1.AmpC ?-lactamases producing escherichia coli and klebsoella pneumoniae:a survey and analysis

Objective To detect AmpC ?-lactamases from clinical isolates of escherichia coli and klebsoella pneumoniae in our hospital.Methods Using confirmatory test recommended by the NCCLS to detect ESBLs producing escherichia coli and klebsoella pneumoniae.Adopting three dimensional extract test and three dimensional depression test to detect AmpC ?-lactamases.Results Among 236 escherichia coli strains collected,104 were ESBLs producing strains.Among 135 klebsoella pneumoniae strains collected,29 were ESBLs producing strains.Among 106 cefoxitin-resistant strains,AmpC ?-lactamases producing strains were found in 3 strains.Conclusion ESBLs are the most important resistant mechanisms of the two bacteria isolates in our hospital.The strains cefoxitin-resistant may result from the loss of the membrane porin.
2.Percutaneous endoscopic lumbar discectomy versus microendoscopic discectomy for lumbar disc herniation: a Meta-analysis

Objective To compare the curative effect of percutaneous endoscopic lumbar discectomy (PELD) and microendoscopic discectomy (MED) in lumbar disc herniation.Methods A literature search was performed in PubMed, Web of Science, Embase, Wanfang, CNKI. Two authors reviewed all articles individually. The methodological quality of RCTs was assessed by the Cochrane risk of bias tool, and the quality of retrospective studies was evaluated by the modified Newcastle-Ottawa scale. The data was extracted by the Review Manager 5.30.Results A total of 19 articles were brought into this Meta-analysis. The outcomes were divided into primary outcomes and secondary outcomes. Excerpt for the short-term VAS score was lower in PELD group (P = 0.010), other index, including long-term VAS score (P = 0.120), ODI score (P = 0.260), complication (P = 0.100) and recurrence (P = 0.100), didn't had significant difference in two groups. The blood loss (P = 0.000), hospital stay (P = 0.000) and the length of incision (P = 0.000) were all superiority in PELD group. Whereas the operative time was shorter in MED group (P = 0.001).Conclusion PELD was a more minimally invasive and secure technique in lumbar disc herniation.

Objective To construct the recombinant yeast expressing PreS2 120-146-hepatitis B surface antigen (HBsAg),and to evaluate the immune effects of whole yeast cells.Methods PreS2 120-146 and HBsAg gene sequence were optimized according to the yeast cell codon preference,and were recombined and cloned into pPIC3.5K yeast expression vector to construct pPIC3.5K/PreS2 120-146 plasmid.After digested and linearized by Bgk Ⅱ restriction enzyme,pPIC3.5K/PreS2 120-146-HBsAg recombinant plasmid was electrotransformed into GS115 strain to screen PreS2 120-146-HBsAg-recombinant Pichiapastoris .The expression of PreS2 120-146-HBsAg was identified by sodium doclecyl sulfate polyacrylamide gel electrophogesis (SDS-PAGE),Western blot and enzyme linked immunosorbent assay (ELISA)analysis. BALB/c mice were vaccinated by inactivated whole recombinant yeast cells expressing target protein. Specific antibodies to HBsAg were detected by ELISA.Cytotoxic T lymphocyte (CTL)response induced by interferon (IFN)-γ was detected by reverse transcription-polymerase chain reaction (RT-PCR)when immune spleen cells of mice were stimulated by CTL epitope on HBsAg.Independent sample t test was used. Results Data of PCR detection,restriction enzyme digestion and sequencing analysis showed that recombinant pPIC3.5K/PreS2 120-146-HBsAg plasmid was successfully constructed.SDS-PAGE,Western blot and ELISA verified the expression of PreS2 120-146-HBsAg in the lysate of the recombinant Pichiapastoris induced by methanol.Levels of specific anti-HBsAg IgG antibodies produced by inactivated yeast cells vaccinated mice were comparable to purified HBsAg immunization (t =0.946,P =0.381 ). Analysis of HBsAg-specific CTL responses revealed that the level of IFN-γwas significantly higher when the immune spleen cells of mice were stimulated by CTL epitope peptides on HBsAg (t =2.305 ,P =0.044).Conclusions PreS2 120-146-HBsAg target protein is successfully expressed by construction of recombinant Pichiapastoris . The specific humoral and cellular immune responses are induced by recombinant whole yeast cells vaccinated mice.
5.Determination of urinary iodine by catalytic spectrophotometry using discrete chemical analyzer

Objective To establish a method for quantitative detection of urine iodine by catalytic spectrophotometry using discrete chemical analyzer.Methods After digestion of urine samples,discrete chemical analyzer was used to control the reaction temperature and time,determine the iodine content,which replaced the urine iodine arsenic cerium catalytic spectrophotometry (standard method) after sample digestion in the testing process,and the linear range,detection limit,precision and accuracy of the method were tested.The method was used to detect the urine iodine and the results were compared with the standard method.Results The urinary iodine was in a linear range of 0-1 000 μg/L,correlation coefficient r =-0.999 7.The detection limit of the method was 4.5 μg/L.Precision:the relative standard deviation (RSD) of urine iodine samples with high,medium,and low concentrations were all < 5%.Accuracy:the contents of urinary iodine in reference materials at high,medium,and low concentrations were within their respective given value ranges.No statistical significant was found in the urinary iodine content detected either by this method or the standard method (t =0.643 8,P > 0.05).Conclusion Catalytic spectrophotometry using discrete chemical analyzer is succeessfully established,this method is simple,rapid,accurate and easy to operate.
6. Open reduction and internal fixation via modified Stoppa approach for pelvic fractures of Tile type C combined with acetabular both-column fractures

Objective:
To evaluate the clinical efficacy of the modified Stoppa approach in the surgical treatment of pelvic fractures of Tile type C combined with acetabular both-column fractures.
Methods:
Between April 2014 and April 2017, a total of 14 patients were treated by open reduction and internal fixation (ORIF) through the modified Stoppa approach at Department of Orthopaedics, Foshan Gaoming Hospital for pelvic fractures of Tile type C combined with acetabular both-column fractures. They were 10 men and 4 women, with an average age of 36.4 years (from 23 to 57 years). The modified Stoppa approach was used exclusively in 4 cases, in combination with the iliac fossa approach in 3 ones, in combination with the Kocher-Langenbeck approach in 4 ones and in combination with the Kocher-Langenbeck and iliac fossa approaches in 3 ones. In all the patients, the pelvic reconstructive plate and screws and lag screws were used. The operation time, intraoperative bleeding volume, postoperative fracture reduction, fracture union time, efficacy and complications were recorded.
Results:
The operation time averaged 170 min (from 110 to 330 min) and the intraoperative bleeding 420 mL (from 240 to 1 100 mL). There were no operative complications. By the Matta evaluation, the postoperative reduction was rated as excellent in the 14 pelvic fractures and 9 acetabular both-column fractures and as good in 5 acetabular both-column fractures. Of this series, 13 patients were followed up for an average of 19 months (from 12 to 29 months) and one was lost to the follow-up. The fractures got united after an average time of 3.4 months (from 2.7 to 4.6 months). Screw loosening was observed in one case and mild limitation to hip flexion in one. Follow-ups found no lateral ventral syndrome or femoral head necrosis. Their Harris hip scores at the last follow-up ranged from 70 to 94 points, averaging 84 points. The function of the affected hip was excellent in 6 cases, good in 5 and fair in 2.
Conclusion
The modified Stoppa approach may be used exclusively or in combination with other approaches to treat effectively the pelvic fractures of Tile type C combined with acetabular both-column fractures, leading to good short-term clinical outcomes.
7.Determination of iodine in salt by arsenic-cerium catalytic spectrophotometry

Objective:To establish a method for determination of iodine in salt by arsenic-cerium catalytic spectrophotometry.Methods:The content of iodine in salt was detected by arsenic-cerium catalytic spectrophotometry with an automatic iodine analyzer. The standard curve linearity, detection limit, precision and accuracy of the method were evaluated. The iodine content of 20 edible salt samples was detected by the newly established method and direct titration, and the results were compared.Results:In the range of 0 - 150 μg/L standard curve, the correlation coefficient ( r) = - 0.999 9, and the detection limit was 1.4 mg/kg. The average iodine contents of iodine composition analysis standard materials GBW10006z and GBW10007z were 12.2 and 22.8 mg/kg ( n = 6), respectively, which were all within the given standard value ranges, and the relative standard deviations ( RSD) were 2.04% and 2.33%, respectively. Iodine composition analysis standard materials GBW10006b, GBW10007b, GBW10006v, GBW10007v, GBW10006z and GBW10007z measurement results (12.0, 24.6, 12.6, 22.8, 12.3, 23.2 mg/kg, n = 2) were all within the given standard value ranges, with good quality control. The iodine content of 20 edible salt samples was detected by arsenic-cerium catalytic spectrophotometry and direct titration, respectively, and the difference was not statistically significant ( t = 1.99, P = 0.060). Conclusion:Arsenic-cerium catalytic spectrophotometry has the characteristics of good linear relationship, low detection limit, good precision and high accuracy in determination of salt iodine content, which is suitable for popularization and application.
