1.Rho kinase and ischemic cerebrovascular disease

Ischemic cerebrovascular disease may result in severe neurological deficits, and even death. Rho kinase is an important downstream substrate of Rho protein. It involves in multiple biological effects, including vascular smooth muscle contraction, cell migration, inflammatory cell infiltration and vascular endothelial dysfunction. It plays important roles in the processes of the occurrence, development and expansion of infarct volume in ischemic cerebrovascular disease. Rho kinase inhibitor can effectively relax vascular spasm, reduce local inflammatory response, protect endothelial function, increase blood flow in ischemic region, reduce infarct volume, and improve neurological function.
2.Upgrading No.1 Military Medical Project Database from Oracle 8.17 to Oracle 10g

Objective To upgrade No.1 Military Medial Project database from Oracle8.17 to Oracle10g. Methods Through the establishment of database link, the database migration was realized based on full test's foundation. Results The database was upgraded and the system's operation speed was enhanced greatly. Conclusion Under the existing technology, it is feasible to upgrade Oracle database from Oracle8.17 in 32 bits to Oracle10g in 64 bits, and the benefit is remarkable.
3.Clinical significance of serum sialic acid in diagnosis and treatment of IgA type multiple myeloma

Objective To investigate the clinical significance of serum sialic acid (SA) in the diagnosis and treatment of IgA type multiple myeloma (MM). Methods The level of serum SA in 50 healthy subjects and 70 patients with MM were determined. Results In MM group, there were 25 cases of IgA type, 51 cases of other than IgA type (40 cases of IgG type, 7 cases of light chain type MM, 4 cases of non-secreting type MM). In healthy subjects, patients with IgA, IgG, light chain, non-secreting types and other than IgA type, the serum SA levels were (570.33 ± 67.72) mg/ L, (1289.24 ± 325.42) mg/ L, (585.88 ± 159.12) mg/L, (600.77 ± 126.90) mg/L, (590.50 ± 100.86) mg/L, and (588.39 ± 150.90) mg/L. The serum SA level of IgA type patients was higher than those of healthy subjects and other types (all P<0.01), and there was no significant difference among the other groups (all P> 0.05). The serum SA level of IgA type patients had positive correlations with serum IgA, globulin, bone marrow smear myeloma cell population and ESR (r values were 0.699, 0.753, 0.504, and 0.732, all P< 0.05); the serum SA level of IgA type patients had negative correlations with serum albumin, IgG, hemoglobin and platelet (r values were -0.712, -0.505, -0.629, and -0.596, all P< 0.05). The serum SA level of IgA type patients had no correlation with leukocyte, serum creatinine, serum IgM, lactic dehydrogenase, serum calcium, C-reactive protein and β2-microglobulin (all P>0.05). The level of serum SA in IgA type patients decreased with the improvement of the condition, and increased with the severity of the disease. Conclusions In MM patients, serum SA level in IgA type is high. The serum SA is an index for reflecting the tumor load capacity, and can evaluate the therapeutic efficiency of IgA type MM patients.
5.Effect Comparison of Omeprazole and Pantoprazole in the Treatment of Patients with Peptic Ulcer Bleeding

Objective:To compare the efficacy and adverse reactions of omeprazole and pantoprazole in the treatment of patients with peptic ulcer bleeding. Methods: Totally 362 cases of peptic ulcer bleeding were randomly divided into the control group and the observation group with 181 ones in each. The control group was treated by omeprazole 40mg in 0. 9% sodium chloride injection 100ml, ivd, bid, and the observation group was given pantoprazole 80mg in 0. 9% sodium chloride injection 100ml, ivd, bid. The treatment course was 3-6 days. The times of vomiting and melena, bleeding situation and bleeding stopping time in the two groups were observed and recorded, and the clinical efficacy, adverse drug reactions during the treatment and the expense of the two groups were also compared. Results:The bleeding amount on the first and third day in the observation group was significantly lower than that in the control group (P < 0. 05), the average bleeding stopping time and the average times of black stools and vomiting in the observation group were also significantly lower than those in the control group(P < 0. 05). The effective rate of the observation group was 92. 27%, that of the control group was 90. 61%, and the difference was not statistically significant (P >0. 05). The average daily cost of the observation group was significantly lower than that of the control group (P <0. 05). The difference in the incidence of adverse drug reactions of the two groups was not statistically significant (P > 0. 05). Conclusion:The efficacy of omeprazole and pantoprazole in the treatment of peptic ulcer bleeding was similar with high security. However, pantoprazole may reduce the times of vomiting and melena, shorten the bleeding time and decrease the treatment cost.
6.Application and Comprehension of Configurable Electronic Medical Record

Objective To establish the structured electronic medical records system based on XML in hospital. Methods The software system of electronic medical record which is suitable for the practical environment of hospital was developed to establish various types of specialized templates and train the clinicians well. Results The establishment of hospital-wide electronic medical record system has achieved the major goals. Conclusion In the current technical conditions, through process reengineering and software development, the hospital can establish the appropriate electronic medical records system to improve the quality of medical records and efficiency of clinicians.
7.Behavioral study of facial inversion effect and priming effect

Objective To compare the cognitive difference of subjects'responses to the same visual stimuli under two different priming conditions,and to discuss the influence of inversion effect and configural changes upon facial recognition.Methods Priming pattern was employed,and subjects were induced to recognize the same stimuli as schematic faces or three English letters under different priming conditions.The participants'accuracy and reaction times were compared in two priming conditions. Results Participants'accuracy of the comparison stimuli in facial priming condition decreased significantly by inversion effect((79.03±10.53)%vs(89.43±9.98)%,P<0.01);reaction times of priming stimuli and comparison stimuli were delayed by inversion effect significantly((3720.40±607.71)ms vs(2998.33±544.02)ms,(3521.80±1038.20)ms vs(2750.87±867.13)ms,P<0.01),and there was no influence of inversion effect upon the reaction times of English letter stimuli(P>0.05).Correlations of accuracy to reaction times reached no significant difference under two conditions.Conclusion Priming effect make sense in the cognition,and there was a priming stage before the facial configuration.Inversion effect shows up in the face priming condition,and vanish in the English letter priming condition,which demonstrate the importance of configuration and inversion effect in the facial cognition.What's more,such effect can't be explained by the changes of cognitive difficulty.
8.Research on Detecting Serum LDH,TBIL,IBIL and Hcy Levels in Clinical Differential Diagnosis of MA and MDS

Objective To investigate the serum lactate dehydrogenase (LDH),total bilirubin (TBIL)and indirect bilirubin (IBIL)and homocysteine (Hcy)levels change in megaloblastic anemia (MA)and myelodysplastic syndrome (MDS)in the differential diagnosis of the clinical application value.Methods Hitachi 7 600 automatic biochemical analyzer was applied to the determination of 40 patients with MA,30 with MDS patients and 40 healthy physical examination (normal control group)of serum LDH,TBIL,IBIL and Hcy levels,and application SPSS16.0 statistical software for data processing were retrospectively analyzed.Results Ma group of LDH,TBIL,IBIL and Hcy four biochemical index level was significantly higher than that of MDS group and normal control group (P<0.01).MDS group of LDH levels higher than normal control group (P<0.01).MDS group of TBIL,IBIL and Hcy,there was no statistically significant difference compared with normal control group (P>0.05).MA group of LDH,TBIL,IBIL,Hcy higher positive rate higher than that of MDS group,LDH and Hcy was most significant,four biochemical indexes between the two groups compared to increase the positive rate of difference was statistically significant (P<0.01).Conclusion Joint detection of LDH,TBIL,IBIL and Hcy four biochemical index level of the differential diagnosis of MA and MDS has certain clinical value.
9.Clinical application of gemstone spectral imaging associated with patient-based low dose of contrast medium protocol for carotid CT angiography

Objective To investigate the clinical value of gemstone spectral imaging (GSI) associated with patient-based low dose of contrast medium protocol in carotid CTA. Methods One hundred and twenty patients who were suspected with stenotic carotid artery or carotid plaque were prospectively enrolled in the study. All of them were divide into two groups by random number table. Group A (routine group): 60 were scanned with 120 kVp after the administration of moderate-concentration CM(320 mg/ml) with 5 ml/s injection velocity, Group B (low dose group):60 were scanned with GSI which was reconstructed using 50%ASiR after the administration of the same CM with 3 ml/s injection velocity. The contrast dose [(test bolus peak time +2 s – 5 s) × injection velocity] was calculated. Images of the two groups were compared in terms of arterial attenuation, signal-noise-ratio (SNR), contrast-to-noise ratio (CNR), and subjective image quality (IQ) score. The value of CT dose index volume (CTDIvol), dose length product (DLP) and effective dose (ED) was recorded, respectively. Data were analyzed by using independent samples t test or Mann-Whitney U test. Results The mean attenuation, noise, SNR, CNR, subjective image quality score, contrast dose, CTDIvol, DLP and ED of routine group was (363 ± 56)HU, (13 ± 4)HU, 30 ± 10, 38±13, 3.0 score, (69 ± 13) ml, 13.61 mGy,527 mGy · cm and 3.11 mSv, respectively. The above variables of low-dose-group was (378 ± 69) HU, (9 ± 4)HU, 48 ± 19, 62 ± 24, 2.0 score,(49 ± 7)ml, 12.72 mGy, 478 mGy · cm and 2.82 mSv, respectively. The mean attenuation and subjective IQ score of carotid artery had no significant differences statistically between two groups (P>0.05), respectively. The noise, SNR, CNR, contrast dose, CTDIvol, DLP and ED had significant differences statistically between two groups (P<0.05), respectively. Conclusion Compared with 120 kVp protocol, the use of GSI associated with patient-based low dose of contrast medium protocol in carotid CTA could provide equivalent image quality and higher SNR and CNR of carotid artery with a smaller amount of iodine and a lower radiation dose.
10.Effects of low-dose of TGF-β1 on maintaining bovine corneal stromal cell growth and retarding extra cellular matrix fibrosis in a three-dimensional culture model

Background Transforming growth factor-β1(TGF-β1) plays an important role in corneal woundhealing.The effects of TGF-β1 on the synthesis of extra cellular matrix (ECM) vary upon different concentrations.Previous studies focused on the effects of high concentration of TGF-β1 on keratocytes under the two-dimensional culture condition,and the effect of low concentration of TGF-β1 on the synthesis of ECM in keratocytes remains unclear.Objective This study was to investigate the growth of Pellet,a three-dimensional model of corneal stroma cells in vitro,and its ECM synthesis under a low concentration of TGF-β1.Methods Bovine corneal stromal cells were isolated from fresh bovine eyeballs by two-step digestion by collagenase and cultured using DMEM/F12 medium with 10% fetal bovine serum (FBS).Pellets derived fresh bovine keratocytes with culture medium containing 0.25 ng/ml TGF-β1 +5% FBS and 0.50 ng/ml TGF-β1 +5% FBS were established,respectively.The morphology of Pellets was observed under the natural light at 48 hours,1 week,2 weeks and 3 weeks after culture.In 3 weeks after culture,the cell structures was observed by hematoxylin-eosin staining,and Calcein-AM/propidium (Calcein-AM/PI) staining was used to assay the cell viability.Real-time flurorescence quantitative PCR and immunofluorescence technology were applied to analyze the expressions of α-smooth muscle actin (α-SMA),fibronectin (FN),type Ⅰ collagen (Col Ⅰ) and type Ⅲ collagen (Col Ⅲ) mRNA and proteins.RT-PCR was employed to detect the expressions of lumican (LUM) mRNA and keratocan (KERA) mRNA in the cells.Results Cells in Pellet clustered throughout the culture duration.Hematoxylin-eosin staining showed the mass red-dyed collagen fibers in both 0.25 ng/ml TGF-β1 +5% FBS group and 0.50 ng/ml TGF-β1 +5% FBS group,and most cells possessed complete structures.The death rate of the cells was (33.60±1.65)% in the 0.25 ng/ml TGF-β1 +5% FBS group and (30.90±0.78) % in the 0.50 ng/ml TGF-β1 +5% FBS group,showing an insignificant difference between them (t =0.144,P=0.887).The expressions of α-SMA,FN and Col Ⅲ proteins in 0.25 ng/ml TGF-β1 +5% FBS group were lower than those in the 0.50 ng/ml TGF-β1 + 5 % FBS group (tα-SMA =4.622,P =0.010;tFN =2.973,P =0.040;tCol Ⅲ =7.845,P<0.001),but the expression of Col Ⅰ in 0.25 ng/ml TGF-β1 +5% FBS group was higher than that in 0.50 ng/ml TGF-β1+5% FBS group (tColⅠ =4.022,P=0.016).The ratio of Col Ⅲ/Col Ⅰ in 0.25 ng/ml TGF-β1+5% FBS group was lower than that in the 0.50 ng/ml TGF-β1 +5% FBS group in both mRNA and protein level (tmRNA =-3.039,P =0.038;tprotein =3.215,P =0.032).The expression of LUM mRNA and KERA mRNA were detected in Pellet at different time points.The expression of LUM mRNA in 0.25 ng/ml TGF-β1 +5% FBS group increased over time.While in 0.50 ng/ml TGF-β1 +5% FBS group,the expression of LUM mRNA peaked at 1 week but declined at 2 weeks.The expression of KERA mRNA in two groups were all peaked at 1 week but declined at 2 weeks.Conclusions Low-dose TGF-β1 in Pellet can maintain the normal growth of keratocytes and synthesize ECM.The expression of ECM tends to the normal condition after reducing the concentration of TGF-β1,implying a scarless expression.
