio>1.735 were 80.00%, 79.17%, 82.67%, and 75.92% respectively, sensitivity, specificity, positive predictive value, and negative predictive value of mortality were 100.00%, 51.02%, 17.23%, and 100.00% respectively.Conclusion Lactate/albumin ratio level is closely correlated with incidence and mortality of MODS in patients with severe sepsis or septic shock.
5.Analysis of short-term outcomes for lobar lung transplantation

Objective:To explore the clinical value of lobar lung transplantation for end-stage lung disease patients in organ donation era.Methods:Clinical data were analyzed retrospectively for 14 cases with lobar lung transplantations between January 2016 and December 2019, including clinical outcomes and postoperative complications.Results:Eleven cases(78.6 %)had a positive etiology examination in bronchial secretion or tissue culture. There were unilateral lobar lung transplantation (n=2), bilateral lobar lung transplantation(n=2)and unilateral lobar lung transplantation and contralateral lung transplantation(n=10). Intra-operative ECMO(n=11)postoperative ECMO(n=5)were required. All survived during a 30-day perioperative period. The median time of postoperative ECMO was 1(1~11)day, the median time of extubation 4.5(0~182)day and the median time of stay in ICU 11(2~186)day. Re-operation was required for 1 case due to active bleeding in thoracic cavity. There were 10 survivors and 4 deaths. The causes of death were bronchus fistula(n=2), pulmonary infection(n=1)and renal failure(n=1)respectively.Conclusions:Lobar lung transplantation is efficacious for selected patients with end-stage lung disease.
