1.Clinical Analysis on Electrolyte Imbalance in 96 Infants with Ⅱ~Ⅲ Malnutrition

Objective To study the status of electrolyte imbalance in infant with moderate to severe malnutrition and discuss reasonable clinical therapy.Methods we examined serum level of electrolyte and analyse retrospectively.96 infants were divided into three group-pneumonia,diarrhea and mixed group,difference of electrolyte imbalance was compared within the three groups.Results Hypokalemia and hyponatremia were the major electrolyte imbalamce.60.4% had hypokalemia,55,2% had hyponatremia;the other imbalance,46.9% had hypochloremia,40.6% had hypocalcemia,32.3% had hypomagnesemia,30.2% had metabolic acidosis and 16.6% had metabolic alkalosis.Conclusions The pneumonia group was more likely to have hyponatremia,and the diarrhea group was more likely to have hypokalemia.Serum level of chloride fluctuated with the change of blood potassium.Hypomagnesemia and hypocalcemia were normal and were influenced by acid-base balance.Acidosis was more likely to be found in diarrhea group,and was metabolic in most situation.There was a trend of emerging metabolic alkalosis in severe dehydration patient.

Objective To study the intervention effect of montelukast on thymus and activation-regulated chemokine(TARC) in the children with asthma.Methods 100 children with asthma were randomly divided into montelukast (LTRA) group ( n = 50) and budesonide (BUD) group ( n = 50), the LTRA group was treated with montelukast, the BUD group was treated with budesonide,50 children without asthma as control group were inhaled NS.Before and after 7 days treatment,the asthma symptoms, FEV1,concentration of TARC were measured.Results Before treatment,the concentration of TARC in asthma group was significantly higher than control group (P < 0.05 ).After treatment, the concentration of TARC in BUD group and LTRA group was significandy decreased( P < 0.05 ), but the difference between these two groups was not significantl( P > 0.05 ), the concentration of TARC in control group was not significantly decreased(P > 0.05 ) ;the symptoms were better after treatment in BUD group and LTRA group,(P <0,05) ,and the pulmonary function was significantly improved after treatment in BUD group and LTRA group ( P < 0.05 ).Conclusion TARC was the important factor in children asthma.Montelukast could block the production of TARC ,and was more convenient and safe for children asthma.
3.Acute heart failure as an initial symptom of pediatric rheumatic carditis: clinical analysis of 10 cases

Objective:To investigate the clinical manifestations, pathological characteristics, treatment and prognosis of pediatric rheumatic carditis with the initial symptom of acute heart failure, and to improve the clinical understanding of the disease.Methods:The clinical data, laboratory examination results, imaging findings, treatment and prognosis data of 10 cases with acute rheumatic carditis were collected and analyzed retrospectively.The patients pre-sented with acute heart failure as the first symptom and were treated in Shenzhen Children′s Hospital from January 2015 to February 2022.Results:There were 4 males and 6 females in this study.The mean age was 9.1 years (3 years and 1 month to 11 years and 3 months), and the average onset age was (9.3 ± 3.2) days (4-14 days). All the 10 cases had circulation symptoms.Besides, 4 cases were also complicated with joint symptoms, 3 cases with neurological symptoms and 2 cases with skin symptoms.Echocardiography revealed mitral valve diseases in all cases.More specifically, combined valve diseases were found in 5 cases, pure mitral regurgitation in 3 cases, and mixed mitral valve diseases (mitral regurgitation complicated with mitral stenosis) in 2 cases.After receiving antibiotic therapy, anti-infection treatment and anti-heart failure therapy, symptoms improved in all patients and valve lesions were alleviated.All patients were followed up for 6 months to 6 years.Six cases had persistent rheumatic heart disease during the follow up.Conclusions:Pediatric rheumatic carditis with the initial symptom of acute heart failure is characterized by early onset and the typical manifestation of rheumatic fever.Mitral valve disease is the commonest lesion.Echocardiography is of great value for the early diagnosis of the disease.Medical treatment is effective, but the incidence of chronic rheumatic heart disease is still high.

Objective To explore the blood lead level and its relationship with behavior in school?age children from rural areas of Chongqing. Methods A total of 697 students from grades 3 to 6 in the fall semester of 2014 from 14 rural townships in one district of Chongqing was selected by using the random cluster sampling method. Blood were sampled to analyze the lead level. Neurobehavioral tests were performed to determine their personal cognitive and memory ability. Questionnaires and physical examinations were administered to obtain the information of confounding factors. All students were divided into Q1-Q4 groups according to the quartile of their blood lead level. The relationship between the blood lead level and behavior was analyzed by multivariate logistic regression model and restricted spline regression model. Results The mean age of 697 students was (10.07±1.36) years old, and the median (interquartile range) of their blood lead level was 44.31 (35.42) μg/L. Multivariate logistic regression model showed that after adjusting for age, gender, body mass index and maternal culture level, compared with Q1 group, the OR (95%CI ) values of high digit symbol substitution test (DSST) scores and high overall memory quotient (MQ) scores in Q 3 group were 1.65 (1.01-2.70) and 2.10 (1.21-3.62), and the OR (95%CI ) value of high long term memory (LTM) scores in Q4 group was 0.53 (0.31-0.92). The results of the restricted spline regression model showed that the dose?response curves between the blood lead level and MQ/LTM test scores were both parabolic (P<0.05). Conclusion The blood lead level of school?age children from rural areas of Chongqing is the same as that from other areas of China, but slightly higher than that from other areas of Chongqing. Children with higher blood lead level have poor long?term memory ability.

7.Construction and eukaryotic expression of recombinant HSA-TP5 fusion gene expression vector

Objective:To construct the expression vector of the fusion protein of human serum albumin (HSA) and thymopentin (TP5) and to express it in Pichia pastoris,and to elucidate the biological activity of fusion protein.Methods:The HSA-TP5 fusion gene was constructed by gene recombination and transfected into Pichia pastoris to construct the eukaryotic expression system of HSA-TP5.The recombinant eukaryotic expression plasmid of PPICZα-HSA-TP5 was obtained by agarose gel electrophoresis and purification reagent.The two step fermentation method was used to ferment gene engineering bacteria of HSA-TP5 in high density,and the fermentation supernatant protein was precipitated and concentrated;the purified fusion protein was obtained by cation exchange chromatography and hydrophobic chromatography and analyzed by SDS-polyacrylamide gel electrophoresis.The effect of the fusion protein on the proliferation of lymphocytes was detected by MTT assay.Results:The HSA target gene fragment with length of 1 845 bp was achieved by PCR method.The HSA-TP5-pPICZαC fusion plasmid was identified by restriction endonuclease digestion,and the fragment length was 707 bp.The sequence analysis showed that the HSA and TP5 sequences of the target genes were identical with the gene sequences reported in GenBank and were fused by forward fusion.PCR method confirmed that the eukaryotic recombinant plasmid PPICZ αC-HSA-TP5 was integrated into the yeast genome,and compared with control group,the target gene PCR product length was found to be 1 860 bp.SDS-PAGE analysis showed that the expression level of HSA-TP5 fusion protein was gradually increased with the induction time within 72 h.HSA-TP5 fusion protein was purified by cation exchange chromatography and AKTA multifunctional protein purification system.The MTT assay results showed that HSA-TP5 fusion protein was consistent with TP5 protein in promoting lymphocyte proliferation activity.Conclusion:HSA-TP5 fusion protein can be obtained by constructing the eukaryotic expression system of Pichia pastoris and owns the biological activity.

Objective:
To investigate the association between aflatoxin exposure and primary hepatocellular carcinoma (PHC) development.
Methods:
From December 2013 to May 2016, we selected 214 patients newly diagnosed with PHC as cases, and 214 patients as controls from three hospitals in Chongqing. Cases were confirmed with PHC diagnosis standard. And cases caused by clear reasons such as drug-induced liver injury, alcoholic liver damage, fatty liver and gallstones etiology, were excluded. Controls were included with no cancer and no digestive system disease, and recruited simultaneously with cases. Cases and controls were frequency-matched (1∶1) by same gender and age (±3 years). Peripheral blood and random urine samples were collected and analyzed for serum HBsAg status by biochemistry analyzer, and serum AFB1-ALB adduct and urinary AFB1-N7-GUA adduct by ELISA. Basic information, living habits and history of disease for patients were obtained by questionnaires. Objective:
To retrospectively analyze the laboratory findings and ultrasonographic features in acute phase of children suffering from Kawasaki disease (KD) with stable hemodynamics and Kawasaki disease shock syndrome (KDSS), so as to provide the evidence for early diagnosis, timely treatment and improvement of prognosis of KDSS.
Methods:
Four hundred and eighteen patients with KD diagnosed at Shenzhen Children′s Hospital from November 2016 to May 2018 were selected, including 23 KDSS patients(KDSS group) and 395 cases with stable hemodynamic(KD without shock group). The clinical characteristics, laboratory index and ultrasonic examination data of the 2 groups were collected and compared for statistical conclusion.
Results:
