1.Effect of Salvia miltiorrhiza Ligustrazine on peripheral lymphocyte neutrophil proportion, neurological function score and prognosis in patients with acute cerebral infarction

Objective To investigate effect of Salvia miltiorrhiza Ligustrazine on peripheral lymphocyte neutrophil proportion, neurological function score and prognosis in patients with acute cerebral infarction. Methods 125 cases of acute cerebral infarction in our hospital from June 2015 to June 2016 were selected as the research object, randomly divided into two groups, the control group was treated with conventional treatment of acute cerebral infarction, patients in the observation group in the control group patients on the basis of Salviae Miltiorrhizae and Ligustrazine Hydrochloride Injection, all patients were detected in the blood leukocyte classification, and the NIHSS evaluation of patients with neurological function and prognosis of the two groups of patients. Results After treatment, the observation group of white blood cells in patients with (×109/L), neutrophil (%), neutrophil / lymphocyte was significantly lower than the control group patients, two groups of patients before treatment, after treatment of 1 day and 7 days, two groups of NIHSS score comparison showed no significant difference, after the treatment of 14d patients in the observation group the NIHSS score was (11.1±3.3), which was obviously lower than the control group, the patients in the observation group were six cases death and disability in three cases, 53 cases of independent life, significantly better than the control group, the difference was statistical significance(P<0.05). Conclusion Salvia miltiorrhiza Ligustrazine can help to reduce the ratio of neutrophil / lymphocyte, improve neurological function and improve the prognosis of patients with acute cerebral infarction, and it is worthy of clinical application.
2.Intranasal dexmedetomidine prevents the emergence delirium in children after sevoflurane-based anesthesia

Objective To investigate whether intranasal dexmedetomidine prevents the emergency delirium after sevoflurane-based anesthesia (ASA) in children.Methods Ninety children (ASA I,2 to 7years) undergoing elective tonsillectomy with or without adenoidectomy,were randomly divided into group D1 (intranasal dexmedemidine 1.0 μg/kg),group D2 (intranasal dexmedemidine 0.5 μg/kg),and group NS (intranasal normal saline).Anesthesia was induced with sevoflurane (30 min after intranasal administration) and maintained with sevoflurane in 50/50% O2/N2O.Heart rate (HR),blood pressure (BP),and hemoglobin oxygen saturation (SpO2) were monitored before intranasal administration to 2 hours after extubation.Time of operation and extubation were recorded.Pediatric anesthesia emergence delirium (PAED) scale,Modified Children's Hospital of Eastern Ontario Pain Scale (m-CHEOPS) Pain scale and the adverse effects were observed.Results The PAED scales of group D1 and Group D2 were significantly lower than group NS.No significant differences were found in hemodynamic and respiratory variables,pain scale,time of extubation and adverse effects among three groups.Conclusions Intranasal dexmedetomidine decreased the severity of emergence delirium after sevoflurane-based anesthesia in children without prolonging the time to extubate and affect hemodynamics and respiratory variables.

With the rapid development of general anesthesia technology and the occurrence of new types of inhalation and intravenous anesthetics,safety and comfort requirements of general anesthesia become increas-ingly higher. Therefore,emergence agitation and its adverse consequences gradually receive more and more atten-tion . Thus the etiology and prevention of emergence agitation is an important issue in pediatric anesthesia at pres-ent. In recent years, some achievement has been made in emergence agitation following general anesthesia in children. This article will summarize the related factors,possible mechanisms and prevention strategics of pediat-ric emergence agitation.
4.The effect of resveratrol on the expression of adiponectin receptors in intermittent high glucose-cultured rat mesangial cells

Objective To study the effects and mechanisms of resveratrol (Resv) on the expression of adiponectin receptors ( AdipoR1 and AdipoR2 ) in rat mesangial cells (MCs) cultured in intermittent high glucose medium.Methods The MCs cultured in normal glucose ( 5.6 mmol/L) medium were transfected by the plasmid vector of the FoxO1 short hairpin RNAs ( FoxO1 shRNA ),and then cultured in intermittent high glucose medium (5.6mmol/L or 30 mmol/L,alternately every 8 hours ) and resveratrol ( Resv 20 μmol/L).The MCs cultured in normal glucose (5.6 mmol/L) medium served as normal glucose control group (NG),and the MCs exposed to iutermittent high glucose medium were divided into five groups:intermittent high glucose group (IHG),IHG+Resv group,IHG+FoxO1 shRNA group,IHG+ Resv + FoxO1 shRNA group,IHG + Negative control group ( shNC ),each group was cultured for 72 h.The level of reactive oxygen species (ROS) was assessed by Fluorescence microplate reader.The mRNA levels of Sirt1,Foxo1,AdipoR1,and AdipoR2 were assessed by RT-RCR.The protein levels of FoxO1,phosphorylation FoxO1 ( p-FoxO1 ),AdipoR1,and AdipoR2 were assessed by Western blotting.Results ( 1 )Compared with NG,intermittent high glucose significantly decreased the mRNA expression of Sirt1,but markedly increased the level of p-FoxO1.Furthermore,the mRNA and protein expression levels of AdipoR1 were obviously decreased while the level of ROS was enhanced in IHG ( all P<0.05 ).(2) Compared with IHG,the mRNA and protein expression levels of FoxO1 and AdipoR1 were inhibited,but the level of ROS was enhanced in group IHG+FoxO1 shRNA ( all P<0.05 ).( 3 ) Compared with IHG,after treating with Resv,the mRNA expression level of Sirtl was increased,whereas the level of p-FoxO1 was decreased.Moreover,the mRNA and protein levels of AdipoR1 was increased while the level of ROS was lowered in group IHG+Resv (all P<0.05 ).(4) Compared with group IHG+Resv,the mRNA and protein levels of FoxO1 and AdipoR1 were inhibited,but the level of ROS was increased in group IHG+Resv+FoxO1 shRNA (all P<O.05).(5) In addition,the mRNA and protein expression level of AdipoR2 showed no significant difference among these groups ( P>0.05 ).Conclusion ( 1 ) Resveratrol significantly increases the expression of AdipoR1 in MCs induced by intermittent high glucose.( 2 ) FoxO1 plays an important role in regulating the expression of AdipoR1 by resveratrol.
5.Effect of dexmedetomidine on postoperative analgesia pump dose and effect

Objective To investigate effect of dexmedetomidine on postoperative analgesia pump dose and effect.Methods 50 cases of patients with abdominal surgery under general anesthesia were selected.According to the postoperative analgesic drugs were divided into control group and experimental group, 25 cases in each group were given corresponding drug treatment.After treatment, the visual analogue scale, comfort score, adverse reaction rate and dosage of analgesic drugs were detected and compared.Results Compared with the control group,the VAS score were lower(P <0.05),the BCS score were higher(P<0.05),the adverse reaction rate were lower(P<0.05),the dosage of analgesic pump were lower(P<0.05). Conclusion Dexmedetomidine can significantly reduce postoperative pain degree of patients, reduce the incidence of adverse reaction, reduce analgesic dosage of the drug pump.
6.Concentrations of ropivacaine for analgesia after total hip arthroplasty

BACKGROUND:Severe pain after total hip arthroplasty is an important factor for successful rehabilitation of postoperative joint function. Analgesic method after total hip arthroplasty is a hot issue. OBJECTIVE:To investigate the analgesic effect of different concentrations of ropivacaine after total hip arthroplasty. METHODS:69 patients undergoing total hip arthroplasty were recruited from Department of Anesthesiology, Suqian People’s Hospital, from January 2012 to June 2014. According to the ASA classification, their physical status was graded I to III. The involved patients were randomly divided into three groups:0.25%ropivacaine group, 0.3%ropivacaine group, 0.35%ropivacaine group. Each group had 23 cases. At 30 minutes after the surgery, different concentrations of ropivacaine, 20 mL, were injected to patients due to continuous fascia iliaca compartment block. The catheter was then connected to a patient-control ed analgesia pump programmed to deliver 10 mL with a lockout interval of 60 minutes, for postoperative analgesia (72 hours). At 12, 24, 48 and 72 hours of blockade, the visual analogous scale (VAS) scores at rest, passive and active activity were recorded. When VAS score at rest ≥ 4 points, parecoxib sodium 40 mg was injected intravenously. The consumption of ropivacaine within 72 hours after the blockade, application of parecoxib sodium, time of ambulation, and adverse reactions during blockade were recorded. The analgesic effect in the three groups was also observed. RESULTS AND CONCLUSION:Compared with 0.25%ropivacaine group, static VAS scores of 0.3%ropivacaine group and 0.35%ropivacaine group showed no significant difference (P>0.05), passive and active VAS scores were significantly decreased (P<0.05), and the consumption of ropivacaine within 72 hours after the blockade was significantly decreased. There was no significant difference in the rest, passive and active VAS scores between 0.3%ropivacaine group and 0.35%ropivacaine group (P>0.05). The ropivacaine consumption of 0.3%ropivacaine group and 0.35%ropivacaine group was not statistical y significant (P>0.05). The usage of parecoxib sodium in 0.3%ropivacaine group and 0.35%ropivacaine group was significantly lower than that in 0.25%ropivacaine group (P<0.05). Day of first walk was earlier in the 0.3%ropivacaine group and 0.35%ropivacaine group. The incidence of complications among the three groups showed no significant difference (P>0.05). Experimental findings indicate that, three different concentrations of ropivacaine has certain analgesic effects after total hip arthroplasty with fewer adverse reactions, and the concentration of 0.3%ropivacaine is the suitable concentration for postoperative analgesia of total hip arthroplasty, it can reduce the amount of parecoxib sodium and shorten the day of first walk.
7.Ultra low molecular weight heparin attenuates glutamate-induced neurotoxicity by reducing intracellular calcium release

Aim To investigate the effect of ultra low molecular weight heparin(ULMWH)to protect primarily cultured rat cortical neurons from the deleterious effects of the neurotoxicant glutamate (Glu)and explore the related mechanism.Methods Cortical neurons of fetal rats were cultured carefully in vitro and treated with 100 ?mol?L-1 Glu so that the protective effects of ULMWH were observed thoroughly. And then the viability of cortical neurons, morphological change together with the number of apoptotic neurons,and the concentration of intracellular free Ca2+([Ca2+]i)were measured by MTT assay, Hoechst33258 staining and Fura-2/AM double wavelength fluoremetry, respectively.Results Brief exposure of cultures to 100 ?mol?L-1Glu led to extensive neuronal death and rapid increase of [Ca2+]i.Pretreatment with ULMWH over the concentration range of 0.01~1 mg?L-1 significantly inhibited the Glu-induced neuronal cell death assessed by a MTT assay and the number of apoptotic nuclei, evidenced by Hoechst 33258 staining.Glu-induced elevation of[Ca2+]i was decreased by pretreatment with 1 mg?L-1 ULMWH, which was indicated by Fura-2/AM not only in assay medium containing Ca2+ but also in Ca2+-free assay medium.Conclusions Those results suggest that ULMWH protects the cultured neurons from Glu-induced neurotoxicity, which may be attributed to its alleviating intracellular free Ca2+ overload via suppressing intracellular Ca2+ release from internal stores induced by Glu.
8.Countermeasure and Practice of Improving Connotative Quality of Electronic Medical Records

Objective To explore connotative quality of how to improve the electronic medical records. Methods Combining with the ways of improving connotative quality in fact, the advantages and common defects of electronic medical records were reviewed and some suggests were brought forward. Results The connotative quality of electronic medical records could be enhanced by specific quality standards, the implementation of quality control of section links and the emphasis quality control suited to expert's specialty, at the same time, the connotative quality of electronic medical records was raised by monitor methods of informationazition platform. Conclusion Improving the whole medical level of hospitals is the crux of raising the connotative quality of medical records.
9.Investigation on the Situation of Self-management in 206 Diabetic Patients

OBJECTIVE: To investigate the impact of self-management of diabetic patients on diabetes mellitus and its complication in Chongqing. METHODS: Hospitalized diabetic patients were surveyed randomly using questionnaire modified and simplified from diabetes care profile(DCP)and diabetes knowledge test(DKT)designed by Diabetes Research and Training Center of Michigan University. RESULTS: Most of diabetic patients showed a low level of self-management. The self-management of young and middle aged diabetic patients was poorer than that of aged patients. The glucose control in patients who received diabetic education was superior to those without receiving diabetic education. The glucose control in patients with regular outpatient follow-up was better than patients followed up by other methods. A large number of diabetic patients combined with cardiovascular and cerebrovascular diseases got the bad habits of drinking and smoking. CONCLUSION: Diabetic education is a important method to improve the self-management of diabetic patients. The conception of diabetic patients on diabetic education in Chongqing should be improved.
10.Characteristics and Evaluation of Oral Dyskinesia and Dysarthria in Children with Cerebral Palsy

Objective To observe the efficacy of Complex Oral Motor Scoring (COMS) and diadochokinetic rate (DR) on evaluation of oral motor dysfunction and speech disorders in children with cerebral palsy. Methods 107 children with cerebral palsy were tested with Chinese-version Articulation Test, Simple Oral Motor Scoring (SOMS), COMS and DR. The correlation among SOMS, COMS and DR was analysesd. Results 85 children were abnormal in SOMS and 94 in COMS. Oral dyskinesia was found in all the children with spastic quadriplegia, dyskinetic, ataxia and mixed type of cerebral palsy, and less in the type of spastic diplegia and hemiplegia. DR was the least in spastic quadriplegia, dyskinetic, mixed type of cerebral palsy, more in the type of ataxia and spastic diplegia, and the most in hemiplegia. The coefficient of correlation among the SOMS, COMS, the longest pronunciation and DR were more than 0.8 (P<0.01). Conclusion The incidence and severity of oral dyskinesia and dysarthria are associated with the types of cerebral palsy. There is good correlation among SOMS, COMS, longest pronunciation and DR.
