1.Observation On the Curative Effect of Tangshentongbao Decoction Combined Benazepril on 31Cases of Diabetic Nephropathy in Clinical Stage

Objective To assess the protective effect of Tangshentongbao decoction combined benazepril on renal function of the patients with diabetic nephropathy(DN) in clinical stage and to make a effective contrast to the therapy with western medicine. Methods 62 patients with DN in clinical stage were randomly divided into a control group and a treatment group, with 31 patients in each group. Both groups were supplied with the same food, adopted glucose control therapy and symptoms orientated treatment. On these basis, the treatment group was administered with Tangshentongbao decoction combined benazepril, and the control group was taken kallikrein enteric tablet combined benazepril. The therapeutic courses for both groups were 8 months. The changes of symptoms and physical signs, 24-hour urinay protein (24hUpro), serum cteatimine (SCr), serum creatinine clearance erate (CCr) , blood urea nitrogen (BUN), triglyceride (TG), cholesterd (CHO) and low density lipoprotein-cholesterol (LDL-C)of the two groups were separately assessed. Results 24hUpro, SCr, TG, CHO and LDL-C of treated group were all reduced after the treatment (P<0.01 or P <0.05), showing significant difference compared with the control group. Conclusion 24hUpro and SCr of patients with DN in clinical stage can be evidently reduced by Tangshentongbao decoction, and the effect of reducing blood lipid is remarkable better than western medicine alone.

3.The Protective Effects of Nitric Oxide Inhalation on Rabbit Lung Ischemia/reperfusion Injury

Objective To investigate the protective effects and mechanism of nitric oxide(NO) inhalation on rabbit lung ischemia/reperfusion injury. Methods Total 32 Japanese white rabbits were randomly divided into four groups, each group containing 8 rabbits. In group A, animals received only thoracotomy. In group B, after animals left chest were opened, their left main pulmonary arteries were occluded for 2h, then reperfused for another 2h. In group C, animal inhaled 20 ppm NO for 30min since the eleventh minute after reperfusion. In group D, animal inhaled 60ppm NO for 30min since the eleventh minutes after reperfusion. The levels of blood PaO 2, cGMP, slCAM-1, XOD and MDA were measured before operation and after reperfusion. The pathologic changes of left lung was observed. 4.Diagnosis and Management of Anatomic Variations of the Cystic Duct during Laparoscopic Cholecystectomy

Objective:To explore the diagnosis and management of anatomic variation of the cystic duct during laparoscopic cholecystectomy(LC) .Methods :From Jan 1999 to Dec 2013 ,4652 cases of LC ,which were performed in Qingpu Branch of Zhongshan Hospital ,Fudan University ,were retrospectively analyzed .A total of 34 cases of anatomic variation of the cystic duct were found .Among the 34 cases of anatomic variation of the cystic duct ,11 cases had thick and short cystic duct ,7 cases had a cystic duct converged into right hepatic duct ,6 cases had cystic duct converged into lower part of common hepatic duct ,1 case had cystic duct pronated forward and converged into common hepatic duct from left side ,3 cases had cystic duct that paral-lel itself with common hepatic duct and then converged into common bile duct ,4 cases were diagnosed with Mrizzi syndrome ,2 cases had situs viscera inversus .Results:Among the 34 cases with anatomic variations of cystic duct ,22 cases successfully un-derwent LC ,the other 12 cases were converted to open cholecystectomy .All the complications were biliary duct injury that re-covered well after treatment .Conclusions :Anatomic variation of the cystic duct is hard to diagnose preoperatively .Paying More attention to the anatomic variation during LC ,dissecting calot′s triangle carefully ,and identifying the relationship between the various pipeline is the key to prevent and reduce the biliary duct injury .

Objective:
To describe the clinical manifestations of central nerve system inflammatory demyelinating disease associated with anti-myelin oligodendrocyte glycoprotein antibody (MOG-IDD) in children, and to explore the clinical characteristics of the children.
Methods:
The clinical and laboratory characteristics of the patients diagnosed in Beijing Children′s Hospital, Capital Medical University, from October 2016 to August 2018 were described, and the clinical data of the patients with unipolar and recurrent diseases were compared.
Results:
A total of 50 patients were included, among whom the ratio of male to female was 24:26, and the average age of onset was (6.7±3.1) years old (0.4-12.6 years old). There was no significant difference in the age of onset between boys and girls(
6.Comparison of Quality of Life of the Patients Three Months after Uniportal and Multiportal Thoracoscopic Lobectomy.

BACKGROUND:
The relationship between quality of life at three months after lung cancer surgery and different surgical approaches is remains unclear. This study aimed to compare the quality of life of patients three months after uniportal and multiportal thoracoscopic lobectomy.
METHODS:
Data from patients who underwent lung surgery at the Department of Thoracic Surgery, Sichuan Cancer Hospital between April 2021 and October 2021 were collected. The European Organization for Research and Treatment of Quality of Life Questionnaire-Core 30 (EORTC QLQ-C30) and Quality of Life Questionnaire-Lung Cancer 29 (EORTC QLQ-LC29) were used to collect quality of life data of the patients. Potential confounding factors in the baseline data were included in a multivariate regression model for adjustment, and the quality of life of the two groups three months postoperatively was compared with traditional clinical outcomes.
RESULTS:
A total of 130 lung cancer patients were included, with 57 males (43.8%) and 73 females (56.2%), and an average age of (57.1±9.5) yr. In the baseline data of the two groups, there was a statistical difference in the number of chest drainage tubes placed (P<0.001). After adjustment with the regression model, at three months postoperatively, there were no significant differences in all symptoms and functional status scores between the two groups (all P>0.05). The multiportal group had longer surgery time (120.0 min vs 85.0 min, P=0.001), postoperative hospital stay (6.0 d vs 4.0 d, P=0.020), and a higher incidence of early ≥ grade 2 complications (39.0% vs 10.1%, P=0.011) compared to the uniportal group.
CONCLUSIONS
Patients undergoing uniportal and multiportal thoracoscopic lobectomy have similar quality of life at three months postoperatively. The uniportal group may have certain advantages in terms of traditional clinical outcome indicators such as operation time, postoperative hospital stay, and early postoperative complications.
