1.The molecular mechanisms of lymphangiogenesis in neoplasms

Many studies have demonstrated the presence of increasing newly formed lymphatic vessels in tumour tissue, which is significantly correlated with the invasion of tumours,lymphatic metastasis, prognosis and so on. Re-cently,with the discovery of specific lymphatic endothelial cells markers and the development of novel experiment methods, turnout lymphangiogenesis become the focusing point for researchers. Here we will review what is known a-bout the reasons, molecular mechanisms and inhibitors of lymphangiogenesis in neoplasms.
2.Clinical efficacy of sorafenib in treatment of primary hepatic carcinoma and its effects on serum VEGFR -2 and PLGFlevels

To investigate the clinical efficacy of sorafenib in the treatment of primary hepatic carcinoma (PHC) and its effects on serum vascular endothelial growth factor receptor -2 (VEGFR -2) and placental growth factor (PLGF) levels.Methods A total of 110 patients with a confirmed diagnosis of PHC who received treatment in Jinshan Hospital Affiliated to Fudan University from July 2014 to March 2016 were randomly and equally divided into observation group and control group .The control group was given routine treatment, while the observation group received sorafenib in addition to the routine treatment .Serum levels of VEGFR -2 and PLGF were measured by ELISA.Liver function parameters, aspartate aminotransferase (AST), prothrombin time (PT), total bilirubin (TBil), albumin (Alb), and alanine aminotransferase (ALT), were also recorded.Comparison of continuous data between groups was made by independent samples t -test, and the changes in continuous data after intervention in each group were evaluated by paired samples t -test.Comparison of categorical data between groups was made by chi -square test.Results The observation group showed significant reductions in serum VEGFR -2 and PLGF levels after treatment (VEGFR -2: 7053.2 ±1836.1 ng/L vs 8721.4 ±2427.8 ng/L, t =4.089, P <0.001; PLGF: 468.4 ±136.5 pg/ ml vs 656.8 ±191.4 pg/ml, t =5.975, P <0.001).After treatment, the observation group had significantly lower serum VEGFR -2 and PLGF levels than the control group (VEGFR -2: 7053.2 ±1836.1 ng/L vs 8097.5 ±2325.4 ng/L, t =2.64, P <0.05; PLGF: 468.4 ± 136.5 pg/ml vs 643.3 ±195.8 pg/ml, t =2.48, P <0.05).The observation group showed significant changes in serum AST and ALT lev - els after treatment (t =4.302 and 3.097, both P <0.05).After treatment, the observation group had significantly lower serum AST and ALT levels than the control group (t =2.56 and 2.39, both P <0.05).Compared with the control group, the observation group had better follow -up results, with a significantly increased disease control rate (27.3% vs 47.3% , χ2 =4.705, P =0.030), and had a significantly higher survival rate at 40 months after treatment (43.6% vs 69.1%, χ2 =7.245, P =0.007).Conclusion Sorafenib is effective in the treatment of PHC patients, as it can significantly reduce the serum levels of VEGFR -2 and PLGF, prolong the survival time of patients, and improve the prognosis of patients.
3.Effect of upper limb nerve block by ultrasound-guided technology

Objective To observe the clinical effect of ultrasound-guided technology in the operation of upper limb in patients with nerve block. Methods Two huandred patients had be performed upper limb operation in our hospital form Jan2012 to Dec 2013 were randomly divided into two groups, 100 cases of each group, group A who was guided by tra-ditional anatomical method;group B who was guided by ultrasound-guided positioning technolog, 1% Lignocaine +0.375%Ropivacaine+10 mg Dexamethasone were injected in two groups. Comparison of the effect of two groups nerve block: the onset time of anesthesia, success rate, the amount, complications. Results Anesthesia operation completion time, group B was shorter than group A (TA=7.7±1.9min, TB=4.6±1.4min);Nerve block work time, group B was longer than groupA(TA=19.4±3.4min, TB=10.7±2.3min);group B anesthesia effect was good, with excellence rate reached 97%and fine rate was 100.00%;group A anesthesia effect was general, excellence rate reached 47.00%,fine rate was 91.00%;group B adverse reactions incidence was 2.00%, group A was up to 9.00%;compared the above group A and B related datas of difference was statistically significant (P<0.05).The duration of anesthesia (TA=417.5±98.7min, TB=406.3±79.1min), the more similar between two groups had no statistical significance (P>0.05). Conclusion Ultrasound-guided technology was applied to the upper limb brachial plexus block, which could enhance the effect of nesthesia, re-duce the dosage of anesthetic drug ,complications and adverse reaction.
4.Probe the syndrome differentiation system of six meridians of circular motion

The internal organs and meridians were associated with Yin and Yang, five elements, six qi, and time and space, based on the holistic view of heaven, earth and human, according to Huangdi Neijing. The syndrome differentiation system of six meridians and Zang Fu meridians were established by Shanghan Zabing Lun, on the basis of the three Yin, three Yang, six meridians, and five Zang system in Huangdi Neijing. We put forward the concept of the six meridians syndrome differentiation system of circular motion, considering that the six meridians syndrome differentiation system actually implies the theory of circular motion. The syndrome differentiation system was constructed with the circular model of one qi circulating around the road, rising left and falling right, corresponding up and down, and maintaining conservation in the middle as the core, integrating Yin and Yang, five elements, six qi, Zang Fu and meridians, qi, blood and body fluid, and the integration of heaven, earth and human, focusing on "disease location and disease nature", taking classical prescriptions as the main treatments, and cooperating with external treatments such as acupuncture and moxibustion. We organically combined the circular motion with the syndrome differentiation of the six meridians, systematically interpreted the physiological bases, pathological changes, progressive patterns, and the treatments, based on syndrome differentiation, by inheriting the classical thinking mode of Hetu, Luoshu,Zhouyi, Huangdi Neijing, ShennongHerbal Classic, and Shanghan Zabing Lun.
