[Objective]To develop a method using HPLC for determination content of saikosaponin a and saikosaponin d of Radix Bupleuri from different areas.[Method]Column:YMC-Pack ODS-A C18(250?46 mm,5?m),mobile phase:acetonitrile-water(43∶57),detected wavelength:203nm.[Results]The regression eguations of saikosaponin a and saikosaponin d were Ya =1.223?106 X - 2.249 8?105(r = 0.999 9),Yd = 2.093?106 X - 4.786 2?105 (r = 0.999 8).The average recovery rates of saikosaponin a and saikosaponin d were 98.13 %,97.99 %.[Conclusion]The HPLC method is simply and suitable to control quantitative of radix bupleurum.

3.Randomized control clinical trial in HJ-RY-1 therapeuticpad of soft stone for osteoarthritis

Objective:To evaluate the clinical efficacy and safety ofHJ-RY-1 therapeutic pad ofsoft stone in treating osteoarthritis.Methods:Patients with osteoarthritis with syndrome ofqi stagnancy and blood stasis were enrolled according to the diagnostic criteria and the evaluation standard ofcurative effect in Instruction Principle ofClinical Research ofNew Chinese Medicines, and were randomly divided into experimental group(treating by HJ-RY-1 therapeutic pad ofsoft stone) and the control group(treating by the paster offar infrared bioactive ceramics).Put them respectively on the afflicted part for more than 12 hours a day, and the treatment course ofboth groups lasted 14 days.The main turnover was determined by the improvement of6 clinical parameter indices and the incidence rate ofwhole curative effect and side effect after a treatment of7 days and 14 days.Results:72 patients were randomly equally divided into two groups, and received predetermined interveneing therapy.There were similar baseline characteristics and comparabilities between them.Totally 68 patients finished 2 weeks follow-up observation and 4 cases were missed(33/36 in experimental group;35/36 in the control group).1 weeks later, there were no significant difference in clinical situation and overall curative effect between two groups.But after two weeks interveneing therapy, there were significant difference on activity pain(t-value was 3.3104), the joint movement range(t-value was 3.1596), self-sensation(t-value was 2.5521) and overall curative effect(t-value was 3.5694) between them(P
4.Study on correlation between the joint detection of CHE,AFU and AFP and primary liver cancer

Objective To explore the correlation between the joint detection of cholinesterase(CHE),alpha-L-fucosidase(AFU) and alpha-fetoprotein(AFP)and primary hepatic cancer(PHC).Methods 56 cases of patients with PHC(PHC group),52 cases of patients with liver cirrhosis(LC group)and 60 cases of healthy individuals(control group)were enrolled in this study,and serum levels of CHE,AFU and AFP were detected and statistically analysed.Results Serum levels of CHE in patients with PHC were negatively correlated with levels of AFU and AFP(correlation coefficient was -0.889 and -0.797 respectively,P <0.05),the ser-um levels of CHE in patients with LC were also negatively correlated with levels of AFU and AFP (correlation coefficient was-0.598 and -0.653 respectively,P <0.05).The positive rates of joint detection of CHE,AFU and AFP in PHC group and LC group were higher than that in the control group,had statistically significant differences(P <0.05 ).Conclusion CHE,AFU and AFP are sensitive indicators of liver lesions,and the joint detection could provide reference value for diagnosis and monitoring pro-gression of PHC.
5.The effects of informed consent on emotion and immunologic function in patients with laryngopharynx cancer

Objective To investigate the effects of informed consent on emotion and immunologic function in patients with laryngopharynx cancer.Methods Sixty patients with laryngopharynx cancer were divided into group A and group B by random digits table with 30 cases each group.In group A patients were informed consent,and in group B family were informed consent.The self-rating anxiety scale (SAS) and self-rating depression scale (SDS) scores,as well as cellular immunity function were measured before informed consent and 2 weeks after surgery.Results The SAS,SDS scores and levels of cellular immunity function between two groups,differences was no statistically significant (P > 0.05).The SAS,SDS scores 2 weeks after surgery in group A were significantly less than those in group B [(43.17 ± 7.63) scores vs.(47.84 ± 8.46) scores,(37.43 ± 8.69) scores vs.(42.67 ± 9.82) scores,P < 0.05].The CD3,CD4,NK cells 2 weeks after surgery in group A were significantly higher than those in group B (0.5762 ±0.0941 vs.0.5160 ± 0.0821,0.3832 ± 0.0751 vs.0.3140 ± 0.0561,0.1521 ± 0.0491 vs.0.1163 ± 0.0422,P < 0.05),CD8 and CD4/CD8 between two groups differences was no statistically significant (P > 0.05).Conclusion The informed consent for patients with laryngopharynx cancer may relieve emotional disorders and avoid immunologic function decreasing.

In 2010,Ruijin Hospital Affiliated to Shanghai Jiaotong University School of Medicine and Chinese Center for Disease Control and Prevention conducted collaboratively a 2010 China Noncommunicable Disease Surveillance,and reported the results of diabetes prevalence and control in Chinese adults,which was published in the Journal of American Medical Association (JAMA) in 2013.This article is the interpretation of that report by original authors.
7.Effects of high natural background radiation on chromosome aberration frequency of peripheral blood lymphocytes of long-term exposed inhabitants: a Meta analysis

Objective To quantitatively analyze the effects of high background radiation area (HBRA) on the frequencies of translocation and unstable chromosome aberrations in human peripheral blood lymphocytes.Methods Based on the data from 9 published articles retrieved from 7 electronic databases,the Meta-analysis was performed to evaluate the effects of high natural background radiation (HNBR) on the frequencies of chromosome aberrations in the peripheral blood lymphocytes in 17 777 persons from HBRA and 10 386 from the control area (CA).Cochrane's Q and I2 statistics were used to evaluate heterogeneity among studies and pooling odds ratio (OR) with 95% confidence intervals (95% CI) were calculated using random-effect models.Publication bias was also calculated by funnel plot,Egger linear regression test and Begg rank correlation test.Results The pooling OR of translocation [OR(95% CI) =1.57(1.10-2.24)] and unstable chromosome aberration [OR(95% CI) =2.04(1.32-3.14)] of long-term exposed inhabitants living in HBRA were higher than control.The subgroup analysis showed that,fortranslocation,the OR (95% CI) were 1.24 (1.09-1.42,I2 =0.00) for male,1.37 (1.17-1.60,I2 =0.00) for female,1.17 (1.05-1.30,I2 =69.50%) for adults,and 1.38 (1.25-1.51,I2 =0.00) for Chinese.For unstable chromosome aberration,the OR (95% CI) were 3.78 (2.40-5.97,I2 =0.00) for female,2.60 (2.25-3.00,I2 =69.60%) for adults,1.03(0.85-1.24,I2 =0.00) for children,3.19 (2.46-4.13,I2 =21.60%) for Iranian,and 1.64 (1.33-2.02,I2 =0.00) for Chinese.HBRA,age and sex differences were the reasons of above heterogeneity.Conclusions To the inhabitants living in HBRA,the frequencies of translocation and unstable chromosome aberration of peripheral blood lymphocytes are higher than those in control area.More comprehensive analysis should be performed to assess the health risk in HBRA inhabitants,which may arise new stragety in radiation protection.
8.Comparison of detection rate of osteoporosis in different sex,age and skeleton location

BACKGROUND:Bone mineral density(BMD) is still regarded as the standard of early diagnosis and treatment of osteoporosis(OP) at present.But it is found in detection that different sex,age and skeleton location have different OP detection rate,so it is necessary to analyze the difference. OBJECTIVE:To compare the difference of OP detection rate at different skeleton location between males and females with the increase of age. DESIGN:A cross-sectional study taking patients as the subjects. SETTING:Endocrine department of an artillery general hospital of Chinese PLA. PARTICIPANTS:A total of 147 patients,including 54 males and 93 females, aged from 50 to 78 years old,who were hospitalized in our outpatient clinic from September 2000 to January 2002,were selected and divided into 3 groups according to age,50 to 59 years old group (n=46,13 males and 33 females),60 to 69 years old group (n=66,26 males and 40 females) and 70 to 79 years old group (n=35,15 accordance with the OP diagnostic criteria recommended by WHO[1]. Exclusive criterion: secondary OP patients caused by chronic disease of liver,kidney, heart, and gastrointestinal tract and some endocrine disease such as diabetes,hyperthyroidism and so on. INTERVENTIONS:Every subject filled in the history questionnaire in detail.Height and body mass were measured accurately and body mass index(BMI) was calculated (kg/m2).A new type of Norland Excell plus dual-energy X-ray absorptiometry(DEXA) was used to detect BMD(g/cm2) of L2- 4 and proximate femur(neck of femur, Ward's triangle,greater trochanter).The detected values were compared with the normal data of young adults of the same sex and the T value(SD) was obtained. RESULTS:OP in lumber vertebra was predominant in female climacteric(χ 2=10.14,P< 0.01),and the detection rate of OP in lumber vertebra and neck of femur increased with age(χ 2=7.41, P< 0.05).OP in simple neck of femur increased significantly in males after 60 yeas old(χ 2=9.11,P< 0.05). Females were more liable to suffer from OP in simple lumber vertebra and in both lumber vertebra and neck of femur(χ 2=8.04,P< 0.05;χ 2=14.26,P< 0.01).Age had significant negative correlation with BMD in neck of femur,Ward's triangle and great trochanter of females(r=- 0.364,- 0.389, P< 0.01;r=- 0.504,P< 0.001),while BMI was positively correlated with L2- 4,neck of femur and great trochanter significantly(r=0.306,0.329,0.338,P< 0.05). CONCLUSION:Detection rate of OP changes with skeleton detecting location and age.It is very significant to recognize and evaluate these objective phenomena correctly for the diagnosis and treatment of OP.
9.Effect of intraoperative wake-up test on postoperative delirium in patients undergoing scoliosis surgery under analgesia with sufentanil

Objective To investigate the effect of the intraoperative wake-up test on the postoperative delirium in patients undergoing scoliosis operation under analgesia with sufentanil. Methods Sixty ASA Ⅰ or Ⅱ patients , aged 12-60 yr, scheduled for scoliosis surgery, were randomly divided into 2 groups ( n = 30 each) : control group (group C) and intraoperative wake-up test group (group W) . The intraoperative wake-up test was not performed during operation in group C. Anesthesia was induced with target-controlled infusion of sufentanil with the target effect-site concentration set at 0.5 ng/ml and iv injection of propofol 1-2 nig/kg. As soon as the patients lost consciousness, tracheal intubation was facilitated with 0.15 mg/kg cisatracurium besylate. The patients were mechanically ventilated. Anesthesia was maintained with target-controlled inhalation of sevoflurane (target end-tidal concentration 0. 8%-1. 5%), target-controlled infusion of sufentanil (target effect-site concentration 0.2-0.3 ng/ml) and iv infusion of cisatracurium besylate 0.1 mg- kg-1·h-1 . In group W, the patients underwent the intraoperative wake-up test. Postoperative analgesia was provided with sufentanil. The occurrence of intraoperative awareness and postoperative delirium was recorded. Results No significant difference was found in the incidences of intraoperative awareness and postoperative delirium between the two groups. Conclusion The intraoperative wake-up test is not the risk factor for postoperative delirium in patients undergoing scoliosis surgery under analgesia with sufentanil, and inhibition of perioperative pain and intraoperative awareness by analgesia with sufentanil may be involved in the mechanism.
10.Optimizing the method in calculating the insulin dosages in the insulin hypoglycemia-growth hormone stimulation test:preliminary exploration

Objective To explore more suitable calculation method of the insulin dosage in insulin hypoglycemia-growth hormone stimulation test(insulin tolerance test, ITT). Methods Fifty-six subjects suspected of growth hormone deficiency were divided into primary and secondary onset groups. All the patients took oral glucose tolerance test and ITT. Homeostasis model of assessment for insulin resistance index ( HOMA-IR) and insulin sensitivity index ( ISI), area under insulin curve ( AUCINS ) and the area under glucose curve ( AUCPG ) were calculated. The insulin dosages during ITT between two groups were compared and the main factors influencing the insulin dosage were analyzed. Results There was no difference in the insulin dosage during ITT between primary and secondary groups. The actual dosage of insulin in this cohort study revealed a significant difference from the initial insulin dosage recommended by the guideline. Multiple linear regression analysis found that AUCINS and body mass index were the independent factors affecting the insulin dosage. Then the optimized coefficient of ITT ( γ) were found. Conclusion The insulin dosage used in our study was inconsistent with the guidelines-recommended ones. In order to make ITT more efficient and safer, a more optimized calculation method to improve the successful rate of insulin-induced hypoglycemia in ITT is proposed.
