1.The application of information sharing platform for obsolete medical equipment

Objective:To explore the information technology to improve the efficiency of information sharing platform for obsolete and eliminated medical equipment.Methods: To design and develop a information sharing platform for obsolete medical equipment. This platform included platform construction, function introduction and application effect based on information characteristics of obsolete and elimination medical equipment, and the key point of information sharing. And to explore the next development direction for this platform.Results:This platform was based on internet technology and it can be used to share relevant information of obsolete and elimination medical equipment in a certain region. This platform included obsolete equipment inquiry, disposal information announcement and service obtaining and so on.Conclusion: The successful application of this platform standardize the management system to obsolete and dispose medical equipment. And it can unify the operation process and improve work efficiency. At the same time, it is a powerful management tool for relevant department and it can achieve preservation and increment of national asset in medical institutions.
2.EVALUATING THE VALUE OF COLOR DOPPLER FLOW IMAGING IN DETECTION OF DEEP VENOUS THROMBOSIS OF THE LOWER LIMBS

Objective To explore the value of 2 dimension and color Doppler flow imaging (2D CDFI) in detection of deep venous thrombosis of the lower limbs. Methods One hundred and one patients, 118 limbs clinically suspected as deep venous thrombosis were examined by CDFI and ascending phlebography. Results The sensitivity of ultrasonogram was 94 4% and accuracy was 98 4%. The highest incidence is 30 4% in the femoral vein. With the ratio 2 2∶1, the incidence of left lower limbs was much higher than the right. Because of different pathological changes in acute, subacute and chronic thrombosis, there are diverse character in 2D CDFI. Conclusions Deep venous thrombosis of the lower limbs can be diagnosed by 2D CDFI, which is a noninvasive, easy and accurate method.
3.Effect of intermittent catheterization on the urinary tract Infection in patients with spinal cord injury

Objective To access the effect of intermittent catheterization (IC) on the urinary tract infection in patients with spinal cord injury. Methods Sixty patients of spinal cord injury combined with urinary tract infec-tion were randomly divided into two groups: treatment group (32 patients) treated with sensitive antibiotics and IC;control group (28 patients) treated with sensitive antibiotics only. Both groups were treated for two weeks. Results In the treatment and control group,the cure rate was 56% and 29% ,respectively,the total effective rate was 91% and 64% in the treatment and control group,respectively,the clearance rate of bacteria was 81% and 57% ,respec-tively. All differences were significant (P<0.05). Conclusions IC can improve the effect of antibiotic on urinary tract infection in patients with spinal cord injury, and might be effective on the treatment of urinary tract infection caused by various neurogenic bladders.
4.Value of ultrasonography in early diagnosis of metatarsal fatigue fractures

Objective To observe the ultrasonic manifestation of metatarsal fatigue fractures and probe into the clinical values of ultrasonic diagnosis and follow-up to the disease.Methods The clinical data and sonographic features of 30 cases with metatarsal fatigue fracture confirmed by X-ray and CT were analyzed retrospectively.Results All the subjects in this study were metatarsals insufficiency fractures located in the second or third diaphysis.2D-ultrasound found microfracture in the cortex and the periosteal elevation by increased vascularity in the early stage.During the 3-month follow-up,thickened cortex and callus shown as the shape of mushroom or cauliflower and a great amount of neovascularisation were visualised with color Doppler flow imaging.Then,the cortex surface became gradually changing from rough to smooth and obvious posterior shadows were observed with the illness progression.Conclusions Ultrasonography could be used to detect the cortical continuity,callus characteristics and blood flow perfusion of fatigue fracture in the different damage stages.
5.The effect of mental abacus calculation training on form recognition of children: an ERP study

ObjectiveTo explore the influence of mental abacus calculation (MAC) training on children's form recognition.Methods 28 children ( 14 children with MAC training and 14 children with non-MAC traning)were investigated by using event-related potential(ERP) technology.The event-related potentials were recorded when subjects were in the different form shape recognition.Results ( 1 ) The amplitudes of the posterior P1 evoked by the children with MAC training ( (9.59 ± 3.58) μV) were significantly greater than that of the children without MAC training ( (7.06 ± 2.84) μV) ( P < 0.05 ).(2) The amplitudes of right temporal-posterior N170 of children with MAC( ( -9.83 ±2.97) μV) were markedly less than that of children with non-MAC( ( - 12.45 ±2.51 ) μV) ( P < 0.05 ).( 3 ) The amplitudes of prefrontal P3 of children with MAC ( (7.65 ± 2.25 ) μV) were remarkably greater than that of children with non-MAC ( (4.89 ± 3.35 ) μV) ( P < 0.05 ).ConclusionThe brain function of form recognition in children is influenced by mental abacus calculation training.
7.Study of expression and clinical significance of Livin and VEGF protein in bladder urothelial carcinoma

Objective To investigate the expressions of Livin protein and VEGF protein in bladder urothelial carcinoma(BUC) ,and theirs relationships with the clinicopathologic parameters of bladder urothelial carcinoma. Methods The expression of Livin and VEGF protein in 69 samples of BUC tissue and 10 samples of normal bladder epithelium tissue were detected by immunohistochemical SP method. Their relationships with clinicopathologic data were statistically analyzed. Results The positive expression rate of Livin and VEGF in BUC tissues were 65.2% (45/69) and 46.4% ( 32/69), but negative in normal bladder epithelium tissues, which showed significant differences in the comparison (Ps < 0. 05 ). We found significant difference in the comparison of Livin positive rate between groups with or without recurrence ( 78. 8 % vs 48. 1%, χ2 = 6. 13, P < 0. 05 ); but no differences in pathological grade,TNM stage and tumor number( Gl 55.6% ,G2 64. 3% ,G3 73.9% ;Ta ~ T1 61.9% ,T2 ~ T4 70. 4%; Single-tumor 59. 6%, multi-tumor 77.3%; χ2 = 1.52,0. 52,2.07, Ps > 0. 05 ). For BUC,the expression of VEGF was correlated with the pathological grade,TNM stage( Gl 16. 7% ,G2 53.6% ,G3 60. 9%, χ2 = 8. 91; Ta ~ T1 33.3%, T2 ~ T4 66. 7%; χ2 = 7. 34; Ps < 0. 05 ), but not the tumor number and recurrence( Single-tumor 57.4% , multi-tumor 59. 1%, χ2 = 0. 01; with recurrence 51.5% , without recurrence 40. 7% ,χ2= 0. 69; Ps > 0. 05 ). We found no relationship between the expression of Livin and VEGF (r =0. 056,P > 0. 05 ). Conclusion The overexpressions of Livin and VEGF protein may play an important role in the occurrence and development of BUC. Combind detection of these two protein can be used in the diagnosis and prognosis of BUC.
8.Formation mechanism and surgical choice of teardrop fracture of the axis

Objective To discuss the formation mechanism of teardrop fracture of the axis and observe the clinical efficacy of anterior or posterior induction operation according to the fracture type.Methods Thirteen patients with teardrop fracture of the axis treated from May 2001 to October 2010 were involved in the study.There were 10 males and 3 females,at age range of 18-55 years (mean,35.5 years).Four patients were combined with cervical spinal cord injury (one patient with grade C and two with grade D according American Spinal Injury Association,ASIA).Injury causes included traffic injury in 11 patients and fall from height in two.After admission,the anteroposterior,lateral and mouth X-ray films,spiral CT sagittal reconstruction and MRI examination of the cervical vertebra were performed in all the patients.According to the injury mechanism,there were 10 patients with extension fractures treated with C2,3 intervertebral bone fusion and internal fixation through submandibular retropharyngeal approach and three with flexion fractures treated with posterior C2,3 intervertebral bone fusion and pedicle screw fixation.The clinical efficacy was also observed. Results All patients received complete exposure and effective reduction and fixation.The average time of anterior exposure and posterior exposure was 77 minutes and 125 minutes,respectively.No surgical complications occurred.All patients were followed up for 9-34 months (mean,13 months).Reexamination at four months after operation displayed bone healing in all patients.There was no lessening or breakage of the internal fixators.The spinal function of three patients with ASIA grade D injury and that of one patient with ASIA grade C injury recovered to normal at postoperative 1.5 and 6 months respectively. Conclusion The surgical approaches for teardrop fracture of the axis developed in the light of the fracture formation mechanism are beneficial and safe.
9.Laparoscopic left renal vein extravascular stenting for treatment of left renal vein entrapment syndrome

ObjectiveTo investigate the surgical management of left renal vein entrapment syndrome.MethodsEight cases with left renal vein entrapment syndrome (5 males and 3 female ; mean age 26 years) with history of gross hematuria for 3 to 46 months were reviewed.Doppler ultrasound reports suggested compression of the left renal vein at mesenteric angle in all cases.And the dilated segment of the left vein was three-fold than the stricture segment in diameter.CT scan showed the abnormal angle between aorta and superior mesentery artery in all cases.Bleeding from the left ureteral orifice was detected by cystoscopy in 6 cases.We treated 8 patients by extravascular stent immobilization with laparoscope.ResultsThe operation was successful in the 8 cases without surgical complications.The average operation time was 63 min.The average blood loss was 14 ml,and the average hospital stay after operation was 6 days.Follow-up of 3 -20 months,there was no hematuria relapse since been relieved in 7 cases,one case remained microscopic hematuria.Color Doppler ultrasound examination in all 8 cases showed the narrowest inner diameter of left renal vein was 7.4 mm (6.5 - 8.7 mm),the blood flow was smooth.The angle between abdominal aorta and superior mesenteric artery become normal.Conclusions Laparoscopic left renal vein extravascular stenting could be a new surgical method to treat left renal vein entrapment syndrome.The method of putting artificial blood vessel around renal vein is simple,safe and effective.
10.Prognosis of the metastatic prostatic carcinoma and analyze the relative factors

Objective To discuss the prognosis of the metastatic prostatic carcinoma and analyze the relative factors. Methods From 2001 to 2002, 32 cases of matastatic prostatic carcinoma were admitted to our hospital, the ages ranging from 54 to 87, with the mean age of 71 years. All the diagnosis was proved by the six cores transrectal biopsy of the prostate. The serum PSA ranged from 63 to 2000 ng/ml. Two cases had the Gleason score of 2 - 6, 19 cases had the Gleason score of 7 and 11 cases had the Gleason score of 8 - 10. Preoperative routine examinations included serum testosterone, pelvic CT or MRI scan, CXR and bone scan. Twenty-seven cases were found to have osseous metastasis, among them 8 cases combined with lymph nodes metastasis, and 5 cases with lymph nodes metastasis only. All of them were treated with gonadectomy and the blockade of the androgen receptor (with Bicalutamide 50 mg/d or Flutamide 250 mg/d). It lasted 7 - 48 months with first endocrine therapy,and mid last time 23 months. After first line endocrine therapy failure, Diethylstilbestrol and Estramustine were used in the androgen-independent stage. Thirteen cases were not effective, and second therapy had effect to 19 cases and it lasted for 3 - 15 months. Results The follow-up periods ranged from 13 to 98 months, with the mean period of 33 months. Twenty-eight cases died, 4 cases survived. The median survial period was 37 months, 1-year survival rate was 100% (32/32), 3-year survival rate was 53%(17/32) , 5-year survival rate was 19%(6/23). As to the survival period, there were negative correlaions between survival and the level of PSA (r= -0. 262, P=0. 045) and the Gl-eason score (r=- 0. 624, P=0. 001). There were positive correlations between survival and the level of testosterone (r=0. 514, P=0. 008) and the age (r=0. 311, P = 0. 032). And there was no correlation between survival and the number of the positive cores of the biopsy (r=0. 211, F = 0. 158) and the clinical stage (r=0. 211, P = 0. 352). Conclusions As to the metastatic prostatic carcinoma, there are correlations between the survival and the levels of testosterone before treatment, age, the Gl-eason score and the level of PSA. There is no relationship between the survival period of the metastatic prostatic carcinoma and the number of the positive cores of the biopsy and the clinical stage.
