[Summary] All patients with cholesterol side-chain cleavage enzyme ( P450scc) deficiency that have been reported presented with early adrenal failure. Here we described a 35-year-old male presented with infertility as the only initial presenting complaint. He had received two separate surgeries to remove bilateral testicular masses. We reevaluated the resected tumors and found testicular adrenal rest tumor ( TART) pathology in the resected tumor. We profiled steroid hormones and found significantly elevated ACTH. CT scan revealed bilateral adrenal hyperplasia. Mutation screening identified compound heterozygous mutations (R353W and P432L) in the P450scc encoding gene (CYP11A1). The patient was finally diagnosed as congenital adrenal hyperplasia.

Objective To investigate the clinical therapeutic effect of citicoline sodium combined with Shenxiong glucose in the treatment of senile hypertensive cerebral infarction in the elders. Methods 80 elderly patients with senile hypertensive cerebral infarctiontreated in our hospital were selected and randomly divided into the single treatment group and the combination treatment group,40 cases in each group. Both groups received the routine treatment. The single treatment group additionally received Shenxiong glucose injection (100 ml/d),while patients in the combination treatment group additionally received Shenqiong glucose injection combined with citicoline sodium intravenous infusion therapy (0.5 g/d),both groups were treated for 2 weeks. The levels of serum inflammatory factors, the neurological deficit score, the cognitive function score were compared and analyzed before and after treatment between two groups. Results After systemic treatment, the blood pressure and blood lipid levels of two groups were significantly improved, but there was no significant difference between the two groups (P > 0.05);The serum levels of interleukin (IL)-6,IL-8 and tumor necrosis factor-α (TNF-α) of the combination treatment group improved more significantly (P ≤0.05).After treatment, the oxidative stress indexes were significantly improved in the two groups (P ≤ 0.05).The content of malondialdehyde (MDA) was decreased, while the superoxide dismutase (SOD) activity and nitric oxide (NO) content were increased significantly (P ≤ 0.05);and the improvement degree in the combination treatment group was better than in the single treatment group (P ≤0.05).The degree of improvement in the Modified Edinburgh-Scandinavia Stroke Scale (MESSS) and Hasegawa Dementia Scale (HDS) scores of the combination treatment group was more significant than those in the single treatment group (P ≤ 0.05).The total effective rate of the combination treatment group was 92.5%,which was significantly higher than that of the single group (75.0%),with statistically significant difference (P ≤ 0.05).No obvious adverse reactions happened in two groups during treatment. Conclusions Combination of citicoline sodium and shenxiang glucose on the basis of routine treatment can significantly reduce oxidative stress and inflammation levels, promote the recovery of neurological and cognitive functions, and improve the clinical efficacy and safety. It is worth popularizing and applying in the clinical treatment of senile hypertensive cerebral infarction.

Objective To investigate the mechanism of Sini San Formula in the treatment of ulcerative colitis and depression through"homotherapy for heteropathy"based on network pharmacology and molecular docking. Methods The TCMSP database was used to obtain the potential active components and their related targets;GeneCards,CTD,and TTD databases were used to screen the disease-related targets of ulcerative colitis and depression;the intersection of the predicted targets of the active components and the disease-related targets was used to obtain the potential targets(shared targets)for the treatment of ulcerative colitis and depression by Sini San Formula, and Cytoscape 3.7.2 software to construct a"Chinese medicinals-active components-diseases-common targets"network to analyze the core components;importing the common targets into the STRING database, constructing a common protein-protein interaction(PPI)network. The GO function and KEGG pathway enrichment of the shared targets were analyzed by DAVID database, and molecular docking between the core components and the key targets was verified. Results A total of 136 active components of Sini San Formula were obtained, and 220 potential targets of action(shared targets)for the treatment of ulcerative colitis and depression by Sini San Formula, involving 657 biological processes,70 cellular components,147 molecular functions and 133 signaling pathways. The screening yielded core active compounds such as quercetin,kaempferol,lignans,naringenin,7-methoxy-2-methylisoflavone,key target proteins such as JUN,MAPK3,STAT3,AKT1,and MAPK1,as well as signaling pathways such as TNF,IL-17,Th17 cellular differentiation,HIF-1,and Toll-like receptor.Five potential key targets have strong binding activity to quercetin, kaempferol, lignans and naringenin. Conclusion Sini San Formula may act on key targets such as JUN,MAPK3,STAT3,AKT1,MAPK1,etc.through active components such as quercetin, kaempferol, lignocerotonin, naringenin, etc.,and play the role of"homotherapy for heteropathy"for ulcerative colitis and depression through the signaling pathways such as TNF,IL-17,HIF-1,Toll-like receptor and Th17 cell differentiation.

Objective To explore the feasibility of tracking migration and distribution of SD-rat adipose derived stem cells (ADSCs) labeled with Polyethylene Glycol/Polyehthyleneimine modified superparamagnetic iron oxide (PEG/PEI-SPIO) in rats with chronic cerebral ischemia using MRI.Methods Thirty female SD rats underwent permanent occlusion of bilateral common carotid arteries 6 months were divided into PEG/PEI-SPIO labeled group and unlabeled group (each n =15).Labeled or unlabeled ADSCs suspension was injected into the right ventricle of rats in two groups, respectively.MR scans were performed at the 7th,14th and 21st day after transplantation for each 5 rats.T2 value of T2mapping sequence in hippocampus, cortex and cerebellum were measured. Then the rats were scarified, and the brains were obtained, and Prussia dyeing was performed.Under high magnification, blue dye cells at each time points and brain area were counted.T2 values and blue dye cells were statistically analyzed. Results Class round hypointensity areas were detected in temporol-parietal cortex and hippocampus in both groups on T2WI,T2* WI and SWI.T2 value of the right temporolparietal cortex and hippocampus in the labeled group was shorter than those of the unlabeled group on the 14th day after transplantation (P=0.013,0.045).T2 value of the right temporol-parietal cortex in the labeled group was shorter than that of the unlabeled group on the 21st day after transplantation (P=0.007).The number of blue dye cell of the right temporol-parietal cortex on the 14th and 21th day, hippocampus on the 14th day in the labeled group were more than those of the unlabeled group after transplantation (P=0.029,0.032,0.043).Conclusion ADSCs labeled with PEG/PEI-SPIO transplanted into lateral ventricle of SD rat could migrate to the damaged areas caused by chronic cerebral ischemia. It is possible to use quantitative MRI to track migration and distribution of ADSCs labeled with PEG/PEI-SPIO in rat brain.
