1.Application of endoscopic retrograde cholangiopancreatography in pancreatic diseases in children

ERCP has revolutionized the treatment of adult pancreatic diseases.After many years of clinical application,the current adult treatment technology has reached a mature state.With the development of pediatric endoscopic techniques,in recent years,therapeutic ERCP techniques in children has been progressing from the research stage to the stage of practical application and has become an important treatment for pancreatic diseases in children.This article mainly reviews on ERCP in diagnosis,treatment and complications of pancreatic diseases in children.
2.Down regulation of NMDA receptor subunits NR2A and NR2B mRNA e xpression after audiogenic seizure in epilepsy-prone rat brain

Objective: To study the expression of N-methl- D -aspartate (NMDA) receptor subunits NR2A and N R2B mRNA after audiogenic seizure in the brain of genetically epilepsy-prone ra t s-P77PMC. Methods: In situ hybridization technique was used to stud y the mRNA expre ssion of NR2A and NR2B at different times after audiogenic seizure in different b rain areas of P77PMC rats. Results: The NR2A mRNA expression showed time-depende nt down-regulation in cerebral motor and sensory cortex, hippocampus CA1-CA3, de ntate gyrus after audiogenic seizure. Down-regulation emerged 30 min after se izure, 2 hours later the mRNA reached the lowest level, 4-8 hours later re turned to the baseline, and 24 hours later decreased lower than baseline again . The mRNA expression of NR2B was quite similar to that of the NR2A af ter seizure. Conclusion: The down-regulation of NR2A and NR2B mRNA indicates that the subunit composition of the NMDA receptor complexes may be altered after aud iogenic seizure. These structural alterations could make the function of NMDA re ceptor complexes change and subsequently influence the excitability of brain as well as the susceptibility of epilepsy.
3.Analysis of colonoscopy results and treatment experience in 108 children

Objective To discuss the value and clinical significance of colonoscopy in children .Methods Colonoscopy was per-formed on 108 children with Olympus(PCF-Q260JI) .All polyps underwent electrocision .A retrospective study was performed in 108 children′s clinical and endoscopic examination data .Results 105 in 108 colonoscopy cases arrived at ileocesus successfully ,in which 98 cases were diseased .And the most common disease was colorectal polyps(65 cases) ,and 216 polyps were found .The 65 patients were performed resection in different ways ,and none bled and perforated;colorectal polyps were mainly single polyps .The lesions were often seen in proctosigmoid ,and mostly were juvenile polyps .Conclusion According to colonoscopy ,colorectal polyp is a important cause of disease .Colonoscopy is safe and reliable ,which can be very useful in diagnosis and treatment .
4.Transesophageal echocardiography guidance for occluding procedure with cardiopulmonary bypass for muscular ventricular septal defects in children

Objective To discuss the application value of transesophageal echocardiography (TEE) in occluding procedure with cardiopulmonary bypass (CPB) for children muscular ventricular septal defects (mVSD).Methods Forty-nine mVSD patients for occluding procedure with CPB were performed guided and monitored by TEE,9 cases were simple mVSD,40 were mVSD combined with other cardiac malformations.The location,size,number,scope and the surrounding anatomy of mVSD were carefully observed before going into cardiac arrest,preparing the operator to choose appropriate occluder devices.The procedure was evaluated after the recovery of heart beat using TEE.Results In all of the 49 patients underwent occluding procedure,44 cases were implanted into one occluder,5 implanted into two occluders.The results of TEE after operation showed that the positions of all occluders were not shifted,2 had trace residual shunt,there were no complications worsening,such as mitral and aortic regurgitation,etc.One patient abandoned treatment for serious infection,the other patients' occluders were all in normal position followed up at 6 months to 2 years after operation,there were no tiny residual shunt and valves regurgitation except 1 patient.Conclusions The technique that TEE guiding and monitoring in occluding procedure with cardiopulmonary bypass for children muscular ventricular septal defects can shorten the time of cardiopulmonary bypass,significantly reducing complications and risks of the surgery,improving the successful rate.
5.Hybrid procedure with cardiopulmonary bypass for muscular ventricular septal defects in children

Objective To summarize the technique and clinical experience of hybrid procedure under cardiopulmonary bypass (CPB) in children with muscular ventricular septal defect (mVSD). Methods From January 2006 to June 2010, 45 cases of mVSD underwent hybrid procedure with CPB. mVSDs were closed with devices under direct vision in 45 cases. Of them, there were 20 males and 25 females. They ranged from 52 days to 12 years [mean (2.05 ±2.48) year] in age and from 3 to 30 kg [(11.93 ±7.70)kg] in body weight. Preoperatively, most of children were highly susceptible to respiratory tract infections. The hybrid approach was used in all patients with CPB under the guidance of transesophageal echocardiography (TEE). The diameter of mVSDs ranged from 2 to 7 mm under TEE. Of 45 cases, 40 patients had increased rates of pulmonary blood flow. 29 patients had left axis deviation and 12 cases had sinus arrhythmia on electrocardiography (ECG). 19 had other congenital heart lesions, including transposition of great arteries in 1 case, tetralogy of Fallot in 2, pulmonary artery stenosis in 3, patent ductus arteriosus in 6, atrial septal defects in 6) and aorta coactation in 1. The quantity of VSDs were from 1 to 7 (single, in 7; two, in 24 case; three, in 8 case; four, in 5 case and seven, in lease. 37 patients were combined with pulmonary hypertension in our cohort. 38 patients with another large VSD and 19 with other congenital heart lesions were required surgical repair at sometime. Results The hybrid procedures were undertaken in all 45 cases of this cohort. All cases were successful and no deaths occurred during operation. A total of 48 devices were implanted in 45 patients, including single devices in 42 cases (device size ranged from 4 to 10 mm) and two devices in 3 cases (device size ranged from 4 -7 mm). The average time on CPB was (58.28 ±20.70) min , while aortic crossclamp time was(34. 94 ± 14.75) min. In addition, the time on mechanical ventilation postoperatively ranged from 2 hours to 6 days. Compared to the older children, 20 infante aged less than 8 monhad a significant difference in cardiac function in the early postoperative period. One infant was given up treatment because of serious infection. Anather cases recovered with the use of supportive treatment, such as using vasoactive agents, digoxin, inhaling nitric oxide, diuresis, and so on. The enteric-coated aspirin was given at dose of 5 mg ? kg -1. day -1 for a period of 3 to 6 months as usual postoperatively. All patients attended follow-up at 1 week, 1 month, 3 months, 6 months, 1 year and 2 years post-procedure. No major complications were encountered during this period. All cases were no instance of migration of any of the devices, residual shunt, aortic regurgitation, atrioventricular valve dysfunction, Ⅲo atrial-ventricular conduction block, new arrhythmia, and so on. There are no death in long-term follow-up. Conclusion Hybrid procedure is safe and effective for the closure of congenital heart defects in children.
6.MSCs-Derived miR-150-5p-Expressing Exosomes Promote Skin Wound Healing by Activating PI3K/AKT Pathway through PTEN

Background and Objectives:
The goal of this study was to investigate the mechanism of mesenchymal stem cell (MSC)-derived microRNA (miR)-150-5p-expressing exosomes in promoting skin wound healing through activating PI3K/AKT pathway by PTEN.
Methods:
and Results: Human umbilical cord (HUC)-MSCs were infected with miR-150-5p overexpression and its con-trol lentivirus, and HUC-MSCs-derived exosomes (MSCs-Exos) with stable expression of miR-150-5p were obtained. HaCaT cells were induced by H2O2 to establish a cellular model of skin injury, in which the expression of miR-150-5p and PTEN and the phosphorylation of PI3K and AKT were evaluated. HaCaT cells were transfected with pcDNA3.1-PTEN or pcDNA3.1 and then cultured with normal exosomes or exosomes stably expressing miR-150-5p. Cell proliferation was inspected by CCK-8. Cell migration was detected by scratch test and cell apoptosis by flow cytometry. The starBase tool was used to predict the binding site of miR-150-5p to PTEN. Dual-luciferase reporter assay and RIP assay were applied to assess the interaction between miR-150-5p and PTEN. In H2O2 -induced HaCaT cells, the miR-150-5p expression decreased, and PTEN expression increased in a concentration-dependent manner. MSCs-Exos promoted the growth and migration of H2O2 -induced HaCaT cells and inhibited their apoptosis. In addition, overexpression of exosomal miR-150-5p enhanced the protective effect of MSCs-Exos on H2O2 -induced HaCaT cells; PTEN overexpression in HaCaT cells partially restrained miR-150-5p-mediated inhibition on H2O2 -induced injury in HaCaT cells. PTEN was a target gene of miR-150-5p. MiR-150-5p regulated PI3K/AKT pathway through PTEN.
Conclusions
MSCs-derived miR-150-5p-expressing exosomes promote skin wound healing by activating PI3K/AKTpathway through PTEN.
