Objective To study the effect on bone formation by intermittent administration of simvastatin. Methods Seventy three month old virgin female rats, weighted (200?10) g each had been divided into two groups ovariectomized (50 rats) and pseudo ovarectomized (20 rats). Ten rats each of Group A and Group B were sacrified at the 30 th day for the confirmation of successful establishment models of the osteoporosis. Then the remaining animals were re divided into 5 groups: Group A, pseudo ovarectomized control, 10 rats; Group B, ovarectomized control, 10 rats; Group C, given Nylestriol 0.1 mg/(kg?d), 10 rats; Group D, 10 rats, given calcium 10 mg and vitamin D3 2 IU/(kg?d); Group E, 10 rats given simvastatin 5 mg/(kg?d) for 14 days; and the drug was suspended for 28 days, then simvastatin was given for another 14 days. All of the animals were given the agents through gastric tube 30 days after surgery. From March 2000 to March 2003,15 cases of post internal fixation of fractures and arthroplasty infection were selected from Department of Traumatic Orthopedics,Tianjin Hospital,including 6 males and 9 females.The average age of re-operation was 56 years old.The average time from the primary surgery to diagnosis of infection was 0-19 months.Of the 15 cases,4 of arthroplasty infection underwent prosthesis extraction and temporary fixation with bone cement containing antibiotic and underwent another arthroplasty 6 months later.Of 11 cases of infection following internal fixation,the internal fixation was effective in 2 cases,and the fixation was removed after fracture healing,and ineffective in 9 cases.The fixation was removed primarily and focus of infection was cleared,lavaged with antibiotic solution,and another internal fixation was performed.The pus samples of 15 cases showed positive.Eleven cases(73.3%) had chronic infection including 9 occurring following fracture fixation and 2 following arthroplasty.Four cases(26.7%) had acute infection,including 2 occurring following arthroplasty and 2 following fracture fixation.No infection caused by blood circulation was found.The Gram-positive bacteria,staphylococcus aurens,staphylococcus epidermidis,and enterococcus occupied the most part of the infections in spite of the location and type of the infections,and Gram-positive bacteria-staphylococcus aurens was the most common.The order of the antibiotics sensitivity was Vancomycin,Rifampicin,Gentamicin,and Cephazoline.In addition,antibiotics sensitivity was varied in different infections.In samples of 15 cases,2 cases were sensitive to two bacteria at the same time following fracture fixation.The staphylococcus aurens,staphylococcus epidermidis,and enterococus are the high risk of injection following internal fixation of fractures and arthroplasty.The sensitive antibiotics and long-term medication are effective methods to treat the infection.

Melorheostosis is a rare disorder of osteopathia. The clinical characteristics of melorheostosis in children is totally different from that in adults. The radiographic features of melorheostosis include the hyperostosis in endosteal on the one side of the cavum medullare and formed streakiness. The soft tissue contracture of the limb and joint deformities are the symptoms of melorheostosis. Here, the authors reported a melorheostosis case of 6 years old girl who suffered from severe valgus deformity of the right knee with permanent patellar dislocation. A surgical stabilization was applied with lateral soft tissue release, medial soft tissue stabilization and transferred the vastus medialis laterally (kinetic stability). After 19 years follow up postoperatively, the limb developed well in satisfied alignment with good function of knee joints, even participated in some sports activities. Other authors reported a similar case of melorheostosis with surgical treatment and achieved good outcomes in limb realignment and reduction. The further suggested that the surgical treatment should be produced before epiphyseal closure.

目的观察MOTOmed智能运动训练系统对脑卒中偏瘫患者上肢运动功能和日常生活活动能力(ADL)的影响，并进行相关费用的分析和评价。方法将60 例脑卒中患者分为对照和治疗组各30 例，均采用常规康复治疗，治疗组在此基础上增加MOTOmed智能运动训练系统训练，并对每组患者在入组时、治疗2 个月和4 个月末分别采用简式Fugl-Meyer 评定法(FMA)、Barthel 指数(BI)对患者的上肢运动功能和ADL进行评定，并进行成本-效果分析。结果治疗后，治疗组FMA和BI 评分优于对照组(P<0.05)。两组患者住院总成本之间无显著性差异(P>0.05)，治疗组各项指标每改善1 分(或5 分)所产生的相关成本费用明显低于对照组(P<0.05)。治疗组与对照组比较，上肢运动功能评分每增加1 分，少花费56.3 元；ADL评分每提高5 分，少花费278.5 元。结论脑卒中偏瘫患者上肢运动功能康复治疗中，应用MOTOmed智能运动训练系统是一种既经济又有效的康复治疗方案。

Objective To assess the relationship between type 2 diabetes and the risk of kidney cancer. Methods PubMed, Medline, CNKI and VIP were used to identify studies which regarded the relationship between type 2 diabetes and risk of kidney cancer up to July 2014. The relative risks (RRs) and the corresponding 95% confidence interals (CIs) were used to estimate the association between type 2 diabetes and the risk of kidney cancer through software STATA. Results 8 studies were included in this analysis. Compared to the popula-tion without type 2 diabetes, there was an increased risk of kidney cancer in the type 2 diabetes patients, the pooled RR (95%CI) was 1. 55 (1. 24,1. 92),P<0. 05. Conclusion Our meta-analysis suggestes that type 2 diabetes can increase the risk of kidney cancer.

Objective To analyze and evaluate the effects of the tissue engineering bladder reconstruction on the upper urinary tract structure and function.Methods The 8 male beagles were randomly divided into the two groups:sham-operation group (group A,n=4)and the collagen scaffold repair group (group B,n=4).The bladder defect animal model was established in the group B by using the collagen scaffold materials to repair the bladder.The renal function related biochemical indicators were detec-ted and the renal Doppler ultrasonic examination was performed in each group before repair and in 23 weeks after repair.The speci-mens from the two groups were performed the gross morphology observation and the histology examination on postoperative 24 weeks.Results The renal Doppler ultrasound examination showed the normal kidney morphology and normal blood flow signal.In the general observation,no calculi and neoplasm were found in the kidney and ureter of the experimental dogs.The renal function related biochemical indicators had no statistically significant differences between the two groups(P>0.05).The histological exami-nation indicated that the organization structure was integrity,the nephrons in each group had no obvious pathological changes.Con-clusion Using the collagen scaffold materials to reconstruct the canine bladder has no adverse influence on the upper urinary tract structure and function,this tissue engineering approach has good feasibility.

Objective To observe the effects of three treatment methods on renal anemia in maintenance hemodialysis patients with hyperparathyroidism secondary to uremia and analyze the influencing factors of erythropoietin (EPO) dosage. Methods A total of 55 maintenance hemodialysis patients with secondary hyperparathyroidism at the hemodialysis center of Huashan Hospital affiliated to Fudan University from January 2015 to December 2016 were retrospectively divided into three groups according to different treatment methods, parathyroidectomy +forearm transplantation group (surgery group, n=16), cinacalcet treatment group (n=6), and calcitriol treatment group (n=33), respectively. The hemoglobin level and erythropoietin dosage were measured before treatment and in the 3rd month, the 6th month and the 12th month after treatment. The changes of hemoglobin and erythropoietin dosage in the three groups before and after treatment were observed, and the mixed effect model was used to analyze the difference of the change of hemoglobin and erythropoietin dosage among three groups. Multiple linear regression analysis was used to analyze the influencing factors of EPO dosage after one year. Results The levels of intact parathyroid hormone (iPTH) in the surgery group and the cinacalcet group before treatment were significantly higher than that in the calcitriol group (both P<0.05). In the 12th month after treatment, the levels of iPTH decreased significantly in the patients of surgery group and the cinacalcet group compared with those before treatment (both P<0.05). The levels of serum alkaline phosphatase, serum calcium and serum phosphorus in the surgery group also decreased significantly compared with those before treatment (all P<0.05). The mixed effect model analysis showed that the hemoglobin level of surgery group was on an upward trend after the treatment, and the overall level was significantly higher than cinacalcet and calcitriol treatment group (P=0.007). There was no significant difference in the dosage change of erythropoietin (EPO) in the three groups over time. However, the intra-group comparison of the mixed effect model showed that the dosage of EPO in the 12th month was significantly lower than that of before the treatment in surgery group (P=0.007). Multiple linear regression analysis showed that dialysis vintage (B=-0.064, P=0.012) and ferritin ≥ 500 μg/L (B=0.645, P=0.032) were independent influencing factors of EPO dosage. The longer the dialysis vintage, the less EPO dosage, and more EPO dosage were observed in patients with ferritin ≥ 500 μg/L. Conclusions Parathyroidectomy and forearm transplantation is more effective in reducing EPO dosage and improving renal anemia in maintenance hemodialysis patients with secondary hyperparathyroidism. Dialysis vintage and ferritin are independent influencing factors for the dosage of EPO.
