1.Impact of C-reactive protein, albumin and bilirubin on elderly patients with acute suppurative cholecystitis with somatostatin

Objective To observe the efficacy of somatostatin on elderly patients with acute suppurative cholecystitis and its impact on the C-reactive protein (CRP), prealbumin (PA) and total bilirubin (TBIL) effects before or after operation. Method 110 elderly patients with acute suppurative cholecystitis, which from January 2011 to June 2013, were randomly divided into observation group and control group, each group had 55 cases, the control group were gave operation, the observation group were treated with somatostatin on basis of control group before and after operation. The operative time, blood loss, postoperative visual analog scale (VAS) score and morbidity were observed. At the same time, the levels of CRP, PA and TBIL were detected before and after operation. Results After treatment, the blood loss, postoperative hospital stay and postoperative complication rate in observation group were significantly lower than those in control group (P<0.01), while the differences in operative time and postoperative VAS score between two groups were not significant . After treatment, the levels of CRP, and TBIL appear significantly lower than those before treatment (P<0.01), while the PA levels appear significantly higher compared with those before treatment (P<0.01), the decreasing or increasing levels in observation group were more obvious compared with control group (P<0.01). Conclusion The adjuvant therapy efficacy of somatostatin were obvious before and after operation, they can significantly reduce the body's inflammatory response, and promote the postoperative recovery.
2.The study on the efficacy of gestrinone and Sanjie zhentong capsule for the adjunctive treatment of ovarian endometriosis cyst after laparoscopic opteration

Objective To study the efficacy of gestrinone and Sanjie zhentong capsule for the adjunctive treatment of endometriesis cyst of ovary after laparoscopic operation.Methods 116 patients with ovarian endometriosis cyst after laparoscopic operation were randomly divided into three groups:A group(gestfinone and Sanjie Zhentong capsule,n=42),after operation were given gestrinone(2.5mg each time,two times per-week for 3~months)and Sanjie zhentong capsule(4 pills each time,three times everyday for 3 months);B group(gestrinone,n=38),after operation were only given gestrinone(2.5mg each time,two times per-week for 3-6 months);C group(Sanjie zhentong capsules,n=36),after operation were only given Sanjie zhentong capsules(4 pills each time,three times everyday for 3 months).All patients were followed up for 6~12 months.Results The follow-up visit rate was 96.5%(112/116).A group,B group and C group,their excellence rate was respectively 97.5%,67.6%and 65.7%,their recurrence rate was respectively 2.5%,24.3%and 22.9%.There was significant difference between A group and B group.C group(P<0.05).Incidence rate of three groups'side effects was respectively 35.0%,29.7%and 14.3%,there was no significant difference among three groups(P>0.05).Conclusion The efficacy of gestfinone and Sanjie zhentong capsule for the adjunctive treatment of endometriosis cyst of ovary after laparoscopic operation was better than that of the only medicine treatment.
3.Antimicrobial Drugs Application in Type Ⅰ and Type Ⅱ Operations During Perioperative Period:Survey and Analysis

OBJECTIVE To find out the situation of the usage of antimicrobials in type Ⅰand type Ⅱ operations during perioperative period and to evaluate the rationality of use. METHODS To check 472 examples of medical record data of discharged patients with type Ⅰand type Ⅱ operations in each department during from Apr to Jun and from Aug to Oct 2006,and make statistics and analysis according to application rate of antibacterial drugs, examining rate of the samples, drug usage by experiences and postoperative infection rate. RESULTS The application rate of antibacterial drugs during perioperative period in our hospital was high;the examining rate of correlated samples was low;the choice of antibacterial drugs mainly depended on experience;the sensitivity of the chosen antibacterial drugs was not strong;the duration of prophylactic drug usage was too long;and the postoperative infection rate was high. CONCLUSIONS It is very important to strengthen standardized administration of prophylactic antibiotic usage during perioperative period in our hospital.
4.Relationship between obesity and insomnia among adults

Objective:
To investigate the association between obesity and insomnia among adults, so as to provide insights into obesity control and sleep health improvements among adult populations.
Methods:
Sixteen communities were randomly sampled from Longgang District, Shenzhen City using a multistage cluster sampling method, and permanent residents at ages of 18 years and older living in these communities were recruited as study subjects. Subjects' gender, age, educational level and marital status were collected using a self-designed questionnaire, and the body mass index ( BMI) was calculated. The sleep quality was evaluated using the Insomnia Severity Index ( ISI ), and the correlation between obesity and insomnia was evaluated using a multivariable logistic regression model.
Results:
A total of 10 434 questionnaires were allocated, and 9 425 valid questionnaires were recovered, with an effective recovery rate of 90.33%. The respondents included 3 448 men ( 36.58% ) and 5 977 women ( 63.42% ), and 6 940 respondents ( 73.63% ) were at ages of 18 to 44 years. There were 778 respondents with underweight ( 8.25% ), 5 522 with normal weight ( 58.59% ), 2 209 with overweight ( 23.44% ) and 916 with obesity ( 9.72% ). Among all respondents, 1 149 respondents were detected with insomnia, with a detection rate of 12.19%, including 839 subjects with mild insomnia ( 8.90% ), 247 subjects moderate insomnia ( 2.62% ) and 63 subjects with severe insomnia ( 0.67% ). Multivariable logistic regression analysis showed a statistically significant correlation between obesity and insomnia ( OR=2.179, 95%CI: 1.824-2.603 ).
Conclusions
There is a significant correlation between obesity and insomnia. Body weight management and sleep health education shall be intensified among adults.
5.Prognostic analysis of neuroendocrine component in colorectal adenocarcinoma

Objective To investigate the differentiation of neuroendocrine component (NEC) in colorectal adenocarcinoma in relation to its significance by comparing the outcome between patients with or without NEC.Methods The paraffin sections from patients with pathologically confirmed colorectal adenocarcinoma were retrospectively collected and screened for those with NEC by morphological examination and immunohistochemistry with neuroendocrine markers.Control patients (n=54) without NEC were selected from colorectal cancer database and 2: 1 matched on the basis of clinical features with NEC positive patients (n=27).Relative analysis was performed between two groups.Results With a median follow-up of 72 months,the 5-year disease free survival was 58.0% (16/27) in NEC positive group and 79.1% (43/54) in control group (P=0.036).Similarly,the 5-year cancer-specific overall survival was significantly lower in NEC positive group than in control group (58.3% versus 81.1%,P=0.037).Cox regression showed that the 5-year cumulative risks of disease recurrence and cancer-caused death in NEC positive patients were 2.38 and 2.41 times higher than those in control patients,respectively.Conclusions NEC appears to bear a poor prognosis in patients with colorectal adenocarcinoma.

A cross-sectional survey of 3589 adolescents was conducted in three cities from different typical geographical zones of Guangdong province. The median urinary iodine concentrations (MUI) of adolescents in Nanxiong, Guangzhou and Maoming were 286.6,204.1 and 166.0μ/L, respectively. The MUI of all these adolescents Was 231.7μg/L, which was slightly higher than the current World Health Organization recommendation.

[Summary] A 47 years old female presented with palpitation, tremble, sweaty, and weight gain. Insulinoma was suspended and confirmed with hypoglycemia caused by endogenous hyperinsulinemia. The CT scan was failed to localize the tumor. MRI and ASVS showed inconsistent results. A nutrient arterial was found by arterial angiography and explained the different results. Robert assistant surgery successfully removed the tumor and the patient was well recovered.
8.Adrenal Castleman′s disease:A case report and literature review

Objective To raise the awareness of adrenal Castleman′s disease by analyzing the clinical features and management of a patient with adrenal Castleman′s disease. Methods A case of adrenal Castleman′s disease of our hospital was retrospectively analyzed, including clinical feature, laboratory findings, pathology, treatment, and follow-up. All the data and pertinent literatures were reviewed and analyzed. Results An incidentaloma measuring 4. 8 cm × 6. 3 cm in the right adrenal gland was observed in a 30-year-old men in a ultrasonography examination performed due to a medical check-up. Laboratory analysis showed that the lesion was not hyperfunctioning. The patient subsequently underwent an exploratory laparotomy. Pathological examination revealed retroperitoneally localized Castleman′s disease of the hyaline vascular type. Conclusion Adrenal Castleman′s disease is a rare cause of lymph node hypertrophy, and it is necessary to keep in mind the possibility of its occurrence and take it into consideration in the differential diagnosis of any solitary, heterogeneous, and hypervascular retroperitoneal mass. The proper cooperation between the clinician and pathologist allows early diagnosis and suitable therapy.
9.The application of multicolor probe melting curve analysis for the prenatal diagnosis ofβthalassemia

Objective To establish a method of multicolor melting curve analysis for the prenatal diagnosis ofβthalassemia.Methods Methodology establishment.A total of 95 cases, including 9 fetal villi samples(10-13 weeks)and 86 amniotic fluid samples(18-24 weeks)were collected by Center for Prenatal Diagnosis of Shenzhen Maternity and Child Healthcare Hospital between January 2014 and December 2014.A double-blind test was done to detect the mutations of beta globin gene by means of reverse dot ( RDB) blot and multicolor melting curve analysis ( MMCA).The consistency of the two methods is compared.Results The results of 93 cases detected by MMCA and RDB are completely consistent.The results of the 2 cases detected by MMCA after correction are the same as the results detected by RDB.Finally, the coincidence rate of the result was 100%.Conclusion MMCA can be applied to the prenatal diagnosis ofβthalassemia as an effective supplement to RDB.
10.A simple index derived from triglycerides and blood glucose for identifying insulin resistance

ObjectiveTo compare the sensitivity and specificity of a new index of insulin resistance (IR) derived from plasma triglyceride and glucose with homeostasis model assessment for insulin resistance index ( HOMA-IR) and insulin sensitivity index from frequent sampling intravenous glucose tolerance test (FSIGT-ISI).Methods A total of 1024 subjects (240 normal control,335 with obesity,312 impaired glucose regulation,and 137 type 2 diabetes mellitus) were included in the present study.Standard oral glucose tolerance test,insulin release test,lipid profiles,and other biochemical markers were measured. Among them 540 subjects were selected to undertake FSIGT.TyG index is derived from plasma triglyceride and fasting glucose.TyG2 index is derived from plasma triglyceride and postprandial 2 plasma glucose.ResultsPearson correlation coefficient between TyG and HOMA-IR or FSIGT-ISI was 0.427 ( P<0.01 ) and -0.100 ( P=0.024 ),respectively,and that between TyG2 and HOMA-IR or FSIGT-ISI was 0.455 ( P<0.01 ) and - 0.162 ( P<0.01 ),respectively.The sensitivity and specificity of TyG index for diagnosis of IR was 68.5% and 63.5% compared with HOMA-IR,and 68.5% and49.5% compared with FSIGT-ISI.For TyG2 index,the slightly higher sensitivity ( 81.7% with HOMA-IR,75.7% with FSIGT-ISI),but lower specificity (51.5% with HOMA-IR,48.2% with FSIGT-ISI) were found.ConclusionsBothTyGandTyG2 indices could be used as a surrogate for assessing IR in Chinese subjects.However,considering its moderately high sensitivity but low specificity,these two indices are limited in the use of large-scale epidemiological screening.
