1.Tracheotomy tube as trocar in the electronic bronchoscope instead of thoracoscope

Objective To investigate the feasibilty of the disposable tracheotomy tube as trocar in the electronic bronchoscope instead of thoracoscope. Methods 86 patients with effusion of unknown origin undergoing medical tho-racoscopy from January 2015 to October 2015, 59 male (68.6%) and 27 female (31.4 %), mean age (49.00 ± 20.00) years (15 ~ 83) years, were randomly divided into two groups, Tracheostomy tube group (group T) and Conventional chest tube group (group C). Group T uses disposable cannula tracheostomy tube as trocar, and group C received conventional medical thoracoscopy chest tube. Then compare the positive rate of pathological diagnosis, operation time, inspection fees, and other adverse reactions between the two groups. Results 40 cases (93.0%) in group T were confirmed diagnosis, including 8 cases of lung pleural metastasis, 31 cases of erculous pleurisy, 1 case of empyema, while 4 cases of unknown diagnosis (7.0%). 33 cases (76.7%) in group C were confirmed diagnosis, in-cluding 4 cases of lung pleural metastasis, 29 cases of tuberculous pleurisy, while 10 cases of unknown diagnosis (23.3 %). The difference was statistically significant between the two groups ( < 0.05). Operative time in group T was (23.86 ± 2.45) minutes, while in group C was (29.88 ± 3.67) minutes, the difference was statistically significant ( <0.05). While the complication rate was no significant difference. Conclusions It is demonstrated that the dispos-able tracheotomy tube as trocar in the electronic bronchoscope instead of thoracoscope which can shorten the opera-tion time, improve the diagnosis rate, reduce costs, worthy of promoting.
2.Suppressive effect of exogenous carbon monoxide on abnormal platelet exocytosis and its molecular mechanism in sepsis

Objective To investigate the suppressive effect of exogenous carbon monoxide (CO) on abnormal platelet exocytosis and its possible molecular mechanism. Methods Venous blood was collected from healthy volunteers. Platelet-rich plasma (PRP) was isolated from the blood by differential centrifugation. The PRP was randomly divided into five groups by random number table, namely normal control group, lipopolysaccharide (LPS) group (challenged with 10 mg/L LPS), inactively exogenous carbon monoxide releasing molecule 2 (iCORM-2) group (given 10 mg/L LPS + 50 μmol/L iCORM-2 for intervention), exogenous carbon monoxide releasing molecule 2 (CORM-2) 10 μmol/L and 50 μmol/L groups (given 10 mg/L LPS + CORM-2 10 μmol/L or 50 μmol/L for intervention). After 30 minutes, enzyme linked immunosorbent assay (ELISA) was used to determine the platelet-derived growth factor BB (PDGF-BB) and matrix metalloproteinase 2 (MMP-2). Chemical fluorescein method was used to determine the platelet adenosine triphosphate (ATP). Flow cytometer was used to determine the expression of P-selectin. The expressions of Toll-like receptor 4 (TLR4), phosphorylation of protein kinase Cθ (PKCθ) and syntaxin binding protein 1 (STXBP-1) were determined by Western Bolt. The soluble N-ethylmaleimide-sensitive factor-attachment protein receptors (SNAREs) complex formation [syntaxin 2-synaptosomal-associated protein 23-vesicle associated membrane protein 8 (STX2-SNAP23-VAMP8)] mediated by STXBP-1 was determined by immunoprecipitation. Results ① Compared with normal control group, the platelet release of PDGF-BB, MMP-2 and ATP was significantly increased after LPS challenge, and the P-selectin expression of platelet was also obviously up-regulated [PDGF-BB (μg/L): 127.53±1.78 vs. 94.35±5.84, MMP-2 (ng/L): 51.87±9.20 vs. 35.83±3.17, ATP (μmol/L): 1.288±0.056 vs. 0.975±0.010, P-selectin: (3.93±0.19)% vs. (0.44±0.10)%, all P < 0.05]. The increases in platelet release of PDGF-BB, MMP-2 and ATP were suppressed by 10 μmol/L or 50 μmol/L CORM-2 administration, as well as high-expression of P-selectin in a dose-dependent manner [PDGF-BB (μg/L): 114.68±1.35, 97.08±6.14 vs. 127.53±1.78, MMP-2 (ng/L): 32.67±8.00, 24.63±1.63 vs. 51.87±9.20, ATP (μmol/L): 0.999±0.015, 0.965±0.008 vs. 1.288±0.056, P-selectin: (1.95±0.27)%, (0.94±0.11)% vs. (3.93±0.19)%, all P < 0.05]. ② Compared with normal control group, LPS challenge resulted in a significant increase in the expression of TLR4 and the phosphorylation of PKCθ and STXBP-1 [TLR4 (gray value): 1.21±0.38 vs. 0.67±0.06, p-PKCθ (gray value): 1.36±0.20 vs. 0.44±0.03, p-STXBP-1 (gray value): 1.13±0.06 vs. 0.59±0.04, all P < 0.05]. The increases in above parameters were suppressed by 10 μmol/L or 50 μmol/L CORM-2 administration in a dose-dependent manner [TLR4 (gray value): 0.76±0.05, 0.65±0.04 vs. 1.21±0.38; p-PKCθ (gray value): 0.71±0.07, 0.47±0.10 vs. 1.36±0.20; p-STXBP-1 (gray value): 0.56±0.02, 0.48±0.01 vs. 1.13±0.06, all P < 0.05]. ③ Compared with normal control group, the SNAREs proteins in platelet that combined with STXBP-1, including STX2, SNAP23 and VAMP8, were obviously increased after LPS challenge [STX2 (gray value): 1.35±0.06 vs. 0.57±0.04, SNAP23 (gray value): 0.97±0.04 vs. 0.30±0.12, VAMP8 (gray value): 1.37±0.12 vs. 0.77±0.10, all P < 0.05]. The increases in SNAREs complex formation were suppressed by 10 μmol/L or 50 μmol/L CORM-2 administration in a dose-dependent manner [STX2 (gray value): 0.77±0.02, 0.39±0.03 vs. 1.35±0.06, SNAP23 (gray value): 0.41±0.03, 0.22±0.08 vs. 0.97±0.04, VAMP8 (gray value): 0.85±0.07, 0.66±0.07 vs. 1.37±0.12, all P < 0.05]. There was no significant difference in the above mentioned parameters between iCORM-2 group and LPS group. Conclusions LPS-induced abnormal secretion of platelet was suppressed by CORM-2 administration. The mechanism may involve the TLR4/PKCθ/STXBP-1 signaling pathway activation and the SNAREs complex formation.
3.Clinical Analysis of 82 Cases of Disulfiram-like Reaction in Our Hospital

OBJECTIVE:To provide reference for rational drug use in the clinic. METHODS:The cases of disulfiram-like reac-tion collected from the First Affiliated Hospital of Guangzhou University of TCM from July 1st in 2012 to June 30st in 2014,were analyzed and compared retrospectively. The epidemiological characteristics,causes,clinical characteristics and treatment methods were summarized. RESULTS:Disulfiram-like reaction mainly occurred in male(86.59%),mainly in young and middle-aged men aged 21-50 years old (78.05%). It might be induced by using cephalosporin antibiotics (as cefoperazone,cefperazone/sulbactam, etc.),nitroimidazole (as metronidazole,tinidazole,etc.),drinking or contacting beverage,food or drug containing ethanol. This reaction involved the skin and its accessories system,circulatory system,digestive system,nervous system,respiratory system, etc. Its clinical manifestations mainly were skin flush,chest tightness,shortness of breath,palpitation,nausea,vomiting,dizzi-ness,headache,etc.,occasional confusion,drowsiness,xerostomia,etc. Response to the severe disulfiram-like reaction,the foun-dation treatment combined with symptomatic treatment showed a good effect with remission of symptoms in 1 to 3 hours and no deaths. CONCLUSIONS:When the drugs with mercapto-1-methyltetrazole perssad as cephalosporin antibiotics,nitro imidazoles, etc.,are used,we should avoid drinking or taking drugs or food containing ethanol,to prevent the occurrence of disulfiram-like re-action.
4.DNA damage of gastric cancer cell line SGC7901 dyed by methylene blue after exposure to the cold light radiation of endoscope

Objective To study the co-effects of methylene blue(MB) and exposure with the cold light source on cell DNA damage, and to explore the mechanism involved. Methods The alkaline single cell gel electrophoresis was used to determine cell DNA damage. Apoptosis of the cells was determined by flow cytometry analysis using Annexin V-FITC/PI staining. DCFH-DA probe was used to determine endocellular Reactive Oxygen Species (ROS). Results The levels of DNA damage in the SGC7901 adenocarcinoma cells treated with methylene blue in the light were significantly increased compared to that of control ( F = 8.39, P<0.05 ). The DNA damage levels were related to the length of time of light exposure, and the damage was recovered to a certain level after light withdrew. Cell apoptosis ( x2=7.71,P <0.05)and endocellular ROS level (F = 34.11, P<0.01= increased significantly in the exposure group. Conclusions Methylene blue chromoendoscopy can induce DNA damage and cell apoptosis, and the mechanism may be associated with ROS produced by the photochemical reaction.
5.Efficacy of combined therapies on keloid

Objective To evaluate comprehensive therapy in the treatment of keloid effect and the patients' satisfaction.Methods A total of 523 patients with comprehensive treatment,according to the treatment,were divided into group A (surgery combined radiotherapy) and group B (surgery plus corticosteroids),group C (hyperbaric oxygen in combination with radiotherapy),and the therapeutic effect of patients with satisfaction was analyzed.Results Total effective rate of three groups of patients were 69.47%,89.13%,90.32%,respectively.Effective rate in group A was higher than that of group B and group C,there was statistically significant difference between groups A and C (P < 0.05),but no significant difference between the group A and group B (P>0.05).Difference was statistically significant between three groups of patients' satisfaction,group B better than group A and group C,the difference between group B and C group was statistically significant (P < 0.05).Conclusions Effectiveness of the three combined therapies is obvious,in which radiotherapy plus hyperbaric oxygen is most effective.
6.Study on Synthesis of Tumor Marker PHPAA Molecular Imprinted Polymers and Its Performance

OBJECTIVE:To synthesize 4-hydroxyphenylacetic acid(PHPAA)molecular imprinted polymers(MIPs),and to provide reference for separation and enrichment of PHPAA in urine of cancer patients. METHODS:With PHPAA as template molecule and azobisisobutyronitrile as evocating agent,MIPs was synthesized by precipitation polymerization using 4-vinyl pyridine(4-V),acrylamide(AM),1-vinyl imidazole(1-V)and o-diaminobenzene as functional monomers,ethylene glycol dimethacrylate(EGDMA)and trimethylolpropane trimethacrylate(TRIM)as crosslinking agent. Using scanning electron microscope,infrared spectrum,static adsorption test and molecular recognition performance test used adopted to characterize the structure and performance of MIPs. RESULTS:MIPs1(4-V,EGDMA)microspheres adhered seriously,and MIPs2(AM, EGDMA)microspheres are aggregated. MIPs3(1-V,TRIM),MIPs4(o-diaminobenzene,TRIM)microspheresare were dispersed and had good sphericity. Characteristic absorption peak of PHPAA was not found in infrared spectrum of MIPs. The adsorption capacity of each MIPs was higher than that of blank imprinted polymer without the template molecule,and increased as the increase of template molecular concentration. The adsorption capacity of MIPs3 was significantly higher than that of other MIPs,and its static distribution coefficient(0.14)of PHPAA was higher than that of other structural analogues [PHPA(0.06),TA(0.01)],selective separation factors of PHPAA to PHPA,PHPAA to TA were 2.3,11.5,respectively. CONCLUSIONS:MIPs synthesized with 1-V as functional monomer and TRIM as crosslinking agent has the ability of specific adsorption and selective recognition. It can used as solid phase extractant to separate and enrich low content of PHPAA in the urine of tumor patients.
7.Construction, characterization, and chromosomal mapping of a fosmid library of the white-cheeked gibbon (Nomascus leucogenys).

Gibbons have experienced extensive karyotype rearrangements during evolution and represent an ideal model for studying the underlying molecular mechanism of evolutionary chromosomal rearrangements. It is anticipated that the cloning and sequence characterization of evolutionary chromosomal breakpoints will provide vital insights into the molecular force that has driven such a radical karyotype reshuffle in gibbons. We constructed and characterized a high-quality fosmid library of the white-cheeked gibbon (Nomascus leucogenys) containing 192,000 non- redundant clones with an average insert size of 38 kb and 2.5-fold genome coverage. By end sequencing of 100 randomly selected fosmid clones, we generated 196 sequence tags for the library. These end-sequenced fosmid clones were then mapped onto the chromosomes of the white-cheeked gibbon by fluorescence in situ hybridization, and no spurious chimeric clone was detected. BLAST search against the human genome showed a good correlation between the number of hit clones and the number of chromosomes, an indication of unbiased chromosomal distribution of the fosmid library. The chromosomal distribution of the mapped clones is also consistent with the BLAST search result against human and white-cheeked gibbon genomes. The fosmid library and the mapped clones will serve as a valuable resource for further studying gibbons' chromosomal rearrangements and the underlying molecular mechanism as well as for comparative genomic study in the lesser apes.

8.The relationship between enteral nutrition-related diarrhea and intestinal flora in critically ill patients and the effect of microflora transplantation

Objective:To investigate the relationship between enteral nutrition-related diarrhea and intestinal flora in critically ill patients and the effect of microflora transplantation.Methods:A total of 60 critically ill patients with enteral nutrition-related diarrhea who were scheduled to undergo microflora transplantation in Taizhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to August 2021 were prospectively and continuously selected as the research group, and 60 critically ill patients without enteral nutrition-related diarrhea were selected as the control group. The bacterial count of 4 kinds of intestinal flora in the feces including bifidobacterium, lactobacillus, enterococcus, and escherichia coli were detected and compared between the two groups, and the value of the fecal colony numbers of 4 kinds of intestinal flora in diagnosing non-enteral nutrition-related diarrhea in critically ill patients was analyzed by receiver operating characteristic (ROC) curve. All patients in the research group received microflora transplantation, and the diarrhea score, hematochezia score, partial Mayo score and European five-dimension health scale (EQ-5D) were detected and compared before treatment, 1 week after treatment and 1 month after treatment to evaluate the treatment effect. The Pearson linear correlation method was used to analyze the relationship between the colony count of 4 kinds of intestinal flora colonies in the feces of the research group at baseline and the therapeutic indexes for 1 week and 1 month after treatment.Results:The number of fecal bifidobacterium and lactobacillus colonies in the study group were lower than those in the control group: (7.12 ± 0.58) × 10 7 cfu/L vs. (11.85 ± 1.25) × 10 7 cfu/L, (8.78 ± 1.05) × 10 7 cfu/L vs. (11.25 ± 1.57) ×10 7 cfu/L. The colony number of enterococcus and Escherichia coli were higher than those of control group: (8.58 ± 0.88) × 10 7 cfu/L vs. (3.84 ± 0.72) ×10 7 cfu/L, (8.25 ± 0.97) ×10 7 cfu/L vs. (3.66 ± 0.63) ×10 7 cfu/L. The differences were statistically significant ( P<0.05). ROC curve analysis results showed that the area under the curve of fecal bifidobacterium, lactobacillus, enterococcus and escherichia coli colonies in diagnosing patients with enteral nutrition-related diarrhea were all >0.7, which had certain diagnostic value. The diarrhea scores, stool blood scores and some Mayo scores of the study group at 1 week and 1 month after treatment were lower than those before treatment: (1.52 ± 0.36) and (1.13 ± 0.24) points vs. (2.45 ± 0.51) points, (0.95 ± 0.28) and (0.77 ± 0.21) points vs. (2.39 ± 0.54) points, (4.17 ± 1.24) and (3.26 ± 0.85) points vs. (7.86 ± 1.82) points, and the EQ-5D score of patients 1 week and 1 month after treatment was higher than that before treatment: (0.66 ± 0.11) and (0.79 ± 0.13) points vs. (0.58 ± 0.08) points, the difference was statistically significant ( P<0.05). Conclusions:The intestinal flora of critically ill patients is closely related to enteral nutrition-related diarrhea, and can affect the therapeutic effect of bacterial flora transplantation and the health status of patients.

10.Short-term blood glucose control improves neurovascular coupling in patients with diabetes mellitus type 2

Objective To investigate the effect of short-term blood glucose control on neurovascular coupling in pa-tients with diabetes mellitus type 2(T2DM).Methods The patients with T2DM who were admitted to Department of En-docrinology,The Second Affiliated Hospital of Hainan Medical University,from December 1,2021 to May 30,2023 were consecutively included in this study,and according to whether examination was performed before or after blood glucose control,they were divided into pre-blood sugar control T2DM group(T2DM-Pre-c group)and post-blood sugar control T2DM group(T2DM-Pro-c group).Healthy volunteers matched for age and sex were enrolled as healthy control(HC)group.Transcranial Doppler was used to monitor cerebral blood flow parameters,including mean blood flow velocity(Vm)of the middle cerebral artery during elbow flexion,and the T2DM group and the HC group were compared in terms of the changes in cerebral blood flow parameters after blood glucose control.Results A total of 52 T2DM patients and 36 healthy volunteers were enrolled in this study.Compared with the HC group,the T2DM-Pre-c group had significantly higher exercise-induced pulsatility index,resistance index,and time to peak of Vm(TTP-Vm)(all P<0.05).The T2DM-Pre-c group had a significantly lower Vm slope than the HC group(P=0.031).The T2DM-Pro-c group had a significant in-crease in Vm slope and a significant reduction in TTP-Vm after blood glucose control(P<0.05).Conclusion Impair-ment of neurovascular coupling is observed in patients with T2DM during the state of hyperglycemia,and short-term blood glucose control can improve neurovascular coupling.
