1.Rapid maxillary expansion combined with fixed orthodontics

BACKGROUND:In the treatment of teeth later fixed appliance, whether the expansion effect can be maintained? Whether the expansion in the process of mandibular rotation could be improved? Whether the high angle cases are suitable for rapid palatal expansion? OBJECTIVE:To sum the effect of rapid maxil ary expansion combined with fixed orthodontic technique on the occlusion craniofacial three-dimensional direction, hard and soft tissues and temporomandibular joint, as wel as its long-term stability. METHODS:The CNKI database, PubMed database and Wanfang database were searched for the academic journals and literatures on rapid maxil ary expansion combined with fixed appliance with the key words of“maxil ary expansion, fixed appliance, high angle, stable”. The analysis function of database was used. RESULTS AND CONCLUSION:Rapid maxil ary expansion can effectively obtain arch expansion, increase the arch width, and provide space of align teeth required. Fixed appliance can effectively control the tooth movement in three-dimensional space, precisely adjust the teeth, and make the tooth rows into the ideal arch form. Combination of these two kinds of appliances can obtain the complementary efficiency advantages of the appliances, and shorten the treatment course. Expansion can cause mandibular clockwise rotation and mandibular plane angle increasing, while with the application of upper and lower fixed appliance, the occluding relation between maxil a and mandible can be adjusted more precisely in order to make the mandibular first molar move to the maxil ary molar buccal, establish good occlusal relationship, so that decrease the mandibular plane angle. So the rapid maxil ary expansion combined with fixed orthodontic can be used appropriately in the treatment of the patients with high angle intraversion.
2.Study on Method for Estimate of Cosmic Radiation Effective Dose Rate at Civil Aviation Altitude in Aerosphere.

Objective To monter radiation doses received in flight and keep aircrew from being radiated,in accordance with "The Basic Standard on Ionization Radiation Protection and Radiation Source Safety(GB 18871-2002) of China".A method for calculating radiation dose rate is described.Methods On the basis of a great deal empirical and calculated data,a fundamental calculating formula was derived to calculate cosmic radiation dose rate in aerosphere from sea level to altitude of 25 km.The cosmic radiation dose rate was a function of altitude,geomagnetic vertical cut off rigidity and Heliocentric potential.Results One hundred and thirty eight position of cosmic radiation effective dose rates of 10 km altitudes,various latitudes and various longitudes were calculated with this method,as well as compared to those calculated with CARI-6 software of Aviation Commission of USA.Their differences were within 12.56%.Conclusion It is showed that this method is sufficient to ensure a precision on the dose estimation for radiation protection of aviation flight.

4.Serum asymmetric dimethylarginine and endothelial function after renal transplantation

Objective To investigate the relation between serum asymmetric dimethylarginine (ADMA) level and endothelial function before and after living donor kidney transplantation in uremic patients.Methods A total of 38 renal transplant patients (21males and 17 females) and 36 healthy controls (20 males and 16 females) were enrolled. Plasma ADMA, symmetric dimethylarginine (SDMA), malondialdehyde (MDA), glutathione peroxidase (SeGSHPx), C-reactive protein (CRP) were mea-sured before transplantation and on Days 1, 3, 7, 14, and 28 posttransplantation. The brachial artery flow mediated dilatation (FMD) was studied before transplantation and on the 28th day after the transplantation. Results Serum levels of ADMA,SDMA,MDA and CRP were significantly increased, and the activities of nitric oxide (NO) and SeGSHPx were decreased in uremic patients compared with age matched healthy subjects (all P<0.01). Serum levels of ADMA,SDMA,MDA and CRP decreased while the activities of NO and SeGSHPx increased significantly at the first day after the transplantation (all P<0.01). The decrement of plasma SDMA normalized on the 28th day (P>0.05). The FMD was lower in the patients than the control group (P<0.01) and improved significantly on the 28th day of posttransplantation (P<0.05). Serum levels of ADMA were positively correlated with MDA (r=0.412, P<0.01;r=0.342,P<0.01) and negatively correlated with the values of SeGSHPx (r=-0.345, P<0.01;r=-0.315, P<0.01) and FMD (r=-0.452,P<0.01;r=-0.416,P<0.01) both before and after kidney transplantation. Conclusion The level of serum ADMA is associated with endothelial function improvement in uremic patients both before and after kidney transplantation.
6.Application of 12Fr catheter in catheteration for patients with prostatic hypertrophy

Objective To investigate the effect of 12 Fr catheter on the catheteration for the patients with prostatic hypertrophy after stroke. Methods One hundred and four sixteen patients with prostatic hypertrophy from urinary detention were divided into the control group (n=80) and the observation group (n=84) based on the odd and even numbers of admission date according to a digit random table. The control group received catheteration with a 14~20 Fr catheter, and the observation group did with a 12 Fr catheter. The two groups were compared in terms of pains, urethral injury and leakage of urine during intubation. Result The pain, urethral injury, leakage of urine in the observation group were all significantly lowered than those of the control group (all P<0.05). Conclusions 12 Fr catheter for catheteration for the patients with post-stroke prostatic hypertrophy can significantly reduce the degree of pain, the urethral injury and leakage of urine. Thus it is worthy of clinical application.
7.Association between elevated visfatin and carotid atherosclerosis in patients with chronic kidney disease.

OBJECTIVE:
To determine the levels of visfatin in patients with chronic kidney disease (CKD) and to explore the relationship between visfatin levels and caroid atherosclerosis in CKD patients.
METHODS:
A total of 180 CKD patients (102 non-dialyzed patients and 78 dialysis patients) and 42 healthy subjects enrolled in this study. The serum levels of visfatin, IL-6, and high sensitive C-reactive protein (hsCRP) were measured by ELISA. Common carotid arteries intima-media thickness (CCA-IMT), cross-sectional calculated intima-media thickness (c IMT) area and atherosclerotic plaque were detected by non-invasive high-resolution B-mode ultrasonography.
RESULTS:
The serum levels of visfatin, IL-6 and hsCRP were significantly increased in CKD patients compared with age matched healthy subjects (P<0.01). The serum levels of visfatin, IL-6 and hsCRP in the dialyzed patients were significantly higher than those in non-dialyzed patients (P<0.05). Visfatin levels increased with the progression of renal dysfunction and inversely related to creatinine clearance (Ccr) in non-dialyzed patients (r=-0.415, P<0.05). Patients with carotid artery plaques showed significantly higher levels of visfatin[(34.22±7.96) ng/mL)] compared with those without plaques [(28.24±6.18) ng/mL, P<0.05]. The serum levels of visfatin were closely correlated with IL-6 (r=0.548, P<0.001), hsCRP (r=0.430, P<0.001), CCA-IMT (r=0.462, P<0.05), and c IMT area (r=0.411, P<0.05). After adjusting for GFR, age, gender, and other risk factors associated with atherosclerosis, multiple stepwise regression analysis showed that serum visfatin is a independent risk factor for CCA-IMT (β=0.433, P<0.01) and c IMT area (β=0.412, P<0.01).
CONCLUSION
The serum level of visfatin increase with the progression of CKD and should be considered to be a new risk factor for atherosclerosis in CKD.

8.Application of nursing safety management mode in disinfection supply room management and analysis of its risk factors

Objective To discuss the application of nursing safety management mode in the management of disinfection supply center and its risk factors. Methods A total of 52 workers from our hospital's disinfection and supply center from April 2017 to April 2018 were randomly divided into two groups. The control group was asked to use the routine management model and the study group was asked to use the nursing safety management model. Results The incidence of adverse events in the study group was lower than that in the control group, and the difference was statistically sig nificant (P<0.05). The biological, chemical and physical risk factors of the study group were lower than those of the control group. The satisfaction of the study group was significantly higher than that of the control group (P<0.05). Conclusion The application of nursing safety management mode in the management of disinfection supply center can reduce the occurrence of adverse conditions, improve the quality of care, and has obvious effects. It is worthy of application and promotion in the management of disinfection supply center.

Diagnostics is one of the most important bridge courses for medical students from basic to clinical. Doctor-patient communication runs through the whole process of patient diagnosis and treatment. How to improve medical students' ability of doctor-patient communication? Our teaching team has carried out continuous reform and explored the scientific effective teaching mode. Recently, through the construction of "doctor-patient communication skills" quality online course, efforts have made to build an online and offline blended learning mode, which has gradually realize the integration with diagnostics teaching, and has achieved remarkable results. It also provides a scientific practical basis for the integration of doctor-patient communication and other clinical courses, which is worthy of promotion.
10.Regional homogeneity in mild cognition impairment patients with amnestic and non-amnesic: a resting-state fMRI study

Objective To investigate regional spontaneous brain activity in mild cognition impairment (MCI) patients with amnesic (aMCI) and non-amnesic (naMCI).Methods Twenty-five aMCI patients,21 naMCI patients and 15 normal controls (NC) underwent resting-state functional magnetic resonance imaging.The regional homogeneity (ReHo) map of the whole brain was obtained by calculating the similarity of each voxel with its nearest 26 voxel time series.The differences of ReHo map across the whole brain among three groups were compared.Results In aMCI group,ReHo values were lower in right frontal lobe and higher in left middle temporal gyrus and left cerebellum compared with NC (P<0.05,Alphasim correction).In naMCI group,ReHo values were higher in anterior cingulate cortex and right middle frontal gyrus and lower in right parahippocampa gyrus,right middle temporal gyrus as well as right precuneus compared with NC (P<0.05,Alphasim correction).Compared with naMCI,the ReHo values were significantly higher in left prefrontal gyrus,left middle temporal gyrus and lower in right cerebellum (P<0.05,Alphasim correction).Conclusion There are differences in spontaneous brain activity of left prefrontal gyrus,left middle temporal gyrus and right cerebellum between aMCI and naMCI,which may be used to differentiate brain function between aMCI and naMCI patients.
